    Managing atrial fibrillation in the global community: The European perspective.

    Atrial fibrillation is a common, global problem, with great personal, economic and social burdens. As populations age it increases in prevalence and becomes another condition that requires careful chronic management to ensure its effects are minimised. Assessment of the risk of stroke using well established risk prediction models is being aided by modern computerised databases and the choice of drugs to prevent strokes is ever expanding to try and improve the major cause of morbidity in AF. In addition, newer drugs for controlling rhythm are available and guidelines are constantly changing to reflect this. As well as medications, modern techniques of electrophysiology are becoming more widely embraced worldwide to provide more targeted treatment for the underlying pathophysiology. In this review we consider these factors to concisely describe how AF can be successfully managed

    Intracardiac Calcification - An Interesting Chest X-ray Report.

    We report a case of chest X-ray finding of mitral annular calcification. Mitral annular calcification is a degenerative process involving the fibrous annulus of the mitral valve. It is generally an incidental finding associated with aging and atherosclerosis. It may lead to significant mitral regurgitation and can rarely cause symptomatic mitral stenosis. In addition, mitral annulus calcification may be associated with atrial fibrillation and cardiac arrhythmia. Calcification of mitral annulus in the chest X-ray generally follows the C-shape of the mitral annulus. Confirmation is by echocardiography. Symptomatic cases require repair by surgery

    EVALUATION OF TOTAL PHENOLIC, FLAVONOIDAL CONTENT, AND DPPH FREE RADICAL SCAVENGING ACTIVITY OF METHANOLIC EXTRACT OF AILANTHUS EXCELSA ROXB.

    Objective: Estimation of total phenol, flavonoid and antioxidant activity of various plant parts (leaves, stem, root, flower and fruit) of Ailanthus excelsa Roxb. methanolic extracts.Methods: Different plant parts were extracted with methanol, then the total phenol content was calculated by using Folin–Ciocalteu reagent, total flavonoid content was estimated by using Aluminum Chloride Colorimetric Method and the antioxidant activity was measured by DPPH (1,1-diphenyl-2-picrylhydrazyl radical) assay.Results: The highest total phenolic content (48.38 mg GAE/gdw) was observed in flower and lowest phenolic content in root (28.56 mg GAE/gdw). The highest total flavonoidal content (21.5 mg QE/gdw) was found in leaf and lowest in root (1.11 mg QE/gdw). The highest radical scavenging activity was found in flower extracts with the IC50 value of 36.85 µg/ml and the lowest scavenging activity was observed in root extract that was found to be 1493.46µg/ml.Conclusion: According to the results of present investigation the plant showed significant antioxidant activity that can be used for medical purpose for the treatment of various diseases.Keywords: Antioxidant activity, Ailanthus excelsa Roxb., Total phenolic content, Total flavonoid content, DPPH (2, 2- diphenyl- 1- picryl hydrazyl), Scavenging activity.                                                              Â

    Genetic diversity among some productive genotypes of tomato (Lycopersicon esculentum Mill.)

    This research was done at Vegetable Experimental Farm, Faculty of Agriculture, Sher-e-Kashmir University of Agricultural Sciences and Technology, Jammu during spring and summer seasons, 2007- 2009. D2-statistics revealed that β-carotene contributed maximally towards the genetic divergence followed by ascorbic acid, total soluble solids, alcohol insoluble solids, pericarp thickness, lycopene content and polygalacturonase activity. The 60 genotypes were grouped into 20 clusters. Fourteen (14) clusters were monogenotypic and cluster I possessed highest number of genotypes numbering 25. Out of 20 clusters, cluster VII is promising for minimum polygalacturonase activity and high average fruit weight, cluster VIII had highest number of locules per fruit, fruit yield per plant and yield per hectare and cluster XVII was superior for ascorbic acid. However, cluster XX was found promising for lycopene content, β-carotene and number of fruits per plants. The highest inter cluster D2 values were estimated between clusters XII and XX, followed by clusters XI and XX, clusters VII and XX, and clusters XV and XX, indicating that there is enough scope for the improvement of tomato crop by hybridization and selection.Key words: Genetic variability, genetic gain, heritability, tomato

    Locoregional therapies in cholangiocarcinoma

    Cholangiocarcinoma is a rare and aggressive malignancy of the biliary tract. Complete surgical resection can be curative, but the majority of patients are diagnosed with advanced disease and usually die within a year of diagnosis. Most deaths are attributable to local disease progression rather than distant metastases, supporting the use of locoregional therapies. There is evidence that locoregional therapies can provide local tumor control resulting in increased survival while avoiding some of the side effects of systemic treatments, increasing potential treatment options for patients who may be unsuitable for systemic palliative treatments. This review considers the evidence for locoregional therapies in cholangiocarcinoma, which can be classified into endoscopic, vascular, percutaneous and radiation oncological therapies. Current guidelines do not recommend the routine use of locoregional therapies due to a lack of prospective data, but the results of ongoing trials are likely to increase the evidence base and impact on clinical practice

    A study on light trap catches of some rice pests in relation to meteorological factors

    The effect of meteorological factors on the population build up of green leafhopper Nephotettix virescens Dist (Cicadellidae, Hemiptera), plant hoppers Cofana spectra Dist (Delphacidae, Hemiptera) and C. yasumatsui Young (Kolla mimica, Hemiptera) and rice gundhi bug Leptocoriza acuta Thunberg (Alydidae, Hemiptera) in rice growing season (July to November) was studied through light trap collection during ten years (1988–1997). Maximum populations of Nephotettix virescens Dist (Cicadellidae, Hemiptera) and C. yasumatsui Young (Kolla mimica, Hemiptera) were recorded in the third week of October during all the years. Cofana spectra Dist had maintained peak activity in respect of population in the last week of September and third week of October. Leptocoriza acuta Thunberg (Alydidae, Hemiptera) had maximum population in second and third weeks of October during the aforesaid period. No meteorological factors have significant effect on the population build up of Nephotettix virescens Dist, Cofana spectra Dist and C. yasumatsui Young in the month of October. In the case of Leptocoriza acuta Thunberg, no other factor but rainfall had positive correlation of order 0.857 with population build up in the fourth week of September.Key words/phrases: Meteorological factors, path analysis, regression analysis, rice insect pests SINET: Ethiopian Journal of Science: Vol. 27 (2) 2004: 165–17

    Minimizing the postoperative complications of severe hypospadias using a simple technique

    Background The aim of this study was to decrease complication rates in proximal hypospadias surgery.Methods A simple method of stenting using a polypropylene stent has been developed for the most severe form of hypospadias during the period from January 2008 to January 2011 in the Department of Pediatric Surgery. The total number of patients was 46. The patients were classified into group 1 (n= 23), in which a polypropylene stent was used, and group 2 (n= 23), in which a polypropylene stent was not used.Results In group 1, complications occurred in three patients (13.04%), whereas in group 2 it occurred in 12 patients (52.2%). The difference in the total number of complications between groups was highly significant (P < 0.001). In group 1, no patient needed redo surgery, and in group 2 four patients (17.39%) needed redo surgery (P < 0.05). All other patients responded to repeated dilatation in the follow-up.Conclusion Although the sample size was small, this simple modification can decrease the complication rate significantly in the most severe form of hypospadias. Keywords: polypropylene stent, proximal hypospadias, surgical complications, urethroplast

    4D-PET reconstruction using a spline-residue model with spatial and temporal roughness penalties

    4D reconstruction of dynamic positron emission tomography (dPET) data can improve the signal-to-noise ratio in reconstructed image sequences by fitting smooth temporal functions to the voxel time-activity-curves (TACs) during the reconstruction, though the optimal choice of function remains an open question. We propose a spline-residue model, which describes TACs as weighted sums of convolutions of the arterial input function with cubic B-spline basis functions. Convolution with the input function constrains the spline-residue model at early time-points, potentially enhancing noise suppression in early time-frames, while still allowing a wide range of TAC descriptions over the entire imaged time-course, thus limiting bias. 
 Spline-residue based 4D-reconstruction is compared to that of a conventional (non-4D) maximum a posteriori (MAP) algorithm, and to 4D-reconstructions based on adaptive-knot cubic B-splines, the spectral model and an irreversible two-tissue compartment ('2C3K') model. 4D reconstructions were carried out using a nested-MAP algorithm including spatial and temporal roughness penalties. The algorithms were tested using Monte-Carlo simulated scanner data, generated for a digital thoracic phantom with uptake kinetics based on a dynamic [18F]-Fluromisonidazole scan of a non-small cell lung cancer patient. For every algorithm, parametric maps were calculated by fitting each voxel TAC within a sub-region of the reconstructed images with the 2C3K model. 
 Compared to conventional MAP reconstruction, spline-residue-based 4D reconstruction achieved >50% improvements for 5 of the 8 combinations of the 4 kinetics parameters for which parametric maps were created with the bias and noise measures used to analyse them, and produced better results for 5/8 combinations than any of the other reconstruction algorithms studied, while spectral model-based 4D reconstruction produced the best results for 2/8. 2C3K model-based 4D reconstruction generated the most biased parametric maps. Inclusion of a temporal roughness penalty function improved the performance of 4D reconstruction based on the cubic B-spline, spectral and spline-residue models.&#13

    One-flow Syntheses of Diverse Heterocyclic Furan Chemicals Directly from Fructose via Tandem Transformation Platform

    The sustainable green chemistry associated with lignocellulosic biomass is of current interest for producing various chemical feedstocks via multi-step transformation processes. Here we introduce a chemical platform system for the multicomponent cascade transformation of natural lignocellulosic biomass resources. We demonstrate the concept by developing an integrated continuous two-step microfluidic system as a tandem transformation platform for direct conversion of fructose to diverse furan chemicals with excellent yields up to 99% via decarbonylation, etherification, oxidation and hydrogenolysis of a 5-hydroxymethylfurfural (HMF) intermediate. A sequential two-step process is utilized to complete the dehydration of fructose in the surface acid catalyst at 150 degrees C for 6 min, which is followed by the four types of HMF conversion in a binary or ternary phase to produce furfuryl alcohol (94% yield), 5-ethoxymethylfurfural (99%), 2,5-diformylfuran (82%) and 2,5-dimethylfuran (90%) with magnetic-based heterogeneous catalysts at 70-150 degrees C for 6-60 min. This innovative tandem microfluidic platform enables precise control of the reaction temperature and time for each individual biomass conversion step in a one-flow manner with no separation and purification steps for intermediates and catalysts.112016Ysciescopu
