    The final examination in medicine : time for change?

    Most medical programmes culminate in a final assessment, in order that participants may be tested and graded. In June 1995, at the University of Malta, a group of 53 medical students sat for their final examination; medicine was one of the three co-equal component subjects of this statutory qualifying examination. The scope of this paper is to analyze the results obtained in the final examination in medicine and to use this data to address such issues as aims of this examination, method and quality assurance of assessment. The result obtained by candidates in the final examination in medicine was correlated with their university entry qualifications. The composite mark for each candidate was split into its components and analysis took the form of description, correlation and clustering. Computation of Cronbach’s alpha facilitated anlaysis of reliability of each of the three parts of the examination. The findings of this paper suggest that there is room for improving the quality of assessment methods. A review of methods and procedures, with the dual purpose of decreasing bias and increasing specificity and sensitivity of this statutory examination will not only benefit candidates, but ultimately also the University of Malta. The final qualifying examination in medicine should have clearly defined objectives and methods of assessment should be aimed specifically at reaching them. It needs to be able to assess the ability to think critically about diagnosis and management and to ensure that the candidate has a satisfactory base of factual knowledge. It also needs to assess objectively the adequacy of basic clinical skills and candidates’ facility of communication.peer-reviewe

    New Star Forming Galaxies at z\approx 7 from WFC3 Imaging

    The addition of Wide Field Camera 3 (WFC3) on the Hubble Space Telescope (HST) has led to a dramatic increase in our ability to study the z>6 Universe. The increase in the near-infrared (NIR) sensitivity of WFC3 over previous instruments has enabled us to reach apparent magnitudes approaching 29 (AB). This allows us to probe the rest-frame ultraviolet (UV) continuum, redshifted into the NIR at z>6z>6. Taking advantage of the large optical depths at this redshift, resulting in the Lyman-alpha break, we use a combination of WFC3 imaging and pre-existing Advanced Camera for Surveys (ACS) imaging to search for z approx 7 over 4 fields. Our analysis reveals 29 new z approx 7 star forming galaxy candidates in addition to 16 pre-existing candidates already discovered in these fields. The improved statistics from our doubling of the robust sample of z-drop candidates confirms the previously observed evolution of the bright end of the luminosity function.Comment: 15 pages, accepted in MNRA

    Maltese radiographers’ attitudes towards continuing professional development : an initial study using concept maps

    Purpose: The Council for the Professions Complementary to Medicine in Malta recently published a draft document regarding the introduction of mandatory continuing professional development (CPD) for radiographers. This study explored the attitudes and motivators of Maltese radiographers prior to the implementation of mandatory CPD in order to provide the necessary information required by management to develop CPD successfully. Concept maps are used as part of a methodology to analyse qualitative data. Methods: All radiographers working in the National Health Service were invited to complete an anonymous web-based questionnaire. Results: The study showed that participants generally had a positive attitude towards CPD but were concerned about the mandatory aspect. The participants were mostly motivated by increasing professional knowledge, updating existing qualifications, and enhancing the status of the profession as a whole. Radiographers identified several difficulties with respect to CPD participation, such as lack of funding, lack of management support, and not enough local CPD opportunities. CPD participation was also negatively influenced by family commitments. Conclusion: The study showed that the majority of radiographers were self-motivated to engage in CPD activities, but there were some concerns. Based on these results, the authors suggest recommendations for allaying apprehension and producing the necessary conditions for a successful mandatory CPD scheme.peer-reviewe

    Malignant lymphoma of the intestine : a report of three cases

    Tumours arising in the small intestine are uncommon; those of them originating in the lymphoid tissues are said to be even less frequent than adennocarcinomas. Three cases hereby are reported, outlining the respective diagnosis and treatment. All the three cases were diagnosed histologically as Lymphosarcoma. It is interesting to note that in published series, Lymphosarcomas and Reticulum Cell Sarcomas have been about equally represented, and together accounted for the vast majority of cases. Various gross features have been reported, the commonest being polypoid, annular, ulcerative and aneurismal. An important feature of these tumours is that they have little connective tissue stroma. This, together with infiltration of the muscle coat, is said to account for the aneurismal varieties of the growth. Age incidence studies show that most cases occur in the age group 45 to 60 years, but no age group is exempt. As for signs and symptoms, it is true to say that no definite syndrome is produced by this disease. In general, it may be said that cases have presented either as surgical emergencies or insidiously. There is general consensus regarding the value of post-operative radiotherapy in unresectable growths, and in those with regional lymph node metastasis. What is uncertain is the value of radiotherapy where no evidence of lymph node spread is present.peer-reviewe

    Atmospheric sulphur dioxide pollution in Malta : a preliminary study

    The results of a preliminary survey of the air quality in Malta during May-December 1990 with respect to sulphur dioxide are presented. One-hour-average S02 concentrations as high as 320 ug m-3 have been measured from areas proximate to the Marsa power station. The concentration of S02 in ambient air was found to be strongly dependent on atmospheric conditions and especially on wind direction. At Paola, measured concentrations varied from below the detection limit of 25 ug m-3 to about 160 ug m-3. These preliminary results suggest that specific areas in Malta which are close to the Marsa power station may have an air pollution problem which is significantly more pronounced than that in other areas. The results also indicate the need for a thorough air monitoring programme which would allow the pollution problem to be assessed more accurately and to be followed continually.peer-reviewe

    Built-in premium effects within exotic FX options

    Purpose: This paper explains the analysis of built-in premium within ‘premium-free’ FX Option strategies, also known as ‘exotic options’. The aim is to analyse whether such an increase in built-in premium would have an effect on the outcome of the strategies. Methodology: The analysis was conducted through three different currency pairs, namely, the EURUSD, EURJPY and EURGBP, throughout a period between 2007 to 2014. The authors used the Bloomberg terminal to design two different option strategies: Window Forward Extra and At Expiry Forward Extra. These strategies are known as low risk hedging strategies within the FX options industry. The authors examined different combinations of changes in built-in premium and analysed the respective outcome with each combination. The outcomes were compared to analyse whether an increase in built-in premium would have an effect on the outcome of the strategy. A test was also conducted should these strategies be used as a speculative tool. The strategies were built on a 1 year tenor which is rolled over every month. Hence, each month a hedge using the strategy for 1 year was conducted. The authors used back dated implied volatilies when performing the back-testing in order for results to be realistic. Findings: In most cases it was found that there was no effect on the outcome of the strategy. However, this was only valid if an expiry at a time is taken. On the other hand, when taking into account the whole sample, even though only 3% of the times there was an effect on the outcome of the strategy, the total result finds that an increase in built-in premium has an effect on the outcome of the strategy. Such result was found to be statistically significant using a paired sample t-test. This applied for all currency pairs under review. When analysing the exotic option strategies for speculative purposes, the authors found that in most cases it would have been better for brokers to take higher risk and receive an upfront Premium.peer-reviewe

    The changing concept of competence and categorisation of learning outcomes in Europe : implications for the design of higher education radiography curricula at the European level

    The Bologna process has made the qualifications framework of the European Higher Educational Area based on three cycles and on learning outcomes central to curriculum development in higher education in Europe. The Tuning Educational Structures in Europe project recommended that learning outcomes be expressed in terms of competences. The expression of educational programme learning outcomes as inventories of competences has since become the norm at the European level. However, the more recent European Qualifications Framework for lifelong learning utilises a tripartite set of categories of learning outcomes, namely, knowledge, skills and competence. In addition, the definition of competence used though overlapping with that used by Tuning, is however not identical. This article reviews and discusses the changing definition of the concept of competence and changes in categorisation of learning outcomes in Europe and their potential impact on curriculum development in radiography at the European level. It is proposed that the shift in the definition of competence and in the categorisation of learning outcomes should be taken into account in the formulation of new European curricula or the updating of present ones so that they may reference in a more direct manner to the levels of the European Qualifications Framework.peer-reviewe

    Organizational commitment and users’ perception of ease of use : a study among bank managers

    Purpose – The purpose of this paper is to look at the issue of perceived ease of use of a web-based customer relationship management system and consider the role of organizational commitment as a possible antecedent. Design/methodology/approach – Data for this study were collected from among managers of a major player in the community banking sector within the EU. A total of 274 valid responses were obtained from 398 managers. Findings – Results have been mixed and partially conditioned by service providers’ willingness to leverage the possibilities that the technology can provide. Research limitations/implications – The study was limited to a single organization and consequently the results should be generalized with caution. Replication studies with improved measures, in other countries and contexts are desirable. Practical implications – The results can be useful for management, since Web-based customer relationship management systems have been adopted by many service providers in their quest to offer better one-to-one marketing possibilities to their customers. Originality/value – This paper demonstrates the importance of fostering a sense of organizational commitment amongst key service providers, as this in turn seems to enable them to overcome many impediments pertaining to technology use.peer-reviewe

    Diversification strategies for the service sector

    An analysis of the American economy shows that the service sector overshadows the manufacturing sector, both in size and in state of tunnoil. The tunnoil is due to Ute rapid expansion of franchise chains, computerization, evolving professional standards, and competitive pressures. From the consumer's perspective, as client sensitivity intensifies and professional malpractice suits become more threatening, the issue of understanding and examining services becomes more important. In line with Wind's (1982) observation, service decisions in a finn are not limited to individual services. A finn should take into account its service line (a number of related services), service mix (combination of service lines), and service portfolio (assortments of service mixes). The lines of demarcation among these tenns are not always clear because they depend on organizational position as well as personal preferences in defining classification systems. At the most basic level, an institution offers only a singular service. Extensions of offerings within a restricted and related group of service categories constitute a service line, an exanlple of which is the tax consulting services market where an accountant may file a tax form on behalf of a client either by mail or through computer services linked up with the IRS office.peer-reviewe
