Postpartum Safety and Satisfaction Following Early Discharge
Two Ontario sites were involved in the evaluation of an obstetrical discharge program. Before program implementation a group of eligible women were enrolled as the preprogram control group (n = 542). During the program, eligible women who agreed to early discharge (ED) became the ED group (n = 319), and those opting not to go home early but consenting to participate in the evaluation became the concurrent group (n = 456). All groups were mailed a self-administered postpartum questionnaire. On demographic characteristics, safety and satisfaction, the ED group was comparable to the concurrent group. Hospital readmission rates did not differ across groups after stratification by site or hospital. Multiple classification analyses revealed a similar pattern for overall satisfaction levels. This unique ED program, which allowed pre- or postnatal enrollment and did not require an initial home assessment, appears to be a safe, effective and flexible approach to obstetrical care
Enhanced Mechanistic Understanding Through the Detection of Radical Intermediates in Organic Reactions
Two applications of a radical trap based on a homolytic substitution reaction (SH2') are presented for the trapping of short-lived radical intermediates in organic reactions. The first example is a photochemical cyanomethylation catalyzed by a Ru complex. Two intermediate radicals in the radical chain propagation have been trapped and detected using mass spectrometry (MS), along with the starting materials, products and catalyst degradation fragments. Although qualitative, these results helped to elucidate the reaction mechanism. In the second example, the trapping method was applied to study the radical initiation catalyzed by a triethylboronoxygen mixture. In this case, the concentration of trapped radicals was sufficiently high to enable their detection by nuclear magnetic resonance (NMR). Quantitative measurements made it possible to characterize the radical flux in the system under different reaction conditions (including variations of solvent, temperature and concentration) where modelling was complicated by chain reactions and heterogeneous mass transfer
An Untranslated cis-Element Regulates the Accumulation of Multiple C4 Enzymes in Gynandropsis gynandra Mesophyll Cells.
C4 photosynthesis is a complex phenotype that allows more efficient carbon capture than the ancestral C3 pathway. In leaves of C4 species, hundreds of transcripts increase in abundance compared with C3 relatives and become restricted to mesophyll (M) or bundle sheath (BS) cells. However, no mechanism has been reported that regulates the compartmentation of multiple enzymes in M or BS cells. We examined mechanisms regulating CARBONIC ANHYDRASE4 (CA4) in C4 Gynandropsis gynandra. Increased abundance is directed by both the promoter region and introns of the G. gynandra gene. A nine-nucleotide motif located in the 5' untranslated region (UTR) is required for preferential accumulation of GUS in M cells. This element is present and functional in three additional 5' UTRs and six 3' UTRs where it determines accumulation of two isoforms of CA and pyruvate,orthophosphate dikinase in M cells. Although the GgCA4 5' UTR is sufficient to direct GUS accumulation in M cells, transcripts encoding GUS are abundant in both M and BS. Mutating the GgCA4 5' UTR abolishes enrichment of protein in M cells without affecting transcript abundance. The work identifies a mechanism that directs cell-preferential accumulation of multiple enzymes required for C4 photosynthesis.We would like to thank the BBSRC for funding.This is the author accepted manuscript. Fatty Acid Copper(II) Carboxylates with Nicotinamide - Characterization and Fungicidal Activity. Crystal Structures of Two Heptanoate Forms and Nonanoate
Several new compounds of the composition Cu2(OOCCnH2n+1)4
(nia)2 (nia = nicotinamide; n = 6 to 11) were synthesized, characterized and tested for fungicidal activity. Crystal structure determinations revealed dinuclear structures of the copper(II) acetate hydrate type for compounds [Cu2(OOCC6H13)4(nia)2]-A (1A), [Cu2(OOCC6H13)4(nia)2]-B (1B) and [Cu2(OOCC8H17)4(nia)2] (3).
Other applied characterization methods indicate dimeric structures for ali synthesized compounds [ÎĽeff (298 K) = 1.43-1.50 BM; characteristic band in UV-Vis spectra in the region λ = 350-400 nm]. The same conclusion may also be deduced from the IR (Δ = νasym(COO-) - νsym(COO-) = 183-189 cm-1) and EPR spectra, though some differences were observed for heptanoate modification 1A, probably due to a different hydrogen bonding scheme. Screening for fungicidal activity against the wood-rotting fungus Trametes versicolor (L. ex Fr.) Pilat shows that the compounds dissolved in DMSO completely stop mycelium growth at a concentration of 1.0 Ă— 10-3 mol L-1. Some of them (n = 8, 9, 10) show strong activity also in more diluted Solutions (1.0 Ă— 10-4 mol L-1)
Quantum Logic for Trapped Atoms via Molecular Hyperfine Interactions
We study the deterministic entanglement of a pair of neutral atoms trapped in
an optical lattice by coupling to excited-state molecular hyperfine potentials.
Information can be encoded in the ground-state hyperfine levels and processed
by bringing atoms together pair-wise to perform quantum logical operations
through induced electric dipole-dipole interactions. The possibility of
executing both diagonal and exchange type entangling gates is demonstrated for
two three-level atoms and a figure of merit is derived for the fidelity of
entanglement. The fidelity for executing a CPHASE gate is calculated for two
87Rb atoms, including hyperfine structure and finite atomic localization. The
main source of decoherence is spontaneous emission, which can be minimized for
interaction times fast compared to the scattering rate and for sufficiently
separated atomic wavepackets. Additionally, coherent couplings to states
outside the logical basis can be constrained by the state dependent trapping

Confidence of UK Ophthalmology Registrars in Managing Posterior Capsular Rupture: Results from a National Trainee Survey
Introduction: To establish the level of confidence amongst UK ophthalmology specialist registrars (residents) in managing posterior capsule rupture (PCR) during cataract surgery. / Methods: An online nine-item questionnaire was distributed to all registrars, recruited nationwide via regional representatives. Data collected included stage of training, number of completed cataract operations, cumulative PCR rate, number of PCRs independently managed, understanding of vitrectomy settings and fluidic parameters and access to simulation. Respondents self-evaluated their confidence in managing PCR with vitreous loss. / Results: Complete responses were obtained from 248 registrars (35% response rate). Mean number of phacoemulsification procedures performed was 386. For senior registrars (OST 6–7), 35 out of 70 (50%) felt confident to manage PCR independently and 55 out of 70 (78.6%) were either quite confident or very confident at deciding when to implant an intraocular lens during PCR management. Lower confidence levels were noted for junior trainees (OST 1–2). Over 65% of survey respondents had access to relevant simulation. / Conclusions: Our results represent the largest UK survey analysing the confidence of PCR management amongst registrars. Confidence improves with duration of training and increased exposure to management of PCR. However, 50% of senior registrars still lacked confidence to independently manage PCR and vitreous loss. A specific competency-based framework, potentially using a simulator or simulating a PCR event, incorporated into the curriculum may be desirable
Tics and developmental stuttering
Abstract Background. Developmental stuttering affects 1% of the population but its cause remains unclear. Recent PET studies of metabolism in the central nervous system suggest that it may be related to dysfunction in the basal ganglia or its connections with regions of the cortex associated with speech and motor control. Objective. To determine the presence and characteristics of involuntary movements (IMs) in people who stutter and to investigate the hypothesis that these movements may be of a very similar nature to the IMs seen in patients with movement disorders due to basal ganglia dysfunction. Methods. Sixteen adults with developmental stuttering and 16 controls matched for sex and age were audio-videotaped while freely speaking 300 words in conversation and reading aloud 300 words. The audio data was inspected for dysfluencies and the video data was scrutinised for the presence and characteristics of IMs. Results. Subjects who stuttered produced more IMs than controls during free speech (354 vs 187, p , 0:05Ăž and reading (297 vs 47, p , 0:001Ăž: Most of the IMs in both groups were tics, with a greater number of both simple and complex motor tics (CMTs) in subjects who stuttered. CMTs were more frequent than simple motor tics in those who stuttered, but not in controls. The combination of repetitive eye blink followed by prolonged eye closure was found exclusively in the stuttering group, as were simple tics consisting of eyebrow raise or jaw movement. Dystonia in the form of blepharospasm was identified in a small number of subjects who stuttered. Choreic movements were not associated with stuttering. Conclusions. Developmental stuttering is associated with the presence of IMs that are predominantly simple and CMTs. This association suggests that tics and stuttering may share a common pathophysiology and supports the view that, in common with tics, stuttering may reflect dysfunction in the basal ganglia or its immediate connections.
