De nieuwe zichtbaarheid: sturing in tijden van marktwerking
Ongeveer een jaar geleden constateerde de Opta, de organisatie die toeziet op het
functioneren van de telefoniemarkt, dat die markt zo langzamerhand de minst
overzichtelijke was geworden (âMarkt mobieltjes is ondoorzichtigâ 2006). Iedereen
die de afgelopen jaren wel eens een telefoonabonnement heeft aangeschaft, en wie
heeft dat niet, kan dat volop beamen. Er is volop informatie beschikbaar over de prijzen
en andere voorwaarden voor bijvoorbeeld mobiele telefoons, maar vergelijkingen
tussen abonnementen zijn nauwelijks te maken en achteraf blijk je toch altijd duurder
uit te zijn geweest dan je dacht. Toch kunnen we niet zeggen dat de telefoniemarkt
niet âwerktâ. Integendeel. Er is volop concurrentie tussen aanbieders; de prijs van
producten per eenheid is de afgelopen jaren steeds gedaald en de sector is innovatief
als nooit tevoren. Blijkbaar is transparantie van de markt geen voorwaarde voor het
functioneren ervan
Studying design and use of healthcare technologies in interaction: the social learning perspective in a Dutch quality improvement collaborative program
Designing technologies is a process that relies on multiple interactions between design and use contexts. These interactions are essential to the development and establishment of technologies. This article seeks to understand the attempts of healthcare organisations to integrate use contexts into the design of healthcare technologies following insights of the theoretical approaches of social learning and user representations. We present a multiple case study of three healthcare technologies involved in improving elderly care practice. These cases were part of a Dutch quality improvement collaborative program, which urged that development of these technologies was not âjustâ development, but should occur in close collaboration with other parts of the collaborative program, which were more focused on implementation. These cases illustrate different ways to develop technologies in interaction with use contexts and users. Despite the infrastructure of the collaborative program, interactions were not without problems. We conclude by arguing that interactions between design and use are not naturally occurring phenomena, but must be actively organised in order to create effect
Leren van toezicht
Toezicht staat onder immer toenemende belangstelling, of het nu gaat om het toezicht op financiële instellingen, de veiligheid van het vliegverkeer of de gezondheidszorg. De oorzaken hiervan zijn divers maar kunnen deels gevonden worden in het terugtreden van de overheid als uitvoerder van publieke diensten. Om controle te kunnen houden op die diensten zijn de afgelopen decennia grote toezichthoudende organen opgericht, die
inmiddels een flink deel van de economie beslaan; volgens sommige berekeningen werken momenteel ongeveer een miljoen mensen in de âtoezichtsindustrieâ (van Waarden 2006).
Daarnaast dringt steeds meer het besef door dat we leven in een ârisicomaatschappijâ (Beck 1992) die, hoe paradoxaal ook, aanleiding heeft gegeven tot een âveiligheidsutopieâ
(Boutelier 2002). Risicoâs worden steeds minder geaccepteerd en overheden en toezichthouders worden daar in toenemende mate op aangesproken. Deze trend brengt met zich mee dat onderzoek naar het functioneren van toezichthouders sterk in maatschappelijk belang heeft gewonnen. De afgelopen jaren is dan ook internationaal te constateren dat dit onderzoek een hoge vlucht heeft genomen. Ook de inspecties zelf hebben inmiddels onderzoeksprogrammaâs opgezet. Dit geldt ook voor de
Inspectie voor de Gezondheidszorg (IGZ). Het voorliggende rapport is een verslag van onderzoek naar de effectiviteit van een van de belangrijke toezichtsinstrumenten van de IGZ, het zogeheten Thematisch Toezicht. Middels casestudie onderzoek beoogt dit rapport de vraag te beantwoorden op welke manier het
thematisch toezicht effectief kan zijn en voor welk type risicoproblemen deze vorm van toezicht het meest geschikt is. Hiermee probeert het rapport een zowel conceptuele als
empirische bijdrage te leveren aan de discussie over het toezicht in algemene zin, en in het bijzonder over het toezicht op en in de gezondheidszorg
Overheidstoezicht door de Inspectie voor de Gezondheidszorg
Het rijksoverheidstoezicht door de Inspectie voor de Gezondheidszorg (IGZ) zoals we dat nu kennen heeft zijn wortels in de eerste helft van de negentiende eeuw. In deze bijdrage gaat het over het verleden en de toekomst van het overheidstoezicht door de IGZ als een van de actoren binnen de complexe sturingsrelaties in de gezondheidszorg. De sectorschets is geschreven in opdracht van de Wetenschappelijke Raad voor het Regeringsbeleid (WRR). Tijdens het schrijfproces heeft de WRR een expertbijeenkomst georganiseerd met de auteurs en deskundigen uit het toezicht, de zorg en de wetenschap. De tijdens deze bijeenkomst gemaakte opmerkingen zijn verwerkt in de tekst. De auteurs zijn verantwoordelijk voor de inhoud van de sectorschets
Patient and family engagement in incident investigations: exploring hospital manager and incident investigatorsâ experiences and challenges
Objective: There is growing recognition among health care providers and policy makers that when things go wrong, the patient or their families should be heard and participate in the incident investigation process. This paper explores how Dutch hospitals organize patient or family engagement in incident investigations, maps out incident investigatorsâ experiences of involving patients or their families in incident investigations and identifies the challenges encountered. Methods: Semi-structured interviews were conducted with managers and incident investigators in 13 Dutch hospitals. Study participants (n = 18) were asked about the incident investigation routines and their experiences of involving affected patients or family members. Interview transcripts were coded and analysed using thematic content analysis. Results: Our findings reveal that patient or family involvement in incident investigations is typically organized as a one-time interview event. Interviews with patients or their families were considered to be valuable and important in their own right and seen as a way to do justice to the individual needs of the patient or their family. Yet, the usefulness and validity of the patient or family perspective for incident investigations was often seen to be limited, with the professional perspective afforded more weight. This was particularly the case when the patient or their family were unable to provide verifiable details of the incident under investigation. Study participants described challenges when involving patients or family members, including in relation to the available timeframe for incident investigations, legal issues, managing trust and working with intense emotions. Conclusions: We propose that by placing patient and family criteria of significance at the centre of incident investigations (i.e. an âemicâ research approach), hospitals may be able to expand their learning potential and improve patient-centeredness following an incident
Mutual powerlessness in client participation practices in mental health care
Background Client participation has become a dominant policy goal in many countries including the Netherlands and is a topic much discussed in the literature. The success of client participation is usually measured in terms of the extent to which clients have a say in the participation process. Many articles have concluded that client participation is limited; professionals often still control the participation process and outcomes. Objective The objective of this study is to gain insight into (i) the practice of client participation within a quality improvement collaborative in mental health care and (ii) the consequences of a Foucauldian conceptualization of power in analysing practices of client participation. Design We used an ethnographic design consisting of observations of national events and improvement team meetings and interviews with the collaborative's team members and programme managers. Results Contrary to many studies on client participation, we found both clients and service providers frequently felt powerless in its practice. Professionals and clients alike struggled with the contributions clients could make to the improvement processes and what functions they should fulfil. Moreover, professionals did not want to exert power upon clients, but ironically just for that reason sometimes struggled with shaping practices of client participation. This mutual powerlessness (partly) disappeared when clients helped to determine and execute specific improvement actions instead of participating in improvement teams. Conclusion Recognizing that power is inescapable might allow for a more substantive discussion concerning the consequences that power arrangements produce, rather than looking at who is exerting how much power
