    The Indiana University Student Outreach Clinic as a Substrate for Interprofessional Education: A Physical Therapy Perspective

    poster abstractPurpose The World Health Organization defines Inter-Professional Education (IPE) as “two or more professions learning about, from and with each other to enable effective collaboration &improve health outcomes.” IPE is being stressed in education to prepare students for Inter-Professional Care (IPC). IPC occurs when multiple health workers from different professional backgrounds provide comprehensive health services. Our purpose is to discuss the Indiana University Student Outreach Clinic (IU-SOC) as an exemplar in IPE/IPC in preparing PT students for clinical practice. Description The IU-SOC has implemented IPE/IPC by collaborating with partners from the schools of medicine, pharmacy, law, social work, dentistry, and most recently, physical therapy. Student leaders collaborate to provide comprehensive patient care. Different committees are in place to facilitate communication between disciplines. One PT student is designated in the IPE role to assist other partners in determining if a patient is appropriate for PT. This model has allowed PT to communicate reciprocally with medicine to ensure best patient care. Summary of Use IU-SOC is unique because it is a student-run pro bono clinic where teaching IPE/IPC is a primary purpose. Students benefit from this model by learning the importance of communication between disciplines early in their education to better prepare for IPC; making them more likely to utilize it during clinical practice. The community has increased access to services and is utilizing healthcare resources more efficiently, with less reliance on emergency services. Occupational Therapy is anticipated to join rehab services in the near future. Importance to Members IPC helps to sustain the health care system, improve outcomes, enhance organizational efficiency, and provide more comprehensive care. IPC is the future of health care; the foundation of which must be established in education. Multi-disciplinary, student-led outreach clinics, such as IU-SOC, can serve as an IPE/IPC substrate to prepare PT students for practice

    Insulin-Like Growth Factor-1 Induces Lipid Production in Human SEB-1 Sebocytes Via Sterol Response Element-Binding Protein-1

    An understanding of the molecular signaling involved in sebaceous gland lipid production is needed to develop therapeutic targets to improve acne. Treatment with methylisobutylxanthine, dexamethasone, and a high dose of insulin (MDI) has been shown to differentiate 3T3-L1 preadipocytes into adipocytes, a differentiation marked by an increase in lipid production. The present study has the following aims: (1) Since high doses of insulin, as found in MDI, will activate the IGF-1 receptor, we sought to determine if IGF-1 is capable of reproducing the lipogenic effect seen with MDI treatment, and (2) to determine if the sterol response element-binding protein-1 (SREBP-1) pathway mediates the increase in lipogenesis. Here we report that MDI increases lipogenesis and that this effect can be attributed wholly to the high-dose insulin in SEB-1 cells. Further, we show that a physiologically relevant dose of IGF-1 or high-dose (1ÎĽm) insulin induces an increase in SREBP-1 mRNA, protein, and total lipid production; while 100nm insulin induces lipogenesis yet the SREBP protein levels remain unchanged. These data indicate that activation of the IGF-1 receptor increases lipogenesis in SEB-1 cells through both SREBP-dependent and SREBP-independent pathways

    Pelvic pain symptoms and endometriosis characteristics in relation to oxidative stress among adolescents and adults with and without surgically-confirmed endometriosis [version 1; peer review: 2 approved]

    Background: While the majority of reproductive-aged females will experience pelvic pain during their lives, biological mechanisms underlying pelvic pain are not well understood. We investigated associations between pelvic pain symptoms and oxidative stress among people with and without surgically-confirmed endometriosis. Methods: Using an enzyme-linked immunosorbent assay, we measured 8-Hydroxy-2’-deoxyguanosine (8-OHdG) in urine samples and corrected for creatinine levels in 434 surgically-confirmed endometriosis participants compared to 605 participants never diagnosed with endometriosis. At enrollment, participants reported details of their pelvic pain symptoms. Linear regression was used to compute geometric mean (GM) creatinine-corrected 8-OHdG levels with 95% confidence intervals (CI) among all participants and those with and without endometriosis separately, adjusting for potential confounders. Interactions by surgically-confirmed endometriosis status were tested by Wald statistics. Results: No trends in 8-OHdG were observed among those with or without endometriosis for severity or frequency of dysmenorrhea, acyclic pelvic pain, dyspareunia or pain with bowel movements. Among endometriosis participants, lower 8-OHdG levels were observed for participants with any white, blue/black, or brown lesions (GM=76.7 versus 82.9 ng/mg; p=0.10), which was primarily driven by lower levels of 8-OHdG for any blue/black lesions (GM=72.8 versus 81.6 ng/mg; p=0.05). Conclusion: While no associations were observed between 8-OHdG and pelvic pain symptoms, future research is needed to assess how other pathways of oxidative damage, e.g. through proteins or lipids, may affect endometriosis-associated symptoms. Additionally, further research is needed to understand differences in oxidative stress among endometriosis lesion sub-phenotypes

A commercial oxidizing disinfectant (potassium peroxomonosulphate 50%, sodium alkyl benzene sulphonate 15%, and sulphamic acid 5%) was introduced throughout the intensive care unit, and its use coincided with cessation of the outbreak
