Nutritional controversies in diabetes
W niniejszym artykule przedstawiono zagadnienia
dotyczące leczenia żywieniowego u chorych na cukrzycę,
które w ostatnich latach stały się przedmiotem
licznych dyskusji. Szczególną uwagę poświęcono
dietom z obniżoną zawartością węglowodanów
oraz wykorzystaniu koncepcji wskaźnika (indeksu)
glikemicznego pokarmów w żywieniu chorych na

in recent years. Particular attention has been paid

Disturbances in angiogenesis and vascular maturation in the skin are associated with diabetic kidney disease in type 1 diabetes
Introduction. The skin, as one of the most accessible tissues, is frequently used for investigations of microcirculation and angiogenesis. The aim of this study was to assess the relationship between the dermal microvessel density (MVD) and maturity and the presence of diabetic kidney disease (DKD) in adults with type 1 diabetes (T1D). Skin as the most accessible organ served as a model for the study of angiogenesis. Materials and methods. 148 consecutive T1D patients (87 men), median age of 41 [interquartile range (IQR): 31–49] years and diabetes duration of 21 (17–30) years, participated in the study. The patients were under the care of the Department of Internal Medicine and Diabetology, Poznan University of Medical Sciences. Diabetic kidney disease was diagnosed in patients with increased albuminuria and at least 10-year duration of diabetes or evidence of diabetic retinopathy. The skin biopsy was performed on distal part of lower leg, using a sterile, disposable 3 mm biopsy punch with plunger (Disposable Biopsy Punches, Integra™ Miltex®). In the immunohistochemical analyses, we used: anti-CD133, anti-CD34, anti-CD31, and anti-von Willebrand factor (vWF) autoantibodies. Microvessel density measurement in all specimens was performed using “hot spots technique”. Slides were scanned using the MiraxMidi scanner (Carl Zeiss) and were viewed using CaseViewer (3DHISTECH Ltd. Budapest, Hungary). Data were analyzed using Statistica v. 13 software. Results. In the study group 21 patients with diagnosis DKD+, as compared to 127 subjects withaout DKD–, had longer duration of diabetes [30 (IQR: 21–36) vs. 21 (16–28) years, p = 0.002], higher prevalence of hypertension [14 (67%) vs. 37 (29%), p = 0.002], lower estimated glomerular filtration rate (eGFR) [66 (55–88) vs. 94 (83–106) mL/min/1.73 m2, p < 0.001]. Median MVD compared between groups with and without DKD, was similar for CD34+ vessels/1 mm2 [123 (100–170) vs. 121 (100–170), p = 0.775], CD133+ vessels/1 mm2 [79 (50–100) vs. 79 (63–93), p = 0.823], and for CD31+ vessels/ 1 mm2 [29 (21–46) vs. 38 (17–58), p = 0.454]. Median MVD vWF+ vessels/1 mm2 was lower in the group with than without DKD: 42 (25–54) vs. 54 (43–71), p = 0.009. The values given above were calculated for both layers of the dermis (papillary and reticular dermis). In multivariate logistic regression analysis presence of diabetic kidney disease was associated with lower median vWF+ MVD [odds ratio: 0.97 (95% confidence interval: 0.95–0.99), p = 0.017], with adjustment for age, gender, eGFR value, diabetes duration and presence of hypertension. MVD did not differ significantly between chronic kidney disease stages. Conclusion. In patients with type 1 diabetes and diabetic kidney disease the disturbances in the angiogenesis and vascular maturation are present. The number of mature blood vessels (vWF+) in the skin is reduced. Disturbances in the angiogenesis occur at early stages of diabetic kidney disease.Introduction. Influence of selected inflammatory markers on occurrence and duration of partial remission in patients with type 1 diabetes
WSTĘP. Celem badania była ocena wpływu wybranych
markerów reakcji zapalnej na wystąpienie i czas
trwania remisji w cukrzycy typu 1.
MATERIAŁ I METODY. W badaniu uczestniczyło 224
pacjentów z nowo rozpoznaną cukrzycą typu 1. Częściową
remisję choroby definiowano jako: HbA1c
poniżej 7%, dobowe zapotrzebowanie na egzogenną
insulinę poniżej 0,5 j./kg masy ciała i stężenie peptydu
C powyżej 0,5 ng/ml. Wystąpienie i czas trwania
remisji określono na podstawie regularnej kontroli
w poradni diabetologicznej. Pacjentów podzielono
na podstawie występowania remisji i czasu jej trwania
(poniżej i powyżej roku). W analizie uwzględniono:
liczbę białych krwinek (WBC), stężenie białka
C-reaktywnego (CRP), odczyn opadania krwinek czerwonych
(ESR) i stężenie mieloperoksydazy (MPO)
w surowicy.
WYNIKI. U 175 pacjentów wystąpiła częściowa remisja.
U osób z remisją stwierdzono niższe wartości
WBC (6,2 ± 1,4 v. 7,1 ± 2,7 103/ul; p < 0,03) i ESR
(5,1 ± 3,3 v. 8,3 ± 12,2 mm/h; p < 0,02). Pacjenci
z krótszym czasem trwania remisji mieli wyższe stężenie
w surowicy MPO (250,6 ± 139,5 v. 163,9 ± 82,7
ng/ml; p = 0,008) niż osoby z remisją trwającą powyżej
roku. W analizie regresji logistycznej wykazano
istotny związek między stężeniem MPO a czasem trwania
remisji (OR = 0,34, 95% CI: 0,12–0,94; p < 0,03).
WNIOSKI. Proces zapalny istotnie wpływa na wystąpienie
i czas trwania remisji u chorych na cukrzycę
typu 1. Mieloperoksydaza wydaje się być prognostycznym
markerem czasu trwania remisji. Im wyższe stężenie
MPO, tym krótszy czas trwania remisji w cukrzycy
typu 1. (Diabet. Prakt. 2010; 11, 3: 94–100)INTRODUCTION. Aim of the study was to asses the
influence of selected inflammatory markers on occurrence
and duration of remission in type 1 diabetes.
MATERIAL AND METHODS. 224 patients with newly
diagnosed type 1 diabetes were qualified to the study.
Partial remission was defined as HbA1c below 7%
and daily exogenous insulin requirement below 0.5
U/kg and positive C-peptide level. The occurrence and
duration of remission was assessed during regular
visits in out-patient clinic. We divided patients according
to occurrence of remission and its duration
(above and below 1 year). The following inflammatory markers were taken into account: white blood
cell count (WBC), serum C-reactive protein concentration
(CRP), erythrocyte sedimentation rate (ESR),
and concentration of serum myeloperoxidase (MPO).
RESULTS. 175 patients achieved partial remission.
When compared with patients who did not experience
remission remittents showed lower WBC (6.2 ±
± 1.4 v. 7.1 ± 2.7 103/ul; p < 0.03) and ESR (5.1 ±
± 3.3 v. 8.3 ± 12.2 mm/h; p < 0.02). Patients with
shorter duration of remission had higher serum MPO
level (250.6 ± 139.5 v. 163.9 ± 82.7 ng/ml; p = 0.008).
Using logistic regression, significant association between
serum MPO level (OR = 0.34, 95% CI: 0.12–
–0.94; p < 0.03) and duration of remission was found.
CONCLUSIONS. The results of this study show that
inflammatory process influences occurrence and
duration of remission in patients with type 1 diabetes.
MPO seems to be a prognostic marker of duration
of remission. The higher serum concentration
of the MPO, the shorter duration of remission in

Factors influencing occurrence and duration of partial remission in patients with type 1 diabetes
WSTĘP. Celem badania była ocena czynników wpływających
na wystąpienie i czas trwania częściowej
remisji u chorych na cukrzycę typu 1.
MATERIAŁ I METODY. Obserwacją prospektywną objęto
136 pacjentów z nowo rozpoznaną cukrzycą
typu 1. Analizie poddano ocenę parametrów wyrównania
metabolicznego cukrzycy oraz liczne dane uzyskane
z przeprowadzonego badania ankietowego.
WYNIKI. Po 3 miesiącach od hospitalizacji remisję
stwierdzono u 98 pacjentów. U 35 osób remisja trwała
powyżej 2 lat, u 24 - 1-2 lata, a u 39 - poniżej
1 roku. Osoby z remisją w porównaniu z grupą bez
remisji przy rozpoznaniu choroby charakteryzowały
się niższymi wartościami glikemii: na czczo, 2 godziny
po posiłku, o godzinie 3.00 w nocy i triglicerydemii
oraz wyższymi wartościami cholesterolu frakcji
HDL. Dzienne zapotrzebowanie na egzogenną insulinę
w dniu wypisania ze szpitala było niższe w grupie
pacjentów z remisją. W modelu regresji logistycznej stwierdzono statystycznie istotną zależność między
wystąpieniem remisji a zapotrzebowaniem na
egzogenną insulinę podczas pierwszej hospitalizacji
oraz stężeniem triglicerydów. Osoby z częściową remisją
dysponowały większymi środkami finansowymi
na leczenie, cechowały się niższym poziomem stresu,
rzadziej piły kawę i paliły tytoń. Pacjenci z ponad
2-letnim okresem remisji, w porównaniu z chorymi
z okresem remisji poniżej 1 roku, charakteryzowali
się krótszym okresem od rozpoznania cukrzycy do
rozpoczęcia edukacji i leczenia metodą intensywnej
czynnościowej insulinoterapii.
WNIOSKI. Stopień zaburzeń metabolicznych przy rozpoznaniu
cukrzycy typu 1, a także styl życia pacjentów
mają istotny wpływ na wystąpienie okresu
remisji. Szybkie rozpoznanie choroby i wdrożenie intensywnej
czynnościowej insulinoterapii zwiększają
szansę na wydłużenie okresu remisji.INTRODUCTION. The aim of this study was to analyze

MATERIAL AND METHODS. 136 patients with newly
diagnosed type 1 diabetes were qualified to the prospective
study. Metabolic control parameters and
several data collected on the basis of questionnaires
were taken into analyze. RESULTS. Three months after hospitalization 98 subjects
achieved partial remission. 35 patients had
a duration of remission above 2 years, 24 patients
between one year and two, and 39 subjects below
one year. When compared with patients who didn’t
experience remission remittents showed lower: fasting
plasma glucose, postprandial plasma glucose,
glycaemia on 3 a.m., serum triglyceride and higher
HDL-cholesterol levels. Total daily insulin requirement
at discharge from hospital was significantly lower
in remission group. Using logistic regression significant
association between insulin requirement level
of triglyceride and occurrence of remission was
found. According to the analysis of questionnaires
group with remission characterized bigger founds
to treatment, less stressful live, more seldom drinking
coffee and smoking. In patients with duration
of remission above 2 years we observed: shorter
duration of diabetes before introduction of education
and intensive functional insulin therapy, in comparison
with subjects with duration below 1 year.
CONCLUSIONS. Metabolic disturbances prior to diagnosis,
as well as style of life determine occurrence
of remission phase in patients with type 1 diabetes.
The faster the diagnosis of type 1 diabetes is made
and the earlier the intensive functional insulin therapy
is introduced the longer the duration of partial
remission is
Częstość występowania zespołu rozrostu bakteryjnego jelita cienkiego (SIBO) u pacjentów z cukrzycą
Introduction. Gastrointestinal symptoms are present in 50–70% of patients with diabetes. Small intestinal bacterial overgrowth (SIBO) is expansion of microorganisms colonizing the large intestine into the small intestine, leading to disorders of digestion and absorption.
Material and methods. The study included 200 patients (91 with type 1 and 109 with type 2 diabetes hospitalized in years 2013–2015 in the Department of Internal Medicine and Diabetology, Poznan University of Medical Sciences. The control group consisted of 20 healthy volunteers. Hydrogen breath tests with Gastro+ Gastrolyzer (Bedfont) were used to assess SIBO. After drinking 20 g of lactulose we checked exhaled hydrogen concentration. The result was considered positive when exhaled hydrogen of first breath was ≥ 20 ppm (parts per milion) or an increase in the output value of the exhaled hydrogen was 12 ppm during the first 60 minutes of the test.
Results. We observed lower prevalence of SIBO in the test group as compared to controls [82 patients (41%) vs. 15 patients (75%); p = 0.0043]. Moreover, patients with type 1 diabetes as well as type 2 diabetes had lower incidence of SIBO as compared to controls [accordingly, 42% vs. 75%, p=0.008, 40% vs. 75%, p=0.006]. In the logistic regression model this relationship was independent of age, gender and BMI (OR: 5,57, 95% CI: 1,78–17,47; p = 0,003).
Conclusion. The prevalence of microbial proliferation of the small intestine in patients with diabetes is lower than in healthy subjects. One possible cause might be the beneficial effect of nutritional therapy in patients with diabetes.Wstęp. Dolegliwości ze strony przewodu pokarmowego mogą występować u 50–70% osób z cukrzycą. Zespół rozrostu bakteryjnego (SIBO) to nadmierny rozplem w obrębie jelita cienkiego drobnoustrojów kolonizujących jelito grube, prowadzący do zaburzeń trawienia i wchłaniania. Celem pracy była ocena częstości występowania SIBO u pacjentów z cukrzycą, a także związek występowania SIBO z wyrównaniem metabolicznym cukrzycy i obecnością przewlekłych powikłań choroby.
Materiał i metody. Grupę badaną stanowiło 200 osób z cukrzycą (typu 1 — 91 osób; typu 2 — 109 osób), hospitalizowanych w latach 2013–2015 w Klinice Chorób Wewnętrznych i Diabetologii Uniwersytetu Medycznego im. Karola Marcinkowskiego w Poznaniu. Grupę kontrolną stanowiło 20 zdrowych ochotników. Obecność rozrostu bakteryjnego oceniano w nieinwazyjnym teście oddechowym przy użyciu urządzenia Gastro+ Gastrolyzer (firmy Bedfont) z wykorzystaniem 20 g laktulozy rozpuszczonej w 200 ml wody, z oceną stężenia wodoru w wydychanym powietrzu. Za wynik dodatni uznano zawartość wydychanego wodoru ≥ 20 ppm (parts per milion — części na milion) lub wzrost wartości wyjściowej wydychanego wodoru o 12 ppm w czasie pierwszych 60 minut badania.
Wyniki. Występowanie SIBO w grupie badanej stwierdzono u 82 pacjentów (41%), natomiast w grupie kontrolnej u 15 osób (75%), p = 0,0043. Istotną różnicę stwierdzono również w oddzielnym porównaniu osób z cukrzycą typu 1 i 2 i z grupą kontrolną. Obecność SIBO stwierdzano istotnie częściej u osób zdrowych (75%) niż u osób z cukrzycą typu 1 (42%, p = 0,008) oraz typu 2 (40%, p = 0,006). W modelu regresji logistycznej obecność cukrzycy wiązała się z rzadszym występowaniem SIBO niezależnie od wieku, płci i wartości BMI (OR: 0,18, 95% CI: 0,06–0,56; p = 0,003).
Wnioski. U pacjentów z cukrzycą częstość występowania zespołu rozrostu bakteryjnego jelita cienkiego jest mniejsza niż u osób zdrowych. Jedną z możliwych przyczyn stwierdzonego związku może być korzystny wpływ leczenia żywieniowego u osób z cukrzycą
Original paper<br>Does early insulin treatment decrease the risk of microangiopathy in non-obese adults with diabetes?
Introduction: In the clinical practice classification of diabetes type in adult patients is often difficult. In non-obese adults distinguishing between Latent Autoimmune Diabetes in the Adult and type 2 diabetes leads to introduction of different treatment strategies. The aim of the present study was to assess relationship between introduction of insulin therapy at the diagnosis and occurrence of chronic microangiopathic complications of diabetes in non-obese patients aged above 35 years. Material and methods: The group of 71 non-obese patients (BMI <30 kg/m2) with diabetes diagnosed after the age of 35 was estimated. At diagnosis 24 patients were treated with insulin (group A) and 47 patients with oral hypoglycaemics (group B). Results: No significant differences in presence of microangiopathic complications were noted between the groups after 6 years of duration of diabetes. In the multivariate logistic regression analysis, risk factors for the development of any type of diabetic microangiopathy were: longer duration of diabetes (RR 1.12, 95%CI: 1.00-1.27, p=0.04), higher HbA1c value (RR 2.00, 95%CI: 1.21-3.31), and higher BMI (RR 1.25, 95%CI: 0.99-0.25, p=0.048). Moreover the risk factors for microangiopathy were: CRP concentration (RR 1.36, 95%CI: 1.03-1.80, p=0.025) and smoking status (RR 4.37, 95%CI: 1.33-14.44, p=0.013). Conclusions: Initial insulin therapy in non-obese patients with diabetes diagnosed above the age of 35, after 6 years of duration of diabetes, does not decrease significantly the risk of micro and macroangiopathic complications. Main risk factors for development of microangiopathic complications are: longer duration of diabetes, worse metabolic control of diabetes, higher body mass index, higher C-reactive protein (CRP) level and smoking
Związek zaburzeń w angiogenezie i dojrzewaniu naczyń w skórze z cukrzycową chorobą nerek w cukrzycy typu 1
Wstęp. Skóra — jako najbardziej dostępny narząd — została wybrana jako model do badania mikrokrążenia i angiogenezy. Celem tego badania była ilościowa i jakościowa analiza związku pomiędzy unaczynieniem skóry (MVD) a obecnością cukrzycowej choroby nerek (DKD) u dorosłych chorych na cukrzycę typu 1 (T1D). Materiał i metody. Badaniem została objęta grupa kolejnych 148 chorych na cukrzycę typu 1 (87 mężczyzn), z medianą wieku 41 lat (IQR: 31–49) i czasem trwania cukrzycy 21 (17–30) lat. Pacjenci byli pod opieką Katedry i Kliniki Chorób Wewnętrznych i Diabetologii Uniwersytetu Medycznego w Poznaniu. Cukrzycową chorobę nerek rozpoznano na podstawie zwiększonej albuminurii u pacjentów z 10-letnim czasem trwania cukrzycy i/lub wykładnikami retinopatii cukrzycowej. Biopsję skóry wykonano na dystalnej części podudzia, przy użyciu jednorazowych 3-milimetrowych sztanc biopsyjnych z suwakiem (Integra™, Miltex®). Do analiz immunohistochemicznych (IHC) wybrano: autoprzeciwciała anty-CD133, anty-CD34, anty-CD31 i anty-vWF. Gęstość naczyń krwionośnych (MVD) wyliczono metodą „gorących miejsc”. Szkiełka skanowano przy użyciu skanera MiraxMidi (Carl Zeiss) i oglądano przy użyciu CaseViewer (3DHISTECH Ltd. Budapeszt, Węgry). Dane analizowano przy użyciu oprogramowania Statistica v. 13. Wyniki. W grupie pacjentów z rozpoznaną DKD (n = 21) w porównaniu z pacjentami bez DKD (n = 127) odnotowano dłuższy czas trwania cukrzycy [30 (IQR: 21–36) vs. 21 (16–28) lat, p = 0,002], częstsze występowanie nadciśnienia tętniczego [14 (67%) vs. 37 (29%), p = 0,002] oraz niższą wartość szacunkowego wskaźnika filtracji kłębuszkowej (eGFR) [66,1 (54,7–87,9) vs. 93,5 (82,5–106,2) ml/min/1,73 m2, p < 0,001]. Mediana MVD naczyń CD34+ na 1 mm2 skóry właściwej wynosiła w grupie DKD+ 123 (100–170), a w grupie DKD– 121 (100–170), p = 0,775; mediana MVD naczyń CD133+ wynosiła odpowiednio 79 (50–100) vs. 79 (63–93), p = 0,823; mediana MVD naczyń CD31+ 29 (21–46) vs. 38 (17–58), p = 0,454; mediana MVD naczyń vWF+ 42 (25–54) vs. 54 (43–71), p = 0,009. Podane wartości obliczono dla obu warstw skóry właściwej (brodawkowatej i siateczkowatej). W modelu wieloczynnikowej regresji logistycznej obecność cukrzycowej choroby nerek była związana z mniejszą MVD dla naczyń krwionośnych vWF+ na 1 mm2 [OR: 0,97 (95% CI: 0,95–0,99), p = 0,017]. Związek ten był niezależny od wieku, płci, wartości eGFR oraz obecności nadciśnienia tętniczego. Grupa badana obejmowała 79 (52,7%) pacjentów z przewlekłą chorobą nerek (CKD) w stadium 1, 58 (39,2%) w stadium 2, 8 pacjentów (5,4%) w stadium 3a i 3 (2%) w stadium 3b. Nie było pacjentów w stadium CKD 4 ani 5. Nie stwierdzono istotnych różnic w MVD w zależności od stadium CKD. Wniosek. U pacjentów z cukrzycą typu 1 i cukrzycową chorobą nerek obserwuje się zaburzenia procesów angiogenezy oraz dojrzewania naczyń. W biopsji skóry obserwowano zmniejszoną liczbę naczyń krwionośnych o wysokiej dojrzałości (vWF+). Zaburzenia angiogenezy w skórze są obecne już we wczesnych stadiach cukrzycowej choroby nerek

The study included 200 patients (91 with type 1 and 109 with type 2 diabetes hospitalized in years 2013–2015 in the Department of Internal Medicine and Diabetology, Poznan University of Medical Sciences. The control group consisted of 20 healthy volunteers. Hydrogen breath tests with Gastro+ Gastrolyzer (Bedfont) were used to assess SIBO. After drinking 20 g of lactulose we checked exhaled hydrogen concentration. The result was considered positive when exhaled hydrogen of first breath was ≥ 20 ppm (parts per milion) or an increase in the output value of the exhaled hydrogen was 12 ppm during the first 60 minutes of the test.
Results. We observed lower prevalence of SIBO in the test group as compared to controls [82 patients (41%) vs. 15 patients (75%); p = 0.0043]. Moreover, patients with type 1 diabetes as well as type 2 diabetes had lower incidence of SIBO as compared to controls [accordingly, 42% vs. 75%, p=0.008, 40% vs. 75%, p=0.006]. In the logistic regression model this relationship was independent of age, gender and BMI (OR: 5,57, 95% CI: 1,78–17,47; p = 0,003).
Conclusion. The prevalence of microbial proliferation of the small intestine in patients with diabetes is lower than in healthy subjects. One possible cause might be the beneficial effect of nutritional therapy in patients with diabetes.Wstęp. Dolegliwości ze strony przewodu pokarmowego mogą występować u 50–70% osób z cukrzycą. Zespół rozrostu bakteryjnego (SIBO) to nadmierny rozplem w obrębie jelita cienkiego drobnoustrojów kolonizujących jelito grube, prowadzący do zaburzeń trawienia i wchłaniania. Celem pracy była ocena częstości występowania SIBO u pacjentów z cukrzycą, a także związek występowania SIBO z wyrównaniem metabolicznym cukrzycy i obecnością przewlekłych powikłań choroby.
Materiał i metody. Grupę badaną stanowiło 200 osób z cukrzycą (typu 1 — 91 osób; typu 2 — 109 osób), hospitalizowanych w latach 2013–2015 w Klinice Chorób Wewnętrznych i Diabetologii Uniwersytetu Medycznego im. Karola Marcinkowskiego w Poznaniu. Grupę kontrolną stanowiło 20 zdrowych ochotników. Obecność rozrostu bakteryjnego oceniano w nieinwazyjnym teście oddechowym przy użyciu urządzenia Gastro+ Gastrolyzer (firmy Bedfont) z wykorzystaniem 20 g laktulozy rozpuszczonej w 200 ml wody, z oceną stężenia wodoru w wydychanym powietrzu. Za wynik dodatni uznano zawartość wydychanego wodoru ≥ 20 ppm (parts per milion — części na milion) lub wzrost wartości wyjściowej wydychanego wodoru o 12 ppm w czasie pierwszych 60 minut badania.
Wyniki. Występowanie SIBO w grupie badanej stwierdzono u 82 pacjentów (41%), natomiast w grupie kontrolnej u 15 osób (75%), p = 0,0043. Istotną różnicę stwierdzono również w oddzielnym porównaniu osób z cukrzycą typu 1 i 2 i z grupą kontrolną. Obecność SIBO stwierdzano istotnie częściej u osób zdrowych (75%) niż u osób z cukrzycą typu 1 (42%, p = 0,008) oraz typu 2 (40%, p = 0,006). W modelu regresji logistycznej obecność cukrzycy wiązała się z rzadszym występowaniem SIBO niezależnie od wieku, płci i wartości BMI (OR: 0,18, 95% CI: 0,06–0,56; p = 0,003).
Wnioski. U pacjentów z cukrzycą częstość występowania zespołu rozrostu bakteryjnego jelita cienkiego jest mniejsza niż u osób zdrowych. Jedną z możliwych przyczyn stwierdzonego związku może być korzystny wpływ leczenia żywieniowego u osób z cukrzycą
