Weidegang in Nederland anno 2008 : Eindmeting Koe & Wij

Het project Koe & Wij werkt aan een sectorbrede bewustwording van het belang van weidegang en enthousiasmeert melkveehouders voor weidegang door het bieden van technische en economische oplossingen voor knelpunten bij weidegang. Het effect van het project Koe & Wij is gemonitord middels een nulmeting bij de start en een eindmeting na afloop. Doel van deze eindmeting is: de verandering (voorjaar 2008 ten opzichte van zomer 2006) van bewustzijn, houding en gedrag van melkveehouders ten aanzien van weidegang vast te leggen; inzicht in de (spreiding van) persoonlijke waarden van melkveehouders; inzicht krijgen in de resultaten van het project Koe & Wij voor een beoordeling van de effectiviteit van het project; ideeën en suggesties voor de invulling van een eventueel vervolgproject. Dit is de rapportage van de eindmeting welke CLM Onderzoek en Advies in het kader van het project Koe & Wij heeft uitgevoerd. Waardering weidegang : en wat drijft jou?

Op basis van de nulmeting van het project Koe & Wij was al geconcludeerd dat de factor ‘persoonlijke waarden’ een zeer belangrijke rol speelt in de afweging om de koeien al dan niet te weiden. Uit het huidige onderzoek blijkt dat ‘persoonlijke waarden’ een rol spelen bij alle drie de andere afwegingen uit het afwegingskader van Koe & Wij: ‘arbeid’, ‘economie’ en ‘maatschappelijke waarden’. We kunnen op basis van dit onderzoek vier ‘typen’ ondernemers onderscheiden: de zakelijke ondernemer; de genietende ondernemer; de traditionele ondernemer; de overtuigde ondernemer. Elke type ondernemer laat andere waarden meewegen in het maken van de keuze voor weidegang of opstallen

Economic evaluation of orphan drug Lutetium-Octreotate vs. Octreotide long-acting release for patients with an advanced midgut neuroendocrine tumour in the Netherlands

OBJECTIVES: Multiple studies showed positive effects of Lutetium-Octreotate (LO) treatment in neuroendocrine tumours. LO has been used in the Netherlands since the 1980s and recently received the orphan status shortly after the acquisition by Novartis. Since then, the official list price has increased sixfold. From a value-based pricing perspective, we analysed the impact of the increase in price on the incremental cost-effectiveness ratio (ICER) of LO treatment compared to optimal best supportive care, a high dose of Octreotide long-acting release (O-LAR), using the clinical data of the NETTER-1 trial. METHODS: A Markov model was developed to evaluate the costs per quality-adjusted life-year (QALY) for LO treatment compared to O-LAR from the healthcare perspective. A scenario analysis was conducted to compare the cost-effectiveness with the initial and increased price level of the LO-treatment. RESULTS: At the increased price level, the cost-effectiveness analysis rendered a deterministic ICER of €53,500 per QALY, while at the initial pricing, the ICER was €19,000 per QALY. The probabilistic sensitivity analysis (PSA) showed that LO had a high probability of being cost-effective at both the increased and initial price level, considering a cost-effectiveness threshold of €80,000. CONCLUSIONS: Even at the increased price level, LO treatment can still be considered cost-effective using the applicable Dutch willingness-to-pay threshold of 80,000 euro per QALY. Considering the public scrutiny in relation to this price increase, these outcomes raise the question whether traditional cost-effectiveness methods are sufficient in fully capturing the societal acceptance of prices of new medicines. Betere kwaliteit luzerne door vroeger maaien

Op proefbedrijf Cranendonck is vanaf 1992 onderzoek gedaan om de invloed van het maaitijdstip op de voederwaarde van luzerne vast te stellen. Parallel aan dit onderzoek liepen voederproeven met jonge energierijke luzerne en oudere energiearme luzerne.Uit de voederproeven bleek dat de voederwaardeberekening voor luzerne moet worden aangepast. Uit de proefvelden waar verschillende maaitijdstippen werden vergeleken bleek dat door vroeger te maaien de kwaliteit verbeterde terwijl de productie afnam ten opzichte van maaien in het algemeen geadviseerde stadium. De voederwaarde opbrengst neemt toe, of blijft gelijk, bij beperkt vroeger maaien

The criterion-related validity of the Northwick Park Dependency Score as a generic nursing dependency instrument for different rehabilitation patient groups

Objective: To investigate the criterion or concurrent validity of the Northwick Park Dependency Score (NPDS) for determining nursing dependence in different rehabilitation groups, with the Barthel Index (BI) and the Care Dependency Scale (C D S).Design: Cross-sectional study.Setting: Centre for Rehabilitation of the University Medical Center Groningen, The Netherlands.Subjects: Patients after stroke, spinal cord injury, multitrauma, head injury, amputation, rheumatoid arthritis, diabetes mellitus, lung diseases, tuberculosis and coronary artery disease. One hundred and fifty-four patients were included.Measures: The Northwick Park Dependency Score (NPDS), the Barthel Index (BI) and the Care Dependency Scale (CDS).Results: The correlation (rho) between the NPDS and the BI for all groups was - 0.87-1 R-2 = 0.76 (n = 154). Per patient group rho varied from - 0.70 (R-2 = 0.49) to - 0.93 (R-2 = 0.86). The overall correlation between the NPDS and CDS was larger than the criterion of rho = 0.60 (r=- 0.74; R-2 = 0.55) but was &lt;0.60 in the rheumatoid arthritis and tuberculosis group. The overall correlation between BI and CDS exceeded the criterion (r = 0.75; R-2 = 0.56).Conclusions: The NPDS is a generic nursing dependency instrument that can be used as a valid measure across various patient groups in rehabilitation.</p

Some studies on the deformation of the membrane in an RF MEMS switch

Radio Frequency (RF) switches of Micro Electro Mechanical Systems (MEMS) are appealing to the mobile industry because of their energy efficiency and ability to accommodate more frequency bands. However, the electromechanical coupling of the electrical circuit to the mechanical components in RF MEMS switches is not fully understood. In this paper, we consider the problem of mechanical deformation of electrodes in RF MEMS switch due to the electrostatic forces caused by the difference in voltage between the electrodes. It is known from previous studies of this problem, that the solution exhibits multiple deformation states for a given electrostatic force. Subsequently, the capacity of the switch that depends on the deformation of electrodes displays a hysteresis behaviour against the voltage in the switch. We investigate the present problem along two lines of attack. First, we solve for the deformation states of electrodes using numerical methods such as finite difference and shooting methods. Subsequently, a relationship between capacity and voltage of the RF MEMS switch is constructed. The solutions obtained are exemplified using the continuation and bifurcation package AUTO. Second, we focus on the analytical methods for a simplified version of the problem and on the stability analysis for the solutions of deformation states. The stability analysis shows that there exists a continuous path of equilibrium deformation states between the open and closed state
