The first eigenvalue of a homogeneous CROSS
We provide explicit formulae for the first eigenvalue of the Laplace-Beltrami
operator on a compact rank one symmetric space (CROSS) endowed with any
homogeneous metric. As consequences, we prove that homogeneous metrics on
CROSSes are isospectral if and only if they are isometric, and also discuss
their stability (or lack thereof) as solutions to the Yamabe problem.Comment: LaTeX2e, 40 page
Laparoscopic Myomectomy for Very Large Myomas Using an Isobaric (Gasless) Technique
Laparoscopic myomectomy using pneumoperitoneum for large myomas (> or = 8 cm) is hindered by several factors, such as the increased operative time, the risk of perioperative bleeding, and the risk of conversion to laparotomy. With the introduction of isobaric laparoscopy using abdominal wall lifting, this procedure can be performed using conventional surgical instruments introduced through small abdominal incisions. The aim of this study was to evaluate the feasibility, reproducibility, and safety of isobaric laparoscopic myomectomy for very large myomas > or = 10 cm using a subcutaneous abdominal wall-lifting device

Transvaginal ultrasound evaluation of the pelvis and symptoms after laparoscopic partial cystectomy for bladder endometriosis
Objective: To evaluate transvaginal sonography (TVS) findings after laparoscopic partial cystectomy for bladder endometriosis and to correlate postsurgical ultrasound findings with symptoms. Material and Methods: A retrospective study including women who underwent laparoscopic partial cystectomy for bladder endometriosis. Within 12 months after surgery, TVS examination was conducted in all patients to evaluate the bladder morphology, and the presence of any postsurgical sonographic findings of the pelvis. Painful symptoms were assessed using a visual analogue scale. Results: A total of 40 women were included. At the follow-up visit, 25 patients were receiving medical treatment while 15 had declined post-surgical therapy and had tried to conceive. The presence of bladder deep-infiltrating endometriosis (DIE) was found in nine (22.5%), fibrotic thickening of the bladder wall was found in 15 (37.5%), and normal bladder morphology was observed in 16 (40%). There was a correlation between anterior adenomyosis and bladder DIE, and fibrotic thickening of the bladder. Patients with TVS signs of bladder DIE and anterior adenomyosis suffered more dysmenorrhea and dysuria than patients with normal bladder. Conclusion: Post-operative TVS can detect the alteration of pelvis and could explain the causes of the persistence of symptoms. (J Turk Ger Gynecol Assoc 2022; 23: 145-53
Case Report A New Case of Prenatally Diagnosed Pentasomy X: Review of the Literature
Pentasomy X is a rare chromosomal abnormality probably due to a nondisjunction during the meiosis. Only four cases prenatally diagnosed were described until now. Our case is the fifth one prenatally diagnosed at 20 weeks of gestational age in a 39-yearsold woman. She underwent invasive prenatal diagnosis for her advanced maternal age without any other known risk factor. Amniocentesis performed at 17 weeks showed a female 49, XXXXX karyotype. The ultrasonographic examination revealed nonspecific signs of a mild early fetal growth retardation and no significant increased nuchal fold. The fetal autopsy and the Xray excluded major malformations. Prenatal diagnosis is often difficult due to the lack of indicative ultrasonographic findings and the rarity of described cases. The influence of the mother's age on the occurrence of penta-X syndrome has not been determined. Considering the lack of correlation between advanced maternal age and increased risk for pentasomy X, as well as the absence of typical echographic signs, evaluation of the inclusion of a noninvasive prenatal test (NIPT) that expands clinical coverage to include the X and Y chromosomes in routine prenatal diagnosis should be considered as well as three-dimensional ultrasound to detect any helpful indicative prognostic signs
Role of transvaginal hydrolaparoscopy in the management of the infertile patient
Transvaginal hydrolaparoscopy (THL) was introduced in the evaluation of infertility to explore the pelvic structures in their natural positions. In our clinical experience, THL was performed in 93.3.% of cases with no complication. The overall accuracy of THL in diagnosis of pelvic pathology was 81.8%. More recently, some interventions during THL were reported. We treated 28 clomiphene-resistant, anovulatory women with polycystic ovary syndrome by transvaginal hydrolaparoscopic ovarian drilling using the bipolar VersaPoint system. In conclusion, THL can be proposed as a first line procedure in the routine evaluation of infertile women without clinical or imaging evidence of pelvic disease. Also the surgical treatment can be realized by this technique, when required. Further clinical trials need to elucidate the precise role of THL in the evaluation and management of infertility
