The use of anticoagulants in morbidly obese patients
Due to its constantly growing incidence, obesity is an increasingly serious social and medical problem. Available data on the use of novel oral anticoagulants in morbidly obese and obese patients are very limited. However, we tried to summarize the available knowledge on the use of anticoagulants in this subpopulation of patients in everyday clinical practice. Studies on the clinical use of anticoagulants provide a poor basis for any adjustment of doses in obese patients as compared to patients with normal body weight. In our opinion, further studies are required in this particular population
Failures of ECG assessment in diagnostic of right heart diseases
Badanie elektrokardiograficzne (EKG) cechuje niedostateczna czułość i swoistość, by mogło stanowić pojedyncze narzędzie w wykrywaniu przerostu lub powiększenia prawej komory serca. Brak typowych odchyleń w EKG nie wyklucza również nadciśnienia płucnego. Objawy kliniczne i obciążający wywiad powinny ukierunkowywać na poszerzenie diagnostyki prawej komory o badanie echokardiograficzne. W pracy przedstawiono 2 przypadki chorych z potwierdzonym przerostem prawej komory i nadciśnieniem płucnym bez typowych dla tych stanów zmian w EKG.An electrocardiogram is characterized by an insufficient sensitivity and specificity to be a single tool in the recognition of hypertrophy or enlargement of the right ventricle. Absence of typical abnormalities in electrocardiography do not exclude pulmonary hypertension either. Therefore clinical symptoms and specific signs in medical past history should indicate to perform echocardiography. The paper presents 2 cases of patients with confirmed right ventricle hypertrophy and pulmonary hypertension without typical electrocardiographic abnormalities
Andersen-Tawil syndrome: report of 3 novel mutations and high risk of symptomatic cardiac involvement.
IntroductionAndersen-Tawil syndrome (ATS) is a potassium channelopathy affecting cardiac and skeletal muscle. Periodic paralysis is a presenting symptom in some patients, whereas, in others, symptomatic arrhythmias or prolongation of QT in echocardiographic recordings will lead to diagnosis of ATS. Striking intrafamilial variability of expression of KCNJ2 mutations and rarity of the syndrome may lead to misdiagnosis.MethodsWe report 15 patients from 8 Polish families with ATS, including 3 with novel KCNJ2 mutations.ResultsAll patients had dysmorphic features; periodic paralysis affected males more frequently than females (80% vs. 20%), and most attacks were normokalemic. Two patients (with T75M and T309I mutations) had aborted sudden cardiac death. An implantable cardioverter-defibrillator was utilized in 40% of cases.ConclusionsKCNJ2 mutations cause a variable phenotype, with dysmorphic features seen in all patients studied, a high penetrance of periodic paralysis in males and ventricular arrhythmia with a risk of sudden cardiac death

Ostra zatorowość płucna prowadzi do gwałtownego wzrostu ciśnienia w naczyniach płucnych
i przeciążenia prawej komory serca. O wyborze metody leczenia decyduje stopień zaburzeń
hemodynamicznych i wynikające z tego ryzyko zgonu. Pacjenta w stanie stabilnym leczy się
przede wszystkim przeciwzakrzepowo, natomiast chorzy z hipotonią, we wstrząsie wymagają
natychmiastowego odblokowania tętnic płucnych, co uzyskuje się, podając leki trombolityczne
lub wykonując embolektomię przeskórną albo chirurgiczną. Drugim istotnym problemem jest
czas wtórnej profilaktyki przeciwzakrzepowej. Decydujące znaczenie ma w tym przypadku
ryzyko nawrotu zakrzepicy, oceniane obecnie głównie na podstawie pierwotnej przyczyny żylnej
choroby zakrzepowo-zatorowej oraz coraz częściej - stężenia D-dimeru miesiąc po odstawieniu
leków przeciwzakrzepowych.Acute pulmonary embolism leads to sudden increase of pressure in pulmonary arteries and
overload of the right ventricle. The management of pulmonary embolism depends on hemodynamic
deterioration and consequent risk of death. All patients should be anticoagulated,
however, subjects with hypotonia, in shock demand immediate resolving of thromboembolic
obstruction of pulmonary arteries with thrombolysis or surgery/percutaneous embolectomy.
The duration of secondary prophylaxis is a second important problem. The assessment of the
risk of recurrent embolism is crucial and currently is based on primary reason of thromboembolic
disease and on concentration of D-dimer one month after cease of anticoagulation
