    Chasing the Dragon Away: Personality as a protective factor and extended-release naltrexone as a treatment for heroin dependence

    Opioid dependence causes severe problems for patients and their family members and imposes an enormous economic burden on society. The main objectives of this thesis were (a) to gain better insight in the process of getting addicted to heroin in order to develop personality-based prevention strategies, by studying distinctive personality patterns in heroin users that became heroin dependent and heroin users that never became dependent; (b) to assess the interest of heroin dependent patients and their treatment providers in treatments directed at total abstinence and specifically in the treatment with extended-release naltrexone (XRNT); and (c) to study whether treatment with XRNT in heroin dependent patients results in decreased striatal dopamine transporter (DAT) availability and in anhedonia and depression as potential side-effects. From this thesis, it can be concluded (a) that there are distinctive personality traits that form a risk for or protect against heroin use and heroin dependence that may be used for the development of personality-targeted personalized interventions; (b) that many patients are interested in becoming fully abstinent and in treatment with XRNT; (c) that treatment with (extended-release) naltrexone seems to have no significant effect on striatal DAT availability and is not associated with a reduced pleasurable response to natural rewarding activities (anhedonia)

    Offshore wind farm optimisation: a comparison of performance between regular and irregular wind turbine layouts

    Layout optimisation is essential for improving the overall performance of offshore wind farms. During the past 15 years, the use of yield optimisation algorithms has resulted in a transition from regular to more irregular farm layouts. However, since the layout affects many factors, yield optimisation alone may not maximise the overall performance. In this paper, a comparative case study is presented to quantify the effect of the wind farm layout on the overall performance of offshore wind farms. The case study was performed to investigate two performance indicators: power performance, using yield calculations with WindPRO and wake-induced tower fatigue, using the Frandsen model. It is observed that irregular wind farm layouts have a higher annual energy production compared to regular layouts. Their power production is also more persistent (less sensitive) to wind direction, improving predictability and thus market value of power output. However, one turbine location in the irregular layout has a 24 % higher effective turbulence level, leading to additional tower fatigue. As a result, fatigue-driven tower designs would require increased wall thicknesses, which would result in higher capital costs for all turbine locations. It is demonstrated in this study that layout optimisation using a minimum inter-turbine spacing effectively resolves the induced wake issue while maintaining high-yield performance.</p

    Facing difficult but unavoidable choices: Donor blood safety and the deferral of men who have sex with men

    Blood service organizations employ various ways to ensure transfusion blood safety, including the testing of all donations for transfusion-transmissible infections (TTI) and the exclusion of donors who are at increased risk of a recent infection. As some TTIs are more common among men who have sex with men (MSM), many jurisdictions (temporarily) defer the donation of blood by sexually active MSM. This boils down to a categorical exclusion of a large group solely on the basis of their sexual orientation, which is seen as unduly discriminatory and stigmatizing. Blood service organizations in the U.K. and the Netherlands have recently changed their deferral policies for MSM. The problem of the MSM deferral involves a conflict between fundamental rights: the right of MSM to equal treatment and the right to health of the recipients of blood and blood products. We distinguish and discuss three broad alternative options to the current categorical deferral of MSM donations: (1) completely abandoning donor selection on the basis of sexual behavior, (2) individual risk assessment of the sexual activities of each potential donor, and (3) individual risk assessment of the sexual activities of MSM only. The new U.K. policy falls within the second category, and the new Dutch policy is in the third category. We argue that each approach comes with moral costs but that the most reasonable option is different from the policies of both the U.K. and the Netherlands

    We present a patient with acute myeloid leukaemia who developed subcutaneously disseminated aspergillosis after allogeneic peripheral stem cell transplantation (PSCT). Disseminated aspergillosis after stem cell transplantation has a high mortality despite treatment with amphotericin B or one of the azoles. Aspergillosis in our patient was refractory to amphotericin B and itraconazole but was successfully treated with caspofungin acetate

    Roadmap to the multidisciplinary design analysis and optimisation of wind energy systems

    A research agenda is described to further encourage the application of Multidisciplinary Design Analysis and Optimisation (MDAO) methodologies to wind energy systems. As a group of researchers closely collaborating within the International Energy Agency (IEA) Wind Task 37 for Wind Energy Systems Engineering: Integrated Research, Design and Development, we have identified challenges that will be encountered by users building an MDAO framework. This roadmap comprises 17 research questions and activities recognised to belong to three research directions: model fidelity, system scope and workflow architecture. It is foreseen that sensible answers to all these questions will enable to more easily apply MDAO in the wind energy domain. Beyond the agenda, this work also promotes the use of systems engineering to design, analyse and optimise wind turbines and wind farms, to complement existing compartmentalised research and design paradigms
