    Severe Ocular Surface abnormalities in a child and Ectodermal Dysplasia: A Case Report.

    Ectodermal Dysplasia is a disorder that occurs due to abnormal development of at least two major ectodermal derivatives in the developing embryo. Author report the case of a 10 year old male child who was referred to our department with complaints of absent sweating, foreign body sensation and watering in both eyes for past few months. The family history could be traced to four generations and there was an observed trend of increase in severity of signs and symptoms occurring at younger age.  The purpose of this case report is to create awareness in the Ophthalmic community about the diagnosis and clinical manifestations of the disorder. This case highlights the role of multidisciplinary approach for management of systemic disease, genetic evolution of affected individual and carriers and genetic counseling

    RUFY1 binds Arl8b and mediates endosome-to-TGN CI-M6PR retrieval for cargo sorting to lysosomes

    Arl8b, an Arf-like GTP-binding protein, regulates cargo trafficking and positioning of lysosomes. However, it is unknown whether Arl8b regulates lysosomal cargo sorting. Here, we report that Arl8b binds to the Rab4 and Rab14 interaction partner, RUN and FYVE domain-containing protein (RUFY) 1, a known regulator of cargo sorting from recycling endosomes. Arl8b determines RUFY1 endosomal localization through regulating its interaction with Rab14. RUFY1 depletion led to a delay in CI-M6PR retrieval from endosomes to the TGN, resulting in impaired delivery of newly synthesized hydrolases to lysosomes. We identified the dynein-dynactin complex as an RUFY1 interaction partner, and similar to a subset of activating dynein adaptors, the coiled-coil region of RUFY1 was required for interaction with dynein and the ability to mediate dynein-dependent organelle clustering. Our findings suggest that Arl8b and RUFY1 play a novel role on recycling endosomes, from where this machinery regulates endosomes to TGN retrieval of CI-M6PR and, consequently, lysosomal cargo sorting

    Measuring patient-perceived quality of care in US hospitals using Twitter

    ABSTRACT Background Patients routinely use Twitter to share feedback about their experience receiving healthcare. Identifying and analysing the content of posts sent to hospitals may provide a novel real-time measure of quality, supplementing traditional, survey-based approaches. Objective To assess the use of Twitter as a supplemental data stream for measuring patientperceived quality of care in US hospitals and compare patient sentiments about hospitals with established quality measures. Design 404 065 tweets directed to 2349 US hospitals over a 1-year period were classified as having to do with patient experience using a machine learning approach. Sentiment was calculated for these tweets using natural language processing. 11 602 tweets were manually categorised into patient experience topics. Finally, hospitals with ≥50 patient experience tweets were surveyed to understand how they use Twitter to interact with patients. Key results Roughly half of the hospitals in the US have a presence on Twitter. Of the tweets directed toward these hospitals, 34 725 (9.4%) were related to patient experience and covered diverse topics. Analyses limited to hospitals with ≥50 patient experience tweets revealed that they were more active on Twitter, more likely to be below the national median of Medicare patients ( p<0.001) and above the national median for nurse/patient ratio ( p=0.006), and to be a nonprofit hospital ( p<0.001). After adjusting for hospital characteristics, we found that Twitter sentiment was not associated with Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) ratings (but having a Twitter account was), although there was a weak association with 30-day hospital readmission rates ( p=0.003).     Cultural Parameters for Man On the Loop Paradigm in the context of Intelligence, Surveillance, and Reconnaissance

    Social reasoning and norms of a group of individuals that share a set of cultural traits are largely fashioned by those traits. We explored a few predominant sociological, cultural traits and developed a methodology for parametrically adjusting them. This exploratory study shows promise toward a capability to deliberately tune cultural group traits in order to produce desired group behavior. In order to validate our methodology, we implemented a prototypical agent based simulated intelligence, surveillance, and reconnaissance scenario. In this simulation, a group of simulated agents traverse a hostile territory while a user adjusts cultural group trait values. The group and individual utilities are dynamically observed against values of selected cultural traits. Uncertainty avoidance index and individualism are the cultural traits we explored in depth. Upon user\u27s training of the correspondence between cultural values and system utilities, the user is able to deliberately produce desired system utilities by issuing changes to trait values. Appropriateness of specific cultural values is not universal but determined by the domain and scenarios. This work heralds a path for control of large systems via parametric cultural adjustments
