Circadian models of serum potassium, sodium, and calcium concentrations in healthy individuals and their application to cardiac electrophysiology simulations at individual level
In the article a brief description of the biological basis of the regulation of human biological clocks was presented in order to introduce the role of circadian rhythms in physiology and specifically in the pharmacological translational tools based on the computational physiology models to motivate the need to provide models of circadian fluctuation in plasma cations. The main aim of the study was to develop statistical models of the circadian rhythm of potassium, sodium, and calcium concentrations in plasma. The developed ion models were further tested by assessing their influence on QT duration (cardiac endpoint) as simulated by the biophysically detailed models of human left ventricular cardiomyocyte. The main results are model equations along with an electronic supplement to the article that contains a fully functional implementation of all models
Acupuncture : could it become everyday practice in oncology?
Acupuncture is a complementary and
alternative medical treatment (CAM)
which is increasingly used in the care
of cancer patients. Traditionally derived
from Chinese medicine, nowadays it is
becoming a part of evidence-based
oncology. The use of acupuncture in
these patients has been recommended
by the American Cancer Society (ACS)
for the treatment of side effects associated
with conventional cancer therapy
and cancer-related ailments. A growing
body of evidence supports the use of
acupuncture in the treatment of cancer-
induced pain and chemotherapy-
related nausea and vomiting. Also
other indications, such as xerostomia,
fatigue, hot flashes, anxiety and peripheral
neuropathy, are being constantly
evaluated. This article summarizes
the most important discoveries
related to the possible usefulness of
this method in contemporary oncology.
Emphasis is placed on the results
of randomized controlled trials with
an adequate level of evidence. However,
explanation of the mechanisms
responsible for these effects requires
confirmation in further studies with an
adequate level of evidence. In future,
acupuncture may become an interesting
and valuable addition to conventional
medicine
Creating and field-testing the questionnaire for the assessment of knowledge about cervical cancer and its prevention among schoolgirls and female students
OBJECTIVE: The aim of this study was to develop and validate a questionnaire used to assess the level of general knowledge about cervical cancer, its primary and secondary prevention, and to identify sources of information about the disease among schoolgirls and female students. METHODS: The questionnaire development process was divided into four phases: generation of issues; construction of a provisional questionnaire; testing of the provisional questionnaire for acceptability and relevance; field-testing, which aimed at ensuring reliability and validity of the questionnaire. Field-testing included 305 respondents of high school female Caucasian students, who filled out the final version of the questionnaire. RESULTS: After phase 1, a list of 65 issues concerning knowledge about cervical cancer and its prevention was generated. Of 305, 155 were schoolgirls (mean age±SD, 17.8±0.5) and 150 were female students (mean age±SD, 21.7±1.8). The Cronbach alpha coefficient for the whole questionnaire was 0.71 (range for specific questionnaire sections, 0.60 to 0.81). Test-retest reliability ranged from 0.89 to 0.94. CONCLUSION: The Cervical-Cancer-Knowledge-Prevention-64 has been successfully developed to measure the level of knowledge about cervical cancer. The results confirm the validity, reliability and applicability of the created questionnaire
HER-3 expression in HER-2-amplified breast carcinoma
AIM OF THE STUDY: To determine whether the expression of HER-3 influences the survival of HER-2 positive patients with breast cancer (BC). MATERIAL AND METHODS: In the present work, the expression of HER-3 in a group of 35 HER-2 positive patients with BC was studied by performing immunohistochemistry (IHC) in formalin-fixed paraffin embedded tissues. RESULTS: Higher HER-3 status if estimated by IHC correlated significantly with older age of the patients. HER-3 expression did not correlate with estrogen or progesterone receptor status, pT or pN. There was also no significant difference in disease-free or overall survival (DFS and OS) between groups with different HER-3 expression, although some tendencies were seen as HER-3 expression in over 50% of cells was a factor of worse 5- and 10-year survival. CONCLUSIONS: Further studies should be performed on a larger group of patients to confirm the prognostic role of HER-3 status determined by IHC in BC
Synchronous extraskeletal Ewing's sarcoma/PNET and gallbladder carcinoma : a case report and literature review
Ewing’s sarcoma (ES) and primitive neuroectodermal tumour (PNET) are now considered to be the same tumour and usually occur in long bones. Extraskeletal Ewing’s sarcoma is an extremely rare neoplasm, accounting for 1% of soft tissue sarcomas, with most common location in the thorax. Gallbladder cancer (GBC) represents the most common type among the biliary tract cancers with a poor prognosis even among patients undergoing aggressive therapy.
We present study of extraskeletal ES/PNET found in the hilus of the liver of an elderly, diagnosed one month prior with GBC woman. The patient underwent two cycles of chemotherapy SAIME/SAVAC for ES and thereafter was operated. During three-year follow-up no recurrence of ES/PNET has been reported. However, two years after chemotherapy the patient suffered a relapse of adenocarcinoma of the gallbladder and thus received palliative chemotherapy of gemcitabine and cisplatin. After 16 months of recurrence she died. To the best of our knowledge, this is the first case of ES/PNET located in the hilus of the liver and as a synchronous neoplasm
Pilot testing and preliminary psychometric validation of the Polish translation of the EORTC INFO25 questionnaire : validation of the Polish version of INFO25-pilot study
The quality of information that oncological patients receive from health care professionals is an underestimated issue in Poland and Eastern European countries. There is lack of sufficient data on this subject. The European Organization for Research and Treatment of Cancer (EORTC) supplies a new tool for measuring the quality of information provided to cancer patients. The purpose of the study is the translation into Polish, pilot testing and preliminary validation of the EORTC information module (INFO25). Following the EORTC translation procedures, forward and back translations of the questionnaire were performed (English → Polish, Polish → English). The intermediate version of the INFO25 was pilot-tested together with the general questionnaire of quality of life (EORTC QLQ-C30). Reliability, validity and known-group comparison tests were performed. A total of 21 patients with different cancer diagnoses were recruited into the study (7 women and 14 men; mean age of 60,2 years, age range 25–73 years). Apart from filling out the INFO25, patients were interviewed about the difficulties with answering every questionnaire item. Patients' comments were analyzed and minor language changes were made to the initial translation. The internal consistency of the INFO25 showed a reliability of 0,78. The final version of the Polish translation of the INFO25 module was obtained and approved by the EORTC Quality of Life Department. It can now be used in clinical setting and for scientific purposes
Comparison of efficacy and safety of first-line palliative chemotherapy with EOX and mDCF regimens in patients with locally advanced inoperable or metastatic HER2-negative gastric or gastroesophageal junction adenocarcinoma : a randomized phase 3 trial
The aim of the study was to compare efficacy and safety of first-line palliative chemotherapy with (EOX) epirubicin/oxaliplatin/capecitabine and (mDCF) docetaxel/cisplatin/5FU/leucovorin regimens for untreated advanced HER2-negative gastric or gastroesophageal junction adenocarcinoma. Fifty-six patients were randomly assigned to mDCF (docetaxel 40 mg/m(2) day 1, leucovorin 400 mg/m(2) day 1, 5FU 400 mg/m(2) bolus day 1, 5FU 1000 mg/m(2)/d days 1 and 2, cisplatin 40 mg/m(2) day 3) or EOX (epirubicin 50 mg/m(2) day 1, oxaliplatin 130 mg/m(2) day 1, capecitabine 1250 mg/m(2)/d days 1–21). The primary endpoint was overall survival. The median overall survival was 9.5 months with EOX and 11.9 months with mDCF (p = 0.135), while median progression-free survival was 6.4 and 6.8 months, respectively (p = 0.440). Two-year survival rate was 22.2 % with mDCF compared to 5.2 % with EOX. Patients in the EOX arm had more frequent reductions in chemotherapy doses (34.5 vs. 3.7 %; p = 0.010) and delays in subsequent chemotherapy cycles (82.8 vs. 63.0 %; p = 0.171). There was no statistically significant difference in the rates of grade 3–4 adverse events (EOX 79.3 vs. mDCF 61.5 %; p = 0.234). As compared with the mDCF, the EOX regimen was associated with more frequent nausea (34.5 vs. 15.4 %), thromboembolic events (13.8 vs. 7.7 %), abdominal pain (13.8 vs. 7.7 %) and grades 3–4 neutropenia (72.4 vs. 50.0 %), but lower incidences of anemia (44.8 vs. 61.5 %), mucositis (6.9 vs. 15.4 %) and peripheral neuropathy (6.9 vs. 15.4 %). In conclusion, the mDCF regimen was associated with a statistically nonsignificant 2.4-month longer median overall survival without an increase in toxicity. The antineoplastic effect of low-molecular-weight heparins : a literature review
There is some evidence for the antitumor
effect of heparins, especially the lowmolecular-
weight ones. The authors
discuss the potential mechanism of
this antineoplastic effect and present
results from several in vitro and in vivo
experiments. The clinical trials concerning
the impact of low-molecularweight
heparins on the tumor and on
the patients’ survival are described. The
objective was to find out if heparins could
be administered as an antitumor drug,
independently of their anticoagulatory
properties.
The antitumor role of tissue factor, heparinase,
chemokines, stromal proteins,
cellular interactions as well as angiogenesis
and immunology seems certain.
The results of the available studies
seem promising but large clinical trials
are necessary in order to confirm the
antineoplastic effect of the low-molecular-
weight heparins and to approve
them for standard anticancer treatment.
