    A Case Report of Multifocal Epithelial Hyperplasia (Heck's Disease) Treated with CO2 Laser

    Focal epithelial hyperplasia or Heck’s disease is a rare benign lesion of oral mucosa caused by human papilloma virus especially subtypes 13 and 32. The disease is predominantly found in children and adolescents with indigenous heritage. This paper reports a unique case of Heck's disease in an 11-year-old Iranian girl with multiple, bilateral and elevated lesions on the buccal and labial mucosa. The lesions had persisted for a year. The disease was diagnosed as focal epithelial hyperplasia through the clinical examination. Considering esthetic and functional problems, CO2 laser (Wavelength: 10600 nm) surgery was determined as a suitable choice of treatment. The patient was followed up over 1 year with no signs of lesion recurrence

    Priorities of Medical Education Research at Mashhad University of Medical Sciences based on the model of World Health Organization-Council on Health Research for Development (COHRED)

    Background: Due to the variety and extent of health fields and the limitation of resources, as well as the inapplicability of many research activities, it is impossible to work in all research fields to respond to all needs of the society. The aim of this study was to determine the priorities of educational research at Mashhad University of Medical Sciences.Method: This cross-sectional study was carried out at Mashhad University of Medical Sciences. The process for determining research priorities was based on five principles including stakeholder participation, analysis of the existing situation and assessment of needs, specifying research topics, scoring based on criteria, and determining priorities based on maximum agreement. The priorities were based on the four domains of the COHRED model.Results: A total of 39 research topics were obtained; after scoring and analysis, 15 educational research topics were introduced as priority. The first priority was teaching clinical skills (44.95), the second and third research priorities were clinical reasoning skills (44.65) and professional competence of students (44.55).Conclusion: It is hoped that the identifying of educational research priorities, in addition to guiding the research proposals towards the real needs, attract the attention of policymakers, reviewers, and approvers of educational research projects. In this case, it can be used as a tool for the optimal use of limited financial resources for applicable projects

    Comparing the Level of knowledge and Satisfaction of Students from Conventional in-Person Education and In-Person Education based on Learning Aids in the Clinical Applications of Lasers in Dentistry Course

    Background: In this study, the effect of conventional in-person education and in-person education based on videos and posters on the level of knowledge and satisfaction of students was compared in the course of clinical applications of laser in dentistry.Materials & Methods: In this comparative applied study, 66 dental students were divided into two groups to take the course on clinical applications of lasers. The first group underwent conventional in-person education, and the second group underwent in-person education based on learning aids. The data collection tool was a researcher-made questionnaire that the students completed after completing the course and included 15 questions in two fields, including knowledge level (9 questions) and satisfaction level (6 questions). The data were analyzed with SPSS software (version 24).  Independent sample T-test and chi-square test were used for statistical analysis. Significance level was set at 0.05. Results: The mean and standard deviation of the knowledge and satisfaction scores of the students who completed the course through conventional in-person education were 24.06 ± 6.8 (out of 36 scores) and 17.09 ± 5.15 (out of 24 scores), respectively. The mean and standard deviation of the knowledge and satisfaction scores of the students who had taken the in-person education based on learning aids were 24.56 ± 7.05 and 17.73 ± 5.1, respectively. Statistical analysis did not show a significant difference between the two study methods (P<0.05).Conclusion: Teaching the course of the clinical applications of lasers was not significantly different between in-person education based on learning aids and conventional in-person education concerning the students' level of knowledge and satisfaction

    Lip Leishmaniasis with Oral-mucosal Involvement: A Case Report

    Statement of the problem: Nowadays leishmaniasis is a common infectious disease around the world which is caused by a protozoan parasite named Leishmania. This parasite is transmitted to human by an infected female sand fly. This disease has three clinical forms; mucosal forms are rare in Iran. Purpose: The purpose of this study was to report a case of leishmaniasis with lip and oral mucosal involvement that healed successfully. Results: This article is about patients with lip leishmaniasis with oral-mucosal involvement, clinical features were diffuse lip swelling and ulcer in right lower lip vermilion and buccal mucosa. A smear was caught which proved the diagnosis of leishmaniasis. Thereafter, the patients were referred to a dermatologist for treatment and follow up showed complete healing of the lesions. Conclusion: Oral mucosal leishmaniasis is rare. However, the swelling may cause different complications like loss of teeth or respiratory obstruction. It should be noted that time is a key point in treatment.  So, it is necessary for us to train the differential diagnosis of chronic ulcerated oral lesions to our dentists

    Controlling gag reflex whit low power laser acupuncture for tooth restoration: A case report

    Introduction: Gag reflex is common during dental procedure such as, taking radiography, oral examination and alginate impression. This reflex has negative impact on the quality of any dental procedure. Acupuncture is traditional alternative medicine by inserting needle in special point in body. Because of cross-infection and other risks; studies have suggested the laser acupuncture that use low level laser (LLLT) or photobiomodulation instead of needle.  Various anti-gagging points have been identified; include Neiguan Pericardium 6 (PC6), large intestine point or He Gu point (LI4), Conception Vessel 24 (CV24) and Shenmen, Stomach and Throat points on ears and P6 point is a one of the best point to control nausea and vomiting and is located at medial of the forearm below the palm. The purpose of this study was to report controlling gag reflex during routine dental procedure whit stimulation P6 acupuncture point with low power laser. A patient who has sever gag reflex and for this reason this reason her oral hygiene level is awful, also clinical examination showed multiple caries. The gag reflex was controlled with stimulation P6 point on her wrist whit 6 J/cm2 energy density and 980 nm wavelengths. Gag reflex was successfully controlled. Conclusion: LLLT acupuncture needs less time, low invasive and much safer than other methods. Because of point P6 location it seems this point is more suitable than other anti-gaging points. Dentist who tried LLLT acupuncture must be trained and also adequate equipment should be in their dental office

Heck’s disease is more prevalent among children or in a family. Despite the self-limitation of lesions, they may progress or cause esthetical and functional discomforts. Hence, early diagnosis is necessary.Case Presentation: A middle-aged man with peripheral multiple soft, elevated, sessile, smooth, lobulated surface and nodular lesions in his right buccal mucosa was referred. The dimensions of the lesions varied from 5 to 25 mm. Through clinical and histopathological examinations, Heck’s disease was diagnosed. The diode laser (wavelengths of 980 nm), 1-W power using continuous waves) was applied serially (in 2 sessions with a 1-week interval) for surgically removing the lesions. The patient was followed up for 6 months with no lesion recurrence, and gradual healing was observed.Conclusion: The laser is an effective treatment with no scar remaining after the procedure and brings satisfaction to patients.     Prevalence of chronic complications of sulfur mustard on the oral soft tissues Zahedan veterans

    Background: Chemical gas, especially sulfate mustard (SM) was used against Iranian forces repeatedly during the Iraq war. Gas effects on various tissues within the body depend on amount and duration of exposure. According to the prevalence of oral lesions, this study was conducted in Zahedan city on the chemical warfare. Material and Methods: This descriptive cross-sectional study was performed on 70 patients. After obtaining the patient's consent form, a demographic questionnaire was completed by patients and the patients were examined. If there was soft tissue lesions, the cases were examined more closely. All data were collected on special forms and statistical analysis was performed by the SPSS computer program. Results: Among the 82 patients, 52 patients were with oral lesions and 18 patients were with healthy oral mucosa. The most common oral lesions seen in the study were pigmentation (52.8 %), ulcer (42.8%) and bleeding and gingival enlargement (34.2 %) and white lesions [picking (28.5 %) and Peel off (22.8 %)], respectively. Conclusion: The most common lesion sites were in the gums and tongue. Many patients referring were not informed of the lesions in their mouth and this problem emphasizes that the periodic oral examinations are required by specialists

    Surgical Treatment of Pyogenic Granuloma by 980 Diode Laser

    Introduction: Pyogenic granuloma is a pseudo-tumor which has a great tendency to attached gingiva. Conservative surgery is a treatment choice for this lesion. Among different laser wavelengths used for oral surgery including CO2, Nd:YAG, Erbium family and diode laser, diode can be the best option due to providing blood less area that make better view for surgeon to remove it completely. Case Report: A 14-year-old girl with a chief complaint of exophytic lesion in lower jaw was referred. The surgery was done by 980 diode laser with output power of 1 W in continuous mode. The lesion was sent to pathology laboratory for further investigation. The patient was advised to keep oral hygiene instructions. Results: After 1 weeks, complete healing was observed.Conclusion: Diode laser can be a treatment choice for this kind of lesions due to diminished infection and bacteremia following surgery, reduction of relapse and bone degeneration, less invasive technique and reduced chair time
