    Association of the location of pancreatic ductal adenocarcinoma (head, body, tail) with tumor stage, treatment, and survival: a population-based analysis

    Background: The association between pancreatic ductal adenocarcinoma (PDAC) location (head, body, tail) and tumor stage, treatment and overall survival (OS) is unclear. Methods: Patients with PDAC diagnosed between 2005 and 2015 were included from the population-based Netherlands Cancer Registry. Patient, tumor and treatment characteristics were compared with the tumor locations. Multivariable logistic and Cox regression analyses were used. Results: Overall, 19,023 patients were included. PDAC locations were 13,451 (71%) head, 2429 (13%) body and 3143 (16%) tail. Differences were found regarding metastasized disease (head 42%, body 69%, tail 84%, p 4 cm: 21%, 40%, 51%, p <.001) and resection rate (17%, 4%, 7%, p <.001). For patients without metastases, median OS did not differ between head, body, tail (after resection: 16.8, 15.0, 17.3 months, without resection: 5.2, 6.1, 4.6 months, respectively). For patients with metastases, median OS differed slightly (2.6, 2.4, 1.9 months, respectively, adjusted HR body versus head 1.17 (95%CI 1.10–1.23), tail versus head 1.35 (95%CI 1.29–1.41)). Conclusions: PDAC locations in body and tail are larger, more often metastasized and less often resectable than in the pancreatic head. Whereas survival is similar after resection, survival in metastasized disease is somewhat less for PDAC in the pancreatic body and tail

    Lanthan-Strontium-Manganite perowskites are the most widespread materials in use for solid oxide fuel cell cathodes. The electrode reaction taking place, i.e. the reduction of oxygen supplied by air, was investigated by electrochemical means to obtain further knowledge about the electrode processes. The high activation energy of this reaction (200 kJ/mol), preventing lower operation temperatures of the SOFC, was the starting point for the investigation. Quasi steady state current voltage measurements and impedance spectroscopy were performed in a three electrode configuration. The electrodes were of circular shape with a diameter of 10 mm. The preparation was made by screen printing as well as wet powder spraying onto plates made of Yttria-stabilized zirconia. Perowskite powders of varying chemical and stoichiometric composition were used. To obtain higher power densities and, more important, lower apparent activation energies, catalytic layers were added at the interface electrode/electrolyte. Additionally, a less complex system, a model electrode/electrolyte setup made from single-crystal YSZ as electrolyte and gold in liquid and solid state as electrode was developed to create a better defined system. This setup was used to investigate the behaviour of the electrode/electrolyte interface. Reliable, reproducible results could be obtained using either setup. The experimental conditions i.e. oxygen partial pressure, temperature and overpotential were varied in order to determine the kinetic properties of the electrodes. Apparent activation energies, pre-exponential factors, apparent charge-transfer coefficients and electrochemical orders of reaction were calculated from the current-voltage data in order to propose possible reaction steps.     The Burden of Peritoneal Metastases from Gastric Cancer: A Systematic Review on the Incidence, Risk Factors and Survival

    The peritoneum is a common metastatic site in gastric cancer. This systematic review provides an overview of the incidence, risk factors and survival of synchronous peritoneal metastases from gastric cancer. A systematic search was performed to identify studies wherein the incidence, risk factors and survival of gastric cancer with peritoneal metastases were investigated. Of all 38 potentially eligible studies, 17 studies were included based on the eligibility criteria. The incidence of synchronous gastric peritoneal metastases was reviewed for population-based studies (10–21%), for observational cohort studies (2–15%) and for surgical cohort studies (13–40%). Potential risk factors for synchronous gastric peritoneal metastases were younger age, non-cardia gastric cancer, female sex, signet ring cell carcinoma, diffuse type histology or linitis plastica, T4 stage, Hispanic ethnicity and more than one metastatic location. Synchronous peritoneal metastases are commonly diagnosed in patients with gastric cancer with an incidence up to 21% in recent population-based studies. Furthermore, prognosis of patients with gastric peritoneal metastases is poor with median overall survival ranging from 2 to 9 months. The high incidence and poor prognosis require intensive research on diagnostic features and effective treatment options to improve survival

    Increase in the incidence of synchronous and metachronous peritoneal metastases in patients with colorectal cancer: A nationwide study

    Introduction: – To investigate the incidence of, factors associated with, and differences between synchronous and metachronous colorectal peritoneal metastases (CPM) in a population-based cohort. Methods: – Data from the Netherlands Cancer Registry were used. All patients diagnosed with colorectal cancer (CRC) between 1 January and June 30, 2015 were evaluated for synchronous or metachronous CPM (diagnosis ≀90 or >90 days after surgery for primary CRC), and survival in 2019 (median follow-up 38.4 months). Results: – Of 7233 included patients, 409 (5.7%) were diagnosed with synchronous CPM. Factors associated with synchronous CPM were mucinous (OR 2.72 [1.90–3.90]) or signet ring cell (SRC) histology (OR 6.58 [3.66–11.81]), T4 (OR 4.82 [3.68–6.32]), N1 (OR 1.66 [1.20–2.30]), or N2 stage (OR 3.27 [2.36–4.52]), and synchronous systemic metastases (SM) (OR 3.13 [2.37–4.14]). After surgery for primary CRC, 326 patients developed metachronous CPM after a median time of 14.7 months (3-year cumulative incidence: 5.5%). Factors associated with metachronous CPM were younger age (HR 1.63 [1.10–2.42]), mucinous (HR 1.84 [1.20–2.82]) or SRC histology (HR 2.43 [1.11–5.32]), T4 (HR 2.77 [2.07–3.70]), N1 (HR 2.90 [2.18–3.85]), N2 (HR 3.19 [2.26–4.50]), and synchronous SM (HR 1.95 [1.43–2.66]). Conclusion: – This population-based study found the highest incidence of CPM currently reported in literature and a strong association between the presence of synchronous SM and both synchronous and metachronous CPM. These findings may contribute to a tailored approach in the follow-up after primary CRC surgery and guide future clinical trials investigating new strategies regarding risk-reduction or early detection of metachronous CPM
