    The roles of government and leadership towards the sustainability of TVET teacher education program

    The low payment, the uncertainty in the systems’ implementation and low status given by the community are among the down factors lingers in TVET teachers. Due to this, the Malaysia government has been actively involved in tackling these issues. In order to improve the role of the government, this paper has aim to identify the domains and elements that can contribute in empowering the government’s roles for sustainability of TVET teacher education program. A qualitative research method has been selected with 10 experts involved. A semistructured interview was carried out individually and recorded for thematic analysis. There are four main domains identified namely systematic funding; constructive partnership; stand-alone politic; and visionary leadership. Each domain consists of several elements that characterized it respectively. Overall, one main theme has been identified which is well organized government. It is in a hope that the outcome of this research will help the government in planning for better future of TVET teacher education program

    Regionalization and harmonization in TVET

    A transversal skill is one of the skills needed in every type of workplace setting. There is limited information about the perceptions of which transferable skills are needed in the workplace. The objective of this study is to explore the transferable skills among students and lecturers in TVET higher learning institutions in Indonesia and Malaysia. In the present study, transferable skills consist of communication skills, collaboration skills, problem-solving skills, entrepreneurship, and learning to learn skills. A qualitative research was conducted using survey. The five Likert scale questionnaire was distributed to lecturers and students from selected Malaysian and Indonesian universities who have bachelor program in engineering. Based on the analysis outcomes, it is clearly shown that Malaysian university engineering students yielded significantly higher scores on the perceptions of transferable skill acquisition in all measured components compared to their counterparts in Indonesia. Similar patterns of data distribution have been observed in Malaysian and Indonesian engineering students. Based on the lecturers’ perception, Malaysian and Indonesian Engineering Lecturers have comparable perceptions in the aspects of problem solving skills and entrepreneurship skills. In contrast, Malaysian university engineering lecturers gained significantly higher scores on the perception of communication skill, collaboration skill, and learning to learn skill compared to the Indonesian lecturers. The results indicate the need to increase transferable skills of both lecturers and students

    The management of heart failure cardiogenic shock:an international RAND appropriateness panel

    Background: Observational data suggest that the subset of patients with heart failure related CS (HF-CS) now predominate critical care admissions for CS. There are no dedicated HF-CS randomised control trials completed to date which reliably inform clinical practice or clinical guidelines. We sought to identify aspects of HF-CS care where both consensus and uncertainty may exist to guide clinical practice and future clinical trial design, with a specific focus on HF-CS due to acute decompensated chronic HF. Methods: A 16-person multi-disciplinary panel comprising of international experts was assembled. A modified RAND/University of California, Los Angeles, appropriateness methodology was used. A survey comprising of 34 statements was completed. Participants anonymously rated the appropriateness of each statement on a scale of 1 to 9 (1–3 as inappropriate, 4–6 as uncertain and as 7–9 appropriate). Results: Of the 34 statements, 20 were rated as appropriate and 14 were rated as inappropriate. Uncertainty existed across all three domains: the initial assessment and management of HF-CS; escalation to temporary Mechanical Circulatory Support (tMCS); and weaning from tMCS in HF-CS. Significant disagreement between experts (deemed present when the disagreement index exceeded 1) was only identified when deliberating the utility of thoracic ultrasound in the immediate management of HF-CS. Conclusion: This study has highlighted several areas of practice where large-scale prospective registries and clinical trials in the HF-CS population are urgently needed to reliably inform clinical practice and the synthesis of future societal HF-CS guidelines

    Effects of Once-Weekly Exenatide on Cardiovascular Outcomes in Type 2 Diabetes.

    Abstract BACKGROUND: The cardiovascular effects of adding once-weekly treatment with exenatide to usual care in patients with type 2 diabetes are unknown. METHODS: We randomly assigned patients with type 2 diabetes, with or without previous cardiovascular disease, to receive subcutaneous injections of extended-release exenatide at a dose of 2 mg or matching placebo once weekly. The primary composite outcome was the first occurrence of death from cardiovascular causes, nonfatal myocardial infarction, or nonfatal stroke. The coprimary hypotheses were that exenatide, administered once weekly, would be noninferior to placebo with respect to safety and superior to placebo with respect to efficacy. RESULTS: In all, 14,752 patients (of whom 10,782 [73.1%] had previous cardiovascular disease) were followed for a median of 3.2 years (interquartile range, 2.2 to 4.4). A primary composite outcome event occurred in 839 of 7356 patients (11.4%; 3.7 events per 100 person-years) in the exenatide group and in 905 of 7396 patients (12.2%; 4.0 events per 100 person-years) in the placebo group (hazard ratio, 0.91; 95% confidence interval [CI], 0.83 to 1.00), with the intention-to-treat analysis indicating that exenatide, administered once weekly, was noninferior to placebo with respect to safety (P<0.001 for noninferiority) but was not superior to placebo with respect to efficacy (P=0.06 for superiority). The rates of death from cardiovascular causes, fatal or nonfatal myocardial infarction, fatal or nonfatal stroke, hospitalization for heart failure, and hospitalization for acute coronary syndrome, and the incidence of acute pancreatitis, pancreatic cancer, medullary thyroid carcinoma, and serious adverse events did not differ significantly between the two groups. CONCLUSIONS: Among patients with type 2 diabetes with or without previous cardiovascular disease, the incidence of major adverse cardiovascular events did not differ significantly between patients who received exenatide and those who received placebo.     Amorphous/crystalline silicon heterojunction solar cells via remote inductively coupled plasma processing

    Low-frequency inductively coupled plasma (ICP) has been widely used to deposit amorphous or microcrystalline Si thin films, but the intrinsic drawback namely ion bombardment effect limits its application in Si heterojunction solar cells. In this letter, we redesigned typical ICP and realized a remote plasma deposition with suppressed ion bombardment effect. This remote ICP system enables the synthesis of high quality amorphous Si layers with a compact network and a high hydrogen content (10.5%). By using this remote ICP system, we achieved amorphous/crystalline silicon heterojunction solar cells with an efficiency of 14.1% without any back surface field or textures.Published versio

    Consensus statements from the International Society for Heart and Lung Transplantation consensus conference: Heart failure-related cardiogenic shock.

    The last decade has brought tremendous interest in the problem of cardiogenic shock. However, the mortality rate of this syndrome approaches 50%, and other than prompt myocardial revascularization, there have been no treatments proven to improve the survival of these patients. The bulk of studies have been in patients with acute myocardial infarction, and there is little evidence to guide the clinician in those patients with heart failure cardiogenic shock (HF-CS). An International Society for Heart and Lung Transplant consensus conference was organized to better define, diagnose, and manage HF-CS. There were 54 participants (advanced heart failure and interventional cardiologists, cardiothoracic surgeons, critical care cardiologists, intensivists, pharmacists, and allied health professionals) with vast clinical and published experience in CS, representing 42 centers worldwide. This consensus report summarizes the results of a premeeting survey answered by participants and the breakout sessions where predefined clinical issues were discussed to achieve consensus in the absence of robust data. Key issues discussed include systems for CS management, including the hub-and-spoke model vs a tier-based network, minimum levels of data to communicate when considering transfer, disciplines that should be involved in a shock team, goals for mechanical circulatory support device selection, and optimal flow on such devices. Overall, the document provides expert consensus on some important issues facing practitioners managing HF-CS. It is hoped that this will clarify areas where consensus has been reached and stimulate future research and registries to provide insight regarding other crucial knowledge gaps
