    The author found a new rickettsial disease clinically in 1984, and named it Japanese spottedfever in 1987.For more than 20 years, the investigations concerning this emerging infectious disease hasrequired the collaborative studies with many researchers, such as internal medicine, infection controlmedicine, dermatology, microbiology, pathology, entomology, zoology, veterinary medicine subsequently.This article presented the current status of Japanese spotted fever and the process of solutionabout clinical based question by cooperation of the basic medical scientist.The cases presented were as follows ;1)Research for the vector study2)Development of the new histopathological diagnostic method3)Discussion about the clinical studies on Japanese spotted fever among medical studentsand pathologist, at the summer school which is organized by the society of pathology.It is a great pleasure to share the intellectual curiosity based on the prominent insight withhigh technology between clinical and basic medicine investigators.That must be attractive to young power and inspires the scientist

    The sarin matter by terrorist, which broke up in Mastumoto City (1994) and in Tokyo (1995), arsenic poison curry matter in Wakayama Prefecture (1998), and following similar affairs gave the ruinous earthquake to not only citizens but also doctors in medical care. The management for such situation has been discussed in Tokushima Medical Association and has constructed the risk management network between prefectural health center, police department of chemical substance analysis and Tokushima University for academic support. In this paper, the place to first contact as there is a possibility of drug poisoning and to correspondence with the network for the crisis management in Tokushima Prefecture are described in detail. In addition to them, the clinical diagnosis and first-aid treatment for acute drug poisoning and basic toxicology are mentioned

    The height and weight of all 74,859 students attending to the primary and junior-highschools in the Tokushima prefecture were gathered for the purpose of data collection for thecommittee for prevention of life style related disease. The measurement was performedbetween April and June, 2000 according to the methods recommended by the Japanesegovernment. The histograms of height of each sex and age group showed clear normaldistribution. On the other hands, the histograms of weight and Body Mass Index (BMI)showed the deviation towards the heavier part. We draw histograms of all males and allfemales, and found the existence of two peaks in both of the histograms. We recognizedthat the middle depressed part, which means the less person of that height, indicates thepeak of growth. The most bottoms were 139cm-140cm in female and 152cm-153cm in male

    Steroid pulse therapy in patients with encephalopathy associated with severe fever with thrombocytopenia syndrome

    Severe fever with thrombocytopenia syndrome (SFTS) is a tick-borne infectious disease caused by the SFTS virus (SFTSV). Clinical symptoms of SFTS often involve encephalopathy and other central neurological symptoms, particularly in seriously ill patients; however, pathogenesis of encephalopathy by SFTSV is largely unknown. Herein, we present case reports of three patients with SFTS, complicated by encephalopathy, admitted to Tokushima University hospital: one patient was a 63-year-old man, while the other two were 83- and 86-year-old women. All of them developed disturbance of consciousness around the 7th day post onset of fever. After methylprednisolone pulse therapy of 500 mg/day, all of them recovered without any neurological sequelae. SFTSV genome was not detected in the cerebrospinal fluid of 2 out of the 3 patients that were available for examination. In these patients, disturbance of consciousness seemed to be an indirect effect of the cytokine storm triggered by SFTSV infection.We propose that short-term glucocorticoid therapy might be beneficial in the treatment of encephalopathy during early phase of SFTSV infection
