    SynthRAD2023 Grand Challenge dataset: generating synthetic CT for radiotherapy

    Purpose: Medical imaging has become increasingly important in diagnosing and treating oncological patients, particularly in radiotherapy. Recent advances in synthetic computed tomography (sCT) generation have increased interest in public challenges to provide data and evaluation metrics for comparing different approaches openly. This paper describes a dataset of brain and pelvis computed tomography (CT) images with rigidly registered CBCT and MRI images to facilitate the development and evaluation of sCT generation for radiotherapy planning. Acquisition and validation methods: The dataset consists of CT, CBCT, and MRI of 540 brains and 540 pelvic radiotherapy patients from three Dutch university medical centers. Subjects' ages ranged from 3 to 93 years, with a mean age of 60. Various scanner models and acquisition settings were used across patients from the three data-providing centers. Details are available in CSV files provided with the datasets. Data format and usage notes: The data is available on Zenodo (https://doi.org/10.5281/zenodo.7260705) under the SynthRAD2023 collection. The images for each subject are available in nifti format. Potential applications: This dataset will enable the evaluation and development of image synthesis algorithms for radiotherapy purposes on a realistic multi-center dataset with varying acquisition protocols.     Improved Model-Data Agreement With Strongly Eddying Ocean Simulations in the Middle-Late Eocene

    Model simulations of past climates are increasingly found to compare well with proxy data at a global scale, but regional discrepancies remain. A persistent issue in modeling past greenhouse climates has been the temperature difference between equatorial and (sub-)polar regions, which is typically much larger in simulations than proxy data suggest. Particularly in the Eocene, multiple temperature proxies suggest extreme warmth in the southwest Pacific Ocean, where model simulations consistently suggest temperate conditions. Here, we present new global ocean model simulations at 0.1° horizontal resolution for the middle-late Eocene. The eddies in the high-resolution model affect poleward heat transport and local time-mean flow in critical regions compared to the noneddying flow in the standard low-resolution simulations. As a result, the high-resolution simulations produce higher surface temperatures near Antarctica and lower surface temperatures near the equator compared to the low-resolution simulations, leading to better correspondence with proxy reconstructions. Crucially, the high-resolution simulations are also much more consistent with biogeographic patterns in endemic-Antarctic and low-latitude-derived plankton, and thus resolve the long-standing discrepancy of warm subpolar ocean temperatures and isolating polar gyre circulation. The results imply that strongly eddying model simulations are required to reconcile discrepancies between regional proxy data and models, and demonstrate the importance of accurate regional paleobathymetry for proxy-model comparisons

    Exposure-in-vivo containing interventions to improve work functioning of workers with anxiety disorder: a systematic review

    <p>Abstract</p> <p>Background</p> <p>Anxiety disorders are associated with functional disability, sickness absence, and decreased productivity. Effective treatments of anxiety disorders can result in remission of symptoms. However the effects on work related outcomes are largely unknown. Exposure in vivo is potentially well fit to improve work-related outcomes. This study systematically reviews the effectiveness of exposure-in-vivo containing interventions in reducing work-related adverse outcomes in workers with anxiety disorders.</p> <p>Methods</p> <p>A systematic study search was conducted in Medline, Cinahl, Embase and Psycinfo. Two reviewers independently extracted data and from each study assessed the quality of evidence by using the GRADE approach. We performed a meta-analysis if data showed sufficient clinical homogeneity.</p> <p>Results</p> <p>Seven studies containing 11 exposure-in-vivo interventions were included. Four studies were focused on Obsessive Compulsive Disorder (OCD), two on Post Traumatic Stress Disorder (PTSD), and one on a mixed group of OCD and severe phobias. The studies were grouped according to type of anxiety disorder and subsequently according to type of comparisons. For OCD, exposure-in-vivo containing interventions can yield better work-related outcomes compared to medication (SSRIs) and relaxation but not better compared to response prevention. The results on anxiety outcomes were similar. The net contribution of exposure in vivo in two OCD intervention programs is also presented as a meta-analysis and shows significant positive results on work role limitations. The calculated pooled effect size with 95% confidence interval was 0.72 (0.28, 1.15). For PTSD, exposure-in-vivo containing interventions can yield better work-related and anxiety-related outcomes compared to a waiting-list but not better compared to imaginal exposure.</p> <p>Conclusions</p> <p>Exposure in vivo as part of an anxiety treatment can reduce work-related adverse outcomes in workers with OCD and PTSD better than various other anxiety treatments or a waiting-list. We recommend that it should be studied how the results of these studies can be transferred to the practice of occupational health professionals and how clinicians can make better use of them to improve work-related outcomes. In future research, priority should be given to high-quality randomised controlled trials (RCTs) in which exposure-in-vivo containing interventions are applied to a variety of anxiety disorders and compared with other clinical anxiety treatments such as SSRIs. Work-related outcomes, in particular work functioning and sickness absence, need to be assessed with reliable and valid measures.</p

    Popular support for enlargement of the European Union is a function of how close or how far removed the member states are from the candidate countries. In the absence of country fixed effects or special bilateral relationships (e.g. adjacency, historic rivalry, religious conformity), we can explain approximately 14% of the variation in attitudes across member states and with regard to specific candidate countries using factors related to trade, distance, and relative economic size and structure. Taking special bilateral relations into account we can explain approximately 30%. Once country fixed effects are incorporated, the level of explanation increases to 80%

    Public opinion and enlargement - A gravity approach

    This paper examines European Union (EU) member state attitudes toward enlargement. We use Eurobarometer data for aggregate national support from 1996 to 2002 for the accession of each of 13 candidate countries - the 10 that will join the EU in 2004 plus Bulgaria, Romania, and Turkey. Analyzing our cross-section time series, we find, first, that the level of support correlates positively with the share of the candidate country in the total exports of the member state and negatively with the distance between the capitals of the member state and the candidate country. This finding supports the hypothesis that common identity, or 'we-feeling,' grows with interaction (Deutsch et al., 1957). Second, support correlates negatively with the relative importance of agriculture in domestic employment. Since agriculture triggers EU financial flows, this finding supports the view that attitudes toward integration are policy related (Gabel, 1998b, 1998c). Third, support is lower when both the member state and the candidate country are predominantly Catholic. This suggests that, although Catholics may desire a deeper Europe (Nelsen et al., 2001), they oppose a wider Europe

    A margin recipe for the management of intra-fraction target motion in radiotherapy

    Background and purpose: Strategies to limit the impact of intra-fraction motion during treatment are common in radiotherapy. Margin recipes, however, are not designed to incorporate these strategies. This work aimed to provide a framework to determine how motion management strategies influence treatment margins. Materials and methods: Two models of intra-fraction motion were considered. In model 1 motion was instantaneous, before treatment starts and in model 2 motion was a continuous drift during treatment. Motion management strategies were modelled by truncating the underlying error distribution at cσ, with σ the standard deviation of the distribution and c a free parameter. Using Monte Carlo simulations, we determined how motion management changed the required margin. The analysis was performed for different number of treatment fractions and different standard deviations of the underlying random and systematic errors. Results: The required margin for a continuous drift was found to be well approximated by an average position of the target at ¾ of the drift. Introducing a truncation at cσ, the relative change in the margin was equal to 0.3c. This result held for both models, was independent of σ or the number of fractions and naturally generalizes to the situation with a residual (systematic) error. Conclusion: Treatment margins can be determined when motion management strategies are applied. Moreover, our analysis can be used to study the potential benefit of different motion management strategies. This allows to discuss and determine the most appropriate strategy for margin reduction

Another advantage of the approach presented in this paper is the fact that every monitoring system can be adapted to specific situations.     Understanding the current practice of design in government: limitations and opportunities

    Design is today suggested as an alternative way of working in government contexts. Manydeveloped nations are trying to embed design in their public organizations. Yet recentstudies have shown that design is not easily permeating into everyday practice of public organizations. This research therefore aims to understand what the current practice ofdesign-embedding in government is like and its limitations by interviewing six experts in the design for government field. The research findings reveal that the changes created by the current design-embedding practice in government are not being actively sustained or amplified. Based on an understanding of organisations as complex systems, we suggest a further practice of design-embedding in which designers steward and stimulate design-led change energy within public organizations. This study shows that embedding design capability in professional organizations is more about design-led organizational change than passing on a design skillset to the organizational members

    Resolution dependency of sinking Lagrangian particles in ocean general circulation models.

    Any type of non-buoyant material in the ocean is transported horizontally by currents during its sinking journey. This lateral transport can be far from negligible for small sinking velocities. To estimate its magnitude and direction, the material is often modelled as a set of Lagrangian particles advected by current velocities that are obtained from Ocean General Circulation Models (OGCMs). State-of-the-art OGCMs are strongly eddying, similar to the real ocean, providing results with a spatial resolution on the order of 10 km on a daily frequency. While the importance of eddies in OGCMs is well-appreciated in the physical oceanographic community, other marine research communities may not. Further, many long term climate modelling simulations (e.g. in paleoclimate) rely on lower spatial resolution models that do not capture mesoscale features. To demonstrate how much the absence of mesoscale features in low-resolution models influences the Lagrangian particle transport, we simulate the transport of sinking Lagrangian particles using low- and high-resolution global OGCMs, and assess the lateral transport differences resulting from the difference in spatial and temporal model resolution. We find major differences between the transport in the non-eddying OGCM and in the eddying OGCM. Addition of stochastic noise to the particle trajectories in the non-eddying OGCM parameterises the effect of eddies well in some cases (e.g. in the North Pacific gyre). The effect of a coarser temporal resolution (once every 5 days versus monthly) is smaller compared to a coarser spatial resolution (0.1° versus 1° horizontally). We recommend to use sinking Lagrangian particles, representing e.g. marine snow, microplankton or sinking plastic, only with velocity fields from eddying Eulerian OGCMs, requiring high-resolution models in e.g. paleoceanographic studies.
