The timing of maternal depressive symptoms and child cognitive development: a longitudinal study.
Background: Maternal depression is known to be associated with impairments in child cognitive development, although the effect of timing of exposure to maternal depression is unclear. Methods: Data collected for the Avon Longitudinal Study of Parents and Children, a longitudinal study beginning in pregnancy, included self-report measures of maternal depression the Edinburgh Postnatal Depression Scale, completed on 6 occasions up to 3 years of age, and IQ of the index child (WISC) measured at aged 8 years. We used these data to assign women to 8 groups according to whether depression occurred in the antenatal, postnatal, preschool period, any combination of these times, or not at all. We compared a model comprising all patterns of depression (saturated model) with models nested within this to test whether there is a relationship between depression and child cognitive development and, if so, whether there is a sensitive period. We then investigated the relationship with child IQ for each model, following adjustment for confounders. Results: Six thousand seven hundred and thirty-five of 13,615 children from singleton births (49.5%, of eligible core sample) attended a research clinic at 8 years and completed a WISC with a score ā„ 70. A total of 5,029 mothers of these children had completed mood assessments over the 3 time periods. In unadjusted analyses, all three sensitive period models were as good as the saturated model, as was an accumulation model. Of the sensitive period models, only that for antenatal exposure was a consistently better fit than the accumulation model. After multiple imputation for missing data (to n = 6,735), there was no effect of postnatal depression on child IQ independent of depression at other times [-0.19 IQ points, 95% confidence interval (CI) -1.5 to 1.1 points]. There was an effect of antenatal depression (-3.19 IQ points, 95% CI: -4.33 to -2.06) which attenuated following adjustment (-0.64 IQ points, 95% CI: -1.68 to 0.40). Conclusions: The postnatal period is not a sensitive one for the effect of maternal depression on child cognitive development. Ā© 2011 The Authors. Journal of Child Psychology and Psychiatry
An old horse revived? In-house use of print books at Seton Hall University.
With limited library budgets and declining circulation of print books, it is important to demonstrate library value to multiple stakeholders and to make informed collection development choices. The aim of this one-year study was to gain a complete picture of print book circulation by identifying titles that were used in the library (āin-houseā) but not checked out. We found that almost 30% of circulation transactions were books that were used in-house. Medical and nursing books showed the highest rate of in-house use in both the reference and main (circulating) collection. A close examination of these subject areas indicated that 46% of potentially circulating medical books used in-house were checked out, and 19% of science books used in house were checked out. This suggests that libraries should not assume that titles used in-house are subsequently checked out, or that check out statistics represent the totality of book use. We recommend including in-house use statistics to obtain an accurate picture of total circulation and library value, and to inform collection development

Moments of the critical values of families of elliptic curves, with applications
We make conjectures on the moments of the central values of the family of all
elliptic curves and on the moments of the first derivative of the central
values of a large family of positive rank curves. In both cases the order of
magnitude is the same as that of the moments of the central values of an
orthogonal family of L-functions. Notably, we predict that the critical values
of all rank 1 elliptic curves is logarithmically larger than the rank 1 curves
in the positive rank family.
Furthermore, as arithmetical applications we make a conjecture on the
distribution of a_p's amongst all rank 2 elliptic curves, and also show how the
Riemann hypothesis can be deduced from sufficient knowledge of the first moment
of the positive rank family (based on an idea of Iwaniec).Comment: 24 page
Doping of inorganic materials in microreactors ā preparation of Zn doped FeāOā nanoparticles
Microreactor systems are now used more and more for the continuous production of metal nanoparticles and metal oxide nanoparticles owing to the controllability of the particle size, an important property in many applications. Here, for the first time, we used microreactors to prepare metal oxide nanoparticles with controlled and varying metal stoichiometry. We prepared and characterised Zn-substituted FeāOā nanoparticles with linear increase of Zn content (ZnxFeāāxOā with 0 ā¤ x ā¤ 0.48), which causes linear increases in properties such as the saturation magnetization, relative to pure FeāOā. The methodology is simple and low cost and has great potential to be adapted to the targeted doping of a vast array of other inorganic materials, allowing greater control on the chemical stoichiometry for nanoparticles prepared in microreactors
The Host Immune Response to Tissue-Engineered Organs: Current Problems and Future Directions
As the global health burden of chronic disease increases, end-stage organ failure has become a costly and intractable problem. De novo organ creation is one of the long-term goals of the medical community. One of the promising avenues is that of tissue engineering: the use of biomaterials to create cells, structures, or even whole organs. Tissue engineering has emerged from its nascent stage, with several proof-of-principle trials performed across various tissue types. As tissue engineering moves from the realm of case trials to broader clinical study, three major questions have emerged: (1) Can the production of biological scaffolds be scaled up accordingly to meet current and future demands without generating an unfavorable immune response? (2) Are biological scaffolds plus or minus the inclusion of cells replaced by scar tissue or native functional tissue? (3) Can tissue-engineered organs be grown in children and adolescents given the different immune profiles of children? In this review, we highlight current research in the immunological response to tissue-engineered biomaterials, cells, and whole organs and address the answers to these questions
Neurohumoral and ambulatory haemodynamic adaptations following isometric exercise training in unmedicated hypertensive patients
Objective:
Hypertension remains the leading modifiable risk factor for cardiovascular disease (CVD). Isometric exercise training (IET) has been shown to be a useful non-pharmacological intervention for reducing resting blood pressure (BP). This study aimed to measure alterations in office BP, ambulatory BP, cardiac autonomic modulation and inflammatory and vascular biomarkers following a programme of IET in unmedicated hypertensive patients.
Methods:
Twenty-four unmedicated stage 1 hypertensive patients (age 43.8Ā±7.3 years; height, 178.1Ā±7 cm; weight 89.7Ā±12.8 kg) were randomly assigned in a cross-over study design, to 4-weeks of home based IET and control period, separated by a 3-week washout period. Office and Ambulatory BP, cardiac autonomic modulation, and inflammatory and vascular biomarkers were recorded pre and post IET and control periods.
Results:
Clinic and 24-hour ambulatory BP significantly reduced following IET by 12.4/6.2 mmHg and 11.8/5.6 mmHg in systolic/diastolic BP, respectively (p<0.001 for both), compared to the control. The BP adaptations were associated with a significant (p=0.018) reduction in the average real variability of 24-hour ambulatory BP following IET, compared to control. Cardiac autonomic modulation improved by 11% (p<0.001), baroreceptor reflex sensitivity improved by 47% (p<0.001), and interleukin-6 and asymmetric dimethylarginine reduced by 10% (p=0.022) and 19% (p=0.023), respectively, which differed significantly to the control period.
Conclusion:
This is the first evidence of durable BP reduction and wider CVD risk benefits of IET in a relevant patient population. Our findings support the role of IET as a safe and viable therapeutic and preventative intervention in the treatment of HTN
Nutritional status of children with Wilmsā tumour on admission to a South African hospital and its influence on outcome
BackgroundIn developing countries up to 77% of children with cancer have been shown to be malnourished on admission. High rates of malnutrition occur due to factors such as poverty and advanced disease. Weight can be an inaccurate parameter for nutritional assessment of children with solid tumours as it is influenced by tumour mass. This study aimed to assess the prevalence of malnutrition amongst children with Wilms tumour (WT), the level of nutritional support received on admission and the influence of nutritional status on outcome.MethodsSeventyāsix children diagnosed with WT and admitted to Inkosi Albert Luthuli Central Hospital between 2004 and 2012 were studied prospectively. Nutritional assessment was conducted using weight, height, midāupper arm circumference (MUAC) and triceps skinfold thickness (TSFT) prior to initiating treatment. Outcome was determined 2 years after admission. Time until commencement of nutritional resuscitation and nature, thereof, were recorded.ResultsStunting and wasting was evident in 12% and 15% of patients, respectively. The prevalence of malnutrition was 66% when MUAC, TSFT and albumin were used. Malnutrition was not a predictor of poor outcome and did not predict advanced disease. The majority of patients (84%) received nutritional resuscitation within 2 weeks of admission.ConclusionsWhen classifying nutritional status in children with WT, the utilisation of weight and height in isolation can lead to an underestimation of the prevalence of malnutrition. Nutritional assessment of children with WT should also include MUAC and TSFT. Early aggressive nutritional resuscitation is recommended.Peer Reviewedhttps://deepblue.lib.umich.edu/bitstream/2027.42/137198/1/pbc26382.pdfhttps://deepblue.lib.umich.edu/bitstream/2027.42/137198/2/pbc26382_am.pd
