    Extramedullary hematopoiesis in ovary: a rare presentation of CML

    Extramedullary hematopoiesis (EMH) is a rare phenomenon, and represents infiltration and proliferation of myeloid, erythroid, and megakaryocytic cells in non-bone marrow sites. Extramedullary hematopoiesis (EMH) is normal during fetal life, but after birth, the presence of EMH is considered to be abnormal.  EMH has been associated with CML (chronic myeloid leukaemia). Lymph node is the most common site of involvement, other sites being the abdomen (liver, spleen) and thorax (bone, mediastinum) but very rarely in the gynecological tract. Here authors report a case of a 20-year young female, a known case of CML who presented with abdominal pain with hemoperitonium, with negative urine pregnancy test. Initially hemorrhagic corpus luteal cyst was thought to be the cause of hemoperitonium and patient was taken for surgery wherein histopathology confirmed the diagnosis of extra medullary hematopoiesis of ovary. Women with CML when presents with hemoperitoneum, extramedullary hematopoiesis should be a differential diagnosis apart from rupture ectopic and other causes of spontaneous hemoperitoneum.  Patient was started on imatinib after diagnosis and is doing well

    Aqueous Bark Extract of Terminalia Arjuna Protects Against Cigarette Smoke-induced Alterations in Serum Lipid Profile of Albino Rats

    The present study aimed to evaluate the protective effect of aqueous bark extract of Terminalia arjuna against cigarette smoke-induced changes in serum lipid profile of albino rats. The rats were divided into three groups. One control group (I) and two experimental groups (II and III), all three groups consisting of five rats each. The control group (I) was unexposed to cigarette smoke. Experimental group (II) was exposed to cigarette smoke (6 cigarettes) for one hour/day for 30 days and experimental group (III) was exposed to cigarette smoke along with oral administration of aqueous bark extract of Terminalia arjuna (5mg/rat/day) for 30 days. The findings showed a significant (P<0.01) increase in serum cholesterol, serum triglyceride, low-density lipoprotein (LDL) and very-low-density lipoprotein (VLDL) with significant (P<0.01) decrease in high-density lipoprotein (HDL) level in cigarette smoke-exposed albino rats in comparison to control rats while a significant (P<0.01) decrease in serum cholesterol, serum triglyceride, low-density lipoprotein (LDL) and very-low-density lipoprotein (VLDL) with significant (P<0.01) increase in high-density lipoprotein (HDL) after oral administration of aqueous bark extract of Terminalia arjuna in comparison to cigarette smoke-exposed albino rat

    Quantum size effects and tunable visible photoluminescence in a-Si:H/nc-Si:H superlattices

    Quantum size effects are commonly observed in semiconductor nanocrystals and quantum dots. Here, we demonstrate unexpected quantum size effects in an unusual bulk system with multiple interfaces, consisting of alternating layers of nanocrystalline silicon (nc-Si:H) and amorphous silicon (a-Si:H) material thin films. The nc-Si:H layers consist of silicon nanocrystals embedded in an amorphous matrix, with an amorphous-crystalline interface separating the two structures. Plasma-enhanced chemical vapor deposition was utilized to grow nanocrystalline-amorphous silicon superlattices with a varying thickness of the nanocrystalline layer. Strong visible photoluminescence at room temperature was deconvoluted into individual peaks. As the nanocrystalline silicon layer thickness was increased from 5 to 20 nm, the photoluminescence spectra red-shifted with the emission wavelength varying as d2 (d is the size of the nanocrystallites), the characteristic signature underlying quantum size effects. The size d of the nanocrystals was estimated by the measured shift of the Raman peak, and could be tuned by varying the thickness of the nc-Si:H layers. High resolution transmission electron microscopy show nanocrystals with a narrow size distribution, in an amorphous matrix. We also observe long wavelength photoluminescence from interfacial states that leads to persistent photconductivity.     Background: The kidney is a structurally complex organ that has evolved to subserve a number of important functions: excretion of the waste products of metabolism, regulation of body water and salt, maintenance of appropriate acid balance, and secretion of a variety of hormones and autacoids. Some clinical disorders affect more than one structure. In addition, the anatomic interdependence of structures in the kidney implies that damage to one almost always secondarily affects the others. Thus, severe glomerular damage impairs the flow through the peritubular vascular system, conversely, tubular destruction, by increasing interglomerular pressure, may induce glomerular atrophy. Thus, whatever the origin, there is a tendency for all forms of chronic renal disease ultimately to destroy all four components of the kidney, culminating in chronic renal failure and what has been called end-stage contracted kidneys. The functional reserve of the kidney is large, and much damage may occur before functional impairment is evident. The circulating immune complexes play a very major role in various types of glomerular nephropathies.Methods: The present study was conducted on renal biopsies referred to Pathology Department of G.S.V.M. Medical College, Kanpur and Regency Hospital Ltd., Kanpur. Frozen section of renal biopsy was taken for IF studies. Renal biopsy tissue was received in IF fluid containing Ammonium sulphate, N-ethyl malcimide, Magnesium sulphate.Results: The lgG class of immunoglobulins was found to be most fatal to the G13M, the 1gM and IgA were also found to cause glomerular damage. This mechanism was seen responsible for most cases of ICGN. In the present study, fluorescent study of renal biopsy tissue was also done using Hollande’s fixative and it was observed that a better diagnosis could be done when used with routine H & E and immunofluorescent studies.Conclusions: The immunofluorescence microscopy proved to be very useful and essential, for proper diagnosis and therapy of a renal disease. IFM comes out to be a very good indicator of the deposition site and class of immunoglobulin involved in the Immune-complex deposit

    Pyrexia of unknown origin: a rare presentation of primary ovarian lymphoma

    It is very rare to have a lymphomatous involvement of ovary. Malignant lymphoma of ovary is a well-known late manifestation of disseminated nodal disease. Primary ovarian lymphoma with ovarian mass as an initial manifestation is a rare entity and may have varied presentations which can cause confusion to the physician and cause delay in diagnosis. Study presents a case of non-Hodgkin’s lymphoma where the initial presentation was fever with weight loss, and was evaluated as pyrexia of unknown origin. When no other cause of fever was identified PET-CT was done showing metabolically active uterine mass with no lymphadenopathy. Exploratory laparotomy was planned followed by hysterectomy with bilateral salpingo ophorectomy with omentectomy. Ovarian malignancy was detected intraoperatively, which was diagnosed as diffuse large B cell lymphoma, NHL double expresser phenotype on histopathology and IHC. Patient was started on chemotherapy and is doing fine

    Physiological, Anatomical and Metabolic Implications of Salt Tolerance in the Halophyte Salvadora persica under Hydroponic Culture Condition

    Salt tolerance mechanism of an extreme halophyte Salvadora persica was assessed by analysing growth, nutrient uptake, anatomical modifications and alterations in levels of some organic metabolites in seedlings imposed to various levels of salinity (0, 250, 500 and 750 mM NaCl) under hydroponic culture condition. After 21 days of salt treatment, plant height, leaf area and shoot biomass decreased with increase in salinity whereas the leaf succulence increased significantly with increasing salinity in S. persica. The RWC% of leaf increased progressively in salt-treated seedlings as compared to control. Na+ contents of leaf, stem and root increased in dose-dependent manner whereas there was no significant changes in K+ content. There was significant alterations in leaf, stem and root anatomy by salinity. The thickness of epidermis and spongy parenchyma of leaf increased in salt treated seedlings as compared to control, whereas palisade parenchyma decreased dramatically in extreme salinity (750 mM NaCl). There was a significant reduction in stomatal density and stomatal pore area of leaf with increasing salinity. Anatomical observations of stem showed that the epidermal cells diameter and thickness of cortex decreased by salinity whereas thickness of hypodermal layer, hypodermal cell diameter, pith area and pith cell diameter increased by high salinity. The root anatomy showed an increase in epidermal thickness by salinity whereas diameters of epidermal cells and xylem vessels decreased. Total soluble sugar content remained unchanged at all levels of salinity whereas reducing sugar content increased by 2-fold at high salinity (750 mM NaCl). The starch content of leaf decreased progressively in NaCl treated seedlings as compared to control. Total free amino acid content did not change at low salinity (250 mM), whereas it increased significantly at higher salinity (500 and 750 mM NaCl). The proline content increased in the NaCl treated seedlings as compared to control. There was no significant changes in polyphenol levels in leaf at all levels of salinity. The results from the present study reveal that seedlings imposed with various levels of salinity experience physiological, biochemical and anatomical modifications in order to circumvent under extreme saline environment.     A comparative study of stool antigen Immunocard STAT HpSA test with biopsy in diagnosis of H Pylori Infection

    The present study was conducted on 78 cases in the Department of Medicine at G.S.V.M Medical College, Kanpur who reported with various gastrointestinal symptoms such as dyspepsia, heartburns, nausea, vomiting and loss of appetite. They all were advised for biopsy by the physician. Patients treated with antibiotics, bismuth or proton pump inhibitors within 4 weeks preceding the study were excluded. Invasive tests using mucosal biopsies including histology and rapid urease test (RUT) were used to establish the gold standard in order to evaluate the accuracy of Immunocard STAT HpSA. Sensitivity, Specificity, Positive Predictive Value and Negative Predictive Value of Immunocard STAT HpSA as compared to gold standard were 95.5%, 81.8%, 96.9% and 75.0% respectively. It is thus concluded that the Immunocard STAT HpSA test has a diagnostic value comparable to the gold standard in detecting H Pylori. The sensitivity and specificity of the test is fair enough to be used as a test for screening purpose and also for diagnosis and treatment of H.Pylori infection in clinical practic

    Molecular profiling of ETS and non‐ETS aberrations in prostate cancer patients from northern India

    BACKGROUNDMolecular stratification of prostate cancer (PCa) based on genetic aberrations including ETS or RAF gene‐rearrangements, PTEN deletion, and SPINK1 over‐expression show clear prognostic and diagnostic utility. Gene rearrangements involving ETS transcription factors are frequent pathogenetic somatic events observed in PCa. Incidence of ETS rearrangements in Caucasian PCa patients has been reported, however, occurrence in Indian population is largely unknown. The aim of this study was to determine the prevalence of the ETS and RAF kinase gene rearrangements, SPINK1 over‐expression, and PTEN deletion in this cohort.METHODSIn this multi‐center study, formalin‐fixed paraffin embedded (FFPE) PCa specimens (n = 121) were procured from four major medical institutions in India. The tissues were sectioned and molecular profiling was done using immunohistochemistry (IHC), RNA in situ hybridization (RNA‐ISH) and/or fluorescence in situ hybridization (FISH).RESULTSERG over‐expression was detected in 48.9% (46/94) PCa specimens by IHC, which was confirmed in a subset of cases by FISH. Among other ETS family members, while ETV1 transcript was detected in one case by RNA‐ISH, no alteration in ETV4 was observed. SPINK1 over‐expression was observed in 12.5% (12/96) and PTEN deletion in 21.52% (17/79) of the total PCa cases. Interestingly, PTEN deletion was found in 30% of the ERG‐positive cases (P = 0.017) but in only one case with SPINK1 over‐expression (P = 0.67). BRAF and RAF1 gene rearrangements were detected in ∼1% and ∼4.5% of the PCa cases, respectively.CONCLUSIONSThis is the first report on comprehensive molecular profiling of the major spectrum of the causal aberrations in Indian men with PCa. Our findings suggest that ETS gene rearrangement and SPINK1 over‐expression patterns in North Indian population largely resembled those observed in Caucasian population but differed from Japanese and Chinese PCa patients. The molecular profiling data presented in this study could help in clinical decision‐making for the pursuit of surgery, diagnosis, and in selection of therapeutic intervention.
