Kimutattuk, hogy a VR1 receptorok a ggl. n. trigeminibĹ‘l eredĹ‘ Ă©s a vertebrobasilaris rendszert ellátĂł szenzoros rostokban megtalálhatĂłk. A VR1 receptorokkal jelölt primĂ©r szenzoros rostokat nyomon követtĂĽk a BA, AICA, SMA, Ă©s az a. radiolaris felszĂnĂ©n. A VR1 receptorok a szenzoros neuronokon kolokalizáciĂłt mutatnak a SP-vel Ă©s trkA receptorokkal. Igazoltuk a trkA receptorok jelenlĂ©tĂ©t a cochleaban a postnatalis idĹ‘szakban. A trkA receptorok szenzoros rostokhoz kötöttek. Igazoltuk, a stria vascularis marginalis setjeiben a VR1 receptorok jelenlĂ©tĂ©t. A VR1 receptor a marginalis sejtekben kolokalizáciĂłt mutat -plazma membran calcium ATPase-val. KolokalizácĂłt igazoltunk a marginalis sejtek basolateralis oldalán a VR1 Ă©s Na+-K+ ATPase között is. A VR1 receptorok Ă©s a Na+-K+ ATPase kolokalizáciĂłt mutatott a stria vascularis capillarisai körĂĽl is. A VR1 receptorok a szöveti pH változásokkal, nyomásváltozásokkal, ill. hĹ‘mĂ©rsĂ©kleti ingadozásokkal hozhatĂłk ingerĂĽletbe, s ily mĂłdon befolyással lehetnek a cochlean belĂĽli ozmotikus változáskora, ill. pH Ă©rtĂ©kek beállĂtására. A VR1 receptorokban bekövetkezett morfolĂłgiai Ă©s funkcionális károsodás a cochlea nyomásviszonyainak megváltozását eredmĂ©nyezhetik, amely eltĂ©rĂ©sek a MĂ©ni?re szindrĂłma Ă©s MĂ©ni?re betegsĂ©g tĂĽnet egyĂĽttesĂ©t hozhatják lĂ©tre. A VR1 receptorok a Ca++-ion Ă©s Na+-K+-ion háztartással valĂł kapcsolata rĂ©vĂ©n oki tĂ©nyezĹ‘vĂ© lĂ©phet elĹ‘ egyĂ©b idegi jellegű halláscsökkenĂ©sek patomechanizmusában is. | This research has demonstrated that VR1 receptors are found sensory fibers originating in ggl. n. trigemini and supplying the vertebrobasilar system. Primary sensory fibers marked with VR1 receptors were tracked on the surface of BA, AICA, SMA, and a. radiolar. VR1 receptors demonstrate co-localization on sensory neurons with SP and trkA receptors. The results have been verified the presence of trkA receptors in the cochlea in the postnatal period. TrkA receptors are bound to sensory fibers. Results have verified the presence of VR1 receptors in marginal cells of the stria vascularis. VR1 receptors show co-localization in the marginal cells with plasma membrane calcium ATPase. Co-localization has also been detected on the basolateral side of the marginal cells between VR1 and Na+-K+ ATPase. VR1 receptors and Na+-K+ ATPase as well demonstrated co-localization around the capillaries of the stria vascularis. VR1 receptors can be stimulated by pH changes in the tissue, changes of pressure, and temperature fluctuations, and this way can affect osmotic changes as well as pH value setting within the cochlea. Morphological and functional impairment in VR1 receptors may result in changes of pressure in the cochlea, which can create the symptoms of MĂ©ni-re-s syndrome and MĂ©ni-re-s disease. Due to their connection with Ca++-ion and Na+-K+-ion balance, VR1 receptors can become causal factors in the patomechanism of other neurological hearing empairments

Jelen munkánkban több Ă©vtized alvásdiagnosztikai, neuropszicholĂłgiai Ă©s funkcionális agyi áramlási vizsgálatok egybevetĂ©sĂ©vel nyert tapasztalatait összegezzĂĽk. Fontos eredmĂ©ny, hogy az obstruktĂv lĂ©gzĂ©szavarok korán, mĂ©g normál oxigenizáciĂł mellett is jelentĹ‘s áramlási deficitet okoznak. MásrĂ©szt igazoltuk, hogy a funkcionális áramlási rezervkapacitás beszűkĂĽlĂ©sĂ©Ă©rt, a hipoxia Ă©s nem az alvásfragmentáciĂł,
vagy a deszaturáció a felelős. Szintén új eredmény
a nappali mentális flexibilitás csökkenése és a rezervkapacitás beszűkülése közötti összefüggés igazolása. Hangsúlyozzuk, hogy a konkrét eredmények a megfelelő páciens szelekció, a rutin számára jól értelmezhető neuropszichológiai, és agyi áramlás
vizsgálatok együttes alkalmazásának köszönhető.
In the present work we give an overview of our cumulating results gained in our sleep laboratory while investigating the occurence and the neuropsychological
consequences of different types of SRBD with extended imaging of regional cerebral brain functioning, and also estimating cerebral functional reserve capacity by SPECT. To our knowledge, it was a crucial step to demonstrate that both normoxemic and hypoxic cases had the capacity to give rise to regional cerebral blood flow alterations by SPECT. Furthermore, by monitoring autoregulation
capacity we have realised, that not fragmented sleep, but hypoxia is responsible for impaired cerebrovascular reserve capacity. Our results have also shed light on
the relationship between failure on mental flexibility test and altered reserve capacity. Follow up results underlined that launching effective therapy was associated only in limited number of cases with
improvement on mental flexibility test. The issue is of importance, because improving knowledge about the mechanism of regeneration could enhance our ability to rehabilitate military personnels having been previously trained at high-cost

The author presents sleep related movement disorders producing differential diagnostic problems, highlighted role
parasomnias and sleep related epilepsies. Wide variety of the movements (from the physiologic body position changes
to different pathologic events) can be seen during the sleep period. The most important types of these movements are the sleep related events (from the parasomnias
to the restless leg related movements), the movement disorders and the epilepsy related events. To differentiate
between these events is required special skill, which is based on appropriate characterisation of the events (for example timing, repetition, pattern), the polysomnographic
and video-EEG examination and validated questionnaires (FLEP
scale). The appropriate differential diagnostics and therapy must be based on the knowledge of the relationship of the sleep architecture and movement events. This review would like to provide guideline for the understanding and recognizing the nature of the sleep related movements

Ez a ritmikus fiziolĂłgiás rendszerek, pĂ©ldául az alvás, Ă©bersĂ©g, teljesĂtmĂ©ny, anyagcsere, valamint a hormonális működĂ©sek, pĂ©ldául a melatonin Ă©s a kortizol deszinkronizáciĂłjához vezet. The internal circadian clock adapts slowly, if at all, to rapid transitions between different shift schedules. This
leads to misalignment (desynchrony) of rhythmic physiological systems, such as sleep, alertness, performance, metabolism and the hormones melatonin and
cortisol, with the imposed work–rest schedule. Consequences include sleep deprivation and poor performance. Clock gene variants may influence tolerance of sleep deprivation. Shift work is associated with an increased risk of major disease (heart disease and cancer) and this may also, at least in part, be attributed to frequent circadian desynchrony. Abnormal metabolism has been invoked as a contributory factor to the increased risk of heart disease. There is recent evidence for an increased risk of certain cancers, with
hypothesized causal roles of light at night, melatonin suppression and circadian desynchrony. Various strategies
exist for coping with circadian desynchrony and for hastening circadian realignment (if desired). The most important factor in manipulating the circadian system is exposure to and/or avoidance of bright light at specific
times of the „biological night”

The key to the successful treatment of sleep-wake disorders is correct and timely diagnosis. However, the realization of this depends on taking into account some specific features of this field. We should be aware that the vast majority of patients do not spontaneously mention this kind of problem, but also explain the
obvious complaints and symptoms for other reasons; for a variety of reasons, sleep deprivation can lead to the same symptoms for a long time (eg. mental, affective deficit symptoms, daytime sleepiness, etc.). Only the roommate, the family, is aware of the accompanying
symptoms during sleep. The healing of a significant number of patients requires the active participation of special examination methods of sleep medicine (sleep diagnostics) and the related team of doctors and specialists. After establishing a well-established
disease, effective and effective therapy should be initiated as soon as possible, as the patient’s performance, working capacity, life expectancy, social

Circadian rhythms and sleep influence a variety of physiological functions, including the digestive system. The digestive system also has intrinsic rhythms that interact dynamically with circadian rhythms. New advances in understanding the interaction of these rhythms and sleep provide the prospect of evaluating their role in normal physiology and the link of their disruption to pathological conditions. Recent work has demonstrated that sleep and circadian factors influence appetite, nutrient absorption, and metabolism. Disruption of sleep and circadian rhythms may increase vulnerability to digestive disorders, including reflux, ulcers, inflammatory bowel issues, irritable bowel disease, and gastrointestinal cancer. As our knowledge of the link between circadian timing and gastrointestinal physiology grows, so do our opportunities to provide promising diagnostic and therapeutic approaches for gastrointestinal disorders.
