    Au cours des années 2002 et 2003, je me suis intéressé aux techniques d’analyses prospectives, en particulier celles développées par Michel Godet dans le cadre de sa chaire de prospective stratégique au CNAM. Cette étude fut l’occasion pour moi de réaliser un exercice de prospective du secteur humanitaire à l’horizon 2020. Sept ans plus tard, il m’a semblé intéressant de revenir sur le cadre de cette étude et, à la lumière de l’évolution de ces dernières années, d’en relire les conclusions et d’en tirer quelques conclusions pour éclairer les démarches dans lesquelles se sont aujourd’hui engagées de nombreuses organisations

    Funded by the Belgian Directorate-General for Development and led by Humanity & Inclusion (HI) under the auspices of the National Mine Action Centre, Haut Commissariat National au DĂ©minage (HCND) in Chad, the Odyssey2025 Project explores ways to accelerate land release for the local population with the combined use of small consumer drones, new survey methods, and mobile data collection. Project partners include Mobility Robotics, Dynergie, InZentive, and Third Element Aviation. A practical, field-driven approach is being undertaken together with partners in the PRODECO project, Mines Advisory Group (MAG), and Fondation Suisse de DĂ©minage (FSD)

    Proof: How Small Drones Can Find Buried Landmines in the Desert Using Airborne IR Thermography

    Hypotheses and speculation have circulated for at least three decades regarding how IR thermography could be viable as a technique for locating buried landmines in arid environments. However, there has been a lack of proof under actual field conditions. Addressing many overall questions regarding small drones in HMA, the Odyssey2025 Project—led by HI and Mobility Robotics— closely collaborated with the national mine action center in Chad, the Haut Commissariat National au Déminage (HCND), to complete activities and objectives. As part of a holistic approach, the primary objective was to determine how small drones could add value in HMA field operations and augment other assets at both simple and advanced perspectives. This research is from an advanced perspective, following on from the work previously published in The Journal of Conventional Weapons Destruction outlining how low-cost consumer drones can augment HMA operations at a simple level using visual imagery and GIS/cartography/photogrammetry. This article aims to advance the field’s understanding of drones and their ability to indicate the location of buried objects

    Impact of Low-Level-Viremia on HIV-1 Drug-Resistance Evolution among Antiretroviral Treated-Patients

    to determine the emergence and evolution of DRAM during LLV in HIV-1-infected patients while receiving antiretroviral therapy (ART).Retrospective analysis of patients presenting a LLV episode defined as pVL between 40 and 500 c/mL on at least 3 occasions during a 6-month period or longer while on the same ART. Resistance genotypic testing was performed at the onset and at the end of LLV period. Emerging DRAM was defined during LLV if never detected on baseline genotype or before.48 patients including 4 naive and 44 pretreated (median 9 years) presented a LLV episode with a median duration of 11 months. Current ART included 2NRTI (94%), ritonavir-boosted PI (94%), NNRTI (23%), and/or raltegravir (19%). Median pVL during LLV was 134 c/mL. Successful resistance testing at both onset and end of the LLV episode were obtained for 37 patients (77%), among who 11 (30%) acquired at least 1 DRAM during the LLV period: for NRTI in 6, for NNRTI in 1, for PI in 4, and for raltegravir in 2. During the LLV period, number of drugs with genotypic resistance increased from a median of 4.5 to 6 drugs. Duration and pVL level of LLV episode, duration of previous ART, current and nadir CD4 count, number of baseline DRAM and GSS were not identified as predictive factors of resistance acquisition during LLV, probably due to limited number of patients.Persistent LLV episodes below 500 c/ml while receiving ART is associated with emerging DRAM for all drug classes and a decreasing in further therapeutic options, suggesting to earlier consider resistance monitoring and ART optimization in this setting

    Self-Reactivities to the Non-Erythroid Alpha Spectrin Correlate with Cerebral Malaria in Gabonese Children

    BACKGROUND: Hypergammaglobulinemia and polyclonal B-cell activation commonly occur in Plasmodium sp. infections. Some of the antibodies produced recognize self-components and are correlated with disease severity in P. falciparum malaria. However, it is not known whether some self-reactive antibodies produced during P. falciparum infection contribute to the events leading to cerebral malaria (CM). We show here a correlation between self-antibody responses to a human brain protein and high levels of circulating TNF alpha (TNFα), with the manifestation of CM in Gabonese children. METHODOLOGY: To study the role of self-reactive antibodies associated to the development of P. falciparum cerebral malaria, we used a combination of quantitative immunoblotting and multivariate analysis to analyse correlation between the reactivity of circulating IgG with a human brain protein extract and TNFα concentrations in cohorts of uninfected controls (UI) and P. falciparum-infected Gabonese children developing uncomplicated malaria (UM), severe non-cerebral malaria (SNCM), or CM. RESULTS/CONCLUSION: The repertoire of brain antigens recognized by plasma IgGs was more diverse in infected than in UI individuals. Anti-brain reactivity was significantly higher in the CM group than in the UM and SNCM groups. IgG self-reactivity to brain antigens was also correlated with plasma IgG levels and age. We found that 90% of CM patients displayed reactivity to a high-molecular mass band containing the spectrin non-erythroid alpha chain. Reactivity with this band was correlated with high TNFα concentrations in CM patients. These results strongly suggest that an antibody response to brain antigens induced by P. falciparum infection may be associated with pathogenic mechanisms in patients developing CM

    Simple scoring system to predict in-hospital mortality after surgery for infective endocarditis

    BACKGROUND: Aspecific scoring systems are used to predict the risk of death postsurgery in patients with infective endocarditis (IE). The purpose of the present study was both to analyze the risk factors for in-hospital death, which complicates surgery for IE, and to create a mortality risk score based on the results of this analysis. METHODS AND RESULTS: Outcomes of 361 consecutive patients (mean age, 59.1\ub115.4 years) who had undergone surgery for IE in 8 European centers of cardiac surgery were recorded prospectively, and a risk factor analysis (multivariable logistic regression) for in-hospital death was performed. The discriminatory power of a new predictive scoring system was assessed with the receiver operating characteristic curve analysis. Score validation procedures were carried out. Fifty-six (15.5%) patients died postsurgery. BMI >27 kg/m2 (odds ratio [OR], 1.79; P=0.049), estimated glomerular filtration rate 55 mm Hg (OR, 1.78; P=0.032), and critical state (OR, 2.37; P=0.017) were independent predictors of in-hospital death. A scoring system was devised to predict in-hospital death postsurgery for IE (area under the receiver operating characteristic curve, 0.780; 95% CI, 0.734-0.822). The score performed better than 5 of 6 scoring systems for in-hospital death after cardiac surgery that were considered. CONCLUSIONS: A simple scoring system based on risk factors for in-hospital death was specifically created to predict mortality risk postsurgery in patients with IE
