Ethische dilemma’s bij eigen regie: oproep tot reflectie

De decentralisaties in zorg en welzijn moeten bijdragen aan een transitie van verzorgingsstaat naar participatiesamenleving. Van burgers wordt verwacht dat zij eigen regie voeren. Ten eerste door langer voor zichzelf te zorgen en een beroep te doen op hun eigen omgeving als ze hulp nodig hebben. Ten tweede door, als er wel professionele ondersteuning nodig is, ook daarin een actieve rol te vervullen. Eigen regie wordt vooral positief gelabeld door beleidsmakers: het levert zorggebruikers meer autonomie op, laat de zorg beter aansluiten bij de vraag en bespaart ook nog eens kosten. Eigen regie is daarmee een hoerawoord waar je met goed fatsoen niet tegen kan zijn. In deze bijdrage laten we echter zien dat het stimuleren van eigen regie zorgverleners confronteert met ethische dilemma’s. Reflectie op deze dilemma’s is van groot belang om op die manier recht te doen aan verschillende waarden in de zorg

Doelgericht deelnemen Achtergrondstudie naar burgerparicipatie bij het toezicht

Sinds de jaren tachtig van de vorige eeuw is er groeiende aandacht voor burgerparticipatie in de gezondheidszorg. Participatie vindt niet alleen op het individuele niveau plaats, waar burgers kunnen kiezen en actief kunnen participeren in beslissingen over hun eigen zorg, maar ook op het collectieve niveau. Patiënten worden gevraagd om het patientenperspectief in te brengen in besluitvorming over bijvoorbeeld richtlijnontwikkeling, onderzoeksagenda’s, overheidsbeleid en kwaliteitsbeleid in ziekenhuizen. Ook bij de IGZ staat burgerparticipatie op de kaart. Er is echter nog weinig bekend over participatie bij toezicht. Om meer inzicht te krijgen in deze materie is door het iBMG

Decentralisatie en Democratische Vertegenwoordiging:een pilot-onderzoek naar vertegenwoordigingsclaims op het lokale niveau

In dit rapport presenteren we de resultaten die zijn voortgekomen uit ons pilot-onderzoek naar democratische vertegenwoordiging in het sociale domein in twee gemeenten. We merkten in het onderzoek hoezeer alle betrokkenen er groot belang aan hechten om de omvangrijke decentralisaties van jeugdzorg, maatschappelijke ondersteuning en arbeidsparticipatie democratisch te verankeren. We merkten ook dat het vaak een worsteling is (geweest) om participatie en vertegenwoordiging in het gemeentelijke beleid invulling te geven in de praktijk. Dit rapport biedt niet alleen inzicht in die worsteling, maar is ook een verkenning hoe diverse vormen van vertegenwoordig

De democratische kwaliteit van gekozen en niet-gekozen vertegenwoordiging in gemeenten

De vaak sombere analyses van democratische vertegenwoordiging op lokaal niveau zijn veelal gericht op problemen met partijen en verkiezingen. Directe participatie is geen goed alternatief, omdat alleen maar bepaalde, politiek toch vaak al actieve mensen daarvan gebruikmaken. Met het begrip ‘vertegenwoordigingsclaim’ laten we in twee kwalitatieve gevalstudies van de decentralisaties in het sociale domein echter zien dat er ook andere vertegenwoordigers dan gekozen politici zijn. The Limits of Patient Power: Examining active citizenship in Dutch health care

Modern health care policy is directed at giving patients an active role in health care instead of keeping them the passive recipients of care they used to be. Today, patients should be active participants in health care decision-making, making informed choices of health care providers, getting involved in shared decision-making with medical professionals, and participate in client councils and patient organisations. Much is expected from this policy. It would both democratise decision-making and increase the effectiveness of health care. Much time, money and energy have been spent on this type of health care reform. Policy-makers seem to think that if patients are provided with information and the opportunity to participate, the benefits of the reform will surely follow. This thesis studies whether this is true and how the policy of active patientship works out in practice. It shows that although this policy has presented opportunities for increasing the empowerment of patients, important negative effects and tensions can be identified. As a result the two goals of active patientship cannot unambiguously be found in practice. This situation leads to the conclusion that there are limits to patient power which should be acknowledged

Working with layers: The governance and regulation of healthcare quality in an institutionally layered system

Institutional arrangements used to steer public policies have increasingly become layered. Inspired by the literature on institutional layering and institutional work, this paper aims to make a contribution to our understanding of institutional layering. We do so by studying an interesting case of layering: the Dutch hospital sector. We focus on the actors responsible for the internal governance (Board of Directors and Supervisory Boards) and the external regulation (the Healthcare Inspectorate) of hospitals. In the paper, we explore the institutional work of these actors, more specifically how institutional work results from and is influenced by institutional layering and how this in turn influences the institutional makeup of both healthcare organizations and their institutional context. Our approach allowed us to see that layering changes the activities of actors in the public sector, can be used to strengthen one’s position but also presents actors with new struggles, which they in turn can try to overcome by relating and using the institutionally layered context. Layering and institutional work are therefore in continuous interaction. Combining institutional layering with a focus on the lived experiences of actors and their institutional work makes it possible to move into the layered arrangement and better understand its consequences
