    Transcatheter Leadless Cardiac Pacing with Limited Venous Access

    Entirely leadless cardiac pacemakers that are delivered transvenously required the use of large diameter delivery sheath and femoral venous approach. The complexity of external femoral and iliac venous anatomy may limit their implantation. We describe a patient without subclavian venous access and a conventional pacemaker with a failed right ventricular lead, who had difficult iliac venous anatomy that was also compressed by an external endovascular abdominal aortic stent. Successful leadless pacing using a Micra™ (Medtronic Inc) was accomplished with a strong support wire, hydrophilic delivery sheath and guided by venography. This article is protected by copyright.     We rebut the recent claim (arXiv:0912.5463) that Einstein-frame scattering in the Higgs inflation model is unitary above the cut-off energy Lambda ~ Mp/xi. We show explicitly how unitarity problems arise in both the Einstein and Jordan frames of the theory. In a covariant gauge they arise from non-minimal Higgs self-couplings, which cannot be removed by field redefinitions because the target space is not flat. In unitary gauge, where there is only a single scalar which can be redefined to achieve canonical kinetic terms, the unitarity problems arise through non-minimal Higgs-gauge couplings.Comment: 5 pages, 1 figure V3: Journal Versio

    Unique Appearance of Lamellar Cleavage Patterns on Fracture Surfaces of Ti-Based Amorphous Matrix Composite

    In order to improve mechanical properties of Ti-based amorphous matrix composites basically composed of ductile beta-Ti dendrites and brittle amorphous matrix by overcoming their inherent brittle nature, their fracture mechanisms should be verified in relation with microstructure, stress intensity factor level, and crack growth rate. In this study, thus, detailed fractographic observations including the unique appearance of lamellar cleavage patterns, which has not been reported in previous studies on conventional metals and alloys, were conducted. According to fractographic results, lamellar cleavage patterns were formed by repeated interruptions of crack propagation on {100} cleavage planes by difference between dendrite orientation and loading direction. Ductile-to-brittle transition phenomenon (ductile dimpled fracture -> lamellar cleavage fracture -> ordinary cleavage fracture in dendrite areas, and vein pattern -> smooth pattern in amorphous matrix areas) occurred with increasing crack growth rate was also plausibly explained by the concept of time required for crack growth as well as dendrite orientation.open1122sciescopu

    Bulk Axions, Brane Back-reaction and Fluxes

    Extra-dimensional models can involve bulk pseudo-Goldstone bosons (pGBs) whose shift symmetry is explicitly broken only by physics localized on branes. Reliable calculation of their low-energy potential is often difficult because it requires details of the stabilization of the extra dimensions. In rugby ball solutions, for which two compact extra dimensions are stabilized in the presence of only positive-tension brane sources, the effects of brane back-reaction can be computed explicitly. This allows the calculation of the shape of the low-energy pGB potential and response of the extra dimensional geometry as a function of the perturbing brane properties. If the pGB-dependence is a small part of the total brane tension a very general analysis is possible, permitting an exploration of how the system responds to frustration when the two branes disagree on what the proper scalar vacuum should be. We show how the low-energy potential is given by the sum of brane tensions (in agreement with common lore) when only the brane tensions couple to the pGB. We also show how a direct brane coupling to the flux stabilizing the extra dimensions corrects this result in a way that does not simply amount to the contribution of the flux to the brane tensions. We calculate the mass of the would-be zero mode, and briefly describe several potential applications, including a brane realization of `natural inflation,' and a dynamical mechanism for suppressing the couplings of the pGB to matter localized on the branes. Since the scalar can be light enough to be relevant to precision tests of gravity (in a technically natural way) this mechanism can be relevant to evading phenomenological bounds.Comment: 36 pages, JHEP styl

    Sudden cardiac death: Prevention and treatment

    Sudden cardiac death remains a major health issue in western countries as well as in Hong Kong. Despite increasing knowledge of the mechanisms and risk factors of sudden cardiac death, methods for identifying high-risk candidates and predicting the efficacy of measures to prevent sudden cardiac death are still inadequate. A significant proportion of patients have known heart disease but are generally considered to be at low risk for this event. More efforts are needed to improve the success rate of out-of-hospital resuscitation through better warning systems, the use of amiodarone for refractory arrhythmias, and the widespread availability of automated defibrillation devices to allow early defibrillation. It is likely that these measures could increase the number of survivors following cardiac arrest. In survivors of sudden cardiac death episodes, treatment of the underlying cardiac disease, especially early revascularisation for myocardial ischaemia, is required. In the majority of patients, implantation of an implantable cardioverter defibrillator, with or without the use of an anti-arrhythmic drug such as amiodarone, would then be used to maintain survival. Furthermore, for individuals at significant risk of sudden cardiac death, primary prevention of sudden cardiac death through the placement of an implantable cardioverter defibrillator is increasingly being used.published_or_final_versio
