Impairment of the digestive system in the novel coronavirus infection

Introducere. Prevalenţa şi prognosticul implicării digestive
în evoluţia patologiei provocate de noul coronavirus
(COVID-19) nu este pe deplin elucidată. Scopul a fost de a
analiza şi de a sistematiza datele disponibile, privind multiplele
aspecte gastroenterologice ale bolii, inclusiv poziţia celor
mai valoroase asociaţii din domeniul în cauză, ca răspuns
la apariţia pandemiei. Material şi metode. Au fost efectuată o cercetare în
PubMed, Medline, Web of Science, Elsiever, folosind motorul
de căutare Google, aplicând cuvinte cheie: „coronavirus”,
„sindrom acut respirator sever coronavirus 2”, „SARS-CoV-2”,
„COVID-19”, „nCoV”, „2019-nCoV”. Au fost analizate articole în
limba engleză, începând cu luna ianuarie până în luna mai,
2020. Au fost incluse toate articolele care raportau date epidemiologice
şi clinice, privind simptomele gastro-intestinale,
leziunile hepatice, prevalenţa comorbidităţilor hepatice şi
gastro-intestinale la pacienţii cu COVID-19.
Rezultate. Manifestările gastrointestinale, precum diareea,
voma, greaţa, durerea abdominală au survenit frecvent la
pacienţii cu COVID-19. Acestea, în unele cazuri, au precedat
simptomatologia respiratorie, întârziind stabilirea diagnosticului
corect şi în mod excepţional, au fost stabilite precum
unica acuză a pacientului. Pacienţii cu maladie COVID-19
severă au avut un risc mai mare de a dezvolta simptome
gastro-intestinale şi leziuni hepatice, manifestate prin elevarea
transaminazelor, hipoalbuminemie, trombocitopenie,
tendinţă spre dezvoltarea stărilor critice şi înrăutăţirea prognosticului.
Riscul de boală severă sau de infectare nu a fost
mai crescut în rândul pacienţilor cu COVID-19 şi comorbidităţi
gastrointestinale sau hepatice existente. Totuşi, s-a observat
faptul că, ciroza hepatică decompensată, carcinomul
hepatocelular, formele severe ale bolilor inflamatorii intestinale
au precipitat dezvoltarea formelor severe ale COVID-19
şi prognostic nefavorabil. Deşi, s-a demonstrat că eliminarea
virusului cu materiile fecale depăşeşte în timp prezenţa
acestuia în frotiul nazofaringian, sunt necesare studii suplimentare
pentru a confirma existenţa căii de transmitere fecal-
orală a SARS-CoV-2.
Concluzii. Studiul a arătat că simptomele digestive şi
leziunile hepatice nu sunt o constatare accidentală, în cazul
pacienţilor infectaţi cu coronavirusul de tip nou, având
relevanţă în diagnosticul precoce al bolii şi în prognosticul
acesteia. O atenţie sporită trebuie acordată îngrijirii acestui
grup de pacienţi.Abstract
Introduction. The prevalence of digestive system involvement
in the evolution of coronavirus disease (COVID-
19) is not fully elucidated. The aim was to analyze and
systematize the available data on the multiple gastroenterological
aspects of the disease, including the position of the
most valuable associations in the field, in response to the
pandemic. Material and methods. It was performed a search in
PubMed, Medline, Web of Science, Elsiever on the Google
search engine, applying keywords: “coronavirus”, “severe
acute respiratory syndrome”, “SARS-CoV-2”, “COVID-19”,
“nCoV”, “2019-nCoV”. There were analysed articles in English
language beginning with January up to May 2020. All
articles, that reported epidemiological and clinical data on
gastrointestinal symptoms, hepatic injury, prevalence of hepatic
and gastrointestinal comorbidities in patients with COVID-
19 were included.
Results. Gastrointestinal manifestations, such as diarrhea,
vomiting, nausea, abdominal pain have occurred frequently
in patients with COVID-19. These, in some cases,
preceded the respiratory symptoms, delaying the establishment
of the correct diagnosis and, exceptionally, were established
as the only complaint of the patient. The patients with
severe COVID-19 had a higher risk of developing gastrointestinal
symptoms and liver damage, manifested by elevated
transaminases, hypoalbuminemia, thrombocytopenia, tendency
towards the development of critical states and the
worsening of the prognosis. The risk of severe disease or
infection, has not been increased in patients with COVID-19
and existing gastrointestinal or hepatic comorbidities. However,
it was observed that decompensated liver cirrhosis, hepatocellular
carcinoma, severe forms of inflammatory bowel,
precipitated the development of severe forms of COVID-19
and with an unfavorable prognosis. Although fecal elimination
of the virus has been shown to outweigh its presence
in the nasopharyngeal smear, further studies are needed to
confirm the existence of the fecal-oral route of SARS-CoV-2
transmission.
Conclusions. This study showed that digestive symptoms
and liver damage are not an accidental finding in patients
with COVID-19, having relevance in the early diagnosis
of the disease and its prognosis. An increased attention
should be offered to the care of this group of patients
Profilul clinico-demografic al pacienților cu hepatită alcoolică
Gastroenterology Discipline, Nicolae Testemitanu SUMPhBackground. Alcoholic hepatitis (AH) is the most severe manifestation of alcohol associated liver
disease with a high rate of morbidity and mortality. It is paramount to establish a screening method
with reasonable accuracy in assessing cases with high risk of developing terminal liver disease
alcohol-induced. Objective of the study. Analysis of demographic and clinical characteristics of
patients with AH and observational study of risk factors involved in development of alcohol induced
end stage liver disease. Material and Methods. In this observational study, it was researched
retrospectively 61 patients with AH admitted in Department of Hepatology of SCR “T. Moșneaga”.
The diagnosis of AH was confirmed clinically by questionnaire (AUDIT C),anamnesis with
supporting laboratory findings. Assessment of liver function and Maddrey discriminant factor (MDF)
was calculated at presentation. Results. The highest frequency was in young adults, the average age of
patients being 54 ± 10.1 years (33-73), with male prevalence (55.73%). But women were more likely
to develop a severe form of AH (48.65%), using a smaller amount of alcohol. The habit of bingedrinking was determined in 62.29% of patients. The most important clinical manifestations were:
asthenic syndrome (96.72%), jaundice (75%), ascites (47.54%), hemorrhagic syndrome (31.14%),
hepatic encephalopathy (29.95%). The death rate among patients with AH was 24.32%, all with
MDF> 32, predominantly females (56.8%). Conclusion. The amount of alcohol, gender, habit of
consumption are risk factors for severe alcoholic disease and patients with MDF> 32 develop more
frequent complications and have a poorer outcome. Creating a screening system in this regard will
help reduce mortality rates.Introducere. Hepatita alcoolică (HA) e cea mai severă formă de boală hepatică alcool-indusă, asociată
cu mortalitate înaltă. Există necesitatea elaborării unui sistem de screening pentru evaluarea riscului
major de dezvoltare a patologiilor hepatice terminale alcool-induse. Scopul lucrării. Evaluarea
caracteristicilor demografici și clinici a pacienților cu hepatită alcoolică cu analiza observațională a
factorilor de risc sporit pentru dezvoltarea patologiei hepatice terminale induse de alcool. Material și
Metode. Au fost studiați retrospectiv 61 de pacienți spitalizați în secția de hepatologie, IMSP SCR
„Timofei Moșneaga” cu diagnosticul de hepatită alcoolică confirmat, a fost evaluată funcția hepatică și
s-a calculat scorul de severitate Maddrey (MDF). Rezultate. S-a determinat afectarea mai frecventă la
tineri, vârsta medie a pacienților fiind de 54±10,1ani (33-73), cu preponderență a sexului masculin
(55,73%), însă femeile sunt mai predispuse să dezvolte o formă severă de hepatită alcoolică (48,65%)
utilizând o cantitate mai mică de alcool. La 62,29% dintre pacienți s-a determinat obiceiul consumului
– chef (binge-drinking). Cele mai importante manifestări clinice au fost: sindromul asteno-vegetativ
(96,72%), icterul (75%), ascita (47,54%), sindromul hemoragipar (31,14%), encefalopatia hepatică
(29,95%). Rata deceselor în grupul pacienților cu HA a constituit 24,32%, toți fiind cu scor Maddrey>32,
cu predominarea sexului feminin (56,8%). Concluzii. Cantitatea de alcool, genul, obiceiul consumului
sunt factorii de risc pentru boala alcoolică severă, iar pacienții cu scor Maddrey>32 au dezvoltat mai
frecvent complicații până la deces. Crearea unui sistem de screening în acest sens va contribui la
diminuarea ratelor de mortalitate
Antiviral treatment of chronic viral hepatitis C in Republic of Moldova

Introducere. Hepatita virală este o problemă majoră de
sănătate publică, atât la nivel mondial, cât şi pentru Republica
Moldova. Infecţia cu virusul hepatitei C (VHC) este una dintre
principalele cauze ale bolilor hepatice cronice, iar prevalenţa
generală a infecţiei cu VHC în 2015 a fost estimată la aproximativ
1,0%. Utilizarea noului tratament cu antivirale cu acţiune
directă (AAD) în hepatita virală C, are un răspuns viral
susţinut (SVR) de 95-100%, acesta fiind un argument pentru
continuarea Programului Naţional pentru controlul hepatitei
virale B, C şi D, pentru anii 2017-2021, în Republica Moldova.
Material şi metode. Acest studiu a avut ca scop evaluarea
a 15.754 dosare ale pacienţilor care au primit tratament cu
AAD împotriva VHC, în cadrul Programului Naţional în perioada
2016-2019. Variabilele studiate au fost vârsta, sexul, încărcătura
virală, rata fibrozei hepatice (determinată de fibroscan),
genotipul VHC, regimul de tratament, durata terapiei şi
răspunsul la tratament.
Rezultate. Distribuiţi în funcţie de vârstă, majoritatea pacienţilor,
în special 10.618 (69,3%), aveau între 25 şi 65 de
ani. Repartiţia conform genotipului: 14.494 pacienţi (92%)
au avut genotipul 1, 205 pacienţi (1,3%) cu genotipul 2, 977
(6,2%) cu genotipul 3, iar restul de 78 pacienţi (0,5%) cu genotipul
4, genotipul mixt sau genotip neidentificat. Nu a fost
identificată o legătură patogenetică între sex şi gradul de fibroză
şi nici nu a existat o inferenţă cauzală semnificativă între
gradul de fibroză şi viremia VHC. Un total de 15.685 (99,5%)
pacienţi au prezentat RVS, dar pentru genotipul 3, RVS a fost
obţinut în 93,6% cazuri.
Concluzii. Rezultatele acestui studiu reflectă marele succes
al Programului Naţional al Republicii Moldova pentru
combaterea hepatitei virale, în special a hepatitei VHC, cu utilizarea
terapiei AAD şi susţin necesitatea implementării unor
noi strategii pentru a minimiza incidenţa şi prevalenţa acestei
boli în Republica Moldova.Introduction. Viral hepatitis is a major public health problem,

disease and the overall prevalence of HCV infection in 2015
was estimated at about 1.0%. The use of the new treatment
with direct-acting antivirals (DAA) in HCV hepatitis has a sustained
virological response (SVR) of 95-100%, this being an
argument for continuing the National Program for the control
of viral hepatitis B, C and D, for 2017-2021 years, in Republic
of Moldova
Material and methods. This study aimed to assess 15.754
files of patients who received treatment with DAAs against
HCV, within the National Program during 2016-2019. The
variables studied were age, sex, viral load, hepatic fibrosis rate
(determined by fibroscan), HCV genotype, treatment regimen,
duration of therapy and response to treatment.
Results. Distributed by age, the majority of patients, specifically
10618 (69.3%), were between 25 and 65 years old.
Repartition by genotype: 14.494 patients (92%) were with
genotype 1, 205 patients (1.3%) with genotype 2, 977 (6.2%)
with genotype 3, and the remaining 78 patients (0.5%) with
genotype 4, mixed genotype or unidentified genotype. A
pathogenetic link between sex and the degree of fibrosis has
not been identified, nor was there a significant causal inference
between the degree of fibrosis and HCV viremia. A total
of 15.685 (99.5%) patients had SVR, but for genotype 3, SVR
was obtained in 93.6% cases.
Conclusions. Results of this study reflects the great success
of the National Program of Republic of Moldova for combating
viral hepatitis, especially HCV hepatitis with the use
of DAA therapy and advocate the need of implementing new
strategies to minimize the incidence and prevalence of this
disease in Republic of Moldova
