    Cuspal Deflection and Adhesive Interface Integrity of Low Shrinking Posterior Composite Restorations

    Kad je riječ o restaurativnim zahvatima u distalnom području trenuačno su u svim istraživanjima u središtu pozornosti nisko-kontrahirajući kompoziti. U vezi s tim obećavaju materijali dobiveni siloranskim kemijskim postupkom s prstenastim monomerima i kondenzabilni dimetakrilati s visokim postotkom punila. Materijali i metode: Na ekstrahiranim gornjim premolarima (n=10) promatran je otklon kvržica nakon svjetlosne polimerizacije transduserom s razlikovanjem direktne struje (DCDT-om) uz čuvanje adhezivne veze elektronskim mikroskopom (SEM-om) i dvama restorativnim sustavima. To su Filtek™ Silorane/Silorane System Adhesive (3M ESPE) i Premise™ Packable /OptiBond FL (KERR). Podaci su analizirani dvostranim t-testom. Rezultati: Vrijednost p< 0,05 postavljena je kao statistički značajna granica. Filtek™ Silorane može smanjiti otklon kvržica uzrokovan polimerizacijskom kontrakcijom, ali sumnja se može li se održati adhezivna veza. Zaključak: Premise™ Packable pokazao je veći otklon kvržica tijekom polimerizacije, ali i bolju adhezivnu vezu.Low shrinking resin composites are in the focus of research in posterior resin composite restoratives. In this context, the silorane-chemistry, incorporating ring-opening monomers and highly filled packable dimethacrylates seem to be most promising. Material and Methods: The goal of this study was to investigate cuspal deflection after light-curing in extracted upper premolars (n=10), using direct current differential transducers (DCDT), and adhesive interface integrity by scanning electron microscopy (SEM) evaluation, of two restorative systems: 1) Filtek™ Silorane/ Silorane System Adhesive (3M ESPE); 2) Premise™ Packable /OptiBond FL (KERR). Data were analysed using a two-tailed t-test. Results: A p-value < 0.05 was considered statistically significant. Filtek™ Silorane may reduce cuspal deflection associated to the polymerization process, but there is concern regarding durability of adhesive interface integrity. Conclusions: Premise™ Packable showed higher cuspal deflection and complete integrity of adhesive interface

Takav stres nastaje u kompozitnoj masi te se prenosi na adhezivnu vezu (2) i zubnu površinu, što stvara otklon kvržica (3) te pukotine u okolnoj caklini i dentinu (4). Sve to pacijent doživljava kao postoperativnu preosjetljivost (5). Polimerizacijski stres može kompromitirati vezu zuba i restauracije te završiti bakterijskim mikrocurenjem (6) i na kraju rubnim obojenjem, sekundarnim karijesom, upalom zubne pulpe i nekrozom (7)

    A Core Curriculum in the Biological and Biomedical Sciences for Dentistry

    INTRODUCTION: The biomedical sciences (BMS) are a central part of the dental curriculum that underpins teaching and clinical practice in all areas of dentistry. Although some specialist groups have proposed curricula in their particular topic areas, there is currently no overarching view of what should be included in a BMS curriculum for undergraduate dental programmes. To address this, the Association for Dental Education in Europe (ADEE) convened a Special Interest Group (SIG) with representatives from across Europe to develop a consensus BMS curriculum for dental programmes. CURRICULUM: This paper summarises the outcome of the deliberations of this SIG and details a consensus view from the SIG of what a BMS curriculum should include. CONCLUSIONS: Given the broad nature of BMS applied to dentistry, this curriculum framework is advisory and seeks to provide programme planners with an indicative list of topics which can be mapped to specific learning objectives within their own curricula. As dentistry becomes increasingly specialised, these will change, or some elements of the undergraduate curriculum may move to the post-graduate setting. So, this document should be seen as a beginning and it will need regular review as BMS curricula in dentistry evolve

    Case Definition in Chronic Periodontitis: the Reliability of Half-Mouth Protocols

    Aim of the study To compare half-mouth with whole-mouth examinations, based on clinical attachment loss (CAL) and periodontal pocket depth (PPD) assessments. Material and methods This pilot investigation was designed as a cross-sectional, pretreatment study. Twenty-one adult subjects with moderate and severe periodontal disease and no systemic diseases were recruited from a private dental clinic, Bucharest, Romania. All patients had at least 20 teeth. A total of 2040 interproximal sites were examined. Obtained data were expressed as number of sites with a specific measurement, per quadrant and per mouth. Finally, based on the percentage of patients, the results were compared between couples of quadrants and full-mouth recordings. Results No statistical significant differences between two-quadrant and full-mouth assessments were recorded, in terms of PPD and CAL measurements. Conclusions In the present study, random half-mouth protocols were considered reliable to reproduce full-mouth examination, giving good estimations for case prevalence

    CHRONIC MARGINAL STRESS AND PERIODONTITIS

    Objectives. The aim of our study was to evaluate salivary cortisol levels and the scores of nicotinic social dependence, as markers of stress, in a group of patients with moderate and profound chronic periodontitis but systemically healthy. Material and method. Our study, of pretreatment cross-sectional design, included 28 adult patients with moderate and severe chronic marginal periodontitis and no systemic diseases. They were recruited from a particular clinic in Bucharest. The questionnaire used gathered questions from the Kano test to assess nicotine dependence. The salivary cortisol was determined using a DSNOV20 kit (NovaTec Immundiagnostica GmbH). Results and discussions. The average Kano total score was 13.28 (+ 4.38; range 4-23). The smoker group recorded an average of 16.2 (range 11-23). There were no statistically significant differences between the scores of the three groups. From the 28 patients who completed the questionnaires, 22 were evaluated for salivary cortisol levels. Of these, 3 were smokers, 6 were former smokers and 13 were non-smokers. The mean salivary cortisol level was 6.65 ng/ml (+ 1.47; range 4.12-9.38). The mean salivary cortisol level was higher in smokers compared to non-smokers and ex-smokers (p> 0.05). Conclusions. Stress, expressed by the average level of salivary cortisol, was higher among smokers compared to the other groups, the highest variability being registered among ex-smokers. In patients with moderate and severe chronic marginal periodontitis, there was a positive, albeit insignificant, association between salivary cortisol levels and nicotinic psychological dependence

    Ecological Study on the Oral Health of Romanian Intellectually Challenged Athletes

    This study aimed to give an image of the oral health status and treatment needs of intellectually challenged athletes taking part in Special Olympics&mdash;Special Smiles (SO-SS) events organized in Romania during 2011&ndash;2019 and to find potential associations with socio-economic factors. An observational ecological retrospective study was conducted, and 1860 oral examinations were performed on participants aged 8 to 30 years in nine SS-SO events. Oral examination was performed under field conditions. Percentage of caries-free subjects, mean DMF-T index and its components (D = decayed; M = missing; F = filled; T = teeth), restoration index RI = [F/(F + D) &times; 100]%, and Plaque Index were calculated for each of the nine groups. Caries-free subjects ranged between 5.90% and 21.70%. DMF-T ranged from 4.27 to 7.71. Higher values for &ldquo;F&rdquo; component (range 0.66&ndash;1.69) and RI (range 13.02&ndash;27.74%) were found in events held in cities from regions with higher reported Gross Domestic Product. Sealants were present in 0 to 8.4% of the subjects. In areas with lower numbers of inhabitants per dentist, more sealants were found (p &lt; 0.001). Romanian SO athletes exhibited relatively poor oral health, limited access to dental treatment, and low level of prevention.
