LâabcĂšs hypophysaire est une pathologie rare et fatale en absence de traitement adĂ©quat .Il est Ă Ă©voquer devant un tableau clinique dâhypertension intracrĂąnienne, une dysrĂ©gulation hormonale hypophysaire et un contexte infectieux. Une conduite thĂ©rapeutique basĂ©e sur une antibiothĂ©rapie pourrait ĂȘtre tentĂ© si diagnostic prĂ©coce. Nous rapportons un cas dâabcĂšs hypophysaire survenant chez un hĂ©modialysĂ© chronique pris en charge par une antibiothĂ©rapie seule dont lâĂ©volution Ă©tait favorable avec un recul de 2 ans
An unusual intracranial metallic foreign bodies and panhypopituitarism
A 49 years old man, with a history of aggression at the age of 18 years by a pair of scissors, who consulted for unilateral migraine headaches look straight. Paraclinical explorations concluded that trauma to anterior pituitary by a metallic foreign body from the right nostril to the sella, responsible for panhypopituitarism and sinusitis. The headaches are frequent causes of consultation, often treated symptomatically but rarely explored. The direct trauma to the pituitary gland, by a metallic foreign body, is exceptional. We report the case of neglected panhypopituitarism, discovered 31 years after injury with a pair of scissors
Une cause rare de recidive de lombosciatalgie
Le terme de textile est un terme non mĂ©dical qui dĂ©signe un objet synthĂ©tique oubliĂ© dans le foyer opĂ©ratoire ainsi que la rĂ©action inflammatoire qui lâentoure. Transient mutism after anterior transcallosal approach
Postoperative mutism is an infrequent complication of brain surgery. We report a third ventricular astrocytoma in a 16-year-old boy. The tumor was totally removed via anterior transcallosal approach. The operation was uneventful. On the second postoperative day he became mute. He could follow verbal commands, and write and read. Histopathological examination revealed a pilocystic astrocytoma. Four days postoperatively, he began to say simple words, and two weeks later he could talk normally. Les metastases meningees solitaires prevalentes
Objectif IntĂ©rĂȘt dâavoir une confirmation histologique, en prĂ©sence dâun processus leptomĂ©ningĂ©e solitaire dont le diagnostic de mĂ©ningiome bĂ©nin est souvent Ă©voquĂ© en premier. Introduction Les mĂ©tastases mĂ©ningĂ©es sont observĂ©es de plus en plus frĂ©quemment chez les patients connus porteur dâune nĂ©oplasie, du fait de lâallongement de la survie des patients et lâamĂ©lioration des moyens diagnostiques ; elles reprĂ©sentent environ 8 % des mĂ©tastases du systĂšme nerveux central.Observation Nous rapportons deux observations originales de patients sans histoire nĂ©oplasique, opĂ©rĂ©s pour un processus leptomeningĂ© solitaire dont le diagnostic prĂ©opĂ©ratoire Ă©tait celui dâun mĂ©ningiome. LâĂ©tude histologique rĂ©vĂ©lait la nature nĂ©oplasique mĂ©tastatique de la lĂ©sion, alors que le bilan radiologique a permis de dĂ©tecter la localisation primitive mĂ©connue. Conclusion et discussion La dĂ©couverte Ă lâoccasion dâune imagerie cĂ©rĂ©brale (TDM et/ou IRM) chez un patient, sans histoire nĂ©oplasique, dâune ou plusieurs lĂ©sions leptomĂ©ningĂ©es, pose un problĂšme diagnostique. Lorsque lalocalisation est unique, le diagnostic de mĂ©ningiome est Ă©voquĂ© en premier ; alors que les lĂ©sions inflammatoires et secondaires des hĂ©mopathies malignes reprĂ©sentent un diagnostic diffĂ©rentiel lorsque les lĂ©sions sont diffuses. Rola dynamicznej tomografii komputerowej zĆÄ cza szczytowo-potylicznego przy ustalaniu postÄpowania w przypadkach stwierdzenia os odontoideum
Abstract
Os odontoideum is an uncommon abnormality of the craniovertebral junction (CVJ) that exists as a separate ossicle apart from a hypoplastic dens. Its genesis and natural history have been debated, and its proper treatment remains uncertain. A 48-year-old woman complained of persistent upper neck pain and paraesthesia of her left side. Magnetic resonance imaging of the CVJ demonstrated an os odontoideum. Dynamic computed tomography scan of the CVJ showed a reduction of the space available for the spinal cord to 50% from extended to flexed position. The patient underwent posterior spinal fusion of C1âC2 using a sublaminar titanium hook and rods fixed in moderate extension.
We discuss the usefulness of the dynamic computed tomography (CT) scan in the evaluation of atlantoaxial motion and the management of this pathology.Streszczenie
