    A Viewpoint of Authenticity to Enhance Teacher’s Abilities in Studying Teaching Materials : Through an Analysis of the Adaptation Process of a Junior High School English Textbook

    本研究は,英語教育における教材の真正性(authenticity)のあり方について検討することにより,教師の教材研究力を涵養するための視座を明らかにすることを目的とする。第二言語習得研究において,できるだけ自然かつ多量の理解可能な目標言語入力にふれることは,必須の条件のひとつとされている。最新のICTや多様な教材ソフトウエアの開発により,学習者が自然な英語に接する機会は質量ともに格段に向上してきた。しかしながら,教材はできるだけ自然なものでなければならないとする主張に対して,自然な教材の教育的効果を疑問視する主張もあるなど,教材の自然さの有用性については今なお多くの議論がある。さらに,いったい何をもって自然さを定義するかも定まっていない。そこで本論では,英語教育における教師のための真正性の視座について,これまでのさまざまな研究者による定義を概観・整理するとともに,教科書教材の真正性に大きな影響を与えると考えられる教材改作(materials adaptation)過程について検討を行う。教材の真正性に関する理論的考察に加え,テキスト原素材から教科書題材に変化する過程の分析を通して,日本のような外国語としての英語教育環境で必要となる真正性の視座を新たに構築することをねらいとする。The purpose of this article is to consider the meaning of authenticity of teaching materials in English language education, and consequently to clarify the viewpoints for developing teachers’ abilities in studying teaching materials. In second language acquisition studies, it is regarded as essential to be exposed to abundant comprehensible input in the target language. With the advancement of ICT and a variety of educational software, second language learners have the great advantage of getting access to a lot of natural English language resources. Under the recent communicative trend, there is a strong belief that teaching materials must be as real as possible; however, there remains certain skepticism about the effectiveness of authentic materials in second language teaching. Furthermore, there is no unified definition of the term. Therefore, this article will review the concepts of authenticity advanced by several applied linguistics researchers and examine efforts to maintain authenticity in textbook materials adaptation processes. By theoretical consideration of material authenticity and practical analysis of the simplifying process from the original to the adapted textbook material in a Japanese junior high school English textbook, this study will try to construct a viewpoint of authenticity necessitated in an English-as-a foreign-language situation like Japan

    Case report: Long-term management of occlusion after surgical-orthodontic treatment for a patient with drug-induced open bite developed after the onset of schizophrenia

    BackgroundSchizophrenia is a major mental disorder, with an estimated incidence of 1%. Since they are sensitive to sensory changes, orthodontic treatment to move teeth should be avoided as aggressively as possible in these patients because of strong concerns about the possibility of causing adverse psychological effects, thus there are few reports on orthodontic treatment for schizophrenia patients. We report a case of severe open bite caused by medication after the onset of schizophrenia, even though the patient’s occlusion had been stable for a long time after surgical orthodontic treatment. Medication control and the use of a minimally invasive orthodontic appliance improved the occlusion without adversely affecting the patient’s mental health.CaseA 22-year-old woman presented to the clinic with a chief complaint of an anterior open bite. Intraoral findings showed an overbite (vertical overlap of the incisor teeth) of −3.0 mm and an overjet (horizontal overlap of the incisor teeth) of −0.5 mm. The preoperative orthodontic treatment included bilateral extraction of the maxillary first premolars. Subsequently, orthognathic surgery was performed to achieve a harmonized skeletal relationship and occlusion. Occlusion was stable for 3 years after surgery. However, 10 years after surgery, the patient returned to the clinic complaining of an anterior open bite (overbite = −4.0 mm). Six years prior to the return, the patient was diagnosed with schizophrenia. We thought that ignoring the patient’s strong desire to treat her open bite might also cause psychological problems; therefore, in addition to medication control, we treated her using a minimally invasive removable orthodontic appliance (retainer with tongue crib). Her anterior open bite improved (overbite, +1.0 mm) to within the normal range.ConclusionIn this case, medication control was thought to be essential to improve her drug-induced open bite. However, minimally invasive orthodontic treatment, such as the use of a removable appliance, might be helpful in promoting her mental stability as well as for improving occlusion. Careful support is required to obtain information about the patient’s mental state and medications through close cooperation with psychiatrists

    Capsaicin, a TRPV1 Ligand, Suppresses Bone Resorption by Inhibiting the Prostaglandin E Production of Osteoblasts, and Attenuates the Inflammatory Bone Loss Induced by Lipopolysaccharide

    Capsaicin, a transient receptor potential vanilloid type 1 (TRPV1) ligand, regulates nerve-related pain-sensitive signals, inflammation, and cancer growth. Capsaicin suppresses interleukin-1-induced osteoclast differentiation, but its roles in bone tissues and bone diseases are not known. This study examined the effects of capsaicin on inflammatory bone resorption and prostaglandin E (PGE) production induced by lipopolysaccharide (LPS) in vitro and on bone mass in LPS-treated mice in vivo. Capsaicin suppressed osteoclast formation, bone resorption, and PGE production induced by LPS in vitro. Capsaicin suppressed the expression of cyclooxygenase-2 (COX-2) and membrane-bound PGE synthase-1 (mPGES-1) mRNAs and PGE production induced by LPS in osteoblasts. Capsaicin may suppress PGE production by inhibiting the expression of COX-2 and mPGES-1 in osteoblasts and LPS-induced bone resorption by TRPV1 signals because osteoblasts express TRPV1. LPS treatment markedly induced bone loss in the femur in mice, and capsaicin significantly restored the inflammatory bone loss induced by LPS in mice. TRPV1 ligands like capsaicin may therefore be potentially useful as clinical drugs targeting bone diseases associated with inflammatory bone resorption

    Ventriculoperitoneal Shunt Outcomes among Infants

    Ventriculoperitoneal shunts (VPSs) are used for the treatment of hydrocephalus. Here we analyzed the outcomes of VPS placements in 24 infants to determine the risk factors for shunt failure. The infants had undergone the initial VPS operation in our hospital between March 2005 and December 2013. They were observed until the end of January 2014. We obtained Kaplan-Meier curves and performed a multivariate Cox regression analysis of shunt failure. Of the 24 cases, the median (range) values for gestational age, birth weight, and birth head circumference (HC) were 37 (27-39) wks, 2,736 (686-3,788) g, and 35.3 (23.0-45.3) cm, respectively. The total number of shunt procedures was 45. Shunt failure rates were 0.51/shunt and 0.0053/shunt/year. Shunt infection rates were 0.13/shunt and 0.0014/shunt/year. The Kaplan-Meier analysis revealed an increased risk for shunt failure in infants <1 month old or in the HC >90オtile. The Cox regression analysis yielded hazard ratios (HRs) of 2.93 (95オ confidence interval (CI), 0.96-10.95, p=0.059) for age <1 month, and 4.46 (95オCI:1.20-28.91,p=0.023) for the HC >90オtile. The multivariate Cox regression analysis showed adjusted HRs of 17.56 (95オCI:2.69-202.8, p=0.001) for age <1 month, and 2.95 (95オCI:0.52-24.84, p=0.228) for the HC >90オtile. Our findings thus revealed that the risk factors for shunt failure in infants include age <1 month at the initial VPS placement

    Changes in the Features of Invasive Pneumococcal Disease after Introduction of the Seven-valent Pneumococcal Conjugate Vaccine in a Regional Core Hospital of Kochi, Japan

    Since the introduction of the seven-valent pneumococcal conjugate vaccine (PCV7) in 2007, invasive pneumococcal disease has declined, but the incidence of Streptococcus pneumoniae serotype 19A has risen worldwide. The present study examined changes in the features of invasive pneumococcal disease since the introduction of the PCV7 in Kochi, Japan. Pediatric cases of invasive pneumococcal disease were investigated before and after vaccine introduction (January 2008 to December 2013). Cases of invasive pneumococcal disease tended to decrease after PCV7 introduction. In addition, before introduction of the vaccine, most serotypes causing invasive pneumococcal disease were those included in the vaccine. However, after the introduction, we found cases infected by serotypes not covered by vaccine. Penicillin-resistant S. pneumoniae was the predominant serotype causing invasive pneumococcal disease before introduction of the PCV7, and the susceptibility of this serotype to antibiotics improved after vaccine introduction. Serotype isolates identified after vaccine introduction were also relatively susceptible to antibiotic therapy, but decreased susceptibility is expected

    Prevalence of Arsenicosis in Terai, Nepal

    This paper summarizes the findings of surveys on arsenicosis cases conducted during 2001-2004 in terai, Nepal, by governmental and non-governmental organizations in their respective project areas and by some national and international institutes. Studies were conducted in six arsenic-contaminated districts of terai, namely Nawalparasi, Bara, Parsa, Rautahat, Rupandehi, and Kapilvastu. In these districts, arsenic contamination in tubewells varied from 2.1% to 25.7%. The prevalence of arsenicosis was, on average, 2.2% and varied from 0.7% in Kapilavastu district to 3.6% in Nawalparasi district. In the community-based study, the highest prevalence (18.6%) of arsenicosis was found in Patkhouli village of Nawalparasi, where 95.8% of tubewells were contaminated with arsenic. The prevalence of arsenicosis was higher in older age-groups (>50 years) of both the sexes. Males suffered more from arsenicosis than females (odds ratio: 2.50, 95% confidence interval 1.80-3.47). Skin manifestations, such as melanosis and keratosis, were the common symptoms of arsenicosis. Most patients were identified in the early or mild stage of the disease. They are expected to recover if further exposure to arsenic could be avoided by providing arsenic-safe drinking-water through intervention measures

    Large Quantum Anomalous Hall Effect in Spin-Orbit Proximitized Rhombohedral Graphene

    The quantum anomalous Hall effect (QAHE) is a robust topological phenomenon that features quantized Hall resistance at zero magnetic field. Here we report the observation of the QAHE in a rhombohedral pentalayer graphene/monolayer WS2 heterostructure. Distinct from all existing QAHE systems, this system has neither magnetic element nor moir\'e superlattice effect. The QAH states emerge at charge neutrality and feature Chern numbers C = +-5 at temperatures up to about 1.5 K. This large QAHE in our system arises from the synergy of the electron correlation effect in intrinsic flat bands of pentalayer graphene, the gate-tuning effect that breaks the layer/spin-degeneracy, and the proximity-induced Ising spin-orbit-coupling (SOC) effect that further lifts the valley-degeneracy. Our experiment demonstrates the great potential of crystalline two-dimensional materials for intertwined electron correlation and band topology physics, and points to engineering chiral Majorana edge states towards topological quantum computation

    Effect of Ambient Temperature on Daily Nebulized Asthma Hospital Visits in a Tropical City of Dhaka, Bangladesh

    The acute effect of temperature on asthma morbidity in Bangladesh is not well understood. As climate varies extensively in different parts of the world, the relation between temperature and asthma might also differ. We investigated the association between temperature and asthma-related hospital visits in the tropical city of Dhaka. We analyzed information from a total of 5989 asthma patients who received ambulatory care in the form of nebulized medication at the National Asthma Center in Mohakhali, Dhaka from February to November 2013. A time-stratified case-crossover study was conducted to estimate the effect of daily temperature, with consideration of delayed effects and possible confounders such as relative humidity and political strikes. An inverse association was observed between temperature and the number of hospital visits. The effect was delayed for approximately a week. A degree centigrade decrease in mean temperature (averaged across lags 0-6) was associated with an increase of approximately 4.5% (95% CI 1.5, 7.5) in all asthma visits. The association was evident in adult males but marginal in elderly males. A positive association (lag 0) was observed among adult females, whereas no association was observed among children. Strikes significantly modified the effect among the elderly. Findings suggest temperature declines affect asthma outcomes in a warm climate, and this effect can be delayed and vary by sex and age group

    BEHAVIORS ASSOCIATED WITHWATER CONTACT AND SCHISTOSOMA JAPONICUM INFECTION IN A RURAL VILLAGE, THE DONGTING LAKE REGION, CHINA

    Although identification of water contact patterns is one of the most important factors for the prevention of Schistosoma japonicum infection, it is still insufficient for clarifying specific high-risk behaviors and their implications. Parasitological studies and behavioral observations were carried out in a rural village, the Dongting Lake region, China. A time-allocation study conducted by a time-saving spot-check method was implemented to quantify the behavioral risks. Of the 122 participants, 18 (14.8%; 95% confidence interval: 8.5, 21.0) were positive for S. japonicum. Among those diagnosed, the median (25-75% quartile) eggs per gram was 8 (8-16). A significant positive correlation with worm intensity was found among people who repair ships on the marshland (p<0.001), and this potential risk was consistent with previous suggestions. Although the parasitological techniques and study design require further improvements, our observational methods may be of use to explicitly identify behaviors at the local level that could be relevant to prevention

    Positive Minimal Residual Disease of FLT3-ITD before Hematopoietic Stem Cell Transplantation Resulted in a Poor Prognosis of an Acute Myeloid Leukemia

    Acute myeloid leukemia (AML) patients with fms-related tyrosine kinase 3 (FLT3)-internal tandem duplication (ITD) often have a poor prognosis, even after hematopoietic stem cell transplantation (HSCT). We report a case of AML with FLT3-ITD identified upon initial diagnosis, who received HSCT at complete remission after 3 consecutive chemotherapies. However, the patient relapsed when the same FLT3-ITD clone emerged, and finally died. Retrospective analysis revealed an allelic ratio of FLT3-ITD/wild type of 1.1 and 0.0096 upon initial diagnosis and before HSCT, respectively. The detection of any minimal residual FLT3-ITD clone before HSCT is useful in the treatment of AML with FLT3-ITD
