    Zidovudine is one of the first-line antiretroviral therapy regimen drug used to treat human immunodeficiency virus infected patients. Nail pigmentation associated with zidovudine therapy is seen in adults and children’s, mainly dark-skinned patients. This case attempts to draw attention among healthcare professionals concerning the non-serious adverse effect of nail pigmentation with zidovudine. In our case, the patient was counseled about this adverse effect without any change in zidovudine therapy. Even though this side effect is harmless and reversible, the psychological aspects of this visible side effect may reduce medication adherence and can also result in inessential investigations and management for misdiagnoses such as cyanosis and melanoma

    Prospective observational study evaluating the outcome of a fixed angle plate (proximal humerus internal locking system) in proximal humerus fractures

    Background: Proximal humerus fractures are common among elderly. The present study assesses the functional outcomes of fixed angle plate (proximal humerus internal locking system (PHILOS)) in fractures of proximal humerus.Methods: 30 participants aged ≥18 years with proximal humerus fracture (2, 3 and 4 part) who underwent PHILOS fixation were enrolled. Undisplaced, open, severely comminuted, metastatic, and pathological fractures and with associated head injury and neurovascular injuries wew excluded. Clinical and radiological evaluation were done pre and post-operatively. Intraarticular extent of fracture geometry was assessed using 3-dimensional computed tomography. Participants were managed preoperatively with analgesics and shoulder immobilizer followed by preanesthetic check-up and routine investigations and surgery was done once participants were stable. Sample size was calculated assuming excellent or satisfactory results in 80% participants 6 months after surgery, relative precision of 20%, α of 5% and 10% attrition rate. Institutional ethics committee approved the study and written informed consent was obtained from all study participants.Results: Mean age of study participants was 62.9 (14.9) years and were predominantly females (66.6%, n=20). No significant difference between type of fracture and duration of recovery was observed (p=0.4). 30% participants had post-operative complications, stiffness was the most common (13.3%, n=4) complication. 76.6% (n=23) participants had good functional outcomes. Significant correlation between type of fracture and NEER score was observed (p<0.0001).Conclusions: PHILOS is a preferential implant in proximal humerus fractures due to angular stability, particularly in comminuted fractures in younger patients, and osteoporotic fractures in elderly, thus allowing early mobilization and satisfactory final functional outcome.

    Extreme star formation events in quasar hosts over 0.5<z<4{\bf0.5<\textit{z}<4}

    We explore the relationship between active galactic nuclei and star formation in a sample of 513 optically luminous type 1 quasars up to redshifts of \sim4 hosting extremely high star formation rates (SFRs). The quasars are selected to be individually detected by the \textit{Herschel} SPIRE instrument at >> 3σ\sigma at 250 μ\mum, leading to typical SFRs of order of 1000 M_{\odot}yr1^{-1}. We find the average SFRs to increase by almost a factor 10 from z0.5z\sim0.5 to z3z\sim3, mirroring the rise in the comoving SFR density over the same epoch. However, we find that the SFRs remain approximately constant with increasing accretion luminosity for accretion luminosities above 1012^{12} L_{\odot}. We also find that the SFRs do not correlate with black hole mass. Both of these results are most plausibly explained by the existence of a self-regulation process by the starburst at high SFRs, which controls SFRs on time-scales comparable to or shorter than the AGN or starburst duty cycles. We additionally find that SFRs do not depend on Eddington ratio at any redshift, consistent with no relation between SFR and black hole growth rate per unit black hole mass. Finally, we find that high-ionisation broad absorption line (HiBAL) quasars have indistinguishable far-infrared properties to those of classical quasars, consistent with HiBAL quasars being normal quasars observed along a particular line of sight, with the outflows in HiBAL quasars not having any measurable effect on the star formation in their hosts.Comment: 12 pages, 6 figure

    A RARE PEDIATRIC CASE OF CHRONIC ARSENIC POISONING FROM AYURVEDIC MEDICATION

    Ayurveda is an indigenous system of medicine becoming more popular around the world in recent years. In various Ayurvedic formulations, the arsenical compounds are intentionally added as the main active ingredients or as an auxiliary agent to assist the efficacy of herbal drugs. The chronic arsenic poisoning following the use of Ayurvedic medication is reported rarely in the literature. This report describes a rare pediatric case of chronic arsenic poisoning from Ayurvedic medication in a 15-year-old girl. She was a known case of seizure disorder and was on treatment with Ayurvedic medication for 1 year. The generalized hyperpigmentation and thickening of soles were noticed in the girl 7 months after taking the herbal tablet. The plasma-coupled atomic absorption spectroscopy of herbal tablet was performed in a laboratory and the result showed 4.3 mg of arsenic present in each herbal tablet. The patient was advised to stop the Ayurvedic medication and as an alternate drug, tablet levetiracetam (250 mg, twice a day) was started for the treatment of seizure disorder. During the follow-up, her cutaneous manifestations improved and is doing well
