Introduction: The objective of this study was to analyse the effects of the first three years of treatment with recombinant human insulinlike growth factor 1 (rhIGF-1) in patients from the Polish population. Material and methods: Twenty-seven children (22 boys and five girls) aged 2.8 to 16.0 years old were qualified for treatment with rhIGF-1 (mecasermin) in different treatment centres, according to Polish criteria: body height below –3.0 SD and IGF-1 concentration below percentile 2.5 with normal growth hormone (GH) levels. Mecasermin initial dose was 40 μg/kg bw twice a day and was subsequently increased to an average of 100 μg/kg bw twice a day. Body height, height velocity, weight, body mass index (BMI), and adverse events were measured. Results: Mecasermin treatment resulted in a statistically significant increase in body height (1.45 ± 1.06 SD; p < 0.01) and height velocity in comparison with pre-treatment values. The biggest change in height velocity happened during the first year and diminished during subsequent years. Body weight and BMI also increased significantly after treatment (1.16 ± 0.76 SD and 0.86 ± 0.75 SD, respectively; p < 0.01). Eight patients reported adverse events. These were mild and temporary and did not require treatment modification except in two patients. Conclusions: Treatment with rhIGF-1 was effective and safe in Polish patients with primary IGF-1 deficiency. It had a clear beneficial effect on the height of the patients and significantly accelerated the height velocity, particularly in the first year of treatment

The Use of Robotic Process Automation (RPA) as an Element of Smart City Implementation: A Case Study of Electricity Billing Document Management at Bydgoszcz City Hall

    No full text
Smart cities are an extremely important, multi-faceted subject, both in terms of their practical aspects and in terms of research. This is expressed, among other things, in the multitude of approaches to this concept. These approaches differ based on the emphasis placed on individual aspects: some focus more on technology, and others put more weight on social issues, while still others value sustainable development issues. Currently, an important topic of discussion about the development of the smart city—the importance of which has become even greater in the context of the COVID-19 pandemic—is the digital transformation of the city. The use of robotic process automation (RPA) tools can be a part of such a transformation, as such tools, using advanced software, enable the automation of those tasks carried out thus far by humans. Although such an approach has, to date, been widespread in the case of enterprises (in particular, those operating in the financial and BPO/SSC sectors, but less often in the utilities sector, the first applications of these solutions in the context of process automation for cities are also beginning to emerge in various parts of the world. This article is based on a case study approach. The implementation conditions (including the constraints) of such an approach, the benefits achieved, and the lessons learned (which can be important for other local government units) are outlined using the example of the Bydgoszcz city hall’s (Poland) electricity billing document management. The results of the case study presented here lead to the conclusion that the use of RPA tools enables, very quickly and at relatively low cost, measurable results to be achieved that are related to the processing of electricity billing documentation for the city of Bydgoszcz. This allows the assertion to be made that robotic process automation can be taken into consideration as one of the tools used to build smart cities

An increased risk for the development of emphysema occurs in all carriers of deficientvariantsofAlpha-1-Antitrypsin (A1AT). A DNA polymorphism in the 3' promoter region of A1AT gene (locus Pi), which is important for a modulation of gene expression in a response to inflammation, seemstobeanadditionalgeneticriskfactor for emphysema. In the study, we present two relatively young patients with panlobular lung emphysema associated with a normal level of A1AT in serum. To contribute to our knowledge about a molecular basis of the pulmonary changes, the evaluation of A1AT phenotype with the use of an isoelectrofocusing of serum proteins and the analysis of DNA polymorphism in the 3' region of A1AT gene (Taq I RFLP) have been performed. The obtained results indicate on a necessity to include procedures of A1AT phenotyping and locus Pi genotyping in both the population screening of patients and the detailed clinical diagnostics of young patients with emphysema and/or COPD.Pneumonol.
