A global multilayer cloud identification with POLDER/PARASOL
The detection of multilayer cloud situations is important for satellite retrieval algorithms and for many climate related applications. In this paper, we describe an algorithm based on the exploitation of the POLarization and Directionality of the Earth’s Reflectance (POLDER) observations to identify monolayered and multilayered cloudy situations along with a confidence index. Our reference comes from the synergy of the active instruments of the A-Train satellite constellation. The algorithm is based upon a decision tree that uses a metric from information theory and a series of tests on POLDER Level-2 products. We obtain a multilayer flag as the final result of a tree classification which takes discrete values between 0 and 100. Values closest to zero (resp. a hundred) indicate a higher confidence in the monolayer (resp. multilayer) character. This indicator can be used as it is, or with a threshold level that minimizes the risk of misclassification, as a binary index to distinguish between monolayer and multilayer clouds. For almost fully covered and optically thick enough cloud scenes, the risk of misclassification ranges from 29% to 34% over the period 2006–2010 and the average confidences in the estimated monolayer and multilayer characters of the cloud scenes are 74.0% and 58.2% respectively. With the binary distinction, POLDER provides a climatology of the mono/multi-layer cloud character that exhibits some interesting features. Improved information about the vertical location and extent of monolayer clouds from POLDER3 measurements in the oxygen A-band
This paper describes new advances in the exploitation of oxygen A-band measurements from POLDER3 sensor onboard PARASOL, satellite platform within the A-Train. These developments result from not only an account of the dependence of POLDER oxygen parameters to cloud optical thickness τ and to the scene's geometrical conditions but also, and more importantly, from the finer understanding of the sensitivity of these parameters to cloud vertical extent. This sensitivity is made possible thanks to the multidirectional character of POLDER measurements. In the case of monolayer clouds that represent most of cloudy conditions, new oxygen parameters are obtained and calibrated from POLDER3 data colocalized with the measurements of the two active sensors of the A-Train: CALIOP/CALIPSO and CPR/CloudSat. From a parameterization that is (μs, τ) dependent, with μs the cosine of the solar zenith angle, a cloud top oxygen pressure (CTOP) and a cloud middle oxygen pressure (CMOP) are obtained, which are estimates of actual cloud top and middle pressures (CTP and CMP). Performances of CTOP and CMOP are presented by class of clouds following the ISCCP classification. In 2008, the coefficient of the correlation between CMOP and CMP is 0.81 for cirrostratus, 0.79 for stratocumulus, 0.75 for deep convective clouds. The coefficient of the correlation between CTOP and CTP is 0.75, 0.73, and 0.79 for the same cloud types. The score obtained by CTOP, defined as the confidence in the retrieval for a particular range of inferred value and for a given error, is higher than the one of MODIS CTP estimate. Scores of CTOP are the highest for bin value of CTP superior in numbers. For liquid (ice) clouds and an error of 30 hPa (50 hPa), the score of CTOP reaches 50% (70%). From the difference between CTOP and CMOP, a first estimate of the cloud vertical extent h is possible. A second estimate of h comes from the correlation between the angular standard deviation of POLDER oxygen pressure σPO2 and the cloud vertical extent. This correlation is studied in detail in the case of liquid clouds. It is shown to be spatially and temporally robust, except for clouds above land during winter months. The analysis of the correlation's dependence on the scene's characteristics leads to a parameterization providing h from σPO2. For liquid water clouds above ocean in 2008, the mean difference between the actual cloud vertical extent and the one retrieved from σPO2 (from the pressure difference) is 5 m (−12 m). The standard deviation of the mean difference is close to 1000 m for the two methods. POLDER estimates of the cloud geometrical thickness obtain a global score of 50% confidence for a relative error of 20% (40%) of the estimate for ice (liquid) clouds over ocean. These results need to be validated outside of the CALIPSO/CloudSat track
Determinants of Acceptance of Cervical Cancer Screening in Dar es Salaam, Tanzania.
To describe how demographic characteristics and knowledge of cervical cancer influence screening acceptance among women living in Dar es Salaam, Tanzania. Multistage cluster sampling was carried out in 45 randomly selected streets in Dar es Salaam. Women between the ages of 25-59 who lived in the sampled streets were invited to a cervical cancer screening; 804 women accepted and 313 rejected the invitation. Information on demographic characteristics and knowledge of cervical cancer were obtained through structured questionnaire interviews. Women aged 35-44 and women aged 45-59 had increased ORs of 3.52 and 7.09, respectively, for accepting screening. Increased accepting rates were also found among single women (OR 2.43) and among women who had attended primary or secondary school (ORs of 1.81 and 1.94). Women who had 0-2 children were also more prone to accept screening in comparison with women who had five or more children (OR 3.21). Finally, knowledge of cervical cancer and awareness of the existing screening program were also associated with increased acceptance rates (ORs of 5.90 and 4.20). There are identifiable subgroups where cervical cancer screening can be increased in Dar es Salaam. Special attention should be paid to women of low education and women of high parity. In addition, knowledge and awareness raising campaigns that goes hand in hand with culturally acceptable screening services will likely lead to an increased uptake of cervical cancer screening

Cloud information content analysis of multi-angular measurements in the oxygen A-band: application to 3MI and MSPI
The vertical distribution of cloud cover has a significant impact on a large number of meteorological and climatic processes. Cloud top altitude and cloud geometrical thickness are then essential. Previous studies established the possibility of retrieving those parameters from multi-angular oxygen A-band measurements. Here we perform a study and comparison of the performances of future instruments. The 3MI (Multi-angle, Multi-channel and Multi-polarization Imager) instrument developed by EUMETSAT, which is an extension of the POLDER/PARASOL instrument, and MSPI (Multi-angles Spectro-Polarimetric Imager) develoloped by NASA's Jet Propulsion Laboratory will measure total and polarized light reflected by the Earth's atmosphere–surface system in several spectral bands (from UV to SWIR) and several viewing geometries. Those instruments should provide opportunities to observe the links between the cloud structures and the anisotropy of the reflected solar radiation into space. Specific algorithms will need be developed in order to take advantage of the new capabilities of this instrument. However, prior to this effort, we need to understand, through a theoretical Shannon information content analysis, the limits and advantages of these new instruments for retrieving liquid and ice cloud properties, and especially, in this study, the amount of information coming from the A-Band channel on the cloud top altitude (CTOP) and geometrical thickness (CGT). We compare the information content of 3MI A-Band in two configurations and that of MSPI. Quantitative information content estimates show that the retrieval of CTOP with a high accuracy is possible in almost all cases investigated. The retrieval of CGT seems less easy but possible for optically thick clouds above a black surface, at least when CGT > 1–2 km

We assessed incidence and mortality of colorectal cancer (CRC) at various ages among women and men in 38 European countries. The ages at which defined levels of incidence and mortality were reached varied between 9 and 17 years between countries. This variation requires consideration in the definition of screening guidelines
Collective effects in spin-crossover chains with exchange interaction
The collective properties of spin-crossover chains are studied.
Spin-crossover compounds contain ions with a low-spin ground state and low
lying high-spin excited states and are of interest for molecular memory
applications. Some of them naturally form one-dimensional chains. Elastic
interaction and Ising exchange interaction are taken into account. The
transfer-matrix approach is used to calculate the partition function, the
fraction of ions in the high-spin state, the magnetization, susceptibility,
etc., exactly. The high-spin-low-spin degree of freedom leads to collective
effects not present in simple spin chains. The ground-state phase diagram is
mapped out and compared to the case with Heisenberg exchange interaction. The
various phases give rise to characteristic behavior at nonzero temperatures,
including sharp crossovers between low- and high-temperature regimes. A
Curie-Weiss law for the susceptibility is derived and the paramagnetic Curie
temperature is calculated. Possible experiments to determine the exchange

Esophageal perforation secondary to malignant gastric outlet obstruction: a case report
Background
Esophageal perforation is a rare presenting sign of gastric cancer. To date, only nine case reports of this phenomenon have been previously published.
Case presentation
Esophageal perforation was diagnosed radiographically during workup for acute chest pain in a 67-year-old man. Emergent endoscopy confirmed esophageal perforation and biopsied a pre-pyloric mass confirmed to be adenocarcinoma. The perforation was managed with endoscopically placed transluminal pleural and mediastinal drains and esophageal stenting. The gastric outlet obstruction was temporized with a transpyloric stent. After the patient recovered from sepsis, distal gastrectomy was performed and he made a full recovery.
Conclusions
Rarely, pre-pyloric gastric cancer can present with Boerhaave syndrome, spontaneous esophageal perforation associated with forceful vomiting. We present the tenth report in the literature of this phenomenon and the first to be initially treated with endoscopic stenting and transluminal thoracoscopic drainage. When endoscopic management is used to treat patients with Boerhaave syndrome, it may be beneficial to examine the entire stomach to evaluate for malignant etiology
Climatologically invariant scale invariance seen in distributions of cloud horizontal sizes
Cloud area distributions are a defining feature of Earth's radiative exchanges with outer space. Cloud perimeter distributions n(p) are also interesting because the shared interface between clouds and clear sky determines exchanges of buoyant energy and air. Here, we test using detailed model output and a wide range of satellite datasets a first-principles prediction that perimeter distributions follow a scale-invariant power law n(p)∝p-(1+β), where the exponent β=1 is evaluated for perimeters within moist isentropic atmospheric layers. In model analyses, the value of β is closely reproduced. In satellite data, β is remarkably robust to latitude, season, and land–ocean contrasts, which suggests that, at least statistically speaking, cloud perimeter distributions are determined more by atmospheric stability than Coriolis forces, surface temperature, or contrasts in aerosol loading between continental and marine environments. However, the satellite-measured value of β is found to be 1.26±0.06 rather than β=1. The reason for the discrepancy is unclear, but comparison with a model reproduction of the satellite perspective suggests that it may owe to cloud overlap. Satellite observations also show that scale invariance governs cloud areas for a range at least as large as ∼3 to ∼3×105 km2, and notably with a corresponding power law exponent close to unity. Many prior studies observed a much smaller range for power law behavior, and we argue this difference is due to inappropriate treatments of the statistics of clouds that are truncated by the edge of the measurement domain.</p
Mammography screening: views from women and primary care physicians in Crete
Background: Breast cancer is the most commonly diagnosed cancer among women and a leading cause of death from cancer in women in Europe. Although breast cancer incidence is on the rise worldwide, breast cancer mortality over the past 25 years has been stable or decreasing in some countries and a fall in breast cancer mortality rates in most European countries in the 1990s was reported by several studies, in contrast, in Greece have not reported these favourable trends. In Greece, the age-standardised incidence and mortality rate for breast cancer per 100.000 in 2006 was 81,8 and 21,7 and although it is lower than most other countries in Europe, the fall in breast cancer mortality that observed has not been as great as in other European countries. There is no national strategy for screening in this country. This study reports on the use of mammography among middleaged women in rural Crete and investigates barriers to mammography screening encountered by women and their primary care physicians.
Methods: Design: Semi-structured individual interviews. Setting and participants: Thirty women between 45–65
years of age, with a mean age of 54,6 years, and standard deviation 6,8 from rural areas of Crete and 28 qualified
primary care physicians, with a mean age of 44,7 years and standard deviation 7,0 serving this rural population.
Main outcome measure: Qualitative thematic analysis.
Results: Most women identified several reasons for not using mammography. These included poor knowledge
of the benefits and indications for mammography screening, fear of pain during the procedure, fear of a serious
diagnosis, embarrassment, stress while anticipating the results, cost and lack of physician recommendation.
Physicians identified difficulties in scheduling an appointment as one reason women did not use mammography
and both women and physicians identified distance from the screening site, transportation problems and the
absence of symptoms as reasons for non-use.
Conclusion: Women are inhibited from participating in mammography screening in rural Crete. The provision
of more accessible screening services may improve this. However physician recommendation is important in
overcoming women's inhibitions. Primary care physicians serving rural areas need to be aware of barriers
preventing women from attending mammography screening and provide women with information and advice in a sensitive way so women can make informed decisions regarding breast caner screening
