Exploring autistic-like traits relating to empathic attitude and psychological distress in hospital pharmacists
BACKGROUND:
Pharmacists are expected to play a key role in modern cancer care. Research suggests that an empathic approach and attitude in medical staff improves the quality of patient care. An empathic attitude and psychological distress are thought to be associated with autistic-like traits, but little is known about such traits.
OBJECTIVE:
In this study, we aimed to clarify the associations among autistic-like traits, empathic attitude in a medical context, and psychological health in hospital pharmacists.
SETTING:
Eligibility criteria for inclusion were certified pharmacists working at hospitals for patient care who returned their questionnaires.
METHOD:
Eight hundred and twenty-three hospital pharmacists completed a number of self-administered questionnaires anonymously by mail.
MAIN OUTCOME MEASURES:
Scores were obtained on the Autism-Spectrum Quotient, the Jefferson Scale of Empathy, the General Health Questionnaire-12, and subscales of the Interpersonal Reactivity Index (Perspective Taking, IRI-Empathic Concern, IRIPersonal Distress). We performed correlation and mediation analyses to confirm that the empathy and general health questionnaires were associated with autism-spectrum quotient scores, and with each IRI subscale.
RESULTS:
Complete responses were obtained from 379 pharmacists comprising 151 males (39.8 %) with a mean age of 37.7 ± 10.8 years (missing data, n = 13) and a median of 11 years after qualification as a pharmacist. Autism-Spectrum Quotient scores were inversely correlated with empathy (r = -0.22, p < 0.001) and positively correlated with general health scores (r = 0.40, p < 0.001). In the models with mediation, the inverse correlation between autism-spectrum quotient and empathy scores was mediated indirectly by IRI-Perspective Taking and IRI-Empathic Concern, and the positive correlation between autism-spectrum quotient and general health was mediated indirectly by IRI-Personal Distress. There were also direct effects, with significant effects of autism-spectrum quotient on empathy and general health scores.
CONCLUSION:
Our findings suggest that autistic-like traits affect both empathic attitude in a medical context and the psychological health of pharmacists. We recommend that to improve empathy in those with high levels of autistic-like traits, we may need to develop specialized interventions, such as improving communication skills training
Lipocalin 2, synthesized using a cell-free protein synthesis system and encapsulated into liposomes, inhibits the adhesion of Porphyromonas gingivalis to human oral epithelial cells
Background and objective: Lipocalin 2 (LCN2), a glycoprotein expressed in epithelial cells and leukocytes, has an antibacterial effect and plays a role in innate immunity. The delivery of LCN2 encapsulated in liposomes to oral epithelium may be useful to prevent oral infectious diseases. This study aimed to investigate the inhibitory effect of LCN2, artificially synthesized using a cell-free protein synthesis (CFPS) system, on the adhesion of Porphyromonas gingivalis to oral epithelial cells in order to approach oral healthcare using LCN2.
Methods: LCN 2 was synthesized using a CFPS system and assayed by Western blotting, mass spectrometry and enzyme-linked immunosorbent assay (ELISA). The bilayer liposomes were prepared by the spontaneous transfer method using 1,2-dioleoyl-sn-glycero-3 phosphocholine (DOPC), 3-sn-phosphatidylcholine from Egg Yolk (Egg-PC), and 1,2-dioleoyl-sn-glycero-3 phosphoethanolamine (DOPE). The cellular and medium fractions derived from the culture of oral epithelial cells with liposome-encapsulated LCN2 were assayed by Western blotting and ELISA. The effect of the synthesized LCN2 on adhesion of the labeled P. gingivalis to oral epithelial cells was investigated as an evaluation of its antibacterial activity.
Results: The synthesized LCN2 protein was identified by Western blotting; its amino acid sequence was similar to that of recombinant LCN2 protein. The additions of DOPE and octa-arginine in the outer lipid-layer components of liposome significantly increased the delivery of liposomes to epithelial cells.When oral epithelial cells were cultured with the synthesized and liposome-encapsulated LCN2, LCN2 was identified in the cellular and medium fractions by Western blotting and its concentration in the cellular fraction from the culture with the synthesized LCN2 was significantly higher than that of a template DNA-free protein. The synthesized LCN2 and liposome-encapsulated LCN2 significantly inhibited the adhesion of P. gingivalis to oral epithelial cells compared with template DNA-free protein.
Conclusion: LCN2 was artificially synthesized by a CFPS system, encapsulated in liposomes and delivered to oral epithelial cells, and demonstrated an antibacterial action against P. gingivalis. This approach may become a useful model for oral healthcare
DNA Lesions Induced by Replication Stress Trigger Mitotic Aberration and Tetraploidy Development
During tumorigenesis, cells acquire immortality in association with the
development of genomic instability. However, it is still elusive how genomic
instability spontaneously generates during the process of tumorigenesis. Here,
we show that precancerous DNA lesions induced by oncogene acceleration, which
induce situations identical to the initial stages of cancer development, trigger
tetraploidy/aneuploidy generation in association with mitotic aberration.
Although oncogene acceleration primarily induces DNA replication stress and the
resulting lesions in the S phase, these lesions are carried over into the M
phase and cause cytokinesis failure and genomic instability. Unlike directly
induced DNA double-strand breaks, DNA replication stress-associated lesions are
cryptogenic and pass through cell-cycle checkpoints due to limited and
ineffective activation of checkpoint factors. Furthermore, since damaged M-phase
cells still progress in mitotic steps, these cells result in chromosomal
mis-segregation, cytokinesis failure and the resulting tetraploidy generation.
Thus, our results reveal a process of genomic instability generation triggered
by precancerous DNA replication stress
Validity and Reliability of the Japanese Version of the 12-item Self-administered World Health Organization Disability Assessment Schedule (WHODAS) 2.0 in Patients with Schizophrenia
It is necessary to assess functional impairment when treating schizophrenia. The World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0) has been adopted as a measure of functional disability in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition. This study was a secondary analysis from a cross-sectional study of health-related behaviors among patients with schizophrenia. We examined the validity and reliability of the Japanese version of the 12-item WHODAS 2.0 when self-administered by such patients. Participants were 350 outpatients with schizophrenia from a psychiatric hospital. The standard six-factor structure of the WHODAS 2.0 showed a good fit for these participants. The Cronbach’s alpha coefficient was 0.858, showing good internal consistency. The WHODAS 2.0 showed moderate correlations with the modified Global Assessment of Functioning and Kessler 6 scales (r=−0.434 and 0.555, respectively). The results of this study show that the Japanese version of the 12-item self-administered WHODAS 2.0 has good internal consistency and convergent validity among patients with schizophrenia. Further exploration of the usefulness of WHODAS 2.0 in clinical settings is needed
An Open-Label Feasibility Trial of Repetitive Transcranial Magnetic Stimulation for Treatment-Resistant Major Depressive Episodes
Repetitive transcranial magnetic stimulation (rTMS) has been reported to be a new treatment option for treatment-resistant depression. In Japan, there has been limited research into its feasibility, efficacy, and tolerability. We have launched a trial of rTMS for treating medication-resistant major depressive disorder and bipolar depression. We are investigating low-frequency rTMS to the right dorsolateral prefrontal cortex and traditional high-frequency rTMS to the left dorsolateral prefrontal cortex, in 20 patients. The primary outcome of the study is the treatment completion rate. This study will provide new data on the usefulness of rTMS for treatment-resistant depression in Japan
Strategic use of new generation antidepressants for depression: SUN(^_^) D protocol update and statistical analysis plan
Background: SUN(^_^)D, the Strategic Use of New generation antidepressants for Depression, is an assessor-blinded, parallel-group, multicenter pragmatic mega-trial to examine the optimum treatment strategy for the first- and second-line treatments for unipolar major depressive episodes. The trial has three steps and two randomizations. Step I randomization compares the minimum and the maximum dosing strategy for the first-line antidepressant. Step II randomization compares the continuation, augmentation or switching strategy for the second-line antidepressant treatment. Step III is a naturalistic continuation phase. The original protocol was published in 2011, and we hereby report its updated protocol including the statistical analysis plan. Results: We implemented two important changes to the original protocol. One is about the required sample size, reflecting the smaller number of dropouts than had been expected. Another is in the organization of the primary and secondary outcomes in order to make the report of the main trial results as pertinent and interpretable as possible for clinical practices. Due to the complexity of the trial, we plan to report the main results in two separate reports, and this updated protocol and the statistical analysis plan have laid out respective primary and secondary outcomes and their analyses. We will convene the blind interpretation committee before the randomization code is broken. Conclusion: This paper presents the updated protocol and the detailed statistical analysis plan for the SUN(^_^)D trial in order to avoid reporting bias and data-driven results. Patients' acceptability and implementation outcomes of a case management approach to encourage participation in colorectal cancer screening for people with schizophrenia: a qualitative secondary analysis of a mixed-method randomised clinical trial
Objectives We examined the efficacy of case management (CM) interventions to encourage participation in colorectal cancer screening for patients with schizophrenia. This study aimed to clarify patients' acceptability of the intervention and the helpful components of the intervention. Simultaneously, the study aimed to determine the acceptability, appropriateness and feasibility of the intervention from the perspective of psychiatric care providers. Study design and setting This study was a secondary qualitative analysis of a mixed-method randomised controlled trial that evaluated the efficacy of the CM approach to encourage participation in cancer screening for people with schizophrenia. The intervention comprised education and patient navigation for colorectal cancer screening. Interviews were conducted with patients who received the intervention and staff from two psychiatric hospitals in Japan who delivered the intervention. Participants Of the 172 patients with schizophrenia who participated in the trial, 153 were included. In addition, three out of six providers were included. Data collection and analysis Using a structured interview, the case manager asked participants about patient acceptability and the helpful components of the intervention. Content analysis was conducted for the responses obtained, and the number of responses was tabulated by two researchers. For the interviews with the providers, opinions obtained from verbatim transcripts were extracted and summarised. Results Forty-three of the 56 patients perceived that the intervention was acceptable. For the intervention component, inperson counselling with an explanation of the screening process by psychiatric care providers was most frequently reported by the patients as helpful (48 of the 68 respondents). Psychiatric care providers evaluated the intervention as acceptable, appropriate and easy to understand and administer. However, providing the intervention to all patients simultaneously was considered difficult with the current human resources. Conclusions This study showed that the CM intervention was perceived as acceptable by patients and acceptable and appropriate by psychiatric care providers
ヒノキ ショクリンチ ニ オケル ヒナカマキリ ノ サンラン バショ ト セイカツシ
静岡県伊豆の国市内のヒノキ植林地に多産するヒナカマキリの産卵場所と生活史を調査した。卵鞘はヒノキ幹上で観察され,地表からの高さは平均値で66.2 cm,全体の90%がめくれたヒノキ樹皮内側に産下されていた。調査地は北西向きの斜面林であり,卵鞘のある方位を調べたところ,斜面下部側(北,北西,西)に数多く産下されていた。斜面上部側に比べ下部側はわずかながら温度,湿度が低く,めくれた樹皮が多かった。野外での幼虫や成虫の主な発見場所は,地表のほかヒノキの根元近くの落葉下や樹皮下,幹上であり,いずれも地表から50 cmまでの高さであった。同じ試験地で幼虫と成虫の季節的発生消長を調査し,併せて室内で幼虫を飼育し生活史を推定した。野外調査と室内飼育の結果から,このヒノキ集団は,卵鞘内で越冬してから主に6月に孵化し,幼虫期を経て9月以降に成虫となり,産卵後年内に死亡するものと思われた。To evaluate the egg-deposition site, oothecae of the praying mantis Amantis nawai were observed in Hinoki cypress plantation in Izunokuni City. Height, direction (compass bearing), microhabitat where oothecae were located were measured. Oothecae were found on trees 66.2 cm above ground in average. More oothecae faced the lower side (north side) under partly separated bark on tree trunks in sloping woodland. The north side showed low temperature and humidity and has more separated barks compared with the opposite side of trunks. In addition, seasonal occurrence of this insect was studied in the field and the laboratory under temperature conditions that were similar to those observed in the natural habitat. The immature larvae hatched during late May to June, and they reached the adult stage during September and October. These insects mainly laid eggs after late September
Orbital Physics in the Perovskite Ti Oxides
In the perovskite Ti oxide RTiO3 (R=rare-earth ions), the Ti t2g orbitals and
spins in the 3d^1 state couple each other through the strong electron
correlations, resulting in a rich variety of orbital-spin phases. The origin
and nature of orbital-spin states of these Mott insulators have been
intensively studied. In this article, we review the studies on orbital physics
in the perovskite titanates. We focus on the following three topics: (1) the
origin and nature of the ferromagnetism as well as the orbital ordering in the
compounds with relatively small R ions such as GdTiO3 and YTiO3, (2) the origin
of the G-type antiferromagnetism and the orbital state in LaTiO3, and (3) the
orbital-spin structures in other AFM(G) compounds with relatively large R ions
(R=Ce, Pr, Nd and Sm). On the basis of these discussions, we discuss the whole
phase diagram together with mechanisms of the magnetic phase transition. We
also show that the Ti t2g degeneracy is inherently lifted in the titanates,
which allows the single-band descriptions of the ground-state and low-energy
electronic structures as a good starting point. Our analyses indicate that
these compounds offer touchstone materials described by the single-band Hubbard
model on the cubic lattice. From this insight, we also reanalyze the hole-doped
titanates. Experimentally revealed filling-dependent and bandwidth-dependent
properties and the critical behavior of the metal-insulator transitions are
