Pojęcie wyrównania metabolicznego często utożsamia się przeważnie z odsetkiem hemoglobiny glikowanej. Wielu lekarzy intensyfikuje terapię wyłącznie
na podstawie tego parametru. Autorzy omawiają
konsekwencje posługiwania się parametrem HbA1c,
jego związki z regulacją stężenia glukozy, a także
odnoszą się do innych parametrów, które powinno
się uwzględnić w przypadku konieczności podjęcia
decyzji o intensyfikacji i wyborze modelu leczenia.
Do czynników tych należą: dobowy profil glikemii,

Cukrzyca typu 2 ma charakter postępujący i w pewnym
momencie jej trwania konieczne jest rozpoczęcie
leczenia insuliną w celu uzyskania właściwej kontroli
glikemii. Rozpoczęcie insulinoterapii niemal zawsze
wiąże się z pytaniem, od jakiego rodzaju insuliny
powinno się zaczynać leczenie cukrzycy typu 2, tak
aby było ono najskuteczniejsze dla pacjenta, a jednocześnie
wiązało się z małym ryzykiem działań niepożądanych.
Z punktu widzenia i lekarza, i pacjenta
równie ważna jest wygoda stosowania insuliny.
W niniejszym artykule poruszono kwestie wytycznych
leczenia cukrzycy 2 oraz podjęto próbę odpowiedzi
na pytanie, od którego z preparatów insuliny należy
rozpocząć terapię: długodziałającego analogu insuliny
czy analogu dwufazowego.Diabetes mellitus is a progressive disease requiring
multiple treatment regimens. To secure good glycaemic control insulin needs to be introduced when oral
antidiabetic drugs fail. When starting insulinotherapy
there are several questions to be answered: what
kind of insulin is optimal for the patient and which
one is the most effective and safe. Another important
aspect is quality of life meaning convenient insulinotherapy
assuring high compliance. In the following
article we attempt to asses which of the following
insulin treatment regimens: long acting insulin
analogue or biphasic insulin analogue is optimal
for starting insulinotherapy in type 2 diabetes
mellitus
Cukrzyca typu 2 u chorych po 70. roku życia — aspekty wyrównania metabolicznego
Background. The differences in the clinical manifestation and course of diabetes observed in older individuals should translate into various options of treatment and management of the elderly diabetic patients. Of particular importance is the early detection of disease complications due to its impact on the physical and mental health of elderly patients. Material and methods. All participants were hospitalised in the Department of Internal Medicine, University Hospital No. 1 between the year 2012–2016. Based on the medical records, the consecutively presenting patients diagnosed with type 2 diabetes were divided into three basic groups: group 1 aged 37–58 years (30 individuals), group 2 aged 70–79 years (30 individuals) and group 3 aged ≥ 80 years (50 individuals). The exclusion criteria were lack of logical verbal contact and substantial impairment of physical activity verified with the Katz Basic Activities of Daily Living (ADL) scale. Based on the medical history and documentation, the duration of diabetes, the BMI and frequency of hypoglycaemia were established. Among patients, as part of their stay and regardless of the study conducted, the main parameters were determined referring to metabolic control of type 2 diabetes mellitus (percentage of glycated haemoglobin, lipid profile) and blood pressure measurement. Results. The particular groups of patients differed in terms of the mean disease duration (p < 0.001), i.e. 5 ± 6.4 years in group 1, 16.1 ± 8.6 years in group 2 and 14.6 ± 9 years in group 3. There were statistically significant inter-group differences in mean body weight (BW) and BMI (patients ≥ 80: BW — 70.7 ± 14.3 (kg), BMI: 27.5 ± 4.3 (kg/m2); 70–79 years of age group: BW: 77.1 ± 16.4 (kg), BMI: 28.9 ± 6 (kg/m2); group 1 (BW: 92.4 ± 21.4 (kg), BMI: 31.5 ± 7.0 (kg/m2). The best glucose control was observed amongst patients ≥ 80 (group 3), as compared to group 1 — 47 patients (84%) vs. 4 patients (13.3%) (p < 0.001). The percentages of HbA1c were as follows: 8.7 ± 2.3 (%) in group 1, 7.3 ± 1.2 (%) in group 2 and 6.9 ± 0.9 in group 3, respectively (p < 0.001). There were no statistically significant differences in the blood pressure between particular groups of patients. Hypoglycaemia did not occur more frequently in any of the studied groups. Conclusions. Elderly patients suffering from type 2 diabetes vary in numerous aspects from younger patients. The basic differences were observed in relation to anthropometric indices and average duration of the disease. Treatment of type 2 diabetes in elderly patients leads to excessive control with respect to the carbohydrate metabolism although patients do not report hypoglycaemia more frequently.
Wstęp. U chorych w starszym wieku obserwuje się pewne różnice zarówno w manifestacji klinicznej, jak i w przebiegu cukrzycy typu 2, które powinny się przekładać na sposób leczenia i postępowania z pacjentem diabetologicznym. Szczególne znaczenie ma wczesne zapobieganie powikłaniom choroby ze względu na ich wpływ na fizyczne i psychiczne zdrowie pacjentów w starszym wieku.
Materiał i metody. Wszyscy pacjenci, którzy wzięli udział w badaniu, byli hospitalizowani w Klinice Chorób Wewnętrznych Uniwersyteckiego Szpitala Klinicznego nr 1 w Lublinie w latach 2012–2016. Na podstawie zgromadzonej dokumentacji medycznej chorych z rozpoznaną cukrzycą typu 2 kolejno zgłaszających się do Kliniki podzielono na trzy zasadnicze grupy: grupa badana 1. — pacjenci w wieku 37–58 lat (30 osób), grupa badana 2. — pacjenci w wieku 70–79 lat (50 osób) oraz grupa badana 3. — pacjenci w wieku ≥ 80 lat (50 osób). Kryteriami wykluczenia z badania były brak logicznego kontaktu słownego z chorym oraz znaczne ograniczenie sprawności fizycznej, weryfikowane przy pomocy Skali Katza (ADL). Na podstawie wywiadu i dokumentacji medycznej ustalono czas trwania cukrzycy, wartość wskaźnika masy ciała (BMI) oraz częstość występowania hipoglikemii. W ramach pobytu w Klinice, niezależnie od przeprowadzanego badania, u pacjentów oznaczono główne parametry odnoszące się do kontroli metabolicznego wyrównania cukrzycy typu 2 (odsetek hemoglobiny glikowanej, lipidogram) oraz wykonano pomiar ciśnienia tętniczego.
Wyniki. Poszczególne grupy chorych różniły się pod względem średniego czasu trwania cukrzycy typu 2 (p < 0,001). W grupie badanej 1. czas ten wynosił: 5 ± 6,4 roku, w grupie badanej 2. — 16,1 ± 8,6 roku, a w grupie badanej 3. — 14,6 ± 9 lat. Wykazano statystycznie istotną różnicę w zakresie średniej masy ciała oraz średniego BMI między grupą chorych ≥ 80 lat (średnia masa ciała: 70,7 ± 14,3 kg, BMI: 27,5 ± 4,3 kg/m2), grupą chorych w wieku 70–79 lat (średnia masa ciała: 77,1 ± 16,4 kg, BMI: 28,9 ± 6 kg/m2) i grupą chorych w wieku 37–58 lat (średnia masa ciała: 92,4 ± 21,4 kg, BMI: 31,5 ± 7,0 kg/m2). Najlepsze wyrównanie gospodarki węglowodanowej odnotowano u chorych ≥ 80. roku życia z grupy badanej 3., najsłabsze zaś — w grupie badanej 1.: 47 chorych (94%) vs. 4 chorych (13,3%) (p < 0,001). Odsetek HbA1c wynosił — odpowiednio — 8,7 ± 2,3% w grupie badanej 1., 7,3 ± 1,2% w grupie badanej 2. i 6,9 ± 0,9% w grupie badanej 3. (p < 0,001). Między poszczególnymi grupami chorych nie zaobserwowano statystycznie istotnych różnic w zakresie rejestrowanych wartości ciśnienia tętniczego.
Wnioski. Chorzy na cukrzycę typu 2 w wieku podeszłym różnią się pod wieloma względami od chorych w młodszym wieku. Podstawowe różnice odnotowano zarówno w odniesieniu do wskaźników antropometrycznych, jak i średniego czasu trwania choroby. Leczenie cukrzycy typu 2 u chorych w starszym wieku prowadzi do nadmiernego wyrównania w zakresie gospodarki węglowodanowej, przy czym chorzy ci nie zgłaszają częstszego występowania hipoglikemii

Badanie IMPROVETM - międzynarodowe badanie obserwacyjne w cukrzycy typu 2: wyniki dotyczące populacji Polski
Introduction: Observational studies are valuable tools for assessing the applicability of results from randomised controlled trials to broader
patient populations. They are especially important in chronic diseases such as diabetes, as they can provide a comprehensive picture
of the safety and effectiveness of a particular therapy across cultures and phenotypes.
Material and methods: Patients with type 2 diabetes who required insulin and whose physician had decided to initiate biphasic insulin
aspart 30 (BIAsp 30) were eligible. A total of 4117 type 2 diabetic patients were recruited to the study in Poland, and 809 primary and
secondary care physicians were involved. The aim of this study was to assess the safety and effectiveness of BIAsp 30 treatment in type 2
diabetes in routine clinical practice.
Results: Baseline glycaemic control was poor in the Polish cohort enrolled in the IMPROVETM study, with a mean HbA1c value of 9.0 ± 1.7%. A very high proportion of patients were thus at risk of macrovascular and microvascular complications. A twice-daily regimen for
the start of BIAsp 30 therapy was the most common choice, including 72.2% of patients at baseline. HbA1c was significantly reduced by
1.66% for the total cohort and by 3.07% and 1.55% in the pre-study no-therapy or oral antidiabetic drug group respectively (p < 0.001). The
rates (episodes per subject year) of overall major hypoglycaemia were 0.012 and 0.12 at follow-up and final visits respectively. For minor
hypoglycaemia rates of 5.12 per subject per year at follow-up visit and 4.54 episodes per subject per year at final visit were recorded.
Conclusions: BIAsp 30 appears to be an effective and flexible treatment approach and can be safely intensified to achieve glycaemic
control in a majority of patients with type 2 diabetes.Wstęp: Badania obserwacyjne stanowią wartościowe narzędzie uzupełniające informacje uzyskane z badań RCT (randomised controlled
trials). Odgrywają one szczególnie ważną rolę w przypadku schorzeń przewlekłych, takich jak cukrzyca, w przypadku których, w odniesieniu
do dużych populacji, dostarczają istotnych danych dotyczących skuteczności i bezpieczeństwa stosowanych terapii.
Materiał i metody: Grupa badana obejmowała chorych na cukrzycę typu 2, którzy w opinii lekarza prowadzącego wymagali rozpoczęcia
insulinoterapii i kwalifikowali się do włączenia dwufazowej insuliny aspart 30 (BIAsp 30) w celu poprawy wyrównania metabolicznego.
Do badania, którego celem była ocena bezpieczeństwa i skuteczności leczenia cukrzycy typu 2 przy użyciu BIAsp 30 zakwalifikowano
w Polsce łącznie 4117 chorych.
Wyniki: Wyrównanie metaboliczne polskiej grupy wchodzącej w skład globalnego badania IMPROVETM, w momencie rozpoczęcia obserwacji,
było wysoce niezadowalające - średnia wartość HbA1c wynosiła 9,0 ± 1,7%. W konsekwencji istotny odsetek pacjentów prezentował
powikłania naczyniowe o typie mikro- i makroangiopatii. Najczęściej stosowanym schematem rozpoczęcia terapii BIAsp 30,
w 72,2% przypadków, było zastosowanie dwóch wstrzyknięć na dobę. W trakcie 26-tygodniowej obserwacji redukcja HbA1c wynosiła
1,66% dla całej populacji oraz odpowiednio 3,07% i 1,55% dla osób wyjściowo nie leczonych farmakologicznie oraz otrzymujących doustne
leki przeciwcukrzycowe. Ryzyko epizodów ciężkiej hipoglikemii w trakcie całego badania (epizody/pacjenta/rok) wynosiło 0,012, natomiast
lekkiej hipoglikemii odpowiednio 5,12/pacjenta/rok w trakcie wizyty kontrolnej oraz 4,54/pacjenta/rok na wizycie końcowej.
Wnioski: Dwufazowa insulina aspart 30 stosowana w codziennej praktyce jest bezpieczną i skuteczną insuliną w terapii cukrzycy typu 2
Polymorphisms of the β1 (ADRB1) and β2 (ADRB2) adrenergic receptor genes in essential hypertension accompanying obesity
Wstęp Wartość ciśnienia tętniczego jest uwarunkowana
rzutem minutowym serca i oporem naczyń
obwodowych. Obydwa te czynniki są regulowane
przez aktywność układu współczulnego. Przypuszcza
się, że polimorfizmy Ser49Gly genu receptora
adrenergicznego β1 (ADRB1) oraz Arg16 Gly receptora
adrenergicznego β2 (ADRB2) są związane
z aktywnością układu współczulnego. Celem pracy
było określenie, czy wymienione polimorfizmy
genetyczne wiążą się z nadciśnieniem tętniczym
u osób otyłych.
Materiał i metody Badano 207 osób z nadciśnieniem
tętniczym z polskiej populacji. Podzielono je
na 2 grupy: 103 osoby otyłe i 104 osoby z prawidłowym
wskaźnikiem masy ciała. Polimorfizm
Ser49Gly genu ADRB1 wykrywano metodą PCRRFLP,
a polimorfizm Arg19 Gly genu ADRB2
- metodą ARMS.
Wyniki Częstość występowania allela 49Gly oraz homozygotycznego
genotypu GG w locus genu ADRB1
nie różniła się istotnie między badanymi grupami.
Dystrybucja allela Gly16 w locus genu ADRB2 oraz
homozygotycznego genotypu GG była istotnie wyższa
u otyłych pacjentów z nadciśnieniem w porównaniu
z grupą osób z nadciśnieniem i prawidłową
masą ciała. Dalsza analiza statystyczna wykazała, że
nosiciele genotypu Gly16Gly z nadciśnieniem tętniczym
charakteryzowali się większym ryzykiem otyłości.
Szczegółowa analiza wykazała, że związek ten
dotyczy głównie kobiet przed 60. rokiem życia. Badanie
haplotypów nie potwierdziło synergistycznego
związku dwóch badanych loci genowych z otyłością
towarzyszącą nadciśnieniu tętniczemu.
Wnioski Nosicielstwo wariantu Gly16 genu ADRB2
może mieć związek z otyłością współistniejącą z nadciśnieniem
tętniczym u kobiet przed 60. rokiem życia.Background The blood pressure level is determined by
cardiac output and peripheral resistance both of which are
regulated by the sympathetic nervous system. Presence of
Ser49Gly polymorphism at beta1-adrenergic receptor gene
(ADRB1) locus and Arg16Gly polymorphism at beta2-adrenergic receptor gene (ADRB2) locus are supposed to
have an impact on the adrenergic receptor functional status.
The aim of this study was to evaluate whether these
polymorphisms are associated with the hypertension in
obese individuals.
Material and methods We recruited 207 patients with essential
hypertension of whom 103 were obese and 104 have
a normal BMI. Genotyping of the Ser49Gly polymorphisms
was underteken by PCR-RFLP, whereas the
detection of Arg16Gly polymorphisms was performed by
using the ARMS method.
Results The allele frequency of Gly49 (38% vs. 45%,
p = 0.50) and Gly49Gly homozygotes (9% vs. 13%, p = 0.50)
at beta1-adrenergic receptor gene locus did not differ between
obese and non-obese subjects with hypertension.
The distribution of the allele Gly16 (66% vs. 53%,
p = 0.005) and homozygotic genotype GG (49% vs. 29%,
p = 0.014) at adrenoreceptor beta2 gene locus were significantly
different in patients with BMI ≥ 30 kg/m2 and hypertension
compared to subjects with normal BMI and hypertension.
Further statistic analysis demonstrated that the hypertensive
bearers of Gly16Gly genotype are of the increased
risk of obesity (OR = 2.33; 95% CI: 1.30-4.17;
p = 0.0043). Additional stratification divulged that presence
of Gly16 variant constitute a risk of obesity in the
hypertensive female subjects younger than 60 years old
(OR = 3.01; 95% CI: 1.35-6.71; p = 0.0073). Analysis of
haplotypes of these two genomic loci did not indicate the
synergistic action in hypertension-associated obesity.
Conclusions The carrier status of Gly16 variant od the
ADRB2 gene may contribute to an increased risk of obesity
in the hypertensive women younger than 60 years old
The use of fixed dose combinations in the treatment of hypertension — resistant hypertension or patient resistant to treatment?
Resistant hypertension (RH) is defined as a condition in which blood pressure remains above 140/90 mm Hg despite the use of at least three antihypertensive drugs, correctly combined and in full doses (including a diuretic). In Poland, the percentage of people suffering from RH is estimated at 10–13% relative to the total number of patients with hypertension. On the other hand, in recent years we have seen a significant increase in the importance of combination products both in Europe and in the United States. Several studies have shown that therapy that includes an average of 3 antihypertensive agents (in the form of combination products) having different mechanisms of action, such as blockers of the renin-angiotensin-aldosterone system (RAAS) in combination with a calcium channel blocker (CCB) or a diuretic, provides blood pressure control in the majority of patients, not only through increased efficacy and tolerability of drugs, but also owing to the simplification of the therapeutic regimen, which promotes the compliance with medical recommendations. The prevalence of arterial hypertension depending on selected demographic factors among adult residents of eastern Poland district
Background. Identification and analysis of risk factors are important aspects of planning the long-term actions toprevent, early diagnose and properly treat hypertension. The aim of this study was to examine the prevalence of hypertension and to determine the relationship between the prevalence of hypertension and selected sociodemographicfactors among adult residents of a typical eastern Poland district.
Material and methods. The study involved 1225 residents of the Janów Lubelski district aged between 36 and 65years. The research methodology included a questionnaire survey. Blood pressure, body weight and height weremeasured, the BMI was calculated. Statistical analysis was performed.
Results. 26.8% of subjects are treated for hypertension. 32.9% of hypertensive subjects are treated properly andwomen have better control of hypertension than man (41.3% vs. 19.7%). Among subjects without treatmentfor hypertension, 37.5% of them have abnormally high blood pressure and it is more common in men (58.9%vs. 43.4%). The prevalence of hypertension increased with age. The percentages are from 8.6% for subjects aged36–40 to 45.7% for subjects aged 61–65. Higher prevalence of hypertension is observed in men below the age of50 compared to women. There is the significant rising prevalence of hypertension in women above the age of 50.The relationship between the prevalence of hypertension and the educational background is demonstrated. Amongsubjects with elementary education, the prevalence of hypertension is higher (45.5%) as compared to subjects withhigher education (18.2%). Area of residence and marital status have no effects on the prevalence of hypertension.
Conclusions. The prevalence of hypertension was lower compared to other epidemiological studies. Improvements in hypertension control have been observed for many years. The educational background has an important influence on the prevalence of hypertension. Further studies are required to establish the influence of an area of residence and marital status on the prevalence of hypertension
