Its size means that it has some interesting points of comparison with Nordic countries. These include not only the relative impact of its own export of IP, and dependency on the import of IP, but also the fact that its society is technologically advanced with a high degree of internet penetration and use of digital technology and digitally stored information and knowledge

    Activation of Akt at T308 and S473 in alcohol, tobacco and HPV-induced HNSCC:is there evidence to support a prognostic or diagnostic role?

    BACKGROUND: Tobacco, alcohol and HPV infection are associated with increased risk of HNSCC. However, little is known about the underlying signaling events influencing risk. We aimed to investigate the relationship between these risk factors and Akt phosphorylation, to determine prognostic value. METHOD: VEGF-positive HNSCC biopsies, with known HPV status, were analyzed by immunohistochemistry (IHC) for Akt, phosphorylated at residues S473 and T308. Comparisons between the tissues were carried out using a Mann–Whitney U test. Associations between the variables and continuous immunohistochemical parameters were evaluated with general linear models. Patient characteristics and pAkt IHC score were analyzed for possible association with overall survival by Cox proportional hazard models. RESULTS: Immunohistochemistry revealed that cancer patients had significantly higher levels of pAkt T308 than S473 (P < 0.001). Smoking and alcohol were found to be independent risk factors for Akt phosphorylation at T308 (P = 0.022 and 0.027, respectively). Patients with tumors positive for HPV or pAkt S473 had a poorer prognosis (P = 0.005, and 0.004, respectively). Patients who were heavy drinkers were 49 times more likely to die than non-drinkers (P = 0.003). Patients with low pAkt T308 were more likely to be HPV positive (P = 0.028). Non-drinkers were also found to have lower levels of pAkt T308 and were more likely to have tumors positive for HPV than heavy drinkers (P = 0.044 and 0.007, respectively). CONCLUSION: This study suggests different mechanisms of carcinogenesis are initiated by smoking, alcohol and HPV. Our data propose higher phosphorylation of Akt at T308 as a reliable biomarker for smoking and alcohol induced HNSCC progression and higher phosphorylation of Akt at S473 as a prognostic factor for HNSCC

    An evaluation of a personalised text message reminder compared to a standard text message on postal questionnaire response rates: an embedded randomised controlled trial

    Abstract Background: Research outcome data is commonly collected using postal questionnaires; however, poor response can introduce bias and reduce statistical power. Text messaging is simple, cost-effective, and can be customised to the individual. Personalised, reminder text messages may improve response rates. Methods: A two-arm, parallel group ‘Study within a Trial’ (SWAT) was embedded within the Occupational Therapist Intervention Study (OTIS), a randomised controlled trial of a home assessment for falls prevention in older people. OTIS participants who provided a mobile phone number were randomly allocated (1:1) to receive either a personalised text message (Title, Surname, plus York Trials Unit (YTU) text) or the standard YTU text alone, prior to receiving their four-month post-randomisation follow-up postal questionnaire. The primary outcome measure was the proportion of participants who returned the questionnaire. Secondary outcomes were: time to response, completeness of response, requirement of a reminder letter, and cost-effectiveness. Binary data were compared using logistic regression and time to response by Cox Proportional Hazards regression. Results: A total of 403 participants were randomised: 201 to the personalised text and 202 to the standard text. Of the 283 participants included in the final analysis, 278 (98.2%) returned their questionnaire; 136 (97.8%) for the personalised text versus 142 (98.6%) for the standard text (adjusted odds ratio 0.64, 95% CI 0.10 to 3.88, p=0.63). The median time to response was nine days in both groups. In total, 271 (97.5%) participants returned a complete questionnaire; 133 (97.8%) in the personalised text versus 138 (97.2%) for the standard text. In total, 21 reminder letters were sent. The additional cost of personalised text messages was £0.04 per participant retained. Conclusions: Personalised texts were not superior to standard texts in any outcome assessed in our study. Further SWATs are needed to perform a meta-analysis and obtain more evidence.     Aboriginal and Non-Aboriginal Students Learn About Natural Health Products from Different Information Sources

    Natural health products (NHPs) include naturally derived botanical and nonbotanical products. Past research indicates a high prevalence of NHPs use amongst adults in the United States and Canada but does not clearly characterize NHPs use amongst students, ethnic variations of such use, or how users learn about NHPs. We hypothesize that there is a difference between Aboriginal and non-Aboriginal students in how they learn about NHPs. To investigate this question, we conducted a cross-sectional study at First Nations University of Canada and the University of Regina, Saskatchewan, Canada, during the fall of 2011. Aboriginal (n=214) and non-Aboriginal (n=749) students participated in the 28 question survey. Our results indicate that Aboriginal students who use NHPs are found in all age groups, are mostly female, are smokers and nonsmokers, and learn about NHPs from Elders and healers. Compared to nonAboriginal students, Aboriginal students rely significantly less on alternative and conventional health providers, electronic media, print media, and advertising as their sources of information about NHPs. Thus, Aboriginal students use Elders or healers as a primary source of information to learn about NHPs, as compared to non-Aboriginal students. Future work should investigate the role of Elder traditional educators to convey NHPs information directed specifically to Aboriginal university students

    Molecularly Resolved Electronic Landscapes of Differing Acceptor-Donor Interface Geometries

    Organic semiconductors are a promising class of materials for numerous electronic and optoelectronic applications, including solar cells. However, these materials tend to be extremely sensitive to the local environment and surrounding molecular geometry, causing the energy levels near boundaries and interfaces essential to device function to differ from those of the bulk. Scanning Tunneling Microscopy and Spectroscopy (STM/STS) has the ability to examine both the structural and electronic properties of these interfaces on the molecular and submolecular scale. Here we investigate the prototypical acceptor/donor system PTCDA/CuPc using sub-molecularly resolved pixel-by-pixel STS to demonstrate the importance of subtle changes in interface geometry in prototypical solar cell materials. PTCDA and CuPc were sequentially deposited on NaCl bilayers to create lateral heterojunctions that were decoupled from the underlying substrate. Donor and acceptor states were observed to shift in opposite directions suggesting an equilibrium charge transfer between the two. Narrowing of the gap energy compared to isolated molecules on the same surface are indicative of the influence of the local dielectric environment. Further, we find that the electronic state energies of both acceptor and donor are strongly dependent on the ratio and positioning of both molecules in larger clusters. This molecular-scale structural dependence of the electronic states of both interfacial acceptor and donor has significant implications for device design where level alignment strongly correlates to device performance

    A qualitative exploration of the barriers and facilitators of community pharmacy PrEP delivery for pharmacists and community members

    BACKGROUNDOral Pre-Exposure Prophylaxis (PrEP) is only available free of charge in the United Kingdom from sexual health clinics. Expanding PrEP delivery to community pharmacies could be an effective way of improving access to PrEP and aligns well with the UK government goals for England to eliminate new cases of HIV by 2030. Using the Capability, Opportunity, Motivation, Behaviour (COM-B) Model, the aim of this research was to explore the barriers and facilitators of community pharmacy PrEP delivery, perceived by pharmacists and community members underserved through current delivery models.METHODCommunity members at risk of acquiring HIV but not currently accessing PrEP and community pharmacists were recruited to participate in semi-structured open-ended interviews. Interviews were online, via phone or in person, were audio recorded, fully transcribed, and analysed using thematic analysis, informed by COM-B. RESULTSA total of 17 interviews with pharmacists (pharmacy owners n=7; employed pharmacists n=6; locums n=4) and 24 with community members (Black African women n=6; other women n=2; young adults aged 18- 25-years n=6; trans people n=6; street sex workers n=4) were conducted. Thematic analysis showed barriers include sub-optimal awareness and knowledge of PrEP, perceptions of pharmacist roles in delivering public health services (capability), lack of staff capacity, pharmacy facilities and privacy (opportunity), concern about being seen accessing PrEP from a pharmacy, a preference to access PrEP from a General Practitioner (GP) and a belief that pharmacy PrEP delivery could increase STIs (motivation). Facilitators included improving PrEP education and awareness (capability), the accessibility of pharmacies, being able to deliver PrEP via a patient group directive (PGD) (opportunity), a general preference for pharmacy PrEP and a belief that this model of delivery would be discrete, help decrease stigma and improve access to PrEP, particularly for those who felt uncomfortable accessing PrEP from sexual health clinics (motivation). CONCLUSIONPharmacy PrEP delivery is acceptable to pharmacists and community members but for it to be feasible, results point to the need for a behaviour change intervention incorporating education, training and awareness raising, for both pharmacists and community members to improve access, stimulate patient activation and de-stigmatise HIV and PrEP. <br/
