Molecular aspects in pathogenesis of cancerogenesis: review

will increase by about 70% in the next two years.
Aim of the study. To evaluate and systematize pathogenetic factors that contribute to
cancerogenesis. Cancerogenesis is defined as the static process by which a normal cell acquires
properties that allow the development of malignant phenotype (uncontrolled proliferation, local
invasion and metastasis), or a cascade of events that lead to the transformation of a normal cell,
often a clonogenic cell (stem cell) into cancer. Cancerogenesis is the multistage process in which
mutations lead to the development of malignant phenotype, which is the result of multiple
interactions between various exogenous and endogenous factors. Cancerogenesis proceeds
through the accumulation of genetic and epigenetic changes that allow cells to break free from
the tight network of controls that regulate the homeostatic balance between cell proliferation and
cell death.
Conclusions. 1. In recent years, the development of genome-wide analytic methods has opened
the possibility of identifying simultaneously multiple changes in gene expression as well as in
genetic or epigenetic alterations affecting the genome of cancer cells. 2. The Mutator Phenotype
can be caused by a number of mechanisms, such as defects in cell-cycle regulation, apoptosis,
specific DNA repair pathways, or error-prone DNA polymerase, and it can have its source in
inherited genetic defects that make subjects prone to specific cancers. 3. Mutations in cancer
cells cover a wide range of structural alterations in DNA, including changes in chromosomes
copy numbers or chromosomal alterations encompassing millions of base-pairs such as
translocations, deletions or amplifications, as well as smaller changes in nucleotide sequences
such as point mutations affecting a single nucleotide at a critical position of a cancer-related gene
(Sugimura et al., 1992). These different kinds of alterations often co-exist within a single tumour.
4. TP53 mutations in plasma DNA have been reported in patients with cancers of the colon,
pancreas, lung, and liver. 5. EGFR and HER2 are often altered in diverse human cancers, by
amplification, point mutation, or both. Amplifications of EGFR have been detected in brain
cancers and in a small proportion of a number of epithelial cancers such as squamous oral or
esophageal cancer. Amplification and overexpression of HER2 are a frequent event in breast and
ovarian cancer (Harari and Yarden, 2000)
JUVENILE LARYNGEAL PAPILLOMATOSIS

Papilomatoza laringiana reprezintă o tumoră benignă, ce rezulta din proliferarea conjunctivo-epitelială a mucoasei laringiene. Este o patologie evolutiva, recurentă și extensivă de papilomavirus uman, în special tipul 6 și 11, fiind cele mai agresive. Primele acuze apar de obicei la vârsta cuprinsă între 2 și 6 ani. Simptomul inițial fiind disfonia progresivă, urmată de stridor și detresă respiratorie. Tratamentul este unul multimodal, de baza fiind microlaringoscopia suspendată cu excizia papiloamelor. Scopul lucrării. Studiul presupune efectuarea unei analize detaliate asupra statisticii pacienților cu papilomatoza laringiană pe parcursul anilor 2020-2022 și relevarea datelor obținute. Materiale și metode. În articolul respectiv au fost utilizate date din literatura de specialitate cu referire la papilomatoza laringiană și datele statistice acumulate pe parcursul anilor 2020-2022 în cadrul Spitalului Clinic “E. Coțaga”, secția ORL. Rezultate. S-au înregistrat 11 pacienți cu diagnosticul de papilomatoza laringiană. Prevalența genului F/M este de 8:5. Din numărul total de adresări vârsta pacienților a fost cuprinsă între 1-13 ani, dintre care la 5 pacienți, a fost vârsta 2 ani. Pacienții cu vârsta de debut 2 ani au prezentat un număr mai mare de recidive, de la 2 până la 5 spitali zări. În urma efectuării examenului histopatologic la fiecare caz, nu s-a depistat trecerea din forma benignă în malignă a patologiei. Concluzii. Papilomatoza laringiană juvenilă este o tumoră benignă. La copii, papiloamele sunt multiple și recidivează frecvent după ablație, comparativ cu adultul, la care este de obicei singular, însă poate degenera malign. În unele cazuri, la copii papiloamele pot retroceda spontan, după pubertate.Background. Это эволюционная, рецидивирующая и распространяющаяся патология вируса папилломы человека, особенно типов 6 и 11, являющихся наиболее агрессивными. Лечение комплексное, в основе микроларингоскопия с иссечением папилломы. Использовались данные из специальной литературы в отношении папилломатоза гортани и статистические данные, накопленные в течение 2010-2022 годов в клинике «Эмилиан Коцага», ЛОР-отделение. Зарегистрировано 56 больных с диагнозом папилломатоза гортани. Преобладание пола Ж/М составляет 8:5. Из общего числа обращений возраст пациентов составлял от 1 до 13 лет, из них 5 пациентов были в возрасте 2 лет. У детей папилломы множественные и часто рецидивируют после удаления, по сравнению со взрослыми, у которых они обычно единичны, но могут малигнизироваться. В некоторых случаях у детей папилломы могут регрессировать спонтанно, после пубертатного периода
