Prenatal diagnosis of congenital malformations of the brain in pregnancies with genetic risc

methods of prenatal diagnosis (PD) and prophylaxis of congenital and hereditary pathologies.
Aim of the study. To highlight the role of medical-genetic counseling and prenatal diagnosis in
pregnancies with risk for malformations of the brain (MB) at early stages of intrauterine
development to reduce the incidence of congenital MB in newborn.
Materials and methods. The medical-genetic counseling of the 657 pregnant women during
2015-2017 years, which were divided into two groups: a) I group - 239 women with medium and
high genetic risk; b) the II group - 418 women with low genetic risk.
Results. All pregnant women in the study performed noninvasive PD: ultrasound and
biochemical screening. In 49 cases the values of serum alpha-fetoprotein were elevated.
Examination of pregnant women on informative terms by non-invasive prenatal diagnosis (fetal
ultrasonography) allowed the diagnosis of MB to fetuses in 33 cases. Cerebral fetal
malformations diagnosed prenatally through the ultrasound examination were: spina bifida - 6
cases, anencephaly - 5 cases, holoprozencephaly - 5 cases, corpus calosum agenesia - 7 cases,
hydrocephaly - 4 cases, Dandy-Walker malformation - 3 cases, schizencephaly - 1 case,
lisencephaly - 1 case. The medical-genetic counseling were provided to couples. The final
decision to interrupt the pregnancy was made by couples. A prophylaxis plan was developed in
families with genetic risk.
Conclusions. PD and medical-genetic counseling help to reduce the frequency of congenital
malformations in newborns also makes it possible to prevent the birth of children with CM and
chromosomal abnormalities diagnosed prenatally until 21 weeks of gestation
Identificarea anumitor gene ale ciclului folaților la mamele copiilor cu malformații congenitale cerebrale
Background. In the Republic of Moldova, according to the
National Register of Congenital Malformations (CM) in
newborns in 2011-2020, the incidence of congenital cerebral malformations (CCM) was 1.92 per 1000 newborns,
accounting for 11% of all birth defects. Objective of the
study. To assess the genetic polymorphism of certain genes involved in the synthesis of folic acid (folate cycle genes
MTHFR677; MTHFR1298; MTR2756; MTRR66) in mothers
of children with folate-dependent MCC, to prevent these
pathologies in offspring. Material and methods. The target group was identified - 150 children up to 1 year of age
with a confirmed MCC diagnosis. A number of 65 mothers of
children with MCC were investigated by the molecular-genetic PCR method to detect 4 polymorphisms of the folate
cycle gene. The statistical processing of the data obtained
in the study was performed by the Quanto program. Results. By the method of clinical-genetic examination, later
by the interpretation of the molecular-genetic results, 21
cases of folate-dependent MCC with heterozygous status
were diagnosed, and in 44 cases - with homozygous status.
It was found that the most common folate-dependent MCC
diagnosed was hydrocephalus - 41 cases (43.4%), followed
by anencephaly - 3 cases (4.7%), with the prevalence of
mothers with homozygous status. Among the non-folate
dependent MCCs, the most common were: isolated spina
bifida - 6 cases (9.4%), microcephaly - 7 (10.9%), agenesis
of the corpus callosum isolated - 8 cases (12.2%), mothers
with heterozygous status prevailed. Conclusions. Insufficiency of folic acid in association with genetic causes leads to
the appearance of folate-dependent MCC in children. In this
regard, it is essential to identify predisposing factors at increased risk for the development of folate-dependent MCC.Introducere. Potrivit datelor Registrului Național din Republica Moldova incidența malformațiilor congenitale cerebrale (MCC) pe perioada 2011-2020 a fost de 1,92 la 1000

Congenital brain malformations in children with cerebrial vascular accident in the Republic of Moldova

La copii cu accident vascular cerebral ischemic (AVCI) deseori se întâlnesc diverse malformații congenitale cerebrale (MCC) ca factori de risc. Scopul. Cercetarea factorilor de risc în AVCI la copii prin examenul imagistic cu scop de identificare a MCC. Material și metode. În 20102022 în Republica Moldova a fost realizat un studiu retro- și prospectiv pe un eșantion de 560 copii cu AVCI cu vârsta de nou-născut până la 18 ani, dintre care la 52 de copii au fost confirmate prin imagistică cerebrală MCC. Prelucrarea statistică a datelor obținute în studiul realizat a fost efectuată prin programul SPSS. Rezultate. Printre MCC diagnosticate la copiii cu AVCI pot fi enumerate: hipogenezia corpului calos – 10 (19,2%) cazuri, agenezia corpului calos – 5 (9,6%) cazuri, malformația Arnold Chiari – 8 (15,4%) cazuri, sindromul Dandy-Walker – 7 (13,5%) cazuri, anomalii ale cerebelului – 4 (7,7%) cazuri, hipogenezia simplă a vermisului – 2 (50%) cazuri, hipogenezia emisferelor cerebeloase – 2 (50%) cazuri, microcefalia congenitală – 4 (7,7%) cazuri, hidrocefalia congenitală – 3 (5,8%) cazuri, dilatarea intrauterină a sistemului ventricular – 11 (21,2%) cazuri. MCC deseori pot fi responsabile de dezvoltarea AVC, dintre cele depistate la copiii din studiul nostru cităm sindromul Dandy-Walker, malformația Arnold Chiari și hidrocefalia congenitală. La unii nou-născuți cu AVC ischemic (AVCI) au fost identificate afecțiuni ale encefalului care au apărut în perioada fetală sau neonatală, sugestive pentru un AVC suportat perinatal, printre care chistul cerebral în 27 (4,8%) cazuri. Concluzii. MCC la copii reprezintă una din cauzele AVCI și reprezintă 8,9% în cazul studiului efectuat de noi. Metodele de prevenire a nașterii copiilor cu MCC reprezintă una din domeniile medicale importante, în special pentru neurologia pediatrică.Background. In children with ischemic stroke (IS), various congenital cerebral malformations (CCM) are often found as risk factors. Objective of the study. Research of risk factors of IS in children through the imaging examination with the aim of identifying CCM. Material and methods. In 20102022 in the Republic of Moldova, a retro- and prospective study was conducted on a sample of 560 children with IS aged from newborn to 18 years. 52 children were confirmed by brain imaging with CCM. Statistical processing of the data obtained in the study was performed using SPSS program. Results. Among the CCM diagnosed in children with IS can be listed hypogenesis of the corpus callosum – 10 (19.2%) or agenesis 5 (9.6%) cases, Arnold Chiari malformation – 8 (15.4%) cases, Dandy-Walker syndrome – 7 (13.5%) cases, malformations of the cerebellum – 4 (7.7%) cases, expressed by simple hypogenesis of the vermis – 2 (50%) cases or hypogenesis of the cerebellar hemispheres in 2 (50%) cases, congenital microcephaly – 4 (7.7%) cases, congenital hydrocephalus – 3 (5.8%) cases, intrauterine dilatation of the ventricular system – 11 (21.2%) cases. CCM can often be responsible for the development of stroke, among those detected in children in our study we founded Dandy-Walker syndrome, Arnold Chiari malformation, and congenital hydrocephalus. In some newborns with stroke, brain disorders that appeared in the fetal or neonatal period, suggestive of a stroke sustained perinatally, were identified, including brain cyst in 27 (4.8%) cases. Conclusions. CCM in children is one of the causes of IS and represents 8.9% in our case study. The methods of preventing the birth of children with CCM represent one of the important medical domain, especially in pediatric neurology in the Republic of Moldova
Frecvența malformațiilor congenitale ale sistemului nervos central în Moldova comparativ cu datele registrului internațional EUROCAT

IMSP Institutul Mamei şi CopiluluiIntroducere. Malformațiile congenitale (MC) ale
sistemului nervos central (SNC) reprezintă defecte ale
tubului neural în timpul embriogenezei şi ocupă unul din
locurile principale printre toate anomaliile congenitale
umane detectate (10 – 30%). Scopul lucrării
Studierea frecvenţei MC ale SNC în Republica Moldova în
baza monitoringului genetic pe perioada anilor 2016-2019
în comparaţie cu datele registrului internaţional EUROCAT.
Material şi Metode În cercetare a fost utilizată o
abordare epidemilogică. Frecvenţa MC era calculată după
formulele EUROCAT la numărul total de nou-născuţi vii
până la vârsta de 1 an. Rezultatele cercetării au fost
prelucrate statictic şi utilizate metode statistice
parametrice standard.
Rezultate În 2016-2019, s-au născut 2575 de copii cu MC,
dintre care 274 (10,6%) cu MC ale CNS cu o frecvență de
2,01 la 1000 de nou-născuți vii (nn).În țările EUROCAT, acest parametru a fost de 1.16/1000 nn.
În structura MC ale CNS, ca și în Europa, hidrocefalie a
ocupat un loc important (0.48/1000 nn), în EUROCAT –
0.27/1000 nn, anencefalie a fost înregistrata de 10 ori mai
des decât în Europa (0.22/1000 nn, respectiv, 0.02/1000 nn).
Spina bifida a fost găsită cu o frecvență de 0,21 / 1000 nn, în
EUROCAT – 0,16/1000 nn. Microcefalia a fost mai puțin
frecventă în țara noastră decât în Europa – 0,14/1000 nn., în
EUROCAT acest indicator a fost de 0,21/1000 nn. Concluzii. Analiza comparativă a datelor privind frecvența și
structura anomaliilor sistemului nervos în Republica Moldova
și țările europene ne permite să tragem concluzii despre
sistemul de înregistrare și prevenire a acestei patologii
severe
COVID-19 infection and liver damage in children. Clinical case study
Introduction. COVID-19 is currently considered a systemic disease affecting the immune system, primarily, lungs,
heart, central nervous system, kidneys, intestines, liver and
spleen. Impaired liver function and the presence of biochemical changes in liver can be found in approximately
14-53% of adults infected with SARS-CoV-2. Impaired liver
function in patients infected with COVID-19 may occur due
to a direct effect of the virus on hepatocytes, as well as being secondary to factors such as a systemic inflammatory
response of the infected host, the onset of hypoxia (associated with lung damage), multiple organ failure, or due to
abusive treatment using overlapping and hepatotoxic drugs.
The purpose of this article is to describe a clinical case
study regarding the clinical and paraclinical manifestations
of liver damage in a 12-year-old child infected with SARSCoV-2, hospitalized at the Municipal Children’s Clinical Hospital „Valentin Ignatenco”, Republic of Moldova.
Material and methods. The epidemiological, clinical
and paraclinical data were used to highlight this study, followed by the conclusions of multidisciplinary specialists,
retrieved from the inpatient medical records of the 12-yearold child with moderate COVID-19 infection, who was admitted for emergency treatment.
Results. A 12-year-old patient F. was admitted to the
„Covid-19” subunit, complaining of severe general malaise,
fever up to 39°C, cough, rhinorrhea. The objective clinical
examination revealed hepatomegaly and lack of splenomegaly. Laboratory findings determined leukocytosis 15.88

/l, neutrophilia 72.2%, lymphopenia 26.1%, increased
ESR (Erythrocyte Sedimentation rate) – 20 mm/h, increased CRP (C-reactive protein) >12.0 mg/l, increased ALT
(alanine aminotransferase) by 16 (50.9-487-764 U/l) values compared to the normal reference and a 3-fold increase
in AST (aspartate aminotransferase) that is higher than the
normal range (55.8 - 113 - 181 U/l), an increased fibrinogen
- 5.3 g/l, increased ferritin - 2834 pmol/l and D-dimer levels
- 762 ng/ml. Hepatomegaly was detected on abdominal ultrasound. Covid-19 infection was confirmed by a rapid test
of nasopharyngeal exudate for SARS-CoV-2 antigens.
Conclusions. Patients with the novel coronavirus (COVID-19) show varying degrees of liver dysfunction, especially
those with increased levels of AST and ALT. A question arises within the clinical practice, as whether the liver damage
occurred due to direct viral hepatotoxicity or due to the
drugs used in COVID-19 treatment
Diagnosticul postnatal al malformaţiilor congenitale cerebrale la copii

Nicolae Testemitanu SUMPhBackground. Early diagnosis of congenital brain malformations (CBM) in children represents one of
the modern medical priorities in particular in medical genetics and neuropaediatrics. In the Republic of
Moldova CBM consists of about 25% among all CM in children , taking the fourth place after heart CM.
Objective of the study. The purpose of this work is to highlight the role of congenital malformations
monitoring in the Republic of Moldovain postnatal diagnosis in children with CBM. Material and
Methods. During the investigation process, there was a medical genetic consultation which was done
with the purpose of examining the group of 457 children within the pediatric age with the suspicion to
CBM , they were referred to CSRGM examination in the Institute of Mother and Child during the period
of 2018-2021. Among all of them , 64 children were diagnosed with CBM. Results. Clinical genetic
examination and interpretation of paraclinical and imagistics results allowed to diagnose CBM in 64
cases. It was established that the most frequently diagnosed CBM was hydrocephalus in 15 cases
(23,4%) followed by anencephaly — 3 cases (4,7%). Among the other CBM the most frequent were:
isolated spina bifida — 6 cases (9,4%), microcephaly — 7 cases (10,9%), agenesis of the isolated corpus
callosum- 8 cases, cerebral abnormalities — 7 cases (10,9%), ventricularmegaly — 11 cases (17,2%),
segmental brain aplasia — 3 cases (4,7%). Through citogenetic methods were diagnosed diverse
sindromes of numerous and structural cromozomes in 21 cases (32,8%). Conclusion. Medical genetic
consultation represents an efficient method of postnatal diagnosis of congenital brain malformations in
children of pediatric age.Introducere. Diagnosticul precoce al malformațiilor congenitale cerebrale (MCC) la copii reprezintă
una dintre prioritățile medicinei contemporane, în special ale geneticii medicale și ale neuropediatriei.
În Republica Moldova MCC constituie circa 25% dintre toate MC la copii, plasându-se pe poziția a
patra. Scopul lucrării. Evidenţierea rolului monitoring-ului malformațiilor congenitale din RM în
diagnosticul postnatal al copiilor cu risc de MCC. Material și metode. În procesul investigaţiei s-a
recurs la consultul medico-genetic, care a avut drept scop identificarea grupului-ţintă – 457 copii de
vârstă pediatrică cu suspecție la MCC, trimiși spre examinare în CSRGM din cadrul Institutului Mamei
şi Copilului, în perioada anilor 2018-2021. Dintre aceștia, 64 de copii au fost diagnosticați cu MCC.
Diagnosticul postnatal citogenetic – cariotiparea. Rezultate. Examinarea clinico-genetică și
interpretarea rezultatelor paraclinice și imagistice au permis diagnosticarea MCC în 64 de cazuri. S-a
constatat că cea mai frecventă MCC diagnosticată a fost hidrocefalia – 15 cazuri (23.4%), urmată de
anencefalia – 3 de cazuri (4.7%). Dintre alte MCC, cele mai frecvente: spina bifida izolată - 6 cazuri
(9.4%), microcefalia – 7 cazuri (10.9%), agenezia corpului calos izolat – 8 cazuri (12.2%), encefalocele
– 4 cazuri, anomalii cerebelare – 7 cazuri (10.9%), ventriculomegalie - 11 cazuri (17.2%), aplazii
cerebelare segmentare – 3 cazuri (4.7%). Prin metode citogenetice s-au diagnosticat, în 21 cazuri
(32.8%), diverse sindroame cromozomiale. Concluzii. Consultul medico-genetic reprezintă o metodă
eficientă de diagnostic postnatal al malformaţiilor congenitale cerebrale, la copiii de vârstă pediatrică
NEUROGENETIC FEATURES IN X-FRAGILE SYNDROME

Sindromul X fragil (SXF) sau Martin-Bell reprezintă o afecțiune genetică cauzată de repetări trinucleotidice CGG în gena FMR1. SXF este cea mai răspândită cauză de dizabilitate intelectuală moștenită cu variat grad de severitate și cea mai frecventă cauză monogenă a tulburării din spectrul autismului (TSA), cu o incidență de 1 la 3.6004.000 băieți și 1 la 4.000-6.000 fete. Scopul studiului: evaluarea manifestărilor clinice și genetice prin estimarea unui caz clinic al sindromului X Fragil la un copil, cu scop de ameliorare a diagnosticului precoce. Rezultate. Băiat de 6 ani, se află la evidență TSA, a fost examinat pe cale neurogenetică, pentru abordare diagnostică. Antecedentele perinatale neagravate. Frecvent – infecții de tip ORL, periodic – reflux gastroesofagian. Examen clinic: urechi mari, fața lungă, platfus, deficit de atenție, hiperactivtate, agresivitate, dereglări de vorbire și limbaj, la fel, modificări sugestive pentru TSA (de socializare, comunicare și comportamentale). Manifestări de același tip, însă sub o forma mai ușoară au fost depistate la fratele mai mic. A fost sugerat diagnosticul de SXF. Imagistica prin RMN cerebrală: fără modificări patologice. Teste molecular-genetice: mutații în gena FMR1 (221 de repetări trinucleotidice CGG). Teste citogenetice: cariotipul - 46,XY. Concluzii. Simptomele pozitive ale TSA deseori pot pune în discuție prezența unui sindrom genetic asociat, care necesită confirmare printr-o o perioadă mai îndelungată de evaluare. Testările genetice contribuie la recunoașterea cazurilor de boli genetice la copiii cu TSA, cel mai des întâlnindu-se SXF. Consilierea genetică antenatală permite planificarea familială și reducerea acestei boli.Introduction. Fragile X syndrome (FXS) or Martin-Bell syndrome is a genetic disorder caused by CGG trinucleotide repeats in the FMR1 gene. FXS is the most prevalent cause of inherited intellectual disability of varying severity and the most common monogenic cause of autism spectrum disorder (ASD), with an incidence of 1 in 3,600-4,000 boys and 1 in 4,000-6,000 girls. Objective of the study. To evaluate clinical and genetic manifestations by estimating a clinical case of Fragile X syndrome in a child, with the aim of improving early diagnosis. Results. A 6-year-old boy, on ASD record, neurogenetically examined for diagnostic approach. Perinatal history not recorded. Frequent – ENT type infections, periodic - gastroesophageal reflux. Clinical examination: large ears, long face, flatfoot, attention deficit, hyperactivity, aggressiveness, speech and language disorders, as well as changes suggestive of ASD (social, communication and behavioral). Manifestations of the same type, but in a milder form were detected in the younger brother. The diagnosis of FXS was suggested. Brain MRI imaging: no pathological changes. Molecular genetic tests: mutations in the FMR1 gene (221 CGG trinucleotide repeats). Cytogenetic tests: karyotype - 46,XY. Conclusions. Positive symptoms of ASD often call into question the presence of an associated genetic syndrome, which needs to be confirmed by a longer period of evaluation. Genetic testing contributes to the recognition of genetic disorders in children with ASD, most commonly FXS. Antenatal genetic counselling enables family planning and reduction of this disorder
Neurogenetic aspects of congenital aminoacidopathies
Introducere. Aminoacidopatiile congenitale au fost denumite
ca tulburări genetice datorate dereglării procesului de
sinteză și/sau degradare al proteinelor. Studiile au determinat
manifestări clinice neurologice (hipotonie, ataxie, retard
fizic/cognitiv, distonie, etc.) din primele zile de viață a
copilului. Scopul studiului. Evidențierea rolului aspectului
neurogenetic în aminoacidopatiile congenitale, analiza manifestărilor
clinice și aprecierea gravității pentru diagnosticarea
precoce și inițierea tacticii de tratament al acestui
grup de patologii. Material și metode. Am utilizat metoda
observațională a studiilor oficiale, sinteza și analiza critică a
peste 30 de articole publicate din anii 2016-2023 pe platformele
PubMed, Medscape, NEJM, GeneCards, Lecturio, AMBOSS.
Rezultate. În urma analizei studiilor clinice au fost
observate multiple modificări funcționale ale sistemului
nervos central. Erorile apărute în expresia genelor responsabile
de metabolismul proteic s-au reflectat prin semne și
simptome atât cognitive, cât și fizice. Astfel acestea au dus
la dezechilibre în neurotransmițători (ex.: dopamina, serotonina,
noradrenalina), neurotoxicitate (ex.: creșterea excesivă
de fenilepinefrină), neurodegenerare (ex.: defectele
de glutamat și arginină). În rezultatul celor enumerate s-au
determinat simptome patologice neurologice: tulburări de
comportament, dereglări ale motricității, dizabilitate intelectuală,
convulsii, encefalopatie, neuropatie optică și retard
psiho-verbal. Concluzii. Studiile recente au arătat că reglarea
epigenetică a expresiei genelor are un rol semnificativ în
dezvoltarea aminoacidopatiilor congenitale. Modificările ce
au loc la nivelul ADN-ului au dus la dereglarea ireversibilă a
funcțiilor celulelor întregului organism, în special la nivelul
țesutul cerebral.Background. Congenital aminoacidopathies have been named
as genetic disorders due to the dysregulation of the
protein synthesis and/or degradation process. Studies have
determined neurological clinical manifestations (hypotonia,
ataxia, physical/cognitive retardation, etc.) from the
first days of the child’s life. Objective of the study. To highlight
the role of the neurogenetic aspect in congenital
aminoacidopathies, analysis of clinical manifestations and
assessment of severity for early diagnosis and initiation
of treatment tactics of this group of pathologies. Material
and methods. We used the observational method of official
studies, the synthesis and critical analysis of over 30 articles
published from 2016-2023 on the platforms PubMed,
Medscape, NEJM, GeneCards, Lecturio, AMBOSS. Results.
Following the analysis of clinical studies, multiple functional
changes of the central nervous system were observed.
Errors in the expression of genes responsible for protein
metabolism were reflected in both cognitive and physical
signs and symptoms. Thus they led to imbalances in neurotransmitters
(e.g.: dopamine, serotonin, noradrenaline), neurotoxicity
(e.g.: excessive increase of phenylepinephrine),
neurodegeneration (e.g.: defects of glutamate and arginine).
Because of those listed, pathological neurological symptoms
were determined: behavior disorders, motor disorders, intellectual
disability, seizures, encephalopathy, dystonia, optic
neuropathy, and psycho-verbal retardation. Conclusion.
Recent studies have shown that epigenetic regulation of
gene expression has a significant role in the development of
congenital aminoacidopathies. The changes taking place at
the DNA level led to the irreversible deregulation of the cell

Neurofibromatoza de tip 1 (NF1) sau boala Von Recklinghausen, este o boală genetică rară caracterizată prin multiple tumori benigne ale nervilor și pielii (neurofibroame) și leziuni maculare pe piele, având ca rezultat manifestări sistemice fenotipic eterogene. Scopul studiului. evidențierea aspectelor clinico-genetice și neurologice ale neurofibromatozei prin studiul unui caz clinic bazat pe determinarea asocierii dintre tabloul neurologic și particularitățile clinico-genetice specifice. Material și metode. Datele anamnestice, clinice și paraclinice au fost prelevate din fișa medicală. A fost studiată literatura de specialitate privind cazurile similare. Rezultate. Pacientă, vârsta 18 ani, s-a adresat cu următoarele acuze: noduli multipli pe cap și spate asociate cu macule și leziuni ale pielii. Primele simptome au apărut în copilărie la vârsta de 7 ani. Manifestări de același tip, dar cu o formă mai ușoară au fost depistate la mama și fratele mai mic. La vârsta școlară a prezentat tulburări de învățare. Examen fizic: hiperpigmentare, leziuni cutanate pe spate, axilare și inghinale; noduli cutanați în regiunea posterioară a capului, o masă în regiunea maleolei mediale > 6 cm. Acuitatea vizuală, retina în normă, noduli Lisch prezenți. Diagnosticul a fost stabilit în prezența a două și mai multe criterii de diagnostic, confirmat prin investigații molecular-genetice: mutație în gena NF1. Concluzii. Neurofibromatoza este o tulburare multisistemică, astfel, diagnosticul precoce poate fi dificil și se bazează pe particularități clinice. Diagnosticul genetic nu ne poate oferi informații despre severitatea manifestărilor neurologice la pacienții compromiși.Background. Neurofibromatosis type 1 (NF1), or Von Recklinghausen’s disease, is a rare genetic disorder characterized by multiple benign nerve and skin tumors (neurofibromas) and macular lesions on the skin, resulting in phenotypically heterogeneous systemic manifestations. Objective of the study. To highlight clinical-genetic and neurological aspects of neurofibromatosis by studying a clinical case based on determining the association between the neurological picture and specific clinical-genetic features. Material and methods. Anamnestic, clinical and paraclinical data were taken from the medical record. The literature on similar cases has been studied. Results. Patient, age 18, presented with the following complaints: multiple nodules on the head and back associated with macules and skin lesions. The first symptoms appeared in childhood at the age of 7. Manifestations of the same type, but in a milder form, were detected in the mother and younger brother. At school age, she showed learning disabilities. Physical examination: hyperpigmentation, skin lesions on the back, axilla, and groin; skin nodules in the posterior region of the head, a mass in the medial malleolus region > 6 cm. Visual acuity, retina normal, Lisch nodules present. The diagnosis was determined in the presence of two and more diagnostic criteria, confirmed by molecular-genetic investigations: mutation in the NF1 gene. Conclusions. Neurofibromatosis is a multisystem disorder, thus an early diagnosis can be difficult and is based on clinical features. A genetic diagnosis cannot give us information about the severity of neurological manifestations in compromised patients
