    Rôle pronostic des anticorps anti-HLA en transplantation rénale (approches en population)

    Contexte : La réponse allo-immune induite par la transplantation à partir d'un donneur génétiquement différent est un obstacle majeur au succès de la greffe. Notre objectif est de caractériser les différents phénotypes de rejet d'allogreffe rénale et d'identifier la façon dont chacun est associé aux anticorps anti-HLA. Nous avons également évalué l impact de certaines propriétés de ces anticorps, comme leur intensité ou leur capacité à fixer le complément, sur l'échec des allogreffes rénales. Pour finir, nous avons étudié l impact pronostic des formes indolentes de rejets ainsi que l apport des nouvelles technologies d analyses transcriptomique des biopsies de patients transplantés. Méthodes : Nous avons utilisé une approche en population, basée sur l étude de larges cohortes de receveurs de greffes rénales. L étude concomitante des données immunologiques et histologiques, nous a permis de corréler les caractéristiques des anticorps anti-HLA circulants aux phénotypes lésionnels. Résultats : Nous avons identifié et caractérisé 4 types distincts de rejet : les rejets vasculaires médiés par les lymphocytes T (9%) et par les anticorps (21%), non reconnus par les classifications internationales, et les rejets cellulaires (46%) et humoraux sans vascularite (24%). Le risque de perte de greffons est le plus important dans les cas de rejet vasculaire médié par anticorps. Les anticorps dirigés contre le donneur (DSA) fixant le complément induisent un phénotype histologique plus sévère, dominé par des lésions inflammatoires et plus de dépôts de la fraction C4d du complément dans les greffons. En leur présence, le risque de perte de greffons est augmenté de 3,7 fois (IC95 1,9-7,2). Les formes indolentes de rejet médié par les anticorps sont également associées à un risque accru de perte du greffon. L utilisation d approches moléculaires permet d améliorer la stratification du risque au sein du groupe des patients présentant des rejets humoraux. Conclusion : Ce travail répond à un besoin clinique pressant dans le domaine de la transplantation, celui de déterminer l impact clinique des anticorps anti-HLA et d améliorer la stratification du risque immunologique en se basant sur leurs propriétés et l utilisation de nouvelles technologies pour mieux caractériser l activité et le stade des rejets humoraux.Background : The alloimmune response induced by transplantation from a donor who differs genetically from the kidney recipient has always been the major obstacle to graft success. The present work aimed to improve characterization of kidney-allograft rejection phenotypes and identify how each one is associated with anti-HLA antibodies. We also sought to determine whether characteristics of these antibodies i.e., their levels or complementbinding ability, might play a role in kidney allograft failure. Finally, we evaluated the clinical relevance of indolent forms of ABMR and the clinical relevance of new genes expression technologies to stratify the kidney recipients at risk for failure. Methods : We used a population-based approach in precisely phenotyped cohorts of kidney recipients. The design of our study, which is based on the concomitant evaluation of immunologic and histologic data, permits a precise connection of circulating anti-HLA antibodies with a phenotype of graft injury. Findings : We identified four distinct patterns of kidney allograft rejection: T cell-mediated vascular rejection (9%), antibody-mediated vascular rejection (21%), not included in international classifications, T cell- (46%) and antibody-mediated rejection without vasculitis (24%). Risk of graft loss was 9.07 times (95CI 3.6-19.7) higher in antibody-mediated vascular rejection than in T-cell mediated rejections (p<0.0001). Patients with post-transplant complement-binding DSA had more severe graft injury phenotype with higher inflammation and increased deposition of complement fraction C4d. They have the poorest graft survival with 3.7 fold increased risk of graft loss (95CI 1.9-7.2). Subclinical ABMR is a truncated for of rejection associated with risk of kidney allograft failure. Gene expression assessment in kidney allografts with early ABMR improves classification of individuals at risk for kidney allograft loss. Conclusion : This work addresses the unmet need of the deleterious impact of anti-HLA antibodies and the improvement of risk stratification in kidney transplantation. Recognition of distinct phenotypes could lead to the development of new treatment strategies.     Atrial fibrillation: the current epidemic.

    Atrial fibrillation (AF) is the most common arrhythmia diagnosed in clinical practice. The consequences of AF have been clearly established in multiple large observational cohort studies and include increased stroke and systemic embolism rates if no oral anticoagulation is prescribed, with increased morbidity and mortality. With the worldwide aging of the population characterized by a large influx of "baby boomers" with or without risk factors for developing AF, an epidemic is forecasted within the next 10 to 20 years. Although not all studies support this evidence, it is clear that AF is on the rise and a significant amount of health resources are invested in detecting and managing AF. This review focuses on the worldwide burden of AF and reviews global health strategies focused on improving detection, prevention and risk stratification of AF, recently recommended by the World Heart Federation

    Evolution de la prise en charge et du pronostic des syndromes coronariens aigus en France entre 1995 et 2010

    Dans les pays développés , les syndromes coronariens aigus (SCA) représentent une pathologie fréquente et grave et les maladies cardiovasculaires restent la première cause de mortalité en Europe. Au cours de la dernière décennie, pourtant, plusieurs travaux épidémiologiques ont suggéré une baisse sensible de l'incidence des infarctus et la mortalité cardiovasculaire est dorénavant en recul dans de très nombreux pays, dont la France. La cardiologie est une des disciplines médicales qui a connu les plus grands bouleversements au cours des 25 dernières années et la prise en charge des SCA ainsi que le profil des patients ont considérablement évolué. Dans ce contexte, il nous a paru intéressant d'étudier la manière dont le devenir des patients présentant un infarctus aigu pouvait participer à cette baisse générale de la mortalité cardio-vasculaire. A partir de quatre enquêtes longitudinales successives répertoriant les SCA (USIK 1995, USIC 2000, FAST-MI 2005, FAST-MI 2010) et de l observatoire national des actes de cardiologie interventionnelle (ONACI), nous avons observé, après standardisation sur les caractéristiques initiales des différentes cohortes, une baisse spectaculaire de la mortalité quel que soit le type de SCA (avec sus-décalage ST [SCA ST+] ou ST-elevation myocardial infarction [STEMI] ; sans sus-décalage ST [SCA ST-] ou non-ST-elevation myocardial infarction [NSTEMI]). Cette évolution peut être expliquée par plusieurs paramètres : amélioration de la prise en charge globale, meilleur suivi des recommandations, changement de profils des patients (pour les STEMI), développement de la stratégie invasive et utilisation de nouvelles thérapeutiques, évolution des techniques de cardiologie interventionnelle Ainsi, il apparaît que l'amélioration du pronostic des patients atteints d'infarctus est bien un des éléments ayant pu contribuer à la baisse de la mortalité cardiovasculaire. L enjeu aujourd hui est de maintenir ces résultats, de renforcer les mesures de prévention et d améliorer le pronostic à long terme en développant notamment les programmes d éducation thérapeutique.In developed countries, acute coronary syndromes (ACS) represent a common and serious disease, and cardiovascular disease remains the leading cause of death in Europe. During the last decade, however, several epidemiological studies have suggested a significant reduction in the incidence of myocardial infarction and cardiovascular mortality in many countries, including France. Over the past 25 years, Cardiology has dramatically evolved and the management of ACS, as well as patient risk profile have substantially changed. In this context, we aimed to evaluate how the outcomes of patients with acute myocardial infarction could participate in the general decline in cardiovascular mortality. From four successive longitudinal surveys including ACS (USIK 1995, USIC 2000, FAST-MI 2005, FAST-MI 2010) and the national observatory of interventional cardiology (ONACI) we observed, after standardization of the cohorts on baseline clinical characteristics, a dramatic decline in mortality regardless of the type of ACS (STEMI, ST-elevation myocardial infarction, NSTEMI, non-ST-elevation myocardial infarction). This evolution can be explained by several factors: overall improvement in organization of care, better implementation of recommendations, substantial change in the patient risk profile (for STEMI), increasing use of invasive strategy and adjunctive therapies, improved technique for Interventional Cardiology ... Therefore, the improved prognosis of patients with myocardial infarction appears to be one of the factors that have contributed to the decline in cardiovascular mortality.     Pocket-sized focused cardiac ultrasound: Strengths and limitations

    SummaryFocused cardiac ultrasound (FCU) has emerged in recent years and has created new possibilities in the clinical assessment of patients both in and out of hospital. The increasing portability of echocardiographic devices, with some now only the size of a smartphone, has widened the spectrum of potential indications and users, from the senior cardiologist to the medical student. However, many issues still need to be addressed, especially the acknowledgment of the advantages and limitations of using such devices for FCU, and the extent of training required in this rapidly evolving field. In recent years, an increasing number of studies involving FCU have been published with variable results. This review outlines the evidence for the use of FCU with pocket-echo to address specific questions in daily clinical practice

    The Citius End: World Records Progression Announces the Completion of a Brief Ultra-Physiological Quest

    World records (WR) in sports illustrate the ultimate expression of human integrated muscle biology, through speed or strength performances. Analysis and prediction of man's physiological boundaries in sports and impact of external (historical or environmental) conditions on WR occurrence are subject to scientific controversy. Based on the analysis of 3263 WR established for all quantifiable official contests since the first Olympic Games, we show here that WR progression rate follows a piecewise exponential decaying pattern with very high accuracy (mean adjusted r2 values = 0.91±0.08 (s.d.)). Starting at 75% of their estimated asymptotic values in 1896, WR have now reached 99%, and, present conditions prevailing, half of all WR will not be improved by more than 0,05% in 2027. Our model, which may be used to compare future athletic performances or assess the impact of international antidoping policies, forecasts that human species' physiological frontiers will be reached in one generation. This will have an impact on the future conditions of athlete training and on the organization of competitions. It may also alter the Olympic motto and spirit

    Heart Rate and Risk of Cancer Death in Healthy Men

    BACKGROUND: Data from several previous studies examining heart-rate and cardiovascular risk have hinted at a possible relationship between heart-rate and non-cardiac mortality. We thus systematically examined the predictive value of heart-rate variables on the subsequent risk of death from cancer. METHODS: In the Paris Prospective Study I, 6101 asymptomatic French working men aged 42 to 53 years, free of clinically detectable cardiovascular disease and cancer, underwent a standardized graded exercise test between 1967 and 1972. Resting heart-rate, heart-rate increase during exercise, and decrease during recovery were measured. Change in resting heart-rate over 5 years was also available in 5139 men. Mortality including 758 cancer deaths was assessed over the 25 years of follow-up. FINDINGS: There were strong, graded and significant relationships between all heart-rate parameters and subsequent cancer deaths. After adjustment for age and tobacco consumption and, compared with the lowest quartile, those with the highest quartile for resting heart-rate had a relative risk of 2.4 for cancer deaths (95% confidence interval: 1.9-2.9, p<0.0001) This was similar after adjustment for traditional cardiovascular risk factors and was observed for the commonest malignancies (respiratory and gastrointestinal). Similarly, significant relationships with cancer death were observed between poor heart rate increase during exercise, poor decrease during recovery and greater heart-rate increase over time (p<0.0001 for all). INTERPRETATION: Resting and exercise heart rate had consistent, graded and highly significant associations with subsequent cancer mortality in men

    Sports-related sudden cardiac arrest in young adults

    AIMS Data on sports-related sudden cardiac arrest (SrSCA) among young adults in the general population are scarce. We aimed to determine the overall SrSCA incidence, characteristics, and outcomes in young adults. METHODS AND RESULTS Prospective cohort study of all cases of SrSCA between 2012 and 2019 in Germany and Paris area, France, involving subjects aged 18-35 years. Detection of SrSCA was achieved via multiple sources, including emergency medical services (EMS) reporting and web-based screening of media releases. Cases and aetiologies were centrally adjudicated. Overall, a total of 147 SrSCA (mean age 28.1 ± 4.8 years, 95.2% males) occurred, with an overall burden of 4.77 [95% confidence interval (CI) 2.85-6.68] cases per million-year, including 12 (8.2%) cases in young competitive athletes. While bystander cardiopulmonary resuscitation (CPR) was initiated in 114 (82.6%), automated external defibrillator (AED) use by bystanders occurred only in a minority (7.5%). Public AED use prior to EMS arrival (odds ratio 6.25, 95% CI 1.48-43.20, P = 0.02) was the strongest independent predictor of survival at hospital discharge (38.1%). Among cases that benefited from both immediate bystander CPR and AED use, survival rate was 90.9%. Coronary artery disease was the most frequent aetiology (25.8%), mainly through acute coronary syndrome (86.9%). CONCLUSION Sports-related sudden cardiac arrest in the young occurs mainly in recreational male sports participants. Public AED use remains disappointingly low, although survival may reach 90% among those who benefit from both bystander CPR and early defibrillation. Coronary artery disease is the most prevalent cause of SrSCA in young adults
