    “Başarılı bir öğretmenin çocukların katılımına önem vermesi gerektiğini düşünüyorum.”: Okulda Çocuk Katılımına İlişkin Fenomenolojik Bir Çalışma

    Öğretmen adaylarının çocuk katılımına ilişkin olumlu bakış açısına sahip olmaları, ileriki meslek yaşamlarında katılım hakkını gerçekleştirme biçimlerini ve uygulamaların niteliğini etkileyecektir. Bu çalışmanın amacı, okul öncesi ve sınıf öğretmen adaylarının eğitim kurumlarında çocuk katılımına yönelik görüşlerinin belirlenmesidir. Araştırmada nitel araştırma yöntemlerinden fenomenolojik yaklaşım kullanılmıştır. Araştırmanın çalışma grubunu, Türkiye’nin iç ege bölgesindeki bir üniversitenin okul öncesi eğitimi ve sınıf eğitimi bölümlerinin son sınıfta öğrenim görmekte olan 8’i okul öncesi, 8’i sınıf öğretmen adayı olmak üzere toplam 16 öğretmen adayı oluşturmuştur. Araştırmada veri toplama aracı olarak, araştırmacılar tarafından hazırlanan “Öğretmen Adaylarının Çocuk Katılımına Yönelik Görüşme Formu” kullanılmıştır. Araştırmadan elde edilen verilerin analizinde içerik analizi kullanılmıştır. Araştırmadan elde edilen veriler, dört tema çocuğun okulda katılımı, sınıfta alınan kararlar, görüşlerin ifade edilme süreci ve öğretmenlik uygulamasındaki deneyimler olmak üzere toplam dört tema altında toplanmıştır. Bu araştırma sonucunda okul öncesi ve sınıf öğretmen adaylarının katılıma yönelik olumlu bir bakış açısına sahip oldukları, katılımı sağlamaya yönelik uygulamalar konusunda deneyim kazanmak istedikleri belirlenmiştir. Öğretmen adayları, öğretmenlik deneyimleri sırasında çocukların görüşlerini soru-cevap yöntemi ile almaya çalıştıklarını belirtmişlerdir. Ayrıca bu araştırmaya katılan öğretmen adaylarının çocuk katılımını sağlamada aile ve öğretmenin önemli olduğunu ve ileriki meslek yaşamlarında katılım süreçlerine yer vermeyi düşündükleri görülmüştür. Öğretmen adaylarının çocuk katılımı ile ilgili bilgi ve deneyimlerinin yetersiz olduğu göz önünde bulundurularak temel eğitim bölümü ders programlarına çocuk katılımı üzerine zorunlu ya da seçmeli dersler eklenebilir. Çocuk katılımının önemi ve rolü ile ilgili yönelik farkındalığın artırılması için temel eğitim bölümü öğrencilerine yönelik, seminerler gerçekleştirilebilir ve öğrenci kulüplerinde farklı uzmanlar tarafından çocuk katılımına yönelik projelerin gerçekleştirilmesine rehberlik edilebilir

    Comparison of Radial Extracorporeal Shock Wave Therapy and Local Corticosteroid Injection Effectiveness in Patients with Carpal Tunnel Syndrome: A Randomized Controlled Study

    Objective The aim of the study was to compare the effectiveness of radial extracorporeal shock wave therapy and local corticosteroid injection on pain, function, and nerve conduction studies in the treatment of idiopathic carpal tunnel syndrome. Design A total of 72 patients who were diagnosed as having carpal tunnel syndrome were included in the study. The radial extracorporeal shock wave therapy group received radial extracorporeal shock wave therapy, the local corticosteroid injection group received local corticosteroid injection, and the control group only used a resting hand splint. The patients were evaluated using a Visual Analog Scale-pain, a Visual Analog Scale-numbness, the Boston Symptom Severity Scale, the Boston Functional Status Scale, and handgrip strength tests before treatment 1 and 12 wks after the treatment. Results Both clinical and nerve conduction study parameters improved with all three groups, and this effect continued at the 12th-week follow-up of the patients. The Visual Analog Scale-pain, Visual Analog Scale-numbness, Boston Symptom Severity Scale, and Boston Functional Status Scale scores in the first week after the treatment, as well as Visual Analog Scale-pain and Boston Functional Status Scale scores in the 12th week after the treatment, were significantly lower in the local corticosteroid injection group compared with the other two groups. Conclusions Our study revealed the success of radial extracorporeal shock wave therapy, splint, and local corticosteroid injection, but symptom relief was greater in the first week and 12th week with local corticosteroid injection. © Wolters Kluwer Health, Inc.     Hemodynamic Monitoring Using A Pulse Counter Vigileo Flotrac Cardiac Output System In Transapical Off-Pump Minimally Invasive Mitral Valve Repair

    The aim of this study was to present our anesthetic management and early hemodynamic recovery after transapical off-pump neochords implantation. Observational prospective study. The perioperative records of 13 patients who underwent mitral valve repair using the Neochord DS1000 system were analyzed. Hemodynamic measurements recorded with the Vigileo Flotrac invasive arterial cardiac output system were evaluated. Thirteen patients were included in the study. Transfusions of a total of 2 U of erythrocytes and 11 U of plasma were performed. After Neochord implantation, the mean cardiac output increased at a statistically significant level according to the values measured after induction p = 0.003. Increased cardiac output measurement after placement of the neochordaes and elimination of insufficiency have emerged as indications of early hemodynamic recover

    Comparison of immunogenicity and reactogenicity of inactivated SARS-CoV-2 vaccine (CoronaVac) in previously SARS-CoV-2 infected and uninfected health care workers

    The effects of inactivated SARS-CoV-2 vaccine (CoronaVac) on previously naturally infected individuals are unknown. This study compared immunogenicity and reactogenicity of CoronaVac in once naturally infected health-care workers (HCWs) and uninfected HCWs. All HCWs were immunized with two doses of CoronaVac (600 U/0.5 ml) intramuscularly at a 28-day interval. Adverse reactions were obtained by web-based questionnaires or telephone calls seven days after each vaccine dose. Detection of antibody levels against the receptor-binding domain (RBD) of SARS-CoV-2 spike protein was done four weeks after the second dose of the vaccine. We enrolled 103 previously naturally infected and 627 uninfected HCWs. The mean time for vaccination after the first nasopharyngeal SARS-CoV-2 positivity was 64 days (range: 15-136 days) in previously naturally infected HCWs. Among the previously naturally infected HCWs, 41 (40%) were asymptomatic, 52 (50%) had mild upper respiratory tract infections, 10 (105) had pneumonia, and only 6 (5%) were hospitalized. Any reported adverse reactions, either from the first dose or the second dose of vaccine administration, did not differ between previously infected and uninfected HCWs. Anti-RBD antibody titers were obtained in 50 (51%) of 103 previously infected HCWs and 142 (23%) of 627 uninfected HCWs. Anti-RBD antibody titers were significantly higher in HCWs with a previous natural infection (median 1220 AU/ml, range: 202-10328 AU/mL) than in uninfected HCWs (median: 913 AU/ml, range: 2.8-15547 AU/mL, p = .032). CoronaVac administration was safe and may elicit higher antibody responses in previously naturally infected individuals

    Investigation of survivin gene polymorphism in patients with gastric carcinoma

    Objectives: Despite decreasing incidence of gastric cancerin worldwide, it is still a major health problem. Everyyear, 30.000 new gastric cancer cases emerging, and itis the second most common cancer in Turkey. Gastriccancer is a complex multifactorial disease, emerging byinteraction between genetic and environmental factors.Survivin, apoptosis inhibitory protein is over-expressed incancer tissue. In this study, association between Survivin-31G/C polymorphism and gastric carcinoma was investigated.Materials and Methods: 46 gastric carcinoma patientswho had been admitted at Düzce University Researchand Practice Hospital, Laboratory of Pathology and 42healthy individuals have been included in the study. Sampleshave been subjected to genetic analysis by PCRRFLPmethod in Medical Genetics Department laboratoryat Düzce University.Results: GG genotype was found in 16 (34.8%), GCgenotype in 21 (45.7%), CC genotype in 9 (19.6%) in patientgroup. In control group, genotype distribution werefound 13 (31%), 26 (61.9%) and 3 (7.1%) respectively.The statistically significant difference was not found whencompared between patient and control groups. However,we observed the increased occurrence of gastric cancerassociated with CC genotype (OR=1.52).Conclusions: In our knowledge, this study is the first toevaluate the relationship between gastric carcinoma andSurvivin -31G/C polymorphism in Turkish population. Ourresults show that there is no any association betweengastric carcinoma and Survivin -31G/C polymorphismin the community which is represented by our study andcontrol groups.     Impact of the COVID-19 Pandemic on Inherited Metabolic Diseases: Evaluation of Enzyme Replacement Treatment Adherence with Telemedicine

    Aim:During the coronavirus disease-2019 (COVID-19) pandemic, visiting the hospital and getting regular infusions can be difficult for patients with chronic illnesses. Telemedicine may offer a good option for the management of chronic diseases such as lysosomal storage diseases (LSD).Materials and Methods:LSD patients at the Unit of Metabolic Diseases of Ege University were contacted by phone between April, 2020 and March, 2021 during the COVID-19 pandemic. Telemedicine appointments were performed at intervals every month or three months, depending on the patients’ compliance with their treatment.Results:Ninety-two LSD patients [Mucopolysaccharidosis (MPS) I, MPS II, MPS IVA, MPS VI, MPS VII, Gaucher, Fabry, and Pompe] were included in this study. The total skipped treatment rate within one year was 17.1%. Most of the months of interruption were consonant with the time of social isolation. The treatment interruption in patients under 18 years was lower than in patients over 18 years. A positive correlation was detected between the age of patients and the interruption of treatment.Conclusion:The curfew periods might be one of the causes of missed treatment sessions. Telemedicine is a good method to improve the continuity of treatment. This study showed that the number of interrupted enzyme replacement treatments could be decreased via ongoing telemedicine appointments

    Nephrotic syndrome in a patient with Glycogen Storage Disease Type IXb.

    Introduction: Glycogen storage disorder (GSD) IXb is characterized by liver and muscle involvement. We present a GSD IXb patient with an incidental union of nephrotic syndrome. Case Report: A 4 year-old-patient was diagnosed with GSD IXb at 13 months of age with mildly elevated transaminases and hepatomegaly. During the follow-up period, there was no hypoglycemia. Development and growth were normal. In the last month, the onset of generalized edema was reported. Urinalysis showed a high protein level. He had low serum albumin, high serum triglycerides cholesterol. Complement levels were normal. The patient was diagnosed as minimal change disease with a renal biopsy. He was treated with oral prednisone. Discussion: Minimal Change Disease is the most common cause of idiopathic nephrotic syndrome cases in children and the first step for therapy is the usage of corticosteroids. This is the first report of nephrotic syndrome associated with GSD IXb disease

    Real-World Outcomes of Anti-VEGF Treatment for Neovascular Age-Related Macular Degeneration in Turkey: A Multicenter Retrospective Study, Bosphorus Retina Study Group Report No: 1

    Objectives: To evaluate the real-world outcomes of intravitreal anti-vascular endothelial growth factor (anti-VEGF) treatment in neovascular age-related macular degeneration (nAMD) patients. Materials and Methods: Multicenter, retrospective, interventional, non-comparative study. The records of nAMD patients treated with an anti-VEGF agent on a pro re nata treatment regimen basis between January 2013 and December 2015 were reviewed. The patients who completed a follow-up period of 12 months were included. Primary outcome measures of this study were the visit and injection numbers during the first year. Results: Eight hundred eighty eyes of 783 patients met the inclusion criteria for the study. Mean number of visits at month 12 was 6.9±2.5 (range: 1-15). Mean number of injections at month 12 was 4.1±1.9 (range: 1-11). Mean visual acuity at baseline and months 3, 6, and 12 was 0.90±0.63 LogMAR (range: 0.0-3.0), 0.79±0.57 LogMAR (range: 0.0-3.0), 0.76±0.57 LogMAR (range: 0.0-3.0), and 0.79±0.59 LogMAR (range: 0.0-3.0), respectively. Mean central retinal thickness at baseline and months 6 and 12 was 395±153 μm (range: 91-1582), 330±115 μm (range: 99-975), and 332±114 μm (range: 106-1191), respectively. Conclusion: The numbers of visits and injections were much lower than ideal and were insufficient with the pro re nata treatment regimen
