The aim of this study is to identify the prevalence of sarcopenia and frailty in older Korean adults with diabetes as compared to non-diabetic adults. Methods We analyzed 1,241 adults aged 70-84 years enrolled in the Korean Frailty and Aging Cohort Study (KFACS). Skeletal muscle mass was assessed by dual-energy X-ray absorptiometry, and muscle strength was measured by a digital grip strength dynamometer. Physical performance was assessed using the Short Physical Performance Battery (SPPB), and the Timed Up and Go (TUG) test. Sarcopenia was defined according to criteria put forth by the Asian Working Group for Sarcopenia (AWGS), the Foundation for the National Institutes of Health (FNIH), and the Korean Geriatric Society (KGS). Frailty was defined using Cardiovascular Health Study Frailty Phenotype (CHS) criteria, the Korean version of the FRAIL Scale (K-FS), and the Korean version of the Frailty Index (K-FI). Diagnosis of diabetes was followed by clinical recommendation, according to the American Diabetes Association. Results The mean age of the study subjects was 76.2 ยฑ 9.3 years, and 46.8% of the subjects were men. Prevalence of diabetes was 31.1% in men and 23.8% in women. In both genders, subjects with diabetes had higher levels of fasting glucose, insulin, HbA1c, and HOMA-IR, compared to the non-diabetic group. Subjects with diabetes were more likely to have hypertension and dyslipidemia. In men, the prevalence of sarcopenia and the mean value of the muscle mass index was not different between diabetic and non-diabetic groups, but the diabetic subjects had lower hand grip strength and gait speed. The diabetic male group also took a longer time on TUG test, compared to the non-diabetic group. In women, the prevalence of sarcopenia defined by AWGS was higher in the diabetic group, compared to the non-diabetic group. Women with diabetes also had a lower gait speed and SPPB score, and took a longer time on the TUG test, compared to the non-diabetic group, although hand grip strength and muscle mass index were not different between the two groups. Men with diabetes showed increased prevalence of the composition of pre-frail and frail, defined by CHS and K-FS, compared to the non-diabetic men. Women with diabetes also showed higher prevalence of frailty defined by K-FI, compared to their non-diabetic counterparts. Conclusion Prevalence of sarcopenia was higher in older women with diabetes compared to the non-diabetic group, but this prevalence was not different in men. Physical performance and muscle strength in older adults with diabetes were reduced compared to the non-diabetic group, whereas muscle mass was relatively preserved in subjects with diabetes. Thus, diabetes might be associated with developing sarcopenia, especially in a form of decreased muscle quality, in older adults. ธํ™” ์ฝ”ํ˜ธํŠธ ์—ฐ๊ตฌ (Korean Frailty and Aging Cohort Stuty, KFACS)์— 2016๋…„์— ๋“ฑ๋ก๋œ 70-84์„ธ ๋… Timed Up and Go (TUG) test๋กœ ํ‰๊ฐ€ํ•˜์˜€๋‹ค. ๊ทผ๊ฐ์†Œ์ฆ์˜ ์ •์˜๋Š” Asian Working Group for Sarcopenia (AWGS), the Foundation for the National Institutes of Health (FNIH) ๋ฐ Korean Geriatric Society (KGS)๋ฅผ ์‚ฌ์šฉํ•˜์˜€๊ณ , ๋‹น๋‡จ์˜ ์ง„๋‹จ์€ ๋ฏธ๊ตญ ๋‹น๋‡จ๋ณ‘ํ•™ํšŒ ์ง„๋‹จ ๊ธฐ์ค€์„ ๋”ฐ๋ž๋‹ค. ๋…ธ์‡ ๋Š” Cardiovascular Health Study Frailty Phenotype (CHS) criteria, Korean version of the FRAIL Scale (K-FS), and Korean version of the Frailty Index (K-FI)๋ฅผ ๋”ฐ๋ผ ์ •์˜ํ•˜์˜€๋‹ค. Contents Chapter 1. an inquiry experiment which was based on scientific research papers. And their word and attitude were observed so that we can know the effect of scientific paper to the students. Although inquiry experiment has been emphasized in the science education, most of inquiry activities consist of manuals which already describe problems and method minutely and these lead students to the 'predictable' result. Scientists spend a lot of time to think of a subject for inquiry and set a resonable hypothesis. On the other hand, it is rare for students to create a hypothesis by themselves and it is hard to experience the inquiry experiments which real scientists experiment. Students can learn easily how real scientists think and experiment through scientific research papers, because these are composed of scientist's thoght, process and method of research in a way of completed form. Scientific research papers not only provide background information which can make people understand research, but also help them to accustom language and society of science. Paying attention to advantages of scientific research paper, students searched and read scientific papers in this program so that they could experience indirectly inquiry experiment which real scientists do. Furthermore, students set the hypothesis by themselves for their experiments and they performed inquiry experiment and organized result in the paper form. These program had been lasted from orientation in April to January for 10 months, 31 times. And variety subjects of scientific research paper had been provided but all of them were related to hair. Before students participated in this program, they wrote down what they wanted to experiment for inquiry and designed how to they perform. After they studied scientific papers, they set hypotheses and experimental methods in the same way. these were used for analysis information of the effect of the scientific papers. Also, discussion of students during the inquiry experiment was recorded and analysis. As a result, there are some effect of scientific papers to students as follow. First, the effect of scientific paper to each stage of inquiry experiment. The experimental hypotheses students set before they participated in this program were unclear, little worth to start to experiment and lack of a scientific basis. Moreover, in accordance with advanced research which showed the hardness of set hypotheses for students, students had some difficulties to suggest variety subjects even after they studied scientific papers. However, after they read scientific papers, they expressed hypotheses with precise and technical words, and they tried to suggest their hypotheses with control variables and independent variables. And also these variables were not wrote down simply but they were related to each other having directionality. When students said their hypotheses, they always considered which scientific bases support their hypotheses. And they did their best to search resonable and reliable resources. Students showed changed attitude when they rewritten experimental design. First of all, they had wrote concretely and expressed their though fully. For example, they used specific numbers and equipments. And they described how they wanted to operate these equipments. Second, specific variables were presented so that they knew what they wanted to check when they experimented. Finally, they considered reproducibility and checked a methodological problem. When students did their experiment, they concerned about independent variables and tried to keep control variables under their control. So they could see the results which were not exactly what they expected, but closely similar ones. Furthermore, they tried to increase reliability of the result of experiment and to make generalizations. They could think of these matters because they learned that each hair has unique character from papers. To reach reliability and generalization highly, they experimented various regions of hairs. While students organized their thought and expressed the results of experiments, they rechecked scientific papers to know how scientists presented theirs. After that, they applied the form of the image and graph but they didn't follow uncritically. They developed forms so that they could emphasize important parts of their results. Second, the effect of scientific paper to cognizance of scientific knowledge. Students learned naturally that basing on scientific papers and using the knowledge from the papers are important. Also, they could get knowledges which are various and reliable from the papers. And they started to think of advanced researches and consider about relationship between each papers. So they realized that papers do not exist by them selves, but they influence each other. Moreover, they said that scientific knowledge is not an eternal truth but they can be changed or improved or discarded. And they checked flow of researches and tried to adopt recent result of papers. Third, the effect of scientific paper to cognizance of scientists and society of science. At first time, students said they felt strange and difficult about scientific papers. These were due to the lack of experience of read papers and difficulty of English which is the way of expression. However, while keeping up their study, they got accustomed to read scientific papers. It meaned they were familiar with society of science. Also, when they ran into some unexpected problems, they referred to the results of advanced researches and compared theirs and paper's. After that they designed again their experiments with consulting papers. As mentioned above, students could learn every part of inquiry process of scientists and get basis knowledge at the same time from scientific research papers. And they could experience indirectly how scientists think and how society of science is driven. Also they emulated scientist's behavior and tried to experiment like real scientists. Through series of observation, teaching based on scientific research paper can influence attitude when student experiment and student's cognizance of scientists and society of science.

[์˜๋ฌธ] Due to changing industrial environment, increasing work-related diseases, aging population and increasing woman workers, an effective health promotion policy is required. In this regard, ''Workplace Health Promotion" (WHP) may be defined as workplace health management activities performed in a systematic and organized way to improve the level of workers'' health. In order to suggest a WHP model, its scope and detailed activities, the researcher surveyed domestic and foreign health promotion programs for their operational conditions and divided them into working environment improvement and risky element management, health promotion programs and welfare management models. In particular, the health promotion programs were examined in terms of health condition checks, classification of health conditions and levels, management of the program, evaluation and analysis of the program performances. Then, the researcher implemented a customized health promotion system based on WEHR, whereby workers'' health levels and conditions could be diagnosed to be prescribed and managed for their health. This customized health promotion system diagnose workers'' health level and conditions based on their electronic record containing working environment, health information and health behaviors and thereby, classify and prescribe them and manage and evaluate the performances.Since the customized health promotion system suggested by this study is not an individualized health promotion program but a comprehensive customized health promotion system, operating its programs in a systematic way to prescribe workers'' health conditions, it is expected to be useful in effective health managements in the industrial health area. Lastly, it is expected that the WHP programs would be more effective if a customized health promotion program equipped with a decision making supporting system should be applied to job-related disease management, management of negatively diagnosed workers, management of harmful environment, prevention of cardiovascular diseases and management of woman workers.ope

Dept. of Physical Therapy/์„์‚ฌThe direction of fiber alignment within a muscle is known to influence the effectiveness of muscle contraction. However, most of the commonly used clinical gluteus maximus (GM) exercises do not consider the direction of fiber alignment within the muscle. Therefore, the purpose of this study was to investigate the influence of hip abduction position on the amplitude and onset time of the GM and hamstrings (HAM) during prone hip extension with knee flexion (PHEKF) exercise. Surface electromyography (EMG) signals were recorded from the GM and HAM during PHEKF exercise in three hip abduction positions: 0ยฐ, 15ยฐ, and 30ยฐ. Thirty healthy subjects (age: 22.8 ยฑ 2.9 yrs, body mass: 66.9 ยฑ 10.8 ใŽ, height: 170.3 ยฑ 4.1 ใŽ) voluntarily participated in this study. EMG amplitude was transformed into the root-mean-square (RMS) and expressed as a percentage of the maximal voluntary isometric contraction (MVIC) for each muscle. The relative onset difference between activation of the GM and HAM was calculated by subtracting the HAM onset from the GM onset (in ms). Repeated measures analysis of variance (ANOVA) with Bonferroni post hoc tests was used to compare muscle activation between the three hip abduction positions. The level of statistical significance was set at p < 0.05. The results show that GM amplitude was greatest in the 30ยฐ hip abduction position, followed by 15ยฐ and then 0ยฐ hip abduction during PHEKF exercise (mean ยฑ SD: 29 ยฑ 11% MVIC, 23 ยฑ 9% MVIC, and 20 ยฑ 8% MVIC, respectively). On the other hand, the HAM amplitude was greatest at 0ยฐ hip abduction, followed by 15ยฐ and then 30ยฐ (mean ยฑ SD: 17 ยฑ 15% MVIC, 15 ยฑ 15% MVIC, and 14 ยฑ 14% MVIC, respectively). Additionally, the relative onset difference between the GM and HAM was positive at 0ยฐ hip abduction, meaning that GM firing was delayed relative to that of the HAM (mean ยฑ SD, 0.17 ยฑ 0.17 ms). In contrast to the 15 and 30ยฐ hip abduction positions, the relative onset difference was negative, which means the GM onset occurred earlier than the HAM (mean ยฑ SD: -0.02 ยฑ 0.11 ms and -0.21 ยฑ 0.20 ms, respectively). These findings indicate that the 30ยฐ hip abduction position maximizes GM amplitude and minimizes HAM amplitude. The EMG onset of the GM was significantly earlier relative to the HAM at 15ยฐ and 30ยฐ hip abduction. Therefore, performing PHEKF exercise in the 30ยฐ hip abduction position may be recommended as an effective way to selectively activate the GM and to reduce the delay in GM firing in asymptomatic individuals. This finding provides preliminary evidence that amplitude and onset time during PHEKF can be modified by the extent of hip abduction.ope
