중국 시장 개혁 담론의 변화: 개혁·개방 이후 국유기업 개혁을 중심으로

학위논문 (석사)-- 서울대학교 대학원 : 사회과학대학 정치외교학부, 2018. 2. 권형기.사회주의 시장경제 선언 이래 지속적으로 확대되었던 중국의 시장경제로의 개혁 담론이 변화하고 있다. 2000년대 중반 과학적발전관과 화해사회론이 주장되면서, 생산력 우선, 경제발전 중심의 개혁 노선보다는 평등과 배분 중심의 노선이 강조되기 시작하였고, 시진핑 집권 이후로는 여기에 당 중앙의 영도 확대를 지지하는 국가자본주의적인 발전전략이 더해져 이전 개혁 노선과 또 다른 차별성을 드러내고 있다. 그렇다면 그동안 강력하게 견지되어오던, 그리고 그 경제적 성과 또한 세계가 주목할 만한 것이었던 시장경제로의 개혁 노선에는 어떠한 변화가 생긴 것인가? 본 연구는 개혁의 실질적인 담론을 강조한다. 개혁과 같은 핵심 이념이 주요 행위자들에게 어떻게 해석되는지, 왜 그렇게 해석되는지를 파악하기 위해서는 해석 간의 상호작용이 일어나는 담론의 분석이 필요할 수밖에 없다. 예컨대 개혁을 둘러싼 담론이 처음부터 하나로 합의되어 있을 수 없다. 개혁이 발생하기 전에 어떤 것을 문제(위기)로 인식해야 하는지, 그 문제(위기)의 원인은 무엇인지, 개혁을 추진한다면 어디까지를 개혁의 범위로 삼아야 할지, 누가 개혁을 이끌어야 할지, 개혁의 목표는 무엇인지 등 개혁과 문제의 해석을 놓고 벌어지는 담론은 매우 다양할 수밖에 없기 때문이다. 1979년 개혁·개방이 시작될 때의 개혁, 1989년 톈안먼 사태가 발생했을 때 당의 개혁, 1992년 덩샤오핑(장쩌민)의 사회주의 시장경제의 개혁, 2006년 후진타오 과학적 발전관의 개혁, 2017년 시진핑 신시대 중국 특색의 사회주의 사상의 개혁 등 중국 공산당은 여전히 개혁을 주장하며 스스로 이전 시기를 계승하고 심화시켰다고 주장하지만 개혁의 의미는 환경과 제도에 대한 여러 인식들이 상호작용하는 담론을 통해 유의미하게 변화해왔다. 예컨대, 80년대 말과 90년대 말(2000년대 초), 불평등 심화라는 비슷한 사회적 갈등과 문제가 시위와 논쟁으로 표출되었으나, 이에 대한 중국 공산당의 대응책은 달랐다. 80년대 말 불평등 심화 문제 뒤에는 신계몽주의, 자유주의의 사상계 담론이 학생운동을 뒷받침하면서 문제에 대한 원인을 개혁을 제대로 시행하지 못한 부패한 관료들과 문화혁명의 잔재로 해석하는 것이 주도적이었고, 그 결과 학생운동은 경제적 개혁을 심화시켜 정치적 개혁까지 요구하는 것으로 발전되었다. 따라서 덩샤오핑의 남순강화와 이후 시장 개혁 심화 담론은 80년대 말 사회적 불만에 반대되는 것이 아니라 오히려 경제적인 측면에서는 이를 긍정하는 결과였다. 반면 90년대 말부터 고조된 불평등 문제에서는 80년대와 다르게 신좌파 지식인들을 중심으로 반(反) 자유주의적 담론이 대중들에게 급격하게 힘을 얻었다. 이러한 담론은 국유기업 재국유화를 부상시킨 랑구논쟁을 점화하면서 개혁 평가 논쟁을 부추겼고, 후진타오의 과학적발전관과 화해사회론의 이론적 토대가 되었다. 그리고 2000년대 말 발생한 충칭 모델 담론은 화해사회론과 과학적발전관의 성공적 모델로 부상하였고, 이후 덩샤오핑의 생산성 발전 우선 발전전략은 평등과 분배의 전면적 소강사회를 건설하는 국가주의적 발전 전략으로 변화하게 되었다. 이처럼 중국의 경제 개혁은 생산력 중심의 합리성만을 가지고 진행되었다거나, 제도와 환경에 따른 일관적이고 경로의존적인 방향으로 진행되었다고만 볼 수 없고, 오히려 내부적 담론을 통해 개혁의 실질적 의미와 해석이 변화하는 과정을 경험했다고 보는 관점이 타당하다. 이 점에서 본 연구는 기존 연구가 시장 경제 개혁의 성과를 긍정하는 신자유주의적 논리, 중국의 제도적 특수성을 강조한 중국식 발전 모델에 집중한 것에 반하여, 중국 개혁의 의미가 내생적으로 변화하는 과정을 간과하고 있음을 지적하고, 이 변화를 분석하기 위해서 담론제도주의적 접근을 제시한다. 그리고 분석 위해 중국 시장 경제 개혁의 주요 쟁점인 공유제와 사유제, 그리고 그 주요 행위자인 국유기업을 둘러싼 담론 논쟁에 초점을 맞추고 있다.제 1 장 서론 1 제 1 절 문제제기 1 제 2 절 연구배경 6 제 2 장 선행연구 및 이론적 검토 12 제 1 절 이론적 검토 12 1. 세계화와 신자유주의 12 2. 대안적 이론: 담론 제도주의 15 제 3 장 개혁·개방 담론의 형성과 발전(1978~2002) 21 제 1 절 개혁·개방 초기 국유기업 개혁과 경제 개혁 담론 23 1. 국유기업 개혁의 역사적 경로 23 2. 개혁 담론의 변화 28 제 2 절 톈안먼 사태와 개혁의 심화(1986~2002) 37 1. 톈안먼 사태와 정치 보수화 37 2. 남순강화와 시장경제 개혁 46 제 4 장 개혁 담론의 분화와 변화(2003~) 57 제 1 절 시장경제 개혁의 심화와 3개대표론 58 1. 국유기업의 시장화 개혁 58 2. 3개대표론과 자유주의 60 제 2 절 개혁 담론의 변화: 과학적발전관과 화해사회론 63 1. 후진타오 집권 1기: 과학적발전관과 화해사회론 65 2. 후진타오 개혁의 담론적 배경과 담론의 심화 72 제 5 장 결론 84Maste

Impact of thyroid cancer on the cancer risk in patients with non‑alcoholic fatty liver disease or dyslipidemia

The raised prevalence of obesity has increased the incidence of obesity-related metabolic diseases such as dyslipidemia (DL) and non-alcoholic fatty liver disease (NAFLD), along with the development and progression of various types of cancer, including thyroid cancer. In this study, we investigated whether thyroid cancer in patients with DL and NAFLD could be a risk factor for other cancers. To achieve our goal, we generated two independent cohorts from our institution and from the National Health Insurance System in South Korea. Based on the ICD-10 code, we conducted exact matching (1:5 matching) and estimated the overall risk of thyroid cancer for other cancers in patients with DL or NAFLD. Univariate and multivariate analyses showed that the hazard ratio (HR) of thyroid cancer was 2.007 (95% Confidence Interval [CI], 1.597-2.522) and 2.092 (95% CI, 1.546-2.829), respectively in the institutional cohort and 1.329 (95% CI, 1.153-1.533) and 1.301 (95% CI, 1.115-1.517), respectively in the nationwide cohort. Risk analysis revealed a significant increase in the HR in lip, tongue, mouth, lung, bone, joint, soft tissue, skin, brain, male cancers and lymphoma after thyroid cancer occurred. Thyroid cancer in patients with DL or NAFLD might be a valuable factor for predicting the development of other cancers.ope

Re-do Operation Using a Robotic System due to Locoregional Recurrence after Initial Thyroidectomy for Thyroid Cancer

Locoregional recurrent thyroid cancer is commonly treated with re-do operation. This study aimed to investigate the feasibility of using robotic system for re-do operation in locoregional recurrent thyroid cancer. Sixty-five patients who underwent re-do robotic operation using trans-axillary approach for locoregional recurrent thyroid cancer from October 2007 to April 2021 at Yonsei University Hospital were analyzed. Completion total thyroidectomy (CTT) was performed in 26 cases, CTT and modified radical neck node dissection (mRND) in 16, and mRND in 23. Most of the re-do robotic operations were performed at site of previous incision. All patients were diagnosed with papillary thyroid carcinoma (PTC). CTT with central compartment neck dissection (CCND) took 117.6 ± 26.3 min, CTT with mRND 255.6 ± 38.6 min, and mRND, 211.7 ± 52.9 min. Transient hypocalcemia occurred in 17 (26.2%) patients and permanent hypocalcemia occurred in 3 (4.6%). There was one case of recurrent laryngeal nerve(RLN) injury. One patient was diagnosed with structural recurrence after re-do robotic operation. Median follow-up duration was 50.7 ± 37.1 months. Re-do robotic operation can be an alternative for patients who are diagnosed with locoregional recurrent thyroid cancer after thyroidectomy, with no increase in morbidity, similar oncologic outcomes, and superior cosmetic satisfaction.ope

Unexpected remission of hyperparathyroidism caused by hemorrhage due to the use of fine-needle aspiration biopsy: two cases report

Hyperparathyroidism is not a rare disease; if a parathyroid adenoma is confirmed, the treatment of choice is the surgical resection. Diagnostic use of fine-needle aspiration biopsy (FNAB) for histological confirmation in patients with hyperparathyroidism is controversial. And spontaneous remission of hyperparathyroidism caused by bleeding or infarction of the adenoma rarely occurs. Here we have reported two cases of hyperparathyroidism in which spontaneous remission occurred due to the use of FNAB for diagnosis. The remission was confirmed after surgical removal and pathological review of the adenoma. The first patient diagnosed with primary hyperparathyroidism (PHPT) had neck pain and severe swelling 4 days after FNAB, and spontaneous remission due to intracapsular hemorrhage was confirmed after surgery. In the second patient receiving hemodialysis treatment for end-stage renal disease, hyperparathyroidism spontaneously resolved after FNAB and the parathyroid hormone (PTH) levels normalized after surgery. The first patient maintained a normal level of PTH for 6 years, and the second patient received kidney transplantation 6 years after surgery, and the normal level of PTH was confirmed for 13 years. Caution is needed while performing FNAB for diagnosis of hyperparathyroidism and during decision making regarding whether to observe the patient or perform surgery after spontaneous remission due to bleeding or infarction.ope

Optimal Cut-Off Values of Lymph Node Ratio Predicting Recurrence in Papillary Thyroid Cancer

Regional lymph node (LN) metastasis has a significant impact for prediction of recurrence in patients with papillary thyroid cancers (PTC); however, the prognostic value of the lymph node ratio (LNR), which is defined as the ratio of the number of metastatic LNs to the total number of investigated LNs, is controversial. In this study, we determined the optimal cut-off values of LNRs for the prediction of recurrence in PTC patients.This large cohort study retrospectively evaluated 2294 patients who had undergone total thyroidectomy for PTC at a single institution from October 1985 to June 2009. The prediction probability of central LNR (cLNR, level VI) and total LNR (tLNR, levels II-VI) were estimated by binominal logistic regression analysis. Hazard ratios of the cut-off LNR values for cancer recurrence were calculated for relevant covariates using multivariate Cox regression analyses. Kaplan-Meier analyses were also utilized to assess the effects of estimated LNR cut-off values on recurrence-free survival (RFS).Of the 2294 patients, 138 (6.0%) presented cancer recurrence during the follow-up period (median duration = 107.1 months). The prediction probability indicated that LNRs of 0.4 and 0.5 for central LN and total LN, respectively, are optimal cut-off values for precise prediction with minimization of outliers. Multivariate Cox regression analyses revealed that cLNR ≥0.4 was independently predictive of recurrence in patients with N0 and N1a PTCs (hazard ratio [HR]: 7.016, 95% confidence interval [CI]: 3.72-12.986, P < 0.001) and that tLNR ≥0.5 indicated a significantly increased risk of recurrence in patients with N1b PTCs (HR: 2.372, 95% CI: 1.458-3.860, P < 0.001). In addition, Kaplan-Meier analyses clearly demonstrated that these LNR cut-off values are precisely operational in RFS estimation.The cut-off LNR values of 0.4 and 0.5 for cLNR and tLNR, respectively, were identified. Risk stratification combined with these LNR cut-off values may prove useful to determine treatment and follow-up strategies for PTC patients.ope

Parathyroid venous sampling for the preoperative localisation of parathyroid adenoma in patients with primary hyperparathyroidism

Preoperative localisation studies are essential for parathyroidectomy in patients with primary hyperparathyroidism. If the location of abnormal parathyroid glands cannot be identified through non-invasive studies, parathyroid venous sampling (PVS) may be employed. In this study, we evaluated the utility of preoperative PVS in parathyroid surgery. Patients with primary hyperparathyroidism who underwent preoperative PVS at Severance Hospital between January 2015 and June 2020 were identified. Patients for whom the results of non-invasive imaging studies were inconsistent or negative underwent PVS. The results of PVS were compared with operative findings and pathologic results. For 14 patients, the results of preoperative ultrasonography and 99mTc-sestamibi single-photon emission computed tomography (SPECT) were negative; for 20 patients, either the result of only one test was positive, or the results of the two tests were inconsistent. With respect to the lateralisation of diseased adenoma, the results of PVS and pathological examination were inconsistent only for one patient in either group (total: 2/34 patients). This study showed that PVS could be used effectively for preoperative localisation in patients with primary hyperparathyroidism in whom the location of diseased parathyroid glands cannot be determined through non-invasive image studies.ope

공기업 기관장의 교체가 경영성과에 미치는 영향

학위논문 (석사)-- 서울대학교 행정대학원 : 공기업정책학과, 2015. 8. 이수영.본 연구의 첫 번째 주제는 공기업 기관장의 교체가 경영성과에 어떠한 영향을 미치는가에 관한 것이고, 두 번째 주제는 기관장이 교체된 경우 후임기관장이 내부승진인지 아니면 외부인사영입인지 그리고 전문성을 보유하고 있는지 아니면 전문성을 보유하고 않은지에 따라 경영성과에 어떠한 영향을 미치는 가에 관한 것이었다. 이는 민간기업과 관련하여 활발하게 연구되고 있는 최고경영자의 교체가 경영성과에 미치는 영향에 관한 연구들을 공기업에 적용시켜 보려는 시도였다. 분석결과 기관장이 교체된 경우 그렇지 않은 경우에 비해 정부경영평가 등급 및 총자산수익율이 낮은 것으로 나타났다. 즉 정부경영평가 등급의 경우 기관장이 교체된 경우 그렇지 않은 경우에 비해 0.594만큼 낮은 것으로 나타났는데, 이는 1%의 유의수준에서 유의하였다. 총자산수익율은 10%의 유의수준에서 기관장이 교체된 경우 그렇지 않은 경우에 비해 0.0225만큼 낮은 것으로 나타났다. 기관장이 임기만료 전에 교체된 경우 또한 그렇지 않은 경우에 비해 정부경영평가 등급 및 총자산수익율이 낮은 것으로 나타났는데, 정부경영평가 등급과 총자산수익율 모두 1%의 유의수준에서 각각 0.714, 0.0397만큼 낮았다. 한편 기관장이 교체된 경우와 기관장이 임기만료 전에 교체된 경우 모두 정부경영평가 등급에 부정적인 영향을 미치는 것으로 나타났다. 기관장이 교체된 경우는 5%의 유의수준에서, 기관장이 임기만료 전에 교체된 경우는 1%의 유의수준에서 정부경영평가 등급에 부정적 영향을 미쳤다. 한편 기관장이 교체된 경우 후임기관장이 내부승진자인 경우 5%의 유의수준에서 부정적인 영향을 미쳤다. 우선 첫 번째 주제와 관련하여 공기업 기관장이 임기만료 전에 교체되는 경우는 물론이고 기관장이 교체되는 경우에는 경영성과에 부정적 영향을 미친다는 사실을 발견하였다. 이를 통해 최고경영자의 교체가 경영성과에 부정적 영향을 미친다는 이론이 공기업에 있어서 타당하다는 것을 알 수 있었다. 특히 공기업의 기관장은 그 임기가 공운법상 3년으로 정해져 있어 기관장의 교체를 쉽게 예측할 수 있고 기관장의 교체에 대한 대비를 할 수 있음에도 불구하고 이와 같은 결과가 발생한 것은 공기업의 특성상 제도적·정치적 환경에 적응할 수밖에 없는 이유가 원인이 된 것으로 판단된다. 정부는 공기업 기관장의 독립성, 전문성 등을 보장하기 위해 2007년 공공기관의 운영에 관한 법률의 제정을 통해 제도적인 노력을 기울이고 있으나 이를 현실적으로 실현하기 위한 강도 높은 노력이 필요한 것으로 보인다. 공기업 기관장이 비경제적인 사유로 교체되거나 법에서 정한 임기를 마치지 못하고 교체된다면 제도의 의미가 사라질 것이기 때문이다. 정부경영평가와 같은 객관적이고 경제적인 기준에 따라 기관장이 교체되고 기관장의 법정 임기도 충실히 지켜지는 등 관련 법에서 규정한 내용에 따라 기관장 교체가 이루어져야 할 것이다. 또한 임기가 만료한 경우라도 무조건적으로 기관장을 교체하는 것이 아니라 재임기간 중의 경영실적을 종합적으로 평가하여 그 실적이 우수한 경우에는 법령상의 연임규정을 적극적으로 적용할 필요도 있을 것이다. 공기업 기관장이 교체되는 경우 경영성과에 부정적인 영향을 미친다면 과연 어떠한 사람이 후임기관장으로 임명되는 것이 바람직할 것인가에 대한 문제제기가 본 연구의 두 번째 주제였다. 외부인사가 부정적인 영향을 미칠 것이라는 예상과는 달리 연구결과 오히려 내부인사가 후임기관장이 되는 경우 정부경영평가 등급에는 부정적인 영향을 미쳤다. 비록 당해 년도 경영성과에 국한되기는 하나, 외부에서 영입된 기관장이라도 경영성과에 미치는 영향이 긍정적일 수 있다는 것이다. 따라서 외부인사를 공기업 기관장으로 임명하는 것을 무조건적으로 비판하는 것은 옳지 않다. 공기업 기관장 임명시 단순히 외부인사라고 하여 부정적인 시각으로만 바라볼 것이 아니라 객관적이고 공정한 기준을 마련하여 해당 공기업에 가장 적합한 인물을 선임하도록 하는 것이 필요하다. 현재 공공기관의 운영에 관한 법률 제30조는 기관장의 자격요건으로 기업경영과 그 공기업의 업무에 관한 학식과 경험이 풍부하고 최고경영자의 능력을 갖춘 자로 규정하여 다소 추상적이기는 하지만 전문성을 요구하고 있다. 기관장 임명에 있어 전문성이 가장 중요한 기준으로 작용하고 있겠지만, 이밖에도 일반경영관리능력, 비전 및 혁신 역량, 대내외 네트워크 등 최고경영자의 능력에 대한 보다 세밀한 기준을 마련하여 적용하는 것이 필요할 것으로 보인다.제 1 장 서 론 1 제 1 절 연구의 필요성 및 목적 1 제 2 절 연구의 대상 및 범위 3 제 3 절 연구의 방법 5 제 4 절 자료의 수집 6 제 2 장 이론적 배경 및 선행연구 7 제 1 절 이론적 배경 7 제 2 절 선행연구 9 1. 최고경영자의 교체가 경영성과에 미치는 영향에 관한 연구 10 2. 최고경영자의 교체시 후임자의 유형이 경영성과에 미치는 영향에 관한 연구 15 3. 선행연구의 검토 21 제 3 장 연구의 설계 22 제 1 절 연구의 모형 22 1. 서론 22 2. 연구가설 23 제 2 절 변수의 정의 30 1. 종속변수 30 2. 독립변수 32 3. 통제변수 34 제 4 장 연구의 결과 36 제 1 절 기초통계 분석 36 1. 종속변수 : 경영성과 36 2. 독립변수 : 기관장의 교체 및 후임기관장의 유형 37 3. 통제변수 : 자산, 매출액, 인원, 연령, 전년도 경영성과 41 제 2 절 기관장 교체 및 후임자 유형에 따른 차이 분석 42 1. 기관장 교체에 따른 경영성과의 차이 42 2. 후임자 유형에 따른 경영성과의 차이 45 제 3 절 기관장 교체 및 후임자 유형이 경영성과에 미치는 영향 분석 47 1. 기관장 교체가 경영성과에 미치는 영향 47 2. 후임자 유형이 경영성과에 미치는 영향 54 제 4 절 소결 58 제 5 장 결론 66 제 1 절 연구결과 요약 66 제 2 절 연구의 시사점 67 제 3 절 연구의 한계와 향후 연구 69 참고문헌 71 Abstract 74Maste
