    Governance of inter-organizational systems: a longitudinal case study of Rotterdam’s Port Community System

    An increasing use of inter-organizational systems, as Port Community System (PCS), can be observed in port collaborations. As multiple organizations often rely on PCS, even for business-critical processes, proper governance of these systems is crucial. This study aims to explain the governance of inter-organizational port collaborations using a lifecycles paradigm. The governance is explored using three points of view – i.e. governance mechanisms, governance aspects, and governance models – and by describing the actors’ roles in collaborations. A case study in the port of Rotterdam is analyzed to explain how these actors affect the governance models through the mechanisms to govern the aspects in each lifecycle stage. The port collaboration in Rotterdam has gone through three full governance lifecycles and has entered the fourth iteration after the set-up of Portbase. During the last two cycles, the collaboration has maintained its Network Administrative Organization governance model. This case study analysis of Rotterdam’s port collaboration provides an example of how a systematic approach could help to discuss and communicate the governance of inter-organizational port collaboration systems and gives some lessons learned for other collaborations

    Laser treatment for liver metastases : thermal and photodynamic therapy

    Medical laser applications are one of the points of interest of the Department of Surgery of the Erasmus University Rotterda._"Il. In 1988. experiments were conducted at the Laboratory for Experimental Surgery to investigate the use of laser in destroying liver metastases. These studies led to the research program "Laser treatment of experimental liver metastases", that was supported by the Netherlands Digestive Disease Foundation from beginning 1990 to the middle of 1992. Within this project, studies were carried out in close collaboration with the Departments of Pathology, Internal Medicine II, and Radiology of the Erasmus University. Joint studies were performed with the groups of the Department of Physics of the Dr. Daniel den Hoed Cancer Center and the Laser Center of the Academic Medical Center Amsterdam. This thesis describes the results of the studies mentioned above. The General Introduction gives an outline of the problem of hepatic metastases, the aims of the study, and the tumor model applied. The following parts on thermal and photodynamic therapy start with an introduction of the fundamentals of these laser applications. The second last part presents initial studies on the complex matter oflaser-tissue interaction. Finally, general considerations are given for possible clinical use

    Waiting Time from Diagnosis to Treatment has no Impact on Survival in Patients with Esophageal Cancer

    Background Waiting time from diagnosis to treatment has emerged as an important quality indicator in cancer care. This study was designed to determine the impact of waiting time on long-term outcome of patients with esophageal cancer who are treated with neoadjuvant therapy followed by surgery or primary surgery. Methods Patients who underwent esophagectomy for esophageal cancer at the University Medical Center Utrecht between 2003 and 2014 were included. Patients treated with neoadjuvant therapy followed by surgery and treated with primary surgery were separately analyzed. The influence of waiting time on survival was analyzed using Cox proportional hazard analyses. Kaplan–Meier curves for short (<8 weeks) and long (≥8 weeks) waiting times were constructed. Results A total of 351 patients were included; 214 received neoadjuvant treatment, and 137 underwent primary surgery. In the neoadjuvant group, the waiting time had no impact on disease-free survival (DFS) [hazard ratio (HR) 0.96, 95 % confidence interval (CI) 0.88–1.04; p = 0.312] or overall survival (OS) (HR 0.96, 95 % CI 0.88–1.05; p = 0.372). Accordingly, no differences were found between neoadjuvantly treated patients with waiting times of <8 and ≥8 weeks in terms of DFS (p = 0.506) and OS (p = 0.693). In the primary surgery group, the waiting time had no impact on DFS (HR 1.03, 95 % CI 0.95–1.12; p = 0.443) or OS (HR 1.06, 95 % CI 0.99–1.13; p = 0.108). Waiting times of <8 weeks versus ≥8 weeks did not result in differences regarding DFS (p = 0.884) or OS (p = 0.374). Conclusions In esophageal cancer patients treated with curative intent by either neoadjuvant therapy followed by surgery or primary surgery, waiting time from diagnosis to treatment has no impact on long-term outcom
