Utilizarea anastomozelor limfatico-venoase în managementul chirurgical al limfedemului postmastectomie
Department of Orthopedics and Traumatology, Nicolae Testemitanu SUMPhBackground. Postmastectomy lymphedema is a debilitating and progressive complication, with few
treatment options, which considerably lowers breast cancer survivors’ quality of life. Its incidence varies
from 0-3% after lumpectomy to 65-70% after radical mastectomy and radiotherapy. Objective of the
study. The purpose of this study is to determine the effectiveness of lymphaticovenous anastomosis in
the prevention and treatment of postmastectomy lymphedema in various stages of the disease. Material
and Methods. For this literature review more than 50 scientific articles have been studied in great detail.
Results. Lymphaticovenous anastomosis has proven to be efficient in the surgical management of
postmastectomy lymphedema in the first and second stage together with conservative treatment or
liposuction and in the third and four stage in conjunction with lymph node transfer or Charles procedure.
The supermicrosurgical technique is more efficient in the incipient or moderate stages of the disease
than in severe lymphedema. Conclusion. Lymphaticovenous anastomosis is a supermicrosurgical
technique useful in both the prevention of postmastectomy lymphedema development and the
prevention of lymphedema progression as well as its management in all stages of the disease.Introducere. Limfedemul postmastectomie este o complicație debilitantă și progresivă, cu puține
opțiuni de tratament, ce scade considerabil calitatea vieții supraviețuitorilor de cancer mamar. Incidența
variază de la 0-3% după lumpectomie, la 65-70% după mastectomie radicală și radioterapie. Scopul
lucrării. Scopul acestei lucrări este de a determina eficacitatea anastomozei limfatico-venoase în
prevenirea și tratamentul limfedemului postmastectomie în diferite stadii de avansare ale bolii. Material
și Metode. Pentru această sinteză a literaturii de specialitate au fost studiate minuțios peste 50 de articole
științifice. Rezultate. Anastomoza limfatico-venoasă s-a dovedit a fi eficace în managementul
chirurgical al limfedemului postmastectomie în stadiul 1-2 împreună cu un tratament conservator sau
liposucție și în stadiul 3-4 în conjuncție cu transferul de nodul limfatic sau procedura Charles. Tehnica
supermicrochirurgicală este mai eficientă în stadiile incipiente sau moderată ale bolii decât în cea severă.
Concluzii. Anastomoza limfatico-venoasă este o tehnică supermicrochirurgicală utilă atât în prevenirea
dezvoltării limfedemului postmastectomie, cât și în prevenirea agravării bolii și managementul acesteia
în toate stadiile ei

which produces soft tissue defects that require reconstructive
plastic surgery.
Purpose: The elucidation of a clinical case of
reconstruction of a massive back defect that
resulted after oncological ablation, using the
latissimus dorsi flap.
Material and methods:The study includes a clinical case of a 56
years old female patient who developed a keratinizing
squamous cell carcinoma. After two rounds of radiotherapy and
two soft tissue ablations, the patient was hospitalized in our
clinic with a massive soft tissue defect on her back, where we
performed secondary surgical debridement and defect
reconstruction using a latissimus dorsi flap of 30 x 25 cm.
Results: The donor site was partially closed in the first step of
the surgery, and fully closed in a second step a week after,
using a skin graft. The postoperative period presented itself
without any vascular complications regarding both the flap and
donor site. At the 45th postoperative day the patient fully
recovered and returned to her usual lifestyle.
Conclusions:Radiotherapy related soft tissue defects present a
challenge for reconstructive plastic surgery and the use of
fasciocutanous axially vascularized flaps offers high quality
soft tissue for solving this problem

celulele canceroase, cât și în cele sănătoase, ducând uneori la ulcerații și necroze de țesuturi moi care
produc defecte tisulare ce necesită intervenții reconstructive. Scopul lucrării. Elucidarea unui caz clinic
de plastie a unui defect actinic masiv al spatelui, utilizând lamboul latissimus dorsi. Material și
Metode. Studiul include un caz clinic al unei paciente de 56 de ani, care a dezvoltat un carcinom
scuamos cheratinizant al pielii. După două cure de radioterapie și două excizii tumorale, pacienta a fost
internată în clinica noastră cu un defect masiv al spatelui, unde a fost efectuată excizia focarului și plastia
defectului restant cu lambou latissimus dorsi cu dimensiuni de 30 x 25 cm. Rezultate. Locul donator a
fost închis parțial în prima etapă a intervenției și închis definitiv la o săptămână după aceasta utilizând
o grefă de piele. Perioada postoperatorie a evoluat fără complicații vasculare la nivelul lamboului și a
locului donator. La a 45-a zi postoperatorie pacienta s-a recuperat, revenind la ritmul său de viață
obișnuit. Concluzii. Defectele actinice reprezintă o provocare pentru chirurgia reconstructivă, iar
utilizarea lambourilor fasciocutanate vascularizate axial ne oferă un țesut de înaltă calitate pentru
rezolvarea acestor defecte
THE USE OF DECELLULARIZED NERVE ALLOGRAFTS IN THE RECONSTRUCTION OF PERIPHERAL NERVE DEFECTS

O parte semnificativă a leziunilor de nervi periferici este reprezentată de leziunile cu defect de nerv periferic. Regula de aur în cazul leziunilor de nerv este sutura epiperineurală a capetelor lezate, fără tensiune. În cazul defectelor de nervi aceasta este imposibilă și se recurge la reconstrucția de nerv. Alogrefele de nervi decelularizate prezintă un avantaj față de restul metodelor datorită numărului de grefe nelimitat, lipsa traumei adiționale a pacientului, lipsa necesității tratamentului imunosupresor și ușurința suturării datorită păstrării arhitecturii nervului. Scopul lucrării. Scopul acestei lucrări este de a determina eficacitatea alogrefelor decelularizate de nerv în reconstrucția de nerv periferic. Material și metode. Pentru această sinteză a literaturii de specialitate au fost studiate minuțios 40 de articole științifice. Rezultate. Alogrefele de nerv decelularizate pot fi utilizate în reconstrucția defectelor de nerv periferic chiar și în cazul defectelor mari (peste 15 mm). Sunt sigure din punct de vedere imunologic și rezultatul poate fi comparat cu cel al autogrefelor în cazul unor anumite protocoale de decelularizare. Cele mai bune protocoale de decelularizare sunt cele care utilizeaza Triton X-100 și acid peracetic sau ADN-aza I și ARN-aza A. Concluzii. Alogrefele de nerv decelularizate prezintă o tehnică promițătoare, cu rezultate satisfăcătoare în experimentele pe animale. Pentru a îmbunătăți performanța procesului de decelularizare ar putea fi introdusă o etapă de recelularizare pentru a produce o grefă care este capabilă să înlocuiască standardul de aur actual.Background. A significant part of peripheral nerve injuries is represented by injuries with peripheral nerve defects. The golden rule in nerve injuries is epiperineural suture of the injured ends without tension. In the case of nerve defects, this is impossible and nerve reconstruction is used. Decellularized nerve allografts present an advantage over other methods due to the unlimited number of grafts, the lack of additional trauma to the patient, the lack of need for immunosuppressive treatment and the ease of suturing due to the preservation of the nerve architecture. Objective of the study. The purpose of this paper is to determine the efficacy of decellularized nerve allografts in peripheral nerve reconstruction. Material and methods. For this literature review 40 scientific articles were thoroughly studied. Results. Decellularized nerve allografts can be used in the reconstruction of peripheral nerve defects even in large defects (15 mm or more). They are immunologically safe and the result can be compared to that of autografts in the case of certain decellularization protocols. The best decellularization protocols are those using Triton X-100 and peracetic acid or DNAse I and RNAse A. Conclusions. Decellularized nerve allografts are a promising technique that holds satisfactory results in animal experiments. To improve the performance of the decellularization process a recellularization step could be introduced to produce a graft that is capable of replacing the current gold standard
