Epidemiology of congenital heart defects in the Knin area during a fi ve-year period (2008-2013)
PriroÄene srÄane grjeÅ”ke (PSG) najÄeÅ”Äi su uzrok smrti u perinatalnom i ranom neonatalnom razdoblju. Cilj je analizirati uÄestalost i
tip PSG-a kod djece koja žive na podruÄju Knina, procijeniti utjecaj okoliÅ”nih Äimbenika i pozitivne obiteljske anamneze na pojavnost
PSG-a i ishod lijeÄenja u razdoblju od 1. 4. 2008. do 1. 4. 2013. U studiju smo ukljuÄili 1068-ero djece roÄene u Kninu u petogodiÅ”njem
razdoblju s mjestom stanovanja u Kninu i okolici u udaljenosti do 30 km od Knina. Popunjavali smo upitnik s pitanjima o populacijskom
kretanju PSG-a. UÄestalost PSG-a kod djece s podruÄja Knina i okolice u navedenom razdoblju znaÄajno je viÅ”a u odnosu na
prevalenciju PSG-a u Hrvatskoj u dvije velike epidemioloÅ”ke studije i iznosi 28,0 promila (p<0,001). TeÅ”ku srÄanu grjeÅ”ku imalo je
16,1% djece. Pozitivnu obiteljsku anamnezu na okolnost PSG-a imalo je 16,1% djece, a pozitivnu obiteljsku anamnezu na neku drugu
nasljednu bolest u obitelji njih 16,1%. Nisu evidentirani Å”tetni Äimbenici ili supstancije vezani za okolinu, odnosno za radno mjesto.
Prevalencija PSG-a na podruÄju Knina u navedenom razdoblju viÅ”a je u odnosu na prevalenciju iz hrvatskih epidemioloÅ”kih studija.
Postojanje nasljedne mase koja pripada srÄanim grjeÅ”kama vodeÄi je razlog veÄe prevalencije PSG-a na ovom podruÄju, a na visinu
prevalencije utjecala je dostupnost medicinskih pretraga.Congenital Heart Diseases (CHD) are the most common cause of death in perinatal and early neonatal period. The aim of this study
is to analyze the frequency and type of CHD among children living in the Knin area in the period between April 1, 2008 to April 1,
2013, to evaluate the eff ect of environment and positive family history on the CHD incidence and treatment outcome The study was
conducted over a fi ve-year period and it involved 1068 children born in this period in Knin and surrounding area. We fi lled in a questionnaire
on CHD trend in the above mentioned population. Frequency of CHD among children in the Knin area in the specifi ed period
is 28 ā° and is signifi cantly (p<0,001) higher than the prevalence of CHD in the rest of Croatia as established by two extensive
epidemiological studies. . Among children with CHD, 16.1% had a severe heart defect 16.1% had positive family history, and 16.1%
had a positive history of some other inherited disease in a family. None harmful factors or substances were registered in the living or
working environment of parents. The higher prevalence of CHD in the Knin area compared to other Croatian regions could be
explained by the genetic load of the refugees coming in the Knin region from the secluded enclaves of Bosnia and Herzegovina and
Kosovo and by above average diagnostic facilities
Patau sindrom
Genetic syndromes caused by chromosomal aberrations involve a recognizable pattern of multiple congenital anomalies with increased neonatal and infant mortality, making care challenging for the family, primary care practitioners, and specialists. About 28% of children born with trisomy 13 die during the fi rst week of life. The median life expectancy is about 2.5 days. We present a 12-year-old girl, the longest living patient with Patau syndrome in Croatia, followed-up from the birth until the age of 12 years. The conventional nonintervention approach has been revised and we suggest changing the traditional view of the condition.Genetski sindromi uzrokovani kromosomnim aberacijama ukljuÄuju prepoznatljivi obrazac viÅ”estrukih priroÄenih anomalija s poveÄanom smrtnoÅ”Äu novoroÄenÄadi i dojenÄadi, Å”to skrb za njih Äini teÅ”kom za obitelj, lijeÄnike primarne zdravstvene skrbi i specijaliste. Oko 28% djece roÄene s trisomijom 13 umire tijekom prvog tjedna života. Srednje oÄekivano trajanje života je oko 2,5 dana. Prikazujemo 12-godiÅ”nju djevojÄicu, najduže živuÄu bolesnicu s Patauovim sindromom u Hrvatskoj, koju pratimo od roÄenja do njezine sadaÅ”nje dobi od 12 godina. Konvencionalni pristup zasnovan na izostanku intervencije doživio je reviziju, a mi predlažemo promjenu tradicionalnog pogleda na ovo stanje
