Transcatheter Arterial Chemoembolization of Hepatocellular Carcinoma: Prevalence and Causative Factors of Extrahepatic Collateral Arteries in 479 Patients
OBJECTIVE: We wanted to investigate the prevalence and causative factors of extrahepatic arterial blood supply to hepatocellular carcinoma (HCC) at its initial presentation and during chemoembolization. MATERIALS AND METHODS: Between February 1998 and April 2000, consecutive 479 patients with newly diagnosed HCC were prospectively enrolled into this study. A total of 1629 sessions of transcatheter arterial chemoembolization (TACE) were performed in these patients (range: 1-15 sessions; mean: 3.4 sessions) until April 2004. For each TACE procedure, we determined the potential extrahepatic collateral arteries (ExCAs) depending on the location of the tumor, and we performed selective angiography of all suspected collaterals that could supply the tumor. The prevalence of ExCAs and the causative factors were analyzed. RESULTS: At initial presentation, 82 (17%) of these 479 patients showed 108 ExCAs supplying tumors. Univariate analysis showed that tumor size (p or =5 cm) was significantly higher than that for those patients with a small tumor (< 5 cm) (p < 0.01). CONCLUSION: The presence of ExCAs supplying HCC is rather common, and the tumor size is a significant causative factor for the development of these collateral arteries.This study was supported by a grant

A comparison of regression models for the ice loads measured during the ice tank test
To evaluate the time-domain positioning performance of arctic marine structures, it is necessary to generate an ice load appropriate for the current position and heading of the structure. The position and orientation angle of a floating body continuously change with time. Therefore, an ice load is required for any attitude in the time-domain simulation. In this study, we present a fundamental technique for analyzing ice loads in the frequency domain based on data measured at various angles in the ice-water tank experiment. We perform spectral analysis instead of general FFT to analyze the ice load, which has the characteristics of a random signal. To generate the necessary ice load in the time domain, we must first interpolate the measured data in the frequency domain. Using the Blackman-Tukey method, we estimate the spectrum for the measured data, then process the data to generate the training set required for machine learning. Based on the results, we perform regression analysis by applying four representative techniques, including linear regression, random forest, or neural network, and compare the results with MSE. Substance P and beta-endorphin mediate electro-acupuncture induced analgesia in mouse cancer pain model
<p>Abstract</p> <p>Background</p> <p>Opioid analgesics are generally used to combat the pain associated with cancerous conditions. These agents not only inhibit respiratory function and cause constipation, but also induce other significant side effects such as addiction and tolerance, all of which further contribute to a reduced quality of life for cancer patients. Thus, in the present study, the effects of electro-acupuncture treatment (EA) on mechanical allodynia were examined in a cancer pain mouse model.</p> <p>Methods</p> <p>In order to produce a neuropathic cancer pain model, S-180 sarcoma cells were inoculated around the sciatic nerve of left legs of Balb/c mice. Magnetic Resonance Imaging (MRI) scanning confirmed the mass of S-180 cancer cells embedded around the sciatic nerve. Mechanical allodynia was most consistently induced in the mouse sarcoma cell line S-180 (2 × 10<sup>6</sup>sarcoma cells)-treated group compared to all the other groups studied. EA stimulation (2 Hz) was administered daily to ST36 (Zusanli) of S-180 bearing mice for 30 min for 9 days after S-180 inoculation.</p> <p>Results</p> <p>EA treatment significantly prolonged paw withdrawal latency from 5 days after inoculation. It also shortened the cumulative lifting duration from 7 days after inoculation, compared to the tumor control. Also, the overexpression of pain peptide substance P in the dorsal horn of the spinal cord was significantly decreased in the EA-treated group compared to the tumor control on Day 9 post inoculation. Furthermore, EA treatment effectively increased the concentration of β-endorphin in blood and brain samples of the mice to a greater extent than that of the tumor control as well as the normal group. The concentration of β-endorphin for EA treatment group increased by 51.457% in the blood and 12.6% in the brain respectively, compared to the tumor control group.</p> <p>Conclusion</p> <p>The findings of this study suggest that a S-180 cancer pain model is useful as a consistent and short time animal model. It also indicated that EA treatment could be used as an alternative therapeutic method for cancer pain due to a consequent decrease in substance P and increase in β-endorphin levels.</p
CHANGE IN MUSCLE ACITIVITY DURING SKIING USING 2 SIMULATORS
This research analyzed the change in lower muscle activity during skiing using two simulators. Participants in this study were 8 male adults who held a certificate issued by the Korea Ski Instructors Association. Kinematic factors when skiing on two types of simulators were compared and analyzed. Most subjects maintained a higher value of %MVIC while repeating the positions on the Pro Ski Simulator than Skiers Edge. Efficient skiing on ski-simulators requires the maximum use of skiing time on each foot and the ability to quickly flex and extend the hip and the knee joints. Furthermore, differences between individuals exist depending on the level of ski technique and the type of simulator and it is believed that the development of an additional adjustment apparatus in the existing equipment is necessary to enable safe and effective motions
COMPARISON STUDY OF EXPERIMENTS AND PREDICTIONS OF WAVE KINEMATICS FOR ROGUE WAVE
To investigate the wave kinematics under the rogue wave crest, a series of experiments were performed in 2-D wave tank with the application of PIV technique to measure the velocities under the free surface. Three different prediction methods of linear extrapolation, Wheeler stretching, and modified stretching were applied to estimate water wave kinematics and compared with PIV experimental results under the highest wave crest of irregular wave trains satisfying with rogue wave criteria. Also, the cut-off frequency dependence for three prediction methods was investigated with varying spectral peak frequencies to estimate wave kinematics including velocities and accelerations in horizontal and vertical directions. It was suggested that the cut-off frequency for the reasonable prediction of the wave kinematics under the rogue wave crest could be chosen three times of spectral peak wave frequency for the linear extrapolation and higher frequency than four times of spectral peak wave frequency for Wheeler stretching and modified stretching method
The use of an interdental brush mitigates periodontal health inequalities: the Korean National Health and nutrition examination survey (KNHANES)
Background
The purpose of this study was to evaluate the mitigating effect of the use of interdental brushes on periodontal health inequality.
Methods
This study was based on the data acquired in the Sixth Korea National Health and Nutrition Examination Survey (KNHANES VI; 2013–2015). A total of 17,583 participants (7,633 males and 9,950 females)) aged 19 years or older completed the KNHANES VI between 2013 and 2015. Multivariable logistic regression analysis was performed using socioeconomic characteristics (sex, age, level of education, individual income), personal health practice (smoking, toothbrushing, dental flossing, interdental brushing, dental clinic visiting), systematic medical factors (diabetes mellitus, hypercholesterolemia, hypertension, obesity) and the community periodontal index. We confirmed differences in the prevalence of periodontal disease with the use of an interdental brushes stratified according to individual income.
Results
Three logistic regression models adjusted for covariates hierarchically. In all models, individuals who used an interdental brush were not significantly different from individuals who did not use an interdental brush. The adjusted odds ratio (OR) for interdental brushing was 0.918 with a 95% confidence intervals (CIs) of 0.797–1.057. When periodontal disease was the outcome of the model, the lowest income group had 1.266 (95% CIs 1.066 to 1.502) times the odds of having periodontal disease than the highest income group. In interdental brush nonusers, the lowest income group had 1.276 (95% CI 1.061–1.533) times the odds of having periodontal diseases than the highest income group. However, in the interdental brush users, there were no significant differences in periodontal disease prevalence among income groups.
Conclusions

Obesity and epithelial ovarian cancer survival: a systematic review and meta-analysis
BACKGROUND: Studies on the association between obesity and ovarian cancer survival have had conflicting results. We reviewed and quantitatively summarized the existing evidence, exploring potentially important sources of variability, such as the timing of body mass index (BMI) assessment, BMI cut points, references used in multivariate analysis, and ovarian cancer stage. METHODS: Eligible studies were searched using MEDLINE (PubMed), EMBASE, and Cochrane Central Register of Controlled Trials, relevant bibliographies were manually reviewed for additional studies. Adjusted hazard ratios (HRs) from individual studies were pooled using a random effects model. RESULTS: 17 cohort studies of 929 screened articles were included in the final analysis. Obesity in early adulthood and obesity 5 years before ovarian cancer diagnosis were associated with poor patient survival (early adulthood: pooled HR 1.67; 95% CI 1.29-2.16; 5 years prediagnosis: pooled HR 1.35; 95% CI 1.03-1.76). However, the results for obesity at diagnosis depended on whether BMI was analyzed as a categorical or continuous variable. Analysis of obesity with BMI as a categorical variable did not affect ovarian cancer prognosis (pooled HR 1.07; 95% CI 0.95-1.21); obesity with BMI as a continuous variable showed slightly poorer survival with each incremental increase in BMI (pooled HR 1.02; 95% CI 1.01-1.04). CONCLUSIONS: Obesity 5 years before ovarian cancer diagnosis and obesity at a young age were associated with poor prognosis. The association between obesity at diagnosis and survival of ovarian cancer patients still remains equivocal. BMI at diagnosis cannot be a prognostic factor for the survival of ovarian cancer patients. Further well-designed studies are needed to elucidate the variety effect of obesity on the survival of ovarian cancer patients
WoLF: Wide-scope Large Language Model Framework for CXR Understanding
Significant methodological strides have been made toward Chest X-ray (CXR)
understanding via modern vision-language models (VLMs), demonstrating
impressive Visual Question Answering (VQA) and CXR report generation abilities.
However, existing CXR understanding frameworks still possess several procedural
caveats. (1) Previous methods solely use CXR reports, which are insufficient
for comprehensive Visual Question Answering (VQA), especially when additional
health-related data like medication history and prior diagnoses are needed. (2)
Previous methods use raw CXR reports, which are often arbitrarily structured.
While modern language models can understand various text formats, restructuring
reports for clearer, organized anatomy-based information could enhance their
usefulness. (3) Current evaluation methods for CXR-VQA primarily emphasize
linguistic correctness, lacking the capability to offer nuanced assessments of
the generated answers. In this work, to address the aforementioned caveats, we
introduce WoLF, a Wide-scope Large Language Model Framework for CXR
understanding. To resolve (1), we capture multi-faceted records of patients,
which are utilized for accurate diagnoses in real-world clinical scenarios.
Specifically, we adopt the Electronic Health Records (EHR) to generate
instruction-following data suited for CXR understanding. Regarding (2), we
enhance report generation performance by decoupling knowledge in CXR reports
based on anatomical structure even within the attention step via masked
attention. To address (3), we introduce an AI-evaluation protocol optimized for
assessing the capabilities of LLM. Through extensive experimental validations,
WoLF demonstrates superior performance over other models on MIMIC-CXR in the
AI-evaluation arena about VQA (up to +9.47%p mean score) and by metrics about
