    Adverse reactions of radiotherapy for head and neck cancers : treatment of radiation reactions in the oral cavity

    Due to聽painless nature and聽poorly specific symptoms, such as hoarseness or聽sore throat, head and聽neck cancers are usually diagnosed when the disease is聽locally advanced. A typical patient is聽older than 50 years. Low social awareness concerning the occurrence of聽these cancers and聽rare appointments with specialist physicians escalate the problem. As a result, patients usually seek medical advice when the disease is聽advanced and聽prognosis poor. The聽risk of聽these cancers increases by regular consumption of聽weak alcoholic beverages, cigarette smoking and聽infection with human papilloma virus. The聽head and聽neck location, which is聽a richly vascularised and聽innervated anatomic region, necessitates the application of聽highly specialised treatment, i.e. intensitymodulated radiation therapy. Radiation reactions can be divided into early (acute) and聽late (chronic) based on the time of聽occurrence. Early reactions include inflammation and聽fibrosis of聽the oral mucosa. Late reactions are more troublesome and聽persistent. They include: mandible necrosis or聽permanent impairment of聽salivary gland secretory function. The聽most common adverse effects of聽radiotherapy include mucositis. Patients irradiated for head and聽neck cancers usually suffer from persistent oral mucosa dryness that requires particular care and聽hygiene. Preventive measures in聽patients undergoing radiotherapy include: systematic plaque removal, using high-fluoride agents for oral hygiene, following a low-sugar diet and聽regular dental check-ups

Dental phobia is a paralyzing fear of dental visits and dental procedures. A person affected by dental phobia decides to make an appointment only when their normal functioning is impossible due to pain or when pressed by family or friends. This phenomenon can be evaluated based on changes in physiological responses (blood prese, skin colour, sweating), behaviour (mainly children) or using psychological methods (questionnaires). Despite advances in dentistry and the increasing awareness of the Polish society, the problem of dental anxiety is still unsolved. This results in high DMFt index, which was 8 (2013) for Poles aged 18 years. Aim. The aim of the study was an attempt to determine the causes and the incidence of dental phobia. Material and methods. A total of 330 patients of an out-patient clinic at the University Hospital of Bia艂ystok were included in the survey. Statistical analysis of the collected data was performed with Statistica 10.0. using the Chi-square test (p < 0.05). Results. Negative past experiences and ideas about dental treatment are the most common causes of dental fear in patients. Fear coexisting with somatic symptoms, which can prevent dental treatment, was more often reported by women. Men are afraid of pain connected with dental procedures more often than women. Conclusions. The phenomenon of dental phobia has a complex aetiology, therefore further studies are needed to investigate this issue.dental phobiapanic feardental visitcauses of dental phobi
