유방암 환자의 적응과정에 관한 연구

간호학과/박사[영문] [한글] 암 진단을 받은 환자는 정서적으로 심한 충격을 받는 것으로 나타나 있으나 임상에서 이루어지는 암 환자 간호는 신체적 안위 간호에 국한되었고 환자와의 대화 없이 간호사 일방의 판단에 의해 간호가 이루어짐을 볼 수 있었다. 이러한 암 환자 간호를 비판적으로 볼 때 여러 가지 요인이 작용하고 있는 것을 알 수 있다. 그것은 암 환자를 전문으로 간 호하는 숙련된 간호사의 부족과 병원에서의 간호인력 부족 및 간호들의 암 환자에 대한 이해와 지식 부족으로 암 환자 간호에 한계를 느끼고 있기 때문이라 생각된다. 특히 우리 나라 실정에는 의학적으로 선진인 서구사회와 사고방식이나 생활습관및 건강 에 관한 인식상의 차이가 있음에도 불구하고 서구의 연구나 문헌에 나타난 지식을 점검의 과정 없이 우리 나라 임상에 적응시키고 있는 실정이므로 암 환자 간호에 필요한 실재적 간호 지식이 부족할 수밖에 없다. 이 연구에서는 암 진단을 받고 치료받는 과정 동안에 암 환자가 겪게 되는 정서적 경험과 적응 과정을 그대로 파악하여 기술하고 설명함으로써 암 환자를 이해하고 간호하는데 도움이 되게 할뿐만 아니라 간호학의 탈 서구화에 미력 이나마 기여하고자 한다. 또한 이 연구는 우리 나라의 유방암 환자를 대상으로 하여 자의생활현장을 중심으로 자 료를 수집, 분석하였으므로 유방암 환자 간호에 필요한 실재적 지식을 제공하여 전인 간 호수준 향상에 기여할 수 있으리라 본다. 연구의 방법론적 측면에서도 심층면접 및 관찰 을 이용한 질적 연구방법을 활용하여 연구함으로써 실무에 적용 가능한 간호 이론 개발과 우리 사회에 적합한 간호 연구 방법 개발에 이바지할 수 있으리라 기대된다. 연구 대상은 유방암으로 착진 받은 환자 중환자 본인이 유방암 환자라는 사실을 알고 있으며 암 진단을 받은 후 1년 이내인 53명의 여성이다. 현장 조사 기간은 1990년 6월부터 1991년 2월까지이며 연구방법은 심충면담.전화면담 및 관찰을 통하여 자료를 수집하였고, 의무 기록 지를 참고로 하였다. 수집된 자료는 자 료 수집과 동시에 비교 분석하여 새로운 조사의 지평을 열어 가는 식으로 실시하였다. 조사 결과 유방에 종양을 발견한 이후부터 치료 진행 시기별로 대상자가 겪는 정서적 경험이 변화되어 가는 특징적 반응을 유형화해 볼 수 있었다. 즉, 유방암환자는 종양 발 견 후 진단 전까지 매우 불안 초조해 하였으며 암 진단을 받은 후 수술 전까지는 절망 상 태에 빠지고, 수술 후 보조 치료 전까지 불안하면서도 약간의 희망을 갖게되나, 그후 보 조 치료 동안에는 질병과의 투쟁의지를 보였다. 치료가 끝난 후에는 희망을 가지고 안정 이 되는 것으로 나타났으나 계속하여 재발에 대해 염려하고 불안해하였으며, 생명 유지에 대한 확신이 없는 불확실한 상황 속에서 인생관과 삶을 살아가는 태도에 변화가 나타났 다. 유방암 환자의 적응에 관련된 변수는 환자의 개인적 특성, 상황적 요소 및 환자가 사용 하는 극복 전략으로 나누어 불 수 있었다. 개인적 특성으로는 질병 과정에 대한 인식, 교 육 정도, 종교, 성격. 신체적 건강 상태, 경제적 상태이며. 상황적 요인으로는 환자가 가 진 개인적 지원 체계, 의료진의 지원.환자 사이의 상호작용이었다. 환자가 질병을 극복하 기 위하여 사용하는 극복 전략으로는 주의환기를 통한 기분 풀이. 육체적 노동을 통한 극 복과 비교를 통한 심리적 안정 얻기가 있었다. 이 요인을 구체적으로 들어보면 암 판자의 암에 대한 인식이 죽음과 연관될수록, 자신 의 상태가 암이 진행되고 있다고 생각할수록, 유방제거 후 느끼는 불구의 느낌이 클수록 적응이 어려웠다. 환자의 교육 정도가 높아 어려운 상황을 해결할 능력이 있을수록 적응 을 잘하는 것으로 나타났으며. 환자가 종교를 가지고 신에게 의지할수록 적응을 잘 하였 다. 환자의 성격이 강하고 담담할수록, 신체적으로 건강할수록, 경제적 상태가 좋을 수록 환자의 적응에 도움이 되었다. 환자의 가족이나 친지, 친구가 육체적 정신적·물질적 도 움을 줄수록. 의료진이 관심을 가지고 지지할수록, 환자 사이의 상호작용이 잘 이루어질 수록 환자는 적응을 잘하였으며, 환자가 사용하는 극복 전략이 효과적일 때 적응을 잘 하 였다. 환자의 적용에는 이러한 변수들이 복합적으로 작용하여 환자의 적용상태를 결정하 는 것으로 나타났다. 특히, 암·죽음·수술·유방제거에 대한 환자의 인식이 환자의 적응 에 크게 영향을 주었으며, 환자가 가진 개인적 지원체계 중에서 판자 남편의 지원이 중요 한 것으로 나타났다. 결과적으로, 잘 적용하는 유방암 환자의 경우는 종양을 발견한 이후 즉시 병원을 찾고, 수술을 받고, 치료 계획을 잘 따르고, 가족이나 다른 사람과의 대인 관계를 원만히 지속 하고. 자신의 감정을 표현하면서 스스로 마음을 안정시킬 행위를 자발적으로 찾아하고, 신체적 회복을 도모하면서, 더 오래 살기 위한 노력을 하고, 치료 중에도 자신이 할 수 있는 정도에서 집안 일을 계속 돌보는 환자로 특징 지을 수 있었다. 따라서 유방암 환자를 간호하는 간호사는 환자가 종양을 발견한 이후 치료 진행 시기별 로 다른 정서적 경험을 하며, 환자의 암·죽음·수술 그리고 유방제거에 대한인식이 환 자의 적응에 영향을 준다는 사실에 주목하여 간호사 독단의 판단에 의해서가 아닌, 환자 의 심층내면을 이해하는 수준에서 필요한 간호를 하여야한 것이다. 또한 환자가 병원에 입원하기 전이나 퇴원 후에도 심각한 부정적 정서의 경험을 하는 것으로 나타났으므로, 환자를 병원 내에서만 간호하는 간호체계에서 탈피하여 간호가 필요한 곳으로 찾아가서 간호할 수 있는 새로운 간호체계의 변화를 유도하여야 할 것으로 본다. 이 조사 결과 환 자의 가족, 특히 환자 남편의 지원과 의료진의 지원 및 같은 동료환자가 환자의 적응에 영향을 주는 것으로 나타났으므로 유방암 환자의 간호계획에 환자의 남편과 의료진을 포 함시켜야할 것으로 보며, 환자들끼리의 지지모임도 필요할 것이라 생각된다. It has been known that cancer patients are confronted with serious psychological problems. But, nursing care for the cancer patient is limited to physiological care and the nurses take care of their patients without au understanding of what their needs are. The purpose of this study was to explore and explain the emotional experience, and adjustment process of women wi th breast cancer. The study was performed from June, 1990 to february, 1991. The subjects were 53 women wi th breast cancer who knew they had breast cancer. They were wi thin one year of their the diagnosis. The data in this study was collected through indepth interviews. telephone interviews as wert 1 as observation. Heal th records of the patient were used for reference. The findings are as fort lows. The patients experienced different kinds of emotional distress accord-ins to the stages of the treatment. It appeared that the patients experienced apprehension and restlessness after finding the lumps in there r breast, hopelessness and blackout between the diagnosis and operation, uncertainty and hopelessness between operation and adjunctive therapy. During the adjunctive therapy, they suffered from the side effects of the therapy but expressed a strong wit 1 to overcome the cancer and its treatment. After the therapy, they experienced a fence of stability. But they were constantly anxious about the recurrence of the cancer. Several factors influenced the adjustment of the patients with breast cancer. Those factors were personal characteristics, circumstances and coping strategies. The personal characteristics included the perception of cancer and the mastectomy, educational level, religion. personal iffy, physical condition and economic status. The circumstances included the support from families. relatives, and others, as well as the attitude of the health workers and the mutual interrelationship with other patients. The coping strategies included change of mood, physical labor and emotional stablization hrougrh comparison with others. Most of all, the perception of cancer. death, operation and body image had an influence on the adjustment. The support from their husbands was very important. The patients who went immediately to the hospital on finding the lumps in their breasts adjusted effectively to the cancer and its treatment. They submited to the doctor's orders. During the adjunctive therapy, they maintained harmonious interrelationships with their families, relatives and others. They sought the coping strategies to ease their minds. They expressed their feelings to others. They carried out actions that improved their conditions. They managed their household duties to some extent The nurses in many different kinds of settings must be aware of the specialized needs of the cancer patients. Trends of Nursing Research in the Journal of Oncology Nursing

Purpose: Purpose of study was to analyze the research articles in the Journal of Korean Oncology Nursing in order to provide an direction for the future research. Methods: This study analyzed 93 studies published in the Journal of Korean Oncology Nursing, from its beginning year to the year 2007, according to the research objectives. The frame of evaluation included years and types of publication, the theoretical frameworks, research design, subject, data collection method, keyword analysis by MeSH. Results: 45.2% of studies was non-degree based studies. 95.7% of studies was not described theoretical framework in the articles. 71.0% was utilized a non-experimental design. 57% of subjects in researches was patients. 76.3% of studies were used the questionnaire for data collection. Concepts as human, nursing, and health were consistently appeared in research. But concepts of environment has been insufficiently conducted. Conclusion: Researches in the Journal of Korean Oncology Nursing has been changed in methodology and the topics of research for the last 7 yr. It progresses in both quantity and quality. But, it is necessary to conduct research founded on theoretical background, various research design, variability of study subjects and topics as supported by scientifically and empirically
