Previously, the diagnostic value of IgG reactivity to lipoarabinomanan(LAM) and PPD antigens in cerebrospinal fluid was presented. However, we could net exclude the possibility of passive transfer of IgG from the systemic circulation to the cerebrospinal fluid through the blood brain barrier. We studied the de nove synthesis of IgG and the diagnostic value of IgG antibody titer to PPD and LAM antigens. The following results were obtained. 1. Intrathecally, de novo synthesis of IgG was existed in tuberculous meningitis, because IgG synthetic rate were higher than control group siglificantlly. 2. Seropositivity to PPD and LAM antigens in cerebrospinal fluid were possible in abnormally high titer Of IgG in peripheral blood although the patients were in control group. 3. To prove that cerebrospinal IgG reactive to LAM and PPD antigens is diagnostic for tuberculous meningitis, demonstration of de novo synthesis of IgG in central nervous system is the essential prerequisite.
