    A Scanning Electron Microscopic Morphological and Semi-Quantitative Evaluation of Rat Stomach Treated with Colloidal Bismuth Subcitrate and Alcohol

    Scanning electron microscopy was utilised to study the effect of absolute alcohol on the normal morphology of the rat stomach, together with the gastroprotective actions of colloidal bismuth subcitrate. Studies on normal gastric morphology revealed that the major portion of the stomach was covered by a protective coating of mucus. However, there was considerable variation in the integrity of the mucosal surface of the control animals, with the loss of surface epithelial cells in some regions which may account for the variation in response to necrotising agents. The long-term administration of the gastrocytoprotective agent colloidal bismuth subcitrate resulted in a marked improvement in normal gastric integrity, compared with control tissue samples. The administration of absolute alcohol was associated with an excessive production of mucus and caused extensive damage to the gastric mucosa of control animals, resulting in destruction of the surface epithelial cells and exposure of the reticular framework. However, there was evidence that repair of this damage was underway by four hours after ethanol treatment, with a significant degree of recovery from damage occurring by 24 hours after treatment. In contrast, treatment with colloidal bismuth subcitrate prior to the administration of alcohol resulted in a significant reduction in the degree of damage induced by alcohol administration, suggesting that colloidal bismuth subcitrate has the ability to protect the stomach from the erosive action of alcohol

    Integration of patient-reported outcome measures with key clinical outcomes after immediate latissimus dorsi breast reconstruction and adjuvant treatment

    Background: linical evidence on patient-reported outcome measures (PROMS) in breast reconstruction is lacking. The aim of this study was to evaluate PROMs in implant-assisted latissimus dorsi (LDI) or tissue-only autologous latissimus dorsi (ALD) flap reconstruction in relation to complications and adjuvant treatments.Methods: this was a prospective cohort study involving six UK centres. Eligible patients had primary early-stage breast cancer. The European Organization for Research and Treatment of Cancer quality-of-life questionnaire (QLQ)-C30 and QLQ-BR23, Functional Assessment of Cancer Therapy—Breast Cancer scale (FACT-B), Body Image Scale, and Hospital Anxiety and Depression Scale were completed before operation and at 3, 6 and 12 months after surgery.Results: a total of 182 patients (82 LDI and 100 ALD) were recruited between 2007 and 2010 with symptomatic (59·9 per cent) or screen-detected (39·6 per cent) cancers. Some 64·3 per cent had lymph node-negative disease; 30 per cent of the LDI group had radiotherapy, compared with 53·0 per cent in the ALD group (P = 0·004). Early complications up to 3 months after surgery were reported in 66 and 51·0 per cent of patients in the LDI and ALD groups respectively (P = 0·062) and long-term complications (4–12 months) in 48 and 45·0 per cent (P = 0·845). Role functioning and pain (P = 0·002 for both) were adversely affected in the ALD group compared with results in the LDI group, with no significant effects of radiotherapy on any health-related quality of life (HRQL). Chemotherapy and early complications adversely affected HRQL, which improved between 3 and 12 months after surgery (P < 0·010 for all).Conclusion: there is evidence of similar HRQL between types of latissimus dorsi breast reconstruction for up to a year after surgery. There appear to be no overarching effects for radiotherapy after mastectomy on the specific HRQL domains studied in the short term. The identification of variables that affect HRQL is important, including their integration into the analysis of PROM

    Reduced-Complexity Transmit/Receive-Diversity Systems

    We consider wireless systems with transmit and receive diversity. For reduction of complexity, we propose to use hybrid selection/maximal ratio transmission at one link end, choosing L out of N antennas. We analyze the performance of such systems, giving analytical bounds and comparing them to computer simulations. Outage probability, symbol error probability, and capacity are shown. We demonstrate that in typical cases, a small number of used antennas L is sufficient to achieve considerable performance gains. We also analyze the infuence of the number of base station antennas, of fading correlation and channel estimation errors. The simulation results confirm that the proposed scheme is effective in a variety of environments

    Moral Distress in Critical Care Nursing: The State of the Science

    Background: Moral distress is a complex phenomenon frequently experienced by critical care nurses. Ethical conflicts in this practice area are related to technological advancement, high intensity work environments, and end-of-life decisions. Objectives: An exploration of contemporary moral distress literature was undertaken to determine measurement, contributing factors, impact, and interventions. Review Methods: This state of the science review focused on moral distress research in critical care nursing from 2009 to 2015, and included 12 qualitative, 24 quantitative, and 6 mixed methods studies. Results: Synthesis of the scientific literature revealed inconsistencies in measurement, conflicting findings of moral distress and nurse demographics, problems with the professional practice environment, difficulties with communication during end-of-life decisions, compromised nursing care as a consequence of moral distress, and few effective interventions. Conclusion: Providing compassionate care is a professional nursing value and an inability to meet this goal due to moral distress may have devastating effects on care quality. Further study of patient and family outcomes related to nurse moral distress is recommended

    Nanopore-based kinetics analysis of individual antibody-channel and antibody-antigen interactions

    <p>Abstract</p> <p>Background</p> <p>The UNO/RIC Nanopore Detector provides a new way to study the binding and conformational changes of individual antibodies. Many critical questions regarding antibody function are still unresolved, questions that can be approached in a new way with the nanopore detector.</p> <p>Results</p> <p>We present evidence that different forms of channel blockade can be associated with the same antibody, we associate these different blockades with different orientations of "capture" of an antibody in the detector's nanometer-scale channel. We directly detect the presence of antibodies via reductions in channel current. Changes to blockade patterns upon addition of antigen suggest indirect detection of antibody/antigen binding. Similarly, DNA-hairpin anchored antibodies have been studied, where the DNA linkage is to the carboxy-terminus at the base of the antibody's Fc region, with significantly fewer types of (lengthy) capture blockades than was observed for free (un-bound) IgG antibody. The introduction of chaotropic agents and its effects on protein-protein interactions have also been observed.</p> <p>Conclusion</p> <p>Nanopore-based approaches may eventually provide a direct analysis of the complex conformational "negotiations" that occur upon binding between proteins.</p

    A novel, fast, HMM-with-Duration implementation – for application with a new, pattern recognition informed, nanopore detector

    <p>Abstract</p> <p>Background</p> <p>Hidden Markov Models (HMMs) provide an excellent means for structure identification and feature extraction on stochastic sequential data. An HMM-with-Duration (HMMwD) is an HMM that can also exactly model the hidden-label length (recurrence) distributions – while the regular HMM will impose a best-fit geometric distribution in its modeling/representation.</p> <p>Results</p> <p>A Novel, Fast, HMM-with-Duration (HMMwD) Implementation is presented, and experimental results are shown that demonstrate its performance on two-state synthetic data designed to model Nanopore Detector Data. The HMMwD experimental results are compared to (i) the ideal model and to (ii) the conventional HMM. Its accuracy is clearly an improvement over the standard HMM, and matches that of the ideal solution in many cases where the standard HMM does not. Computationally, the new HMMwD has all the speed advantages of the conventional (simpler) HMM implementation. In preliminary work shown here, HMM feature extraction is then used to establish the first pattern recognition-informed (PRI) sampling control of a Nanopore Detector Device (on a "live" data-stream).</p> <p>Conclusion</p> <p>The improved accuracy of the new HMMwD implementation, at the same order of computational cost as the standard HMM, is an important augmentation for applications in gene structure identification and channel current analysis, especially PRI sampling control, for example, where speed is essential. The PRI experiment was designed to inherit the high accuracy of the well characterized and distinctive blockades of the DNA hairpin molecules used as controls (or blockade "test-probes"). For this test set, the accuracy inherited is 99.9%.</p

    Infrared Spectroscopic Study of a Selection of AGB and Post-AGB Stars

    We present here near-infrared spectroscopy in the H and K bands of a selection of nearly 80 stars that belong to various AGB types, namely S type, M type and SR type. This sample also includes 16 Post-AGB (PAGB) stars. From these spectra, we seek correlations between the equivalent widths of some important spectral signatures and the infrared colors that are indicative of mass loss. Repeated spectroscopic observations were made on some PAGB stars to look for spectral variations. We also analyse archival SPITZER mid-infrared spectra on a few PAGB stars to identify spectral features due to PAH molecules providing confirmation of the advanced stage of their evolution. Further, we model the SEDs of the stars (compiled from archival data) and compare circumstellar dust parameters and mass loss rates in different types. Our near-infrared spectra show that in the case of M and S type stars, the equivalent widths of the CO(3-0) band are moderately correlated with infrared colors, suggesting a possible relationship with mass loss processes. A few PAGB stars revealed short term variability in their spectra, indicating episodic mass loss: the cooler stars showed in CO first overtone bands and the hotter ones showed in HI Brackett lines. Our spectra on IRAS 19399+2312 suggest that it is a transition object. From the SPITZER spectra, there seems to be a dependence between the spectral type of the PAGB stars and the strength of the PAH features. Modelling of SEDs showed among the M and PAGB stars that the higher the mass loss rates, the higher the [K-12] colour in our sample.Comment: 14 pages; accepted in MNRAS, 200
