    Severe anaphylactic shock due to methylene blue dye

    AbstractTo identify the sentinel lymph node in melanoma patients, intradermal injection of a radiocolloid tracer and a blue dye are commonly used. Life-threatening side effects of isosulfan blue and Patent Blue V have been well described. However, to the extent of our knowledge, only two life-threatening events with intradermal methylene blue dye have been reported, and none has been reported in the pediatric population. We report a case of a 6-year-old white girl with spitzoid melanoma on her right forearm. She had lymphoscintigraphy under general anesthesia and was taken to the operating room intubated. Intradermal methylene blue (0.2 ml) was injected around the lesion, and after 5 min, wide complex bradycardia was noted and progressed to asystole within less than 1 min. Cardiopulmonary resuscitation was started. Multiple doses of resuscitative drugs were administered, and electrical cardioversion was given twice as well. She recovered completely and transferred to the intensive care unit

    Banff 2022 liver group meeting report: monitoring long term allograft health.

    The Banff Working Group on Liver Allograft Pathology met in September 2022. Participantsincluded hepatologists, surgeons, pathologists, immunologists and histocompatibility specialists.Presentations and discussions focused on the evaluation of long-term allograft health, including noninvasive and tissue monitoring, immunosuppression optimisation and long-term structural changes.Potential revision of the rejection classification scheme to better accommodate and communicate lateT cell-mediated rejection patterns and related structural changes, such as nodular regenerativehyperplasia, were discussed. Improved stratification of long-term maintenance immunosuppression tomatch the heterogeneity of patient settings will be central to improving long-term patient survival.Such personalised therapeutics are in turn contingent on better understanding and monitoring ofallograft status within a rational decision-making approach, likely to be facilitated in implementationwith emerging decision support tools. Proposed revisions to rejection classification emerging fromthe meeting include incorporation of interface hepatitis and fibrosis staging.     XAF1 as a modifier of p53 function and cancer susceptibility

    Cancer risk is highly variable in carriers of the common TP53-R337H founder allele, possibly due to the influence of modifier genes. Whole-genome sequencing identified a variant in the tumor suppressor XAF1 (E134*/Glu134Ter/rs146752602) in a subset of R337H carriers. Haplotype-defining variants were verified in 203 patients with cancer, 582 relatives, and 42,438 newborns. The compound mutant haplotype was enriched in patients with cancer, conferring risk for sarcoma (P = 0.003) and subsequent malignancies (P = 0.006). Functional analyses demonstrated that wild-type XAF1 enhances transactivation of wild-type and hypomorphic TP53 variants, whereas XAF1-E134* is markedly attenuated in this activity. We propose that cosegregation of XAF1-E134* and TP53-R337H mutations leads to a more aggressive cancer phenotype than TP53-R337H alone, with implications for genetic counseling and clinical management of hypomorphic TP53 mutant carriers.Fil: Pinto, Emilia M..     Immune-related adverse events from immune checkpoint inhibitors in pediatric patients: A literature review and case series

    Background: Immune checkpoint inhibitors (ICIs) are used to treat many adult cancers and are now being used to treat select pediatric cancers. Many toxicities resulting from ICIs, also known as immune-related adverse events (IRAEs), have been reported in the adult literature. IRAEs in the pediatric population have not been as well-characterized as those in the adult population, making management more difficult for clinicians. Methods: Herein we briefly summarize the existing literature about ICI-related toxicities in the pediatric/young adult oncology population. We then describe four patients who were treated at our institution with ICIs and experienced significant IRAEs—pneumonitis, polyneuritis cranialis, antibody-mediated diabetes, and colitis. Results: Patients were initially managed with supportive care, close monitoring, and thorough diagnostic workups to exclude other causes of clinical signs and symptoms (i.e., infection). Additional treatments included corticosteroids, intravenous immunoglobulin (IVIG), vedolizumab, infliximab, and/or discontinuation of ICI therapy. Conclusion: Pediatric/young adult patients being treated for cancer with ICIs should be monitored closely for IRAEs so that appropriate interventions are initiated promptly. Cases of IRAEs in this population should be reported in the literature
