    Surgical Treatment for Biliary Carcinoma Arising After Pancreatoduodenectomy

    The clinicopathological features and surgical treatment of biliary carcinoma around the major hepatic duct confluence arising after pancreatoduodenectomy (PD) due to initial bile duct carcinoma are described in three patients. Occurrence of biliary carcinoma more than 12 years after initial surgery and a histological finding of cholangiocellular carcinoma mixed with hepatocellular carcinoma suggested metachronous incidence of biliary carcinoma after PD. Extended right hemihepatectomy with complete removal of the residual extrahepatic bile duct and segmental, resection of the jejunal loop were carried out safely without operative death or severe postoperative complications. Two patients died of tumor recurrence 6 months after surgery, and the remaining patient is currently living a normal life without evidence of recurrence 17 months after surgery. These surgical procedures are a therapeutic option in patients with biliary carcinoma around the major hepatic duct confluence arising after PD

    Strategy-Proof and Non-Wasteful Multi-Unit Auction via Social Network

    Auctions via social network, pioneered by Li et al. (2017), have been attracting considerable attention in the literature of mechanism design for auctions. However, no known mechanism has satisfied strategy-proofness, non-deficit, non-wastefulness, and individual rationality for the multi-unit unit-demand auction, except for some naive ones. In this paper, we first propose a mechanism that satisfies all the above properties. We then make a comprehensive comparison with two naive mechanisms, showing that the proposed mechanism dominates them in social surplus, seller's revenue, and incentive of buyers for truth-telling. We also analyze the characteristics of the social surplus and the revenue achieved by the proposed mechanism, including the constant approximability of the worst-case efficiency loss and the complexity of optimizing revenue from the seller's perspective

    Report of a study using phantom materials, and clinical experience with simultaneous radio-hyperthermotherapy.

    Simultaneous radiohyperthermotherapy (SRH) is a combined hyperthermia-radiation therapy in which irradiation is given during heating. Mutual interference between the high energy radiotherapy system (Toshiba LMR-15A) and the 13.56 MHz capacitive heating system (Omron HEH-500C) was tested with phantom materials prior to a clinical trial with SRH. The energy and flatness of irradiation were not affected by the heating system within the range of clinical use. The high energy radiotherapy system did not affect the increase or distribution of temperature during simultaneous treatment. The results of this phantom study indicated that these apparatuses would not produce clinically significant mutual interference during SRH. A clinical trial was performed on a 57-year-old woman with postoperative recurrence of rectal cancer. This is the first reported clinical case treated with true SRH in which external irradiation was administered during mid capacitive heating. Twelve SRH treatments were performed on the recurrent lesion at a frequency of twice a week for six weeks using the apparatuses described above. There was a significant reduction in pain after treatment. The tumor marker carcinoembryonic antigen (CEA) level decreased after treatment. On CT images taken after treatment, the tumor site became a low density area which indicated necrosis. There were no side effects. These results suggest that further clinical study of SRH should be performed to clarify its advantages.</p

    Optimization of quantum noise in space gravitational-wave antenna DECIGO with optical-spring quantum locking considering mixture of vacuum fluctuations in homodyne detection

    Quantum locking using optical spring and homodyne detection has been devised to reduce quantum noise that limits the sensitivity of DECIGO, a space-based gravitational wave antenna in the frequency band around 0.1 Hz for detection of primordial gravitational waves. The reduction in the upper limit of energy density ΩGW{\Omega}_{\mathrm{GW}} from 2×10152{\times}10^{-15} to 1×10161{\times}10^{-16}, as inferred from recent observations, necessitates improved sensitivity in DECIGO to meet its primary science goals. To accurately evaluate the effectiveness of this method, this paper considers a detection mechanism that takes into account the influence of vacuum fluctuations on homodyne detection. In addition, an advanced signal processing method is devised to efficiently utilize signals from each photodetector, and design parameters for this configuration are optimized for the quantum noise. Our results show that this method is effective in reducing quantum noise, despite the detrimental impact of vacuum fluctuations on its sensitivity.Comment: 12 pages, 5 figure
