Effect of gastrointestinal hormones and neurotransmitters on hunger and satiety
Otyłość i jej powikłania stanowią narastający problem zdrowia publicznego. Celem wielu badańjest poznanie mechanizmów homeostatycznych regulujących masę ciała. Dużą uwagę zwracasię na kontrolę poboru pokarmu przez hormony przewodu pokarmowego i neuroprzekaźniki.Duża część z nich stanowi sygnały prowadzące do zakończenia przyjmowania posiłku. Należądo nich m.in.: cholecystokinina (CCK), glukagopodobny peptyd 1 (GLP-1), peptyd YY (PYY),polipeptyd trzustkowy (PP), glukozależny peptyd insulinotropowy (GIP), oksyn tomodulina(OXM), leptyna, insulina, kortykoliberyna (CRH), nesfatyna-1, ksenina, transkrypt regulowanyprzez kokainę i amfetaminę (peptyd CART) i melanokortyna. Jak dotychczas nie wynalezionoskutecznego leku na otyłość, dlatego pogłębianie wiedzy na tema t wymienionych związkówmoże stworzyć nowe pola działań terapeutycznych w zakresie leczenia nadwa gi i otyłości.Obesity and its complications are a still increasing problem of public health. Many studies arefocused on understanding the homeostatic mechanisms which regulate body weight. Muchattention is paid to the control of food intake by gastrointestinal hormones and neurotransmitters.Many of them act as signals to the end meal consumption and they are called “satiety signals”.Satiety signals include: cholecystokinin (CCK), glucagon-like peptide-1 (GLP-1), peptide YY(PYY), pancreatic polypeptide (PP), glucose-dependent insulinotropic polypeptide (GIP), oxyntomodulin(OXM), leptin, insulin, cor ticotropin-releasing hormone (CRH), nesfatin-1, xenin,CART peptide (cocaine- and amphetamine-regulated transcript) and melanocortin. The effectiveanti-obesity drug have not been invented yet, therefore, advancing our knowledge about thesehormones and neuropeptides can be a new fields of therapeutic actions in the overweight andobesity treatment
Effect of treatment with reducing diet on body composition and modification of nutrients in daily food intake of obese women
WSTĘP. Podstawową metodą leczenia otyłości jest zmiana nawyków
żywieniowych i zwiększenie wydatku energetycznego. Celem
niniejszej pracy była ocena skuteczności rocznej terapii kobiet zbilansowaną
dietą 1500 kcal.
MATERIAŁ I METODY. Program obejmował ocenę sposobu żywienia,
badania antropometryczne oraz badanie składu ciała. Ocenę
ilościową dziennych racji pokarmowych przeprowadzono za pomocą
programu komputerowego Dieta 2. Badaniem objęto
60 kobiet (średnia wieku wynosiła 46 ± 12,42 roku) i oceniono ich
wskaźnik masy ciała (BMI, body mass index [kg/m²]). Skład ciała
oznaczano metodą bioimpedancji elektrycznej. Wszystkie otrzymane
wyniki poddano analizie statystycznej z wykorzystaniem programu
STATISTICA 8.0.
WYNIKI. Wyodrębniono dwie grupy: I - kobiety, które po 12 miesiącach
stosowania diety uzyskały redukcję masy ciała i II - kobiety,
u których po rocznej dietoterapii masa ciała nie zmieniła się lub
wzrosła powyżej wartości wyjściowej. Po roku średnia redukcja masy
ciała u kobiet z grupy I wynosiła 4,4 ± 2,9 kg, natomiast kobiety
z grupy II przytyły średnio o +1,8 ± 1,7 kg. U kobiet z grupy I istotnie
zmniejszyła się procentowa zawartość tkanki tłuszczowej średnio
o 3,9 ± 4,1%. Osiągnięte efekty mogły być zależne od wieku respondentek
oraz ich wyjściowej masy ciała. Nie stwierdzono różnic istotnych
statystycznie między jadłospisami kobiet z grupy I i II zarówno
na początku badania, jak i po roku stosowania diety niskoenergetycznej
(zobserwowano jednak różnice nieistotne statystycznie
w kaloryczności stosowanej diety i zawartości węglowodanów).
Zmniejszenie wartości energetycznej diety u kobiet z grupy I korelowało
dodatnio z uzyskaną redukcją zawartości tkanki tłuszczowej.
WNIOSKI. Niniejsze badanie wskazuje na konieczność prowadzenia
większej liczby szkoleń w zakresie żywienia podczas terapii
odchudzającej. Do zaleceń dietetycznych należy także dodatkowo
wprowadzić regularne ćwiczenia fizyczne w celu zwiększenia
wydatku energetycznego.

179-188INTRODUCTION. The primary treatment of obesity is changing
eating habits and increasing energy expenditure. The purpose of
this study was to evaluate the effectiveness of the annual treatment
of women by 1500 kcal balanced diet. MATERIAL AND METHODS. The program included an assessment
of diet, anthropometric and body composition measurements.
Quantitative assessment of daily food rations were performed using
the computer program Dieta 2. The study included 60 women
(average age was 46 ± 12.42 years) and assessed them body
mass index (BMI kg/m²). Body composition was determined by
bioelectrical impedance. All results were statistically analyzed using
STATISTICA 8.0.
RESULTS. There were identified two groups: I - the women who
after 12 months of diet gained weight reduction and II in which
after the annual diet therapy body weight remained unchanged or
increased above baseline. After one year, the average weight loss
in women from group I was 4.4 ± 2.9 kg, while in women in group
II body weight increased on average of 1.8 ± 1.7 kg. In women
from group I the percentage of body fat significantly decreased by
an average of 3.9 ± 4.1%. The effects may be dependent on the
age and initial body weight of the respondents. There were no
statistically significant differences between women menu in group
I and II, both at baseline and after low energy diet (however, statistically
insignificant differences in the calorie diet and carbohydrate
content were observed). Reducing dietary energy value of women
in group I correlated positively with reduction in body fat.
CONCLUSIONS. This research shows that more dietetic training is
needed in dietary weight loss therapy. In addition to the diet also
regular physical activity should be recommended, to increase energy
expenditure.

Postprandial metabolic and hormonal response to meals with varying carbohydrate content (preliminary examination)
WSTĘP. Zaburzenia homeostazy energetycznej organizmu zależną
od czynników genetycznych, hormonalnych/metabolicznych i środowiskowych
oraz ich wzajemnych interakcji. W wielu badaniach
potwierdzono wpływ diety o różnej zawartości podstawowych
składników odżywczych na centralne i obwodowe mechanizmy
regulacji łaknienia. Celem pracy była ocena zmian metabolicznych
i hormonalnych po posiłkach o różnej zawartości węglowodanów.
MATERIAŁ I METODY. Badaniem objęto 20 zdrowych mężczyzn
w wieku 28–52 lat, ze wskaźnikiem masy ciała (BMI) 19,6–43,0 kg/m2
(średnie BMI 28,0 ± 5,8 kg/m2). Badani w 2-tygodniowym odstępie
czasowym otrzymali standaryzowane izokaloryczne (450 kcal)
posiłki: Nutridrink Fat Free, w którym 89% energii pochodziło z węglowodanów,
oraz Cubitan, w którym 45% energii pochodziło
z węglowodanów. Na czczo, 30, 60, 120, 180 i 240 minut po posiłku
oznaczano stężenia glukozy, insuliny, triacylogliceroli, leptyny
i greliny całkowitej oraz, metodą kalorymetrii pośredniej, oceniono
oksydację substratów energetycznych.
WYNIKI. Po posiłku, w którym 89% energii pochodziło z węglowodanów,
wykazano wyższe stężenia glukozy (p < 0,01) i insuliny
(p < 0,01). W 180. minucie po posiłku wysokowęglowodanowym
u 15% (3 osób) oraz w 240. minucie u 25% (5 osób) badanych odnotowano
glikemię poniżej 60 mg/dl (hipoglikemie). Po posiłku, w którym
45% energii pochodziło z węglowodanów, wykazano wyższe stężenia
triacylogliceroli (p = 0,02) i jednoczesne zwiększenie oksydacji kwasów
tłuszczowych (p < 0,01). Obserwowano też tendencję (p =0,12)
do zmniejszenia stężenia greliny po posiłku, w którym 45% energii
pochodziło z węglowodanów, czego nie stwierdzono po posiłku wysokowęglowodanowym.
Odnotowano różne stężenie leptyny
(p = 0,03), której stężenia wzrastały istotnie (p = 0,01), począwszy od
60. minuty po spożyciu posiłku o mniejszej zawartości węglowodanów.
WNIOSKI. Uzyskane wyniki wskazują, że następstwa metaboliczne
i hormonalne po spożyciu posiłku wysokowęglowodanowego mogą zaburzać równowagę między czynnikami biorącymi udział w regulacji łaknienia. Zaobserwowane zmiany mogą sprzyjać występowaniu
otyłości.INTROCUCTION. Disorders of energy homeostasis depend on interactions
between genetic, hormonal/metabolic and environmental
factors. Several studies have confirmed the influence of diet with
different nutrients content on central and peripheral mechanisms
that regulate energy balance.The aim of the study was to evaluate
metabolic and hormonal response to meals with different carbohydrate
content.
MATERIALS AND METHODS. Twenty healthy men aged 28–52
years and body mass index (BMI) 19.6–43.0 kg/m2 (average BMI
28.0 ± 5.8 kg/m2) were examined. Subjects received (in two weeks
interval) standardized (450 kcal) meals: Nutridrink Fat Free (89%
of energy from carbohydrate) and Cubitan (45% of energy from
carbohydrate). Plasma glucose, insulin, triacyloglicerol, leptin and
total ghrelin levels were measured at 0’, 30’, 60’, 120’, 180’ and
240’ min after meal ingestion. Substrate oxidation was determined
by indirect calorimetry.
RESULTS. After the meal containing 89% of energy from carbohydrates
we found higher glucose (p < 0.01) and insulin (p < 0.01)
concentrations. At 180 min after the high-carbohydrate meal in 15%
(3 men), and at 240 minutes in 25% (5 men) of subjects glucose
levels < 60 mg/dl (hypoglycemia) were noted. After the meal
containing 45% of energy from carbohydrates, triacyloglicerols
levels were higher (p = 0.02) and lipid oxidation was increased
(p < 0.01). We observed tendency (p = 0.12) to reduce plasma
ghrelin levels after meal with 45% of total energy as carbohydrates,
which was not observed after high-carbohydrate meal. Moreover,
varying secretion of leptin (p = 0.03) was noted. Concentrations of
leptin increased significantly (p = 0.01) from 60 minutes after meal
with lower carbohydrate content.
CONCLUSIONS. The results indicate that metabolic and hormonal
response after a high-carbohydrate meal ingestion may affect
unfavourably the balance between factors involved in appetite regulation.
Observed changes may favor obesity
Intake of Meals Containing High Levels of Carbohydrates or High Levels of Unsaturated Fatty Acids Induces Postprandial Dysmetabolism in Young Overweight/Obese Men
Postprandial metabolic response depends on the meals’ components and can be different in normal weight and obese people. However, there are some discrepancies between various reports. The aim of this study was to determine the metabolic response after intake of standardised meals with various fat and carbohydrate contents and to determine the differences among normal weight and overweight/obese individuals. The study group comprised 46 healthy men. The participants were divided into two groups and study was carried out using a crossover method. Group I received high- and normal-carbohydrate meals, whereas group II received high-carbohydrate and high-fat meals. Glucose, insulin, triglyceride, and free fatty acids levels were measured at fasting state and at 30, 60, 120, 180, and 240 minutes after meal intake. Despite the lack of differences in glucose levels, insulin levels were higher among overweight/obese individuals after each meal. TG and FFA levels were higher after normal-carbohydrate and high-fat meals. Moreover, in overweight/obese young men after high-fat meal intake postprandial hypertriglyceridemia was observed, even if meals contained predominantly unsaturated fatty acids, and fasting triglycerides levels were in normal range. The conducted study showed that postprandial metabolic response depends not only on the meal macronutrient content but also on the current body mass index (BMI)
